My Mom had surgery at St Joes and was discharged to Clearwater Healthcare Center. She was sent there to help her infection clear, maintain dialysis treatments and keep her diabetes in check. Here's out it went: - They repeatedly failed to give her meds. The first time this happened, she was sent BACK to St Joes with a blood sugar reading OVER 400 and blood pressure reading over 200. St Joes managed to get her back on track, and she was returned to Clearwater for medicine management and recuperation. - Our second go-around with Clearwater, and both my sister and I explained the need for Mom to get her meds (insulin, BP meds and dialysis) at each and every prescribed time. Again, they failed to give Mom her meds. I called both at 5pm when she arrived and they were "working on the orders". I called at 5am the next day and she had no insulin whatsoever. - My Mom calls me and explains that a man is vomiting and screaming across from her room and asking for help, and no one from the facility is helping this man. I can hear him screaming on the phone. - Someone tried to climb into my Mom's bed in the middle of the night. Literally, I am without words. - I called on the fourth day to ASK THEM to do their job and administer meds, they left me on hold for THIRTY MINUTES. They came back on the phone and said, "We explained everything to another family member, we're not going to explain again, call them." And they HUNG UP ON ME. Terrible service. Terrible response. You literally have one job, and it's taking care of people - and they end up BACK in the hospital because you won't administer the meds that are prescribed. WHERE DO THOSE MEDS GO?

The social worker Mia…read moredischarged him after 6 weeks to a facility in Lathrop stating that he's independent and walks with a walker. He's in a wheelchair due to having one leg. She lied to the new facility about his condition. Their driver left him outside the facility. I tried contacting the Social Worker and the director and keep getting told they are in a meeting or not in. Please dont send your family there!

    This review is specifically for the skilled nursing and rehabilitation services. After a 1 week…read morestay at the local hospital my grandmother was recommended to Meadowood at O'Connor. She was septic with pneumonia. She was alert, moving, talkative and cheerful when she arrived here and within her 1 week stay she took a drastic turn for the worse. I feel this place is severely understaffed. Each day we noticed the lack of care she was receiving from the staff. We could also hear other patients in rooms nearby shouting for help and nothing was done for them either. The staff also mistook her for another patient so the fact that they could not even remember or keep track of her needs was alarming. They were giving her all the wrong foods, liquids, and hardly any medication to improve her health and get her strong enough to come home. Even after filling out their little cards for specific food and drink instructions the changes were not made. Within 1 week she went from cheerful and talkative to being so weak that she was unable to feed herself or formulate full sentences. The PT staff said she was progressing so well she was walking herself around to the dining area and would be released soon while the actual nurses were saying she was so weak they couldn't even assist her to the bathroom anymore. Talk about inconsistent and polar opposite feedback from the staff about the same person... since they couldn't help her to the restroom anymore she was going in the bed and constantly needed to be changed which they would take 1-2 hours to complete which is a bit dehumanizing and humiliating to have to sit in and after they changed her and the sheets they did not put her blankets back on her - mind you this was in the middle of winter. She was shivering and curled up in a ball with blankets on the floor when we saw her. She was severely dehydrated yet the entire time they did not have her on any IV fluids to get her body the nutrients it so desperately needed. After claiming she would be released for HOME (again I have no idea how they were able to say this to us when they we were clearly not talking about the same person) she was actually transferred back to the local hospital. From there the local hospital deemed her health was so irreversible that all there was left to do was to take her to hospice care. She passed away only 4 days later. I blame this facility for her rapid decline and I also blame myself for allowing her to stay here. We should've taken her out after all the mistakes and neglect here. When she arrived here she was her full self and when she left here she looked skeletal. She lost her figure and you could tell she was dying. Please think twice before taking your loved ones here and keep a close eye on the care they are receiving. If you feel like this is not the right fit, listen to your gut and find a different facility. It may look nice from the outside but the care is some of the worst I have seen.

He's very convincing but all he wants is your money. He'll make promises all day long, but delays on them until your loved one leaves and no more money is coming in. He'll even tell you the chef worked in a 5-star hotel! If the cook worked in such a place, he was probably a bus boy, not a chef! I will eat almost anything, but I tried my wife's food and spit it out. Every day you complete a form saying what you do NOT want...and that is exactly what they serve. Not once in over a month did they get it right! The rehab staff is nice and good when they work you. But they only work you a half hour; not an hour...or they talk to you the entire time and you get no exercise. I would give the facility a 2-star and the administrator and HR person a negative 5-star. Dom said he was in charge of several different facilities...so don't go to any of them where he exists.

    My mother has recently been discharged from the hospital following a severe urinary tract infection…read more(UTI). A contributing factor appears to be the failure to change her promptly during shifts--sometimes not at all for an entire shift or even two. Since her return, I have been present and have observed the same issue recurring. If this were the cause of her hospitalization, one would expect the Certified Nursing Assistants (CNAs) to consistently maintain her hygiene to prevent a recurrence of the infection. Additionally, I find it inappropriate that CNA Paul informed my mother that his discomfort in changing her was due to her not wearing two diapers to make his job easier. This comment caused her significant distress, leading her to tears. Furthermore, today my mother was not changed throughout the entire day. When Paul began his shift, he was instructed to change her immediately. He responded that he would seek assistance to do so. After approximately thirty minutes, I checked to see if help had arrived. Upon asking him if he intended to change my mother, he inquired whether she wished to be changed now, despite having been instructed to do so promptly at the start of his shift. Such actions contribute to the neglect and continued risk of UTIs and infections among nursing home residents. It is only because of dedicated professionals such as nurses Lola and Edward, who genuinely care for their patients, that my mother has not been entirely neglected. Prior to this incident, I contacted Edward about a week earlier to report that my mother had again not been changed during Paul's shift, and her alert light had been on for over an hour. Edward promptly notified Lola, who took corrective action to ensure my mother was attended to. I urge the facility to implement strict protocols for regular patient care and staff accountability to prevent neglect and infections. I ask you to consider how you would feel if your mother were treated in this manner. It is imperative that measures are taken; without prompt action, I may be compelled to escalate this matter publicly. I believe that The Oak Grove Post Acute in Stockton, CA, could be a better environment--particularly if staffed by compassionate and dedicated individuals like Edward, Lola, EJ, Trishell, Araceli, Jezzryl, Ashley, and others who genuinely care for their patients. My mother and I can discern the difference between staff members who perform their duties merely for a paycheck and those who genuinely care about their patients and find fulfillment in their work. I am requesting that this situation be addressed and rectified to ensure that residents like my mother experience more moments of happiness, feel safe, and receive the appropriate level of care in a facility that should be their sanctuary, not a place of neglect. Lola and Edward are saints in my book and could do no wrong. I don't want to, but I will continue to write these reviews and raise concerns until they are taken seriously, and measures are taken to keep my mother and other patients safe, taken care of, and not neglected. I can't even imagine how the patients who cannot speak up for themselves have been treated and neglected. My heart goes out for those patients. The new company is named Oak Grove Post Acute, not Stockton Nursing Center anymore.

The SNF nurses and administrative staff were exceptional in their calls and overall communication with me (a daughter of a former resident there). Staffing levels and food quality were at par with other nursing homes. There are large spacious rooms such as the rehab gym and dining room plus a courtyard with live greenery and even a few cats. Dr. Smith, the SNF MD, is exceptional - a great listener, very practical, with strong and solid medical advice and care. He shared excellent - research-based rationales for his recommendations. The reason for the 4 stars is that the facility is in need of a refresh such as bright & clean interior painting, new curtains, and updated closets & cabinets - as they've seen better days... Nevertheless, Stockton Nursing Center has wonderful nurses and nursing assistants and is a good facility.

They also put a diaper on every single patient as soon as they arrived. Which they also took their time to change most of the time. Nurses never answered the call buttons. And if they did-- it took at least 20 minutes to an hour. My mom was a healthy human being when she arrived here...they sent her to St Joseph's (never informed the family btw. We found out from her clinic bc she missed an appointment) ... when she arrived there she was found to be extremely weak and have a pretty bad case of pneumonia as well as a blood infection. Long story short...my mom suffered for another month until she passed away this past week. So NO. SO NOT GO HERE. STAY AWAY.

    What a busy little world in itself.  My very first experience with convalescent environment…read more  So much involved.  So much to consider.  So many staff hustling and bustling doing their parts.  Most of which appeared happy with their parts .  It was a little rough at first.  Looking out for my brother (Mr Apilado).  On the other side of a stroke and brain surgery, he didn't know what he needed. After asking many questions and establishing a good communication, all of his needs were pretty well accommodated.  I did learn about the daily menu ordering. Writing "diabetic" and "low sodium" can help the preparation proper process. The Physical Therapy people were so awesome.  This business can be depressing to be away from home, strange place, strange bed, strange people everywhere, many just as unhappy about individual discomforts, all kindness extended from staff can go a long way. Thank you Windsor staff. Oh, one more thing. Whom ever is in charge of answering email on the website, must not have a daily reminder to check it.  Today is Feb 26, 2019, Its been couple few weeks since I emailed, have yet to receive any response 3/11/19.  One information request.   One nursing concern. My grandfather had hip surgery and I fell for the "new name and management" of this place!!! This was my biggest mistake they had my grandfather on a wheelchair on strong pain killers literally sleeping with his bad leg that he just got surgery on "resting" on a Damm plastic trash can! They tried taking him to "PT" while he was on these strong pain meds that literally made him unconscious. That is a fall risk itself. He was at this facility for 3 days and that's only because I was afraid of not having everything set up at home for him. I took him out as soon as I could there was no way I was leaving him. He had a UTI because he had to hold his pee for hours because no one would go help him up. We picked him up to the restroom not the "nurses" this place is heartbreaking I literally cried those 3 days when I had to leave. They have roaches and they expect for him to use broken walker with tape all over it. Are you insane ? They need to shut this place down. So many people yelling for help and no one would help them. The van that transports is only doing there job of dropping off at the doctors appointments not one person from this facility went with my grandfather if you don't go they send them on their own and if they miss the van they have no clue. This happened to us but thankfully I drove behind this van and went with him to his appointment and that day I took him home. He recovered fantastic at home even after the "nurses" and the "director" told me I was making a mistake taking him because I didn't have a special bed ? They don't have anything that was helpful for him I'm so happy I followed my heart and took him out !

    My mom was at this facility for about 11 days after having a stroke. I say first couple of days…read morewhen mom Needed higher level of care staff was attentive as cna's followed rn's orders real well. When her care was less critical I would say it was bordering on neglect . A cna said she was in charge of 15 patents and almost half had to be spoon fed including my mother. Hence, myself and Family made sure to be there so she could eat. My observations were patients who didn't have family step in for them might of not ate well. I heard one rn supervisor talk extremely rough to like a dementia patient to be still and stop moving. It appeared as work burn out. There were many times mom was soiled and staff would not come to help. I had to change mom myself and do best I can to get her to the restroom, changed and brought back to bed. They had no sense of urgency in helping despite telling me to not take mom alone. I waited but when it started approaching two hours I had to do it . I also Had to help cna put mom To bed as she had no help as mom was too weak and a two person job. They got mom's diet order wrong as moved up to solids and kept on doing a blender diet for days. We had to Go out and buy food so she can eat as she hated blended diets and I don't blame her. I was told this happened cause dieticians use a different system and it don't data share with the facility but nobody could make a phone call. There was like a comittee but I don't think they were proactive (except supervisor of nursing) as made excuses For some of my concerns . I am not going to say this facility is the worse but definitely not the best. I was told this place was the best in town. I am Just going to say it's like any other nursing facility from what I hear. When I thought mom was well enough to go home I demanded she leave immediately as they wanted to keep mom longer. I overheard admin mentioning mom was a "good" patient as had dual insurance. I told them at my house mom is not neglected and will get better care as I had all her bags packed. I don't recommend this facility for your loved one and it's definitely not the best in town but can't say the worse cause I haven't seen many . It missed my high expectations and them trying to say get better care cause they are small is garbage.
