I just spent 3 weeks here after a knee injury. The staff here is incredible. From administration to the nurses, the aids who care for us daily, the OT and PT staff. Housekeeping who go out of their way to help. Special thanks to Nelda and Av who supplied me with wakeup coffee and smiles in the mornings. My father was placed in Fulton Gardens for end of life care. We had not heard much about this place and were unsure. Both of my parents insurances accepted and recommended. We are and were so happy with ALL and I mean all the care he received from the moment he got there. We could not be more pleased with all the nurses in this facility. No matter what we needed and when we needed it ... it was given. The care and compassion these workers had was remarkable. End of life wasn't a positive experience but they made my dad comfortable. My grandpa had a bad fall and needed Therapy after St. Joseph's Hospital couldn't keep him longer. They were straightforward with the cost of his stay and you can tell they were organized because he already had his medication & a sweet professional girl from the Admissions department already came to talk to him when he came in. I didn't understand much about this facility, but at least they kept my grandpa clean and he actually wanted to work with the therapy team. They could update their drawers, but I cared more about their clean walls and clean bathrooms. Their shower rooms had marble-you could tell the most important things were remodeled. They even had a wheelchair for him so I didn't have to buy one. I felt reassured leaving him there, but I'm glad he's home feeling stronger and more confident! I don't usually write reviews for a place like this but this was more serious because this was for my grandpa. read more

Fulton Gardens Is extremely clean. The staff/nurses were always very kind, compassionate and always went beyond the call of duty to make my dad comfortable during his final days. I can't say enough good things about this facility. read more

My husband lasted 2 1/2 days in this supposed care home before he died. He wasn't given meds unless he asked for them!!! I repeatedly asked for them to send him back to the hospital. They only called an ambulance when they found him unresponsive. A sick dog would receive better care. Please don't leave your loved one here read more

A family member broke a hip and needed rehab after surgery. I checked on the Medicare ratings for rehab and none in my areas ad five stars. Most were deficit in at least one area and another had Covid 19 death. As a result we chose to go 35 miles away to Fulton Gardenss. Virus protocol. No inside visitors. iI was very apprehensive about what might take place in the facility and how the patient who has a hearing deficiency might fare without visitors. I could be happier. The patient exceeded my expectations for sPT results, the staff were all accessible, kind, informative and caring. Fulton Gardens is a well run rehab place. Questions which in this case had to be left on message machines were always answered the same day. Any medical changes were reported to the nearest of kin. No one wants to be in these facilities, but when the need arises check out this place. read more

On my recent visit to Fulton Gardens Post Acute Facility, overall, I found the staff to be very nice, with the exception of one employee uttering something as she walked straight past a sobbing despondent woman in a wheelchair completely invalidating her as a human being. I understand staff are busy, but should never be too busy to show just a moment of acknowledgment and kindness. The witnessing of that will forever be seared in my memory. Can COMPASSION be taught to these type of employees?! They can begin by practicing! BEDROOMS...Although the Facility was remodeled two years ago, and appears nice and clean, etc..I was struck by how ugly and barren which means, "lacks any sign of life" my relatives room appears. Not one picture or any color on the cold white walls. I feel sad about this. How would I feel if I had to live in this room??? Depressed!!! LAUNDRY...when a person is admitted into this facility, someone other than the frail Patient/Resident unless capable, preferably a relative needs to be responsible for writing a COMPLETE Inventory of items, and mark ALL clothing including socks and underwear with a marking pen, or you can expect not everything will be listed, and after laundering to find someone else wearing your loved ones clothes, and or, items disappearing with no recourse, because they were not marked. Administrator Elizabeth is aware that these issues need to be addressed right away. BOUNDARIES...Staff should NEVER enter a Residents room, and go through their belongings without their permission, or the Resident present! This is a violation of Patient/Resident boundaries regardless of the reason. How would you feel??? FOUNTAIN...When a broken Fountain, or one that appears to be broken, is the first thing a visitor sees when they approach the building, it is a clear sign and public perception that other issues are probably not being attended to. 5 Star Facilities are aware of this! Lastly, when a Patient rings their buzzer for assistance, employees must NEVER take their time or assume the patients need is not urgent. Lying on the floor after a fall is unacceptable. LASTLY...My hope is that these issues are addressed as soon as possible in Staff Meetings and expectations are expressed to make these needed changes. Once these changes are solidified, I would be happy to give a 5 Star Rating. Thank you read more

Can't say much about this place. Seen one nursing home seen most, but, not all. Mainly because they are not all the same. Some are not so good, some are good and some are very good. And then you have the occasional one that is excellent. This one is very clean. That nauseating odor wasn't there when I walked in the front door. Floors were clean. Restrooms were clean. The staff was pleasant and the rehab staff was very professional. So why the 3 stars?? Well I'm not a fan of the place and it's not as good as it gets, It's just OK. read more

Lasalette is decent place to be if you are in need of placement of a love one or in need of some rehab .due to illness. The rehab. staff is excellent the nurses are good the nursing assistants are top notch for the most part .There is a new administrator very nice respectful woman. If you need a place to bring a loved one this is a place to bring them the building is clean . The staff is friendly and welcoming so have no fear in bringing them here there have been many resident living here for years it's a home away from home read more

If I could give this place negative stars I would. I'm not someone who would usually make complaints or leave negative reviews for a business / company but my grandmother almost DIED in the care of Fulton gardens. She already has congestive heart failure, and she is on 24/7 oxygen. She has caught covid TWICE in their care, and she is now in the hospital fighting for her life, because of covid she now has pneumonia. She also has swelling all over her body and fluid around her heart. Her blood pressure is dangerously low, and because it is so low, the hospital can't even give her medication for the swelling. My grandmother also had to use the restroom (#2) for 2 days and no one would help her to the bathroom. When it was brought up to the staff, Josephine Valdez laughed about it. They are going to have a lawsuit on their hands, believe that. This place is UNPROFESSIONAL. DO NOT leave your loved ones in this facility. Not if you want them to live. read more

Had to make a yelp account just to let the public know the truth! If there's one thing I advise in life this would be it. PLEASE DONT PUT YOUR LOVED ONES HERE. The nurses working here hate their life and simply just work here for their paycheck. They give no care for the patients and would rather have them die and suffer in their last few days of their lives. Everyone here is rude especially the Administrator to the Director of Nursing and Director of nursing assistant. If you ask them for little things they simply make a fuss about it and give you a hard time. They made the last few days of my grandfather life miserable. They go out of their way to blame things on you and not them. Like is the whole point of a care home is to care for them?? Why neglect them? Again please don't put your love ones here. I'm normally not someone that writes reviews but I wanted to share my experience at Fulton Gardens. My mother was admitted there for therapy after a pretty severe stroke. She needed quite a bit of therapy and we were not sure she would ever be able to return home, but Fulton Gardens staff was there every step of the way. They explained and support us from the insurance, and financial questions, to the health and rehabilitation concerns. The nurses and CNAs were so attentive and caring throughout Mom's stay. After several weeks of therapy she had made an amazing recovery. She was able to return to her own home and is doing well! Thank you Fulton Gardens staff for all your help! read more

I never write reviews but this place I have to! I would give this place -stars if I could! My mother had a massive stroke, was sent here to help her regain her mobility. She wasn't there but 2 days before falling and busting her head wide open! She was sent back to the hospital in critical condition! Her room was the size of a closet, she fell into the hallway and it still took the staff about 15 minutes to see her! Her blood pressure was also very high and her current doctor doesn't think the staff was even giving her, her medication. Please don't send your loved one here. LaSalette sadly needs attention from the Owner of the facility. At a charge of $8k a month it is not fit for any soul trying to make any kind of healthy recovery. I can tell you that animals in kennels have better air conditioning conditions than the patients in this facility. I do have to say that the staff is one of the best I have ever encountered. From the greeting to every person in their capacity demonstrating genuine when they are approached for assistance. Unfortunately it is well beyond the staff to make "things happen " like ensuring they have a good air conditioner in every room. A few top line directors mentioned that the condition of the facility is in their words deplorable because the facility needs updating. So, if you don't mind the heat that each patient is experiencing and an occasional piece of fuzz or lint cooked into the food that is served this may fit your need. Everyone who actually WORKS in this facility is enjoying their A/C but the people that are paying through Medicare/Medicaid or personal insurance should be wary of putting loved one in this facility. read more

The facility is clean, there is no smell when you walk in which is a big plus for a skilled nursing facility, and the patients seem clean and taken care of. The furniture isn't the best, but at least the patients have small flat screen TV's in their room. However, apparently the AC doesn't work well in her room and a fan had to be brought in to keep them cooler. I would hate to be living there in the summer. My grandmother entered their facility last Saturday, and by Tuesday, she was in bad condition and placed on hospice care. When I arrived on Thursday, to meet with the hospice nurse, my grandma was laboring in her breathing. She was on oxygen (and they have a oxygen concentrator that makes this loud humming noise to dispense the oxygen, I guess they do not have the updated equipment like they had at my dad's previous nursing home, which came from a panel on the wall) and the Hospice nurse said she needs morphine. She went to go find out about it and said that the nurse is going to bring it in. The Hospice Nurse steps out to make a phone call and then my grandma's nurse walks in and gives her the med and said rudely, "she got this two hours ago" and walked out. I was shocked by this attitude, my grandmother is dying and she is upset that I am bothering her. I asked the Hospice Nurse, is this place safe for my grandma, is she going to get good care here. She said the nurse was eating when she found her and she offered to get my grandma the medication even after the Hospice Nurse said she would do it. The Hospice Nurse said she left her break early to give the pain med so my Grandma could get it. I felt she did it because she wanted to look good in front of the Hospice Nurse, and not from the kindness of her heart. The attitude nurse was on Thursday and Friday and she would come in and check on my grandma's roommate and never checked in on my grandma, even though she was her nurse too. She would walk by our room and look at us and then kept walking. On Friday, I stayed two to three hours at a time and we did not see anyone check in on her accept when the shift changed, and when the CNA came in on the swing shift to introduce herself and did a quick check on my grandma. My mom who was there all times of the day commented that the staff was not very attentive compared to the two skilled nursing facilities, my father stayed at in the past. When my grandma was getting closer to her time, we wanted to get her a dose of morphine to lessen her labored breathing and stuck our head out and didn't see anyone. This was about 7:30 p.m. We waited to see if we could find anyone but decided to ring the bell and that took about 10 minutes which wasn't so bad and thank goodness we got another nurse besides our attitude nurse, who quickly got my grandma the medicine and she gave us the bad news that her time would be coming. She was kind and the CNA who was helping was so sweet, and she even came to give my brother and I hugs before she left for the evening. Her and the LVN were the only bright spots of this facility. I would never send anyone else I loved to this facility. We were not given a choice because the lousy social worker at St. Joseph said this was the only facility available for long term care. I would want a facility that was more attentive and considering my grandma's quick decline, I would hope that the staff would be checking in on her more to make sure she was comfortable. During her last night, I had left my brother and mom, to drop off my sister at home, and when I came back it was 9:30, after visiting hours. You were suppose to ring to be let in. I rang and waited for five minutes, no answer and no staff walking about. I texted my brother to open the door because there was no one around. I gave two stars because I was touched by the two people who showed my family and I compassion during one of our toughest times. However, I felt that more attention should be given by the staff especially when the patient is very sick and near death to make sure that they are comfortable and not ignored because they will be dead soon. read more
