Five years ago my husband was in "Assisted Living" at Hartsfield Village for 13 months. Additionally, we employed a person who was with him 24 hours/day and slept on a couch in his apartment there. I cannot say if I would have felt that he got all of the care he needed without her but the facility was clean and the food was good. The public spaces were modern and nicely furnished. The billing was accurate and everyone was pleasant. There were some programs that enriched life there and there was free access to the "Independent Living" part of Hartsfield Village . read more

PLEASE READ ALL THESE NEGATIVE VIEWS BEFORE YOU GO IN THERE. I was in the rehabilitation section. Absolutely horrible experience at this place. I was there for 7 days of hell. I had a major spine surgery and was not able to get out of bed for at least two days. When I needed to use the bathroom, I turned on the nurse's alarm and no one came and 30 minutes passed. And I ended up urinating in the bed at three different occasions due to no one coming to help me. I just feel as an adult, and you're urinating in your bed that it is just a disgusting feeling. That should have never happened!!!! I gave them plenty of time to come and help me. At one time it was a forty five minute wait, and still no one came. I ended up urinating in my underwear and in my clothes, and so I had to put those in a bag for someone to help me with my laundry. Well that's another story, after a week of being there, I had a full bag of laundry. Every time I asked one of the aides, is anyone going pick up my laundry and they said, let me check and they never came back. So I had to leave my laundry sit in my room. Finally, I ended up giving it to an aid tossing it out into the hallway to her and asking her to take that to laundry. I had my name and my room number written huge, in black marker on both sides of the white bag.Well, needless to say, my laundry got lost!!! At that point I only had one pair of underwear left because I had limited amounts of clothes there. I had nothing to put on because everything was dirty. I tried my best to find out where my clothes were. And why they weren't coming back and went through many channels of talking to people. And nobody was able to help me or find my laundry when I finally went home after 2 days of being home. I called and talked to the administrator and inquired about my lost laundry bag. She said she would look into it. I did not hear from them. After another day, I called back .And they said, oh, we found your laundry. Well, I was surprised, and I asked them to please send it back to me and they insisted I should come and pick it up.Well, this is an hour away from my home.And I still wasn't driving due to my surgery and I told them I cannot. After practically begging them, two days later they sent someone to drop it off to my doorway. The worst part of the stay that there was... I was an extreme pain. I knew the pain medicine had to be given to me by a nurse, not by an aide. I knew I had a wait till specific time to get my medicine. So when that time came, I pulled the buzzer and the aid came about a half hour later, and I asked her for my medicine, and she said she'll get the nurse well about a half hour after that, the nurse brought me my medicine. So by then, it was an hour later. There were times that so much time passed that I was in tears.So I wobbled down to the end of the hall at nurses station three times during my stay and ask them for my medicine.Because no one came to answer my calls. When I got to the nurse station, they all looked at me like I was nuts.Why am I in the hall. I explained that i've been trying to get someone for over an hour. But when I got to the nurse station, I realized they were all just standing there talking.One person was on the computer. There's something very wrong with that picture!! I do not recommend this place at all! There are so many other things that had happened. I could not wait to get out of there! For any older people that are not understanding the way they are being taken care of and not able to speak up. But to just lay there and hope that they get help, that's bad. I had to give this review 1 start because it would not let me give it a negative number. We had to send my dad to a skilled nursing facility for physical therapy rehab due to a diagnosis of heart failure. I did my research and although most facilities had horrible reviews, we settled on Hartsfield because it was the "least of all evils", although I had my reservations. My dad was there (1) week and we took him out "against medical advice". Since he was weak and could not walk on his own, they would not give him a walker and said he needed to push the call button for assistance getting to the restroom. What a joke that was. No one would ever come. I actually would go down to the nurses' station to let them know we needed assistance and witnessed several of them eating cookies and talking while multiple call lights all around them were going off. I first hand witnessed the rooms around my dad's wait upwards of 60 minutes before someone would come. It was pitiful to watch the neglect of all of the patients - truly gut wrenching. I understand that staffing is a problem, but there should be a law that says you can only take so many patients per staff member. Do not send your loved one to this facility. Hard to review this rehab facility objectively since the medical field has changed so much since Covid. Not sure if it's insurance limitation or staffing issues that are the biggest problem here. The more important is attentive staffing. Big plus ----- This facility was exceptionally clean so you weren't worried about getting infections from unclean facility conditions and staff wore gloves when handling you or touching other areas in the room or your equipment. Was not evaluated by staff at admission. The problem was waiting hours for assistance from anyone. You could ring the buzzer multiple times over numerous hours snd family members would repeatedly go down to the nurses station to get assistance and never happened within a reasonable time frame - 6 family member visits in 4 hrs is not a reasonable timeframe to wait. Their negligence caused antibiotics to be prescribed because bandages left on an incision during an unauthorized shower which got sopping wet and wasn't changed for almost 8 hours only after numerous family inquires and complaints to nursing personnel. Patients had to wait long time periods for assistance in the bathroom which may be why patients never were offered water except for times of medication. It appears most of the nursing staff are from registries and work occasionally or part time at this facility so no loyalty to the facility or patients. 18 patients for 1 nurse is unsafe. The medical assistants who came for checks on vitals were pleasant but when asked to get a nurse for help that didn't happen. Learned the drill quickly. The PT and OT staff were nice and helpful to some extent but seemed to rush through sessions and often combined them. Wouldn't let family do anything to help with mobility or on weekends when there was no therapy. Seems like nothing would happen for patient needs if family members didn't oversee for most of the day. Even when a family member was there not much happened. Administration was helpful with suggestions for insurance related issues. read more

Was a patient there for rehab barely received therapy. Was promised 6 days a week by kelly ( who is no longer there ). I was lucky to get a few times per week. And of note this was not 1:1 with therapist but in a group. Poor communication with nurses barely had interaction with nurses. Aides were not much better. This would be the last place I would take a loved one. A very good friend of mine was there for less than a week and fell in the care of an aid. As this can happen anywhere, after she broke her hip and had surgery, it was time for her to go back to a facility. Heartsfield Village accepted her but stated they did not have a room at the time. so the family waited for a call for availability. A week had passed and while trying to reach someone with no response a family member went to facility where they were told we are not accepting anyone due to flu outbreak, another Lie! this nonsence has gone on for weeks now with no resolve. I think this is such horrible actions on the part of staff. Why would they just not say we are not accepting you and let the family move on? Instead she is now used up reserve hospital stay due to their lack of care. Reading other reviews , this seems to be consistant practice with this establishment. Poor service and liars that have absolutely no compassion for patients or family members. I would rate them a 0 if I could! I am not sure how this staff and sleep at night doing what they do to people. Karma will come back and bite them one day when they have a family member or themselves need a place to go. read more

When they put their light on for help it takes more than 30 minutes for someone to come and then if it's not their job they say I'll get someone and no one shows up. They give pain medicine and Xanax to make you sleep so they don't have to be bothered!!! PLEASE DON'T TAKE YOUR LOVED ONE THERE, I wouldn't take my worst enemy there!!!! read more

Realizing reviews are subjective and each persons experience is different, my family has a lot of experience with this facility. My Dad has been in Hartsfield multiple times. Until now my family was satisfied with his care. Since starting rehab in Nov. "19", things have changed dramatically. Staff morale is poor, patient care is nonchalant, and nobody cares. My Father waits hours to urinate, get in and out of bed, change of clothes, etc. Two weeks ago they forgot to give him prescribed pain medicine for 24 hrs which set him way back. How do I know? An Aid told us at dinner. All you hear in the hallways are employees complaining about each other. Nobody works...Employees stand around talking about each other and patients, and ignore their responsibilities. Try to get attention and staff ignores you. Beware this will probably be your loved ones experience as well. Good luck! read more

Vitals - such as oxygen and temperature - seem not to be checked regularly. My father was on oxgyen and three times he was not receiving t due to the garbage can being left on the tubing, cracked tubing, and failure to connect. When we visited with our own oximeter, his oxygen levels were at 89% and he was taken by ambulance to the hospital. If family does not intervene consistently, proper care does not seem to be provided. Likely the nursing facility will claim short staffing, and that might be true. But the owners of the facility have made a choice to save money over providing basic care. Better off to go home and take your chances. At least you'll be able to eat a proper diet and water will be provided. read more

It would seem my brother may have a case for negligence and Medicare/Medicaid fraud. Really shouldn't boot patients out before completing discharge requirements. No appropriate rationale for the discharge. No one covered discharge instructions. No pain meds. No resources. I'm sure M/M will be thrilled to hear from me.... Who is the case manager and the social worker at this place? Is this how facilities work now? I have to wonder if the employee that tried stealing his cell phone still has his job? Thought it was bad when I left bedside 5 years ago. Guess I was wrong, it's definitely worse now. That star is a negative. read more

This facility seriously needs new leadership to set a tone for professionalism. they take forever to answer the call lights, one day the ice machine was broken on the second floor and I waited six hours to get ice, this is not appropriate given that they have another level they could have gotten done from. , and they also have a kitchen somewhere that should've had ice. they had a medical emergency and I waited four hours for a PRN pain med this is not a place for postop patients they do very little for. I had to repeatedly ask them to help me get my clothes on or get a shower. but I would just say this run don't walk away from this facility it is awful shameful nursing for sure. read more

Be careful here. We put down a huge deposit and when my Mom moved out they held the money the entire 90 days and even then they dragged their feet and came up with excuses why they couldn't return the deposit. As of this writing I STILL don't have the refunded deposit. "oh the Accounts Payable department has it." "It has to be signed off on" "Maybe next week". This after they all but forced my Mom to move out of the independent living facility by putting ridiculous restrictions on her which no one could possibly actually follow. We were forced to move her to another facility. Actually the individual staff members themselves were not the problem. They were quite helpful. It was the management with which we had a problem. follow up- they did eventually refund the money, but 2 weeks late. Not happy. read more

I heard mixed reviews about Hartsfield Village. My mom has been at other rehab/skilled nursing facilities following a hip fracture and a stroke. Last week, my mom fell, which led to a hospital stay and being referred to rehab. I thought we would try Hartsfield. I regret this decision. After two days, I moved my mom to a rehab facility where she was before. We were in the rehab program. We arrived at Hartsfield early evening on Friday 11-23-18. Upon arriving, my mom and I received no instructions or orientation to the facility. I requested to go through the medications for my mother. Each time my mom has been transferred there is always corrections /updates. The nurse that took vitals for my mom said she would be off shift and that the next nurse would go through the meds with me. After two days, a nurse never went through meds with me which led to medical staff at times bringing medication that was either stopped some time ago or the dose changed. Though correcting it with whoever brought it, it was not corrected in the system and kept coming. Had nursing reviewed discharge medication with me upon my mom's arrival from Franciscan St. Margaret's Hospital, this medication chaos would not have happened. Nursing staff seemed resistant and unwilling to converse and collaborate about care. I am a medical provider myself and it was surprising how avoidant nursing staff were here to work with patients/patient's family. It's negligent. My siblings and I bought my mom an Amazon Echo Show, a video/audio device, that allows my mom and her children to communicate with picking up a phone. My mom has hand tremors related to Parkinson's disease. Hartsfield noticed my mom talking to me on the Echo Show. On Saturday 11-24-18, I got a series of calls from Hartsfield. When I did speak with them, they asked that we remove the Echo Show, apparently fearful that we were using the Echo Show for surveillance under the guise of the Echo Show being a violation of privacy. When trying to discuss and negotiate continued use of the Echo Show, they offered to connect it whenever one of my siblings from out of town wanted to speak with my mom. The nursing staff put the Echo Show in "pause" and turned it to the way. I heard nothing until yesterday morning. On Monday morning 11-26-18, my mom shared that three nursing staff came into her room gestapo style around 11:45 pm (shortly after I left) to discuss the Echo Show, unplug it, and apparently to intimidate my mother to get her children to remove it. The nursing staff were whispering about trying to figure out how to disconnect the Echo Show alternating with questions directed at my mother "Why do you need this device?" "Don't you have a cell phone?" "Why can't you just use your cell phone?" After hearing about this incident, I was concerned that nursing staff at Hartsfield must have something to hide because they are so worried about a device they know nothing about and are willing to breach care and privacy by touching devices that don't belong to them and neglecting patient care of all patients (including my mom) while doing it. The proper professional way to do this would be to put patient care first and learn about how the Echo Show is helpful to seniors and can be used as an assistive device. Demanding its removal and then implementing some in the dark of night gestapo operation tells me they have something to hide and aren't doing their providing quality care. On Sunday night 11-25-18, a male aid at Hartsfield told my mom that Hartsfield had run out of coffee. My mom had not been getting coffee on her tray. There is a Starbucks coffee machine in the lobby at Hartsfield that has coffee ongoing, other aids had gotten my mom coffee from here. He basically lied to my mom to save himself a trip to the lobby. The aid later asked me to put the liners in the two trash pales in the room after he grabbed the garbage out of them. When I asked him to bring back liners, he never came back. I've been in hospital and medical facilities numerous times with my mom and I've worked in these settings as a provider, I've never had an aid breach a boundary and ask me to do his job. Prior to my mom being transferred to Hartsfield Village, I met with the admissions person. She also arranged for me to meet with the medical director, nursing supervisor, and rehab director. Most importantly, the nursing director (Susan Ryan, I believe her name is) assured me that nursing services were competent. The three of these people in leadership were either dishonest for the purpose of hardselling a mediocre program or are completely out of touch with what nursing is doing and/or what competent, collaborative nursing actually looks like. For anyone contemplating moving/transferring their loved one to this facility, I recommend looking elsewhere. Don't let the admissions or heads of programming convince you that the program and nursing services are competent. read more

The decor and overall appearance of the facility is the only thing that this place has going for it. Unfortunately, we werent left with much time to find a place for my mother in law after a fall that broke her hip. Community hospital staff (they are now connected) pressured us while we were looking at other facilities to make the best choice and settled with them; they kept telling us we didnt have time and they had a bed available now but could be gone. On a positive note, we werent unhappy with the treatment at the hospital so Hartsfield (being connected to them) should be as positive, right? WRONG! Sure, it looks like a hotel, but they make you feel like just another body, until they drain your insurance days, then out you go, but lets not stop there. The food, is beyond disgusting and WORSE than any hospital food you have ever had, Dont be fooled by the picture of a chef. There isnt one at nursing care. Not sure about the rest of the place. What is up with that tuna fish? It was gritty and the worst I tasted (I didnt believe her until I tried it myself) I promise. Not sure where they get their food from but my MIL lost 20 (TWENTY) pounds in the month she was there; dangerous for someone her age and with cancer. Everything is opposite of what would be healthy and help to boost your strength to help transition out. MANY patients sit in the center hall, staring at nothing or the TV that is on 24/7, like vegetables. No one moves them until meal time. The long wait when pushing the call button is also unacceptable, especially at night. My MIL stated that when she couldn't sleep, she would wander in her wheelchair in the halls and find staff members sleeping. Seriously? The activities are less than desirable, be aware that some activities are sermons under the guise of a totally different activity. She couldn't even leave and said it begins a few minutes after arrival, terrible. The other activities are not stimulating and just difficult to find out about. Therapy at Hartsfield NEVER even connected with her therapist at the hospital to find where she was at as far as progress after the surgery, three weeks after admission to Hartsfield, she was only doing HALF of what she was doing at the hospital. When I asked, why, I was told that therapists don't have access to records. This prompted me to request a meeting, that they called a "care plan", almost a month after admission to which the dietitian and social worker were present only. I expressed concerns about my MIL diet and how she has not progressed in the weeks she had been there. Several days after, we got a call from their billing department that insurance ran out (no notice) and that if she couldn't pay in full (before we even received a bill), that she would have to leave. I could go on and on. Thankfully, we were able to transfer her to Spring Mills in Merrillville. Our original, first choice and they took her and very good care of her for the duration of her recovery. I would steer clear of this place, its NOT worth the price per day. read more

While my mother lived there about 10 years ago, based on another yelp review, I can see that nothing has changed. In her 9 months of living at Hartsfield she "fell" about half a dozen times and got half a dozen urinary tract infections. I could go on and on about all the negative aspects of this place, but won't. My advice: do your homework and look around, there truly are places out there that actually treat the residents with care and compassion. This is not one of them. The staff are Friendly and personable, professional. Everyday they have something going on either a trip to Walts or Sunday church outings, 2pm Bingo, Sunday movie night with popcorn, yoga in the morning on Tuesdays, Thursday is Happy hour. The Village grounds have nice walkways around the Independent and Assisted living facilties. Beautiful flowers and landscaping are lush around the grounds. read more

Hello I Ttwas a patient at Hartsfield Village Re-Hab from mid January to March 1st of 2018 There are four corridors some of which are busiier and have more patients I was on ''D ' corridor There is one CNA for about 12-to 15 people Some of the patients are quite eldery and spend a lot of time in bed If they have an accident they have to be bathed and cleaned up In addition to caring for them and keeping everybody clean the CNAs have bring out meals a and serve meal to tho come to the dining room I never noticed any patients just standing arround during my seven weeks there A few were by the nurses station their choice The CNA's do a great job the nurses who are their on the floor or at the station are caring and do a fantastic job read more

WOULD NOT SEND MY WORST ENEMY HERE TO DIE!!!!! Wish I could give it zero stars. I agree with another reviewer that I can't count the times I went to visit my father and had other patients ask for help or to find a nurse. My family and I were VERY engaged (someone was there with Dad from 7:30 am through 8 pm, EVERY day) and still had problems. I arrived one morning for breakfast, as usual, and could hear my Dad crying in pain from down the hall. When I asked the nurse about it (she was in the hall with a trainee dispensing morning meds), she told me she would "get there when she could", then seemed to make a point of taking a full 30 minutes to get there (she was already at the room next door when I asked). I also observed this same nurse thrice walk past a patient in the hall in obvious pain asking for assistance. The patient asked when she would be getting her arthritis medicine as she was already 45 minutes past when she should have gotten it. The nurse ignored her on her first two passes then told her "I'm busy and you will get your pills when I'm done with what I'm doing". I later found out that this was one of the overnight nurses "covering" the day shift. The overnight nurses appear to be the worst and laziest which is a great concern as that is when the patients are most defenseless and alone. I can't tall you how many times I walked into the room in the morning and found pills on the window sill, bed sheets, floor. The day nurses were generally pretty good but we also had someone there every day, all day. The Social worker there is a joke. For someone who is SUPPOSED to be there as a patient advocate, he appears to be most concerned with a) covering the facility's butt b) coffee-clutching with his gal-pals in the front reception area and c) turning beds over and getting their 30-day medicare pay offs with as little work and rehab done as possible. My Dad had not even been there for two days before we were told that he needed to show continued progress or he would be out in 30 days. However, as neither he nor ANYONE from the therapy team met with is to discuss goals, baseline setting, etc, the determination of "progress" was completely up to the facility and that threat was dangled in front of us every time we DARED to intrude on his "socializing" with pesky concerns about my Dad's treatment and health. As for the "Medical Director", I would not let that quack treat a skinned knee because I'm sure it is beyond his skill level. How do you not immediately order oxygen and/or an ER transport for a patient whose O2 is in the 80's? Then to try and sneak out of the facility through a back hallway when you see the patient's family is onsite rather than even speak to them? That pretty much says it all regarding the level of medical care you can expect at Hartsfield. Don't be fooled by the pretty facade. I'll take quality care over "Starbucks coffee in the machine" any day of the week. read more

People should specify if they are talking skilled nursing, memory care, or assisted living. My mom is in memory care and we couldn't be happier with the staff and the facility. I have recommended it to others as well. However, I have heard that the skilled nursing facility needs to get better. Not enough staff read more

Our first mistake was allowing him to be transferred on a weekend! No one is there! No one can answer a phone! No one can give you updates! We went over 2 days without knowing exactly what was going on! There was a wait for 6 hours to receive a call about results! And still we had to call for them. read more

Please read the other reviews on Heartsfield. Nothing had changed over the years. In room therapy ordered by the doctor is seldom done. No one can give a straight answer. Patients in the hall at night cry for help and are ignored. Medication is not consistent. They give sleeping pills at 7:30am and then tell you the patient sleeps all day. They took the walker out of the room so they can confine the patient. Just forces them to get out of bed and use the wheelchair. They answer the buzzer hours later. I would not put an animal in there. Getting my Mother out asap. They also have the nerve to ask if my Mom is confused like this at home. They are looking to make waves. If you drug people, of course they are going to be confused. Please stay away from Heartsfield. Just because it is beautiful doesn't mean it is worth a shit. read more

I am very disappointed by the experiences and interactions I have had with the staff at the Rehabilitation Facility this past month. I had heard from others that this was supposed to be the best place in NWI. In my opinion, this was not the case, with the only caring people I talked to during my visits being two receptionists at the front desk. The other CNAs (RNs are very hard to come by in this facility) appeared to me to have no idea what was going on. During my walk down the hallway, I would be asked by at least 2-3 patients to "help them" or "try and find a nurse". I will be sure to share my experiences with not only the board, but associates at this facility including the director of nursing, coworkers, family, and friends. It disheartens me to think this is one of the best rehab facilities in NWI. I encourage everyone to do their research prior to sending their family members to the Rehab facility or to any facility at all in that matter. This includes reviewing the the Investigations of Complaints that are available to the public via a quick search on the Web. read more

I moved my dad in April of this year to the assisted living building. The staff is wonderful. My dad is well taken care of, everyone is so attentive and he has flourished with their care. There are so many activities to let him meet people and keep him busy. The food is delicious! He loves it and can't believe all the entertainment they have. They have really given us piece of mind to know my dad is safe and well taken care of. read more
