I regret switching hospices. This hospice went above and beyond for my mom. They provided i.v. fluids and antibiotics..Whereas PiH Palliative/Hospice in La Mirada does not and withholds comfort care over profits. I switched because respite was too far and i needed respite to be closeby for my surgery. Lesson: never receive hospice if it is affiliated with PIH hospitals or any hospital. read more

I came across this on the internet. I never had a chance to thank them. My grandmother was diagnosed with bone cancer and was sent home to pass away in peace at her own home (granny's request). Kaiser recommended Heart 2 Heart Hospice Care to us. Were in denial and thought us girls can do it all by ourselves but discovered it was a lot of work. So we called Heart 2 Heart and the professionals took good care of her and were with her 24/7 to make her as comfortable as possible in her last days. This was our first experience with hospice. My father had congestive heart failure for a long time so the move to finally put him in palliative/hospice care was a tough one. We ended up with Heart2Heart because his primary care dr. recommended them. We got a ton of grief from the assisted living facility because they wanted us to use THEIR recommended hospice provider. I recommend Heart2Heart as they provided care management for my Dad all of last year. All of the staff that came to the house to bathe him, monitor his health, deliver medical supplies, set up medical equipment were very respectful and courteous. They always answered my call, day and night, and visited when I had an emergency to allay my fears and concerns. The two weeks that my husband was on hospice under Heart 2 Heart Hospice were some of the most stressful weeks out of the 25 years of caring for him with Parkinson's Disease. As a Registered Nurse myself, I was repeatedly dismayed, upset, or incredulous at the Hospice providers' lack of knowledge and how unprofessional their conduct was. Our daughter, a healthcare compliance attorney, was also upset at how poorly Heart 2 Heart cared for him. We lasted 16 days with them, and I revoked Hospice two weeks before my husband died. I requested the Hospice Medical Records (MR). My daughter and I were shocked at their documentation. They did not document any phone calls, noted incorrect diagnoses and meds, and manufactured notes for visits that we know did not occur. Heart 2 Heart provided substandard care and falsified their medical records. Official complaints have been made and have resulted in a few citations so far. They did not provide adequate pain management. They took no action the first two times I reported his pain. I had to make REPEATED phone calls to them (which were not documented) and he did not receive pain management until SEVEN DAYS after we first spoke to the LVN about it. The Hospice agency took no action when my husband suffered a seizure. Several hours after the first call to them I called back, and they said they "were trying to find someone". That is when I revoked Hospice, and had another physician immediately order emergency seizure meds in case he had another seizure, which we had within a few hours. They did not coordinate with us in developing his care plan, they gave me little advance notice of their visits and had little or no flexibility with regard to scheduling visits. Staff told me they were part-time and had other "real jobs". They were consistently understaffed, and unable to actually deliver the care that they promised and that we needed. The Hospice staff referred to ALL of their providers as "nurses", even their (uncertified) health aids; this violates state licensure rules. Their staff was not adequately trained to care for someone who needed end-of-life care. The health aide who came to give him a shower did not know how to do this and announced that he "didn't have time for him" and gave him a rough bed bath instead: when I saw my husband his hair was standing up, his shirt was mis-buttoned, he was not shaved, he asked me to put deodorant on him, and he was almost in tears. The LVN had no idea how to manage some types of pain and told me they "could only report the pain" (which she did not do), and told my husband that he "HAD to eat", when his body was clearly shutting down and he could no longer eat. They did not provide me with copies of any of the paperwork that I had signed despite repeated requests for it. They assigned him a physician that not the one they told me he had. They also failed to have the physician see him or give him any exam at all, for which they have been cited. His medical history showed an incorrect diagnosis, meds listed that he wasn't on; and they noted that he was an amputee (he was NOT). They never connected me with a social worker, despite my multiple requests. They did document two social work visits to my very confused husband that I know did not occur. They falsified information about my husband's religious beliefs. The intake nurse wrote down the wrong religion and made a referral for Spiritual Care, despite my being very clear we did not want that. They documented a spiritual counselor visit that falsely documented me as the person he got the info from (I never knew about it and was not there). This SC documented that he "read scripture". The thought of my poor confused bedridden husband enduring this type of unwanted proselytizing is still very upsetting to me. I strongly encourage everyone to avoid this Hospice Agency at all costs. If you did use this agency and had problems, please make written complaints to the appropriate agencies. read more

It was a little rocky start. Our Mom came home from the hospital around 3pm. The evaluation nurse was there. He left around 4pm. At that time the medication supplies had not arrived (they did fiannly arrive after that, but we did not receive any training on how to disburse the medication). The skilled nurse was to arrive at 7pm (per the evaluation nurse). At 7:20 pm, we called and were told that the nurse would not be arriving until 8pm. The night nurse did a good job, was personable and very caring. She was replaced by the day nurse at 8am. This review is past due, but we wanted to let you know that Ruth (the day nurse) is an amazing nurse. She did a wonderful job taking care of our Mom (swabs of water, repositioning her, removing EKG stickers, sponge bath, pain medication, taking vitals, changing her clothes and etc). She was caring and extremely professional. In short, she was amazing!!! FIVE STARS!! My loved one qualified for service do to CHF and she was given a devastating report coming out of the hospital. The Doctor basically said she would not make it through the month . We brought her home that weekend and the loving and caring staff of Heart 2 Heart hospice was there for every step of the way. Christin was very informative from the office and very organized she had ordered everything we needed and was delivered within hours. When someone loves what they do it really shows. Our family was very pleased with Heart 2 Heart Hospice. Every time we would call the office Christin was very helpful and informative. While getting home visits from Vahe we were shocked how loving and caring they were. Vahe put his time was never rushing and was very calm. His tone of voice made everyone be at ease as we knew. That he knew what he was doing. I hope no one has the need for a hospice care until they are in there 90s but when you do rest assure that Heart 2 Heart will be there taking care of you loved ones. I wish that I could give this hospice agency ZERO STARS. We engaged them to care for my father in his nursing home and had a terrible experience with them from start to finish. To begin with, their marketing director- while nice- completely overpromised on how quickly they could actually initiate hospice care for my father. It took days when she said that the nursing assessment could occur the next day. They have poor internal communication. When we called to report that my father was in pain, they said that they would make a note and call us back. They did not call; they did not visit; they did not even contact the nursing home. When we called them back (after more than a whole day of waiting), they could not even tell us whether there was a record of our earlier call. Do they not have a centralized medical record? Do they have one, but their employees never check it? It took five days before we even heard from someone with a pain management plan (one that we eventually suggested ourselves after extensive research), and a whole additional day for him to even get the medication. All in all, HE WAS IN PAIN FOR 6 DAYS WHILE WE WAITED. In some ways it was our fault- we should have fired them after that first 24 hours of NO RESPONSE from them. But, stupidly, we continued to trust them day after day until it was too much. In addition, their medical records are inaccurate. When we saw the notes from the nursing assessment, it said that he felt better after moving from the bed to his wheelchair. THE OPPOSITE WAS TRUE. The whole point was that moving to the wheelchair HURT HIM, and we needed pain management to enable him to last a few minutes at a time out of his bed. We were trying to give him some quality of life- the chance to sit outside for a few minutes- but the hospice agency didn't care about that at all. Not only that, but NOT ONCE did they reach out to my mother and I with a single social or supportive service. Aren't hospice agencies supposed to help families navigate this unbearably difficult, sad time? Instead of feeling comforted, we were stressed out because everything had to go through this hospice- and they WOULD NOT GET BACK TO US. And, their CNAs are LAZY. Part of hospice care in a nursing home is that the patient gets two additional baths or showers each week. So with the two showers from the nursing home, he could theoretically get 4 showers a week. But when the hospice CNAs came, they either (1) gave him a cursory sponge bath- NOT A REAL SHOWER, like we asked for, or (2) asked the nursing home CNA to do it. The last straw was when we fired them. Someone who readily admitted to "not knowing anything about this patient" (excuse me? the patient has a name, also maybe read the freaking file) called back and told us that they could come have us sign the discharge paperwork between 5pm and 7pm. We asked if they could come now- we wanted him discharged. "Oh no, the earliest I can be there is 6pm," she said, as if she had not just literally said 5pm to 7pm. "Ok, let's do 7pm," we replied. She said "oh no, I can't be out that late, I have to pack for a trip." WHAT THE HECK. After much longer than necessary, she finally told us she could come from 6:00 to 6:30- the only time we were not going to be there after being there around the clock for 4 days. "Maybe he can sign the papers himself," she suggested, with the naive air of someone thinking they had solved a problem. Excuse me? My father has not had the legal ability to sign any documents for YEARS. He has advanced Parkinson's. He has dementia. He is END OF LIFE. He is starving to death. He's not eating or drinking by himself. He cannot even remember how to brush his own teeth- or that teeth brushing is a thing. And she thinks he can sign himself out of hospice? To everyone reading this- please do yourself a favor and AVOID THIS TERRIBLE BUSINESS. To the hospice agency- You should be ashamed. You need to improve your terrible practices, total lack of communication, shoddy documentation, and you need to STOP LYING TO FAMILIES THAT YOU WILL HELP THEM. You caused my family absolutely nothing but pain. To the employees of the hospice agency- Wake up. Two of you told us that you had never had any complaints. Well, here is one. In writing. You know how to reach me if you need more information. Read the bad other reviews on here- they exist. You can no longer claim plausibly deniability. This employer is jeopardizing your licenses. If I were you, I would find a different place to work. read more

The nurses Olivia and Mary were wonderful. However, the intake nurse did not provide for or discuss how my partner would be fed through his stomach G-tube (a syringe would later be ordered), and, of course she did not teach me how to feed him, as were her orders. She left at 4 PM. After repeated calls to the office, finally the nurse named Angel left her office post and came to my home at 2:00 THE NEXT DAY to feed him and teach me how to take care of him. This meant my partner had had nothing to eat or drink in 24 hrs. She was alarmed that the intake nurse had not done her job of making sure I could feed him. . That night I phoned the 24 hr. call nurse at about 1:00 am to report that my partner's breathing was very short and rapid. He was gasping. I pleaded with her to talk me through how to give him oxygen using the mask. I put the phone up to his mouth so she could hear his breathing. She had been sleeping when I phoned her, and her voice was still sleepy. She said he sounded fine, he was just catching his breath. She assured me that someone would be there in the morning. At 10 am the next morning Mary arrived, and immediately phoned the office to place my partner on 24 hr continuous care. His oxygen level and heart rate were bad, and he had a fever of 101. She gave him a breathing treatment and he wore the oxygen mask from then on. She was alarmed that the previous nurse had not taught me how to apply the oxygen mask. I laughed that every nurse who came over was alarmed at something the previous nurse did not do. Their social worker came by and gave me a price for cremation of $655., then asked me if I could afford it. I shrugged and said it didn't fit in my budget, but that I'd deal with it. She then questioned whether I could get his family members to help. I said his family members had not been a part of his life and I would never ask them. I told her he was a combat veteran, on SSI and medi-cal. She said she could offer me a lower price of $465. I agreed to that price. She said the name of the mortuary, D--, and she was writing things down. The next evening when my partner showed signs of actively dying, the nurse searched for instructions on who to call to pick up the body. The social worker had not left any instructions, but she had given me her cell phone #. We called, but her message center was full and not accepting any more messages. Rather than making things easier, she had put us in that frantic situation. When I was able to speak with the social worker, it was 2 days later and her message center was still full, but she called my number back. I asked her to confirm the price and she claimed not to have any recollection of the conversation. She insisted it must have been someone else who told me the lower price. The social worker had also pressed me to allow a spiritual counselor to visit. I had informed her we were Buddhists philosophically, though we had Christian backgrounds. We read Eckhart Tolle, the Dalai Lama, and 'new age' spirituality. She kept assuring me that the chaplain was spiritual, for all faiths. So I said OK. When I spoke to the chaplain over the phone he said the same thing, but when he showed up, he had no grasp of any religious idea outside the purview of evangelical Christianity. His agenda was to present his beliefs, over and above to provide peace of mind to the dying. He'd never heard of the gnostic gospel of Thomas, he'd never heard of Eckhart Tolle. He'd heard of the Dalai Lama, but didn't know anything about him. Everything he said about death was negative and fear-based. Sin was his overarching concern. He spoke about the evil of Adam's sin in a very literal way. He repeated that death was punishment for the wages of sin. I disagreed and cited that all things have the same life cycle, including the stars and planets. Death was as normal as birth, it is not punishment, it is transition to a different state. Despite my telling him that he could not present his particular scary scripture selections or say anything about sin, he began praying in a loud, trembling voice, commanding my partner to repent and confess his sins, and to accept Jesus Christ as his savior, and that only by believing in Jesus could he have everlasting life. It was abusive. I interrupted him, and tried to pray my own prayer, but my voice is softer. I was traumatized. My experience with Heart2heart hospice is they have hired some people who are dishonest and negligent, whose behavior negatively impacts the suffering of their patients. read more

Following my last review, I called for a continuous care nurse at 11:30 am on Thursday, March 17 because she was in distress (as defined by them). They called back and argued with me. Finally they called to say one would be there at 8 pm. At midnight no one had arrived and I laid down on the floor. The continuous care nurse arrived at 7 am on Friday, March 18. She had been dead for an hour. read more

After having a bad experience with abother local hospice. We decided it was time to change. We changed to Heart 2 Heart hospice and sure enoghy they were great. Rubina came out to do an EOB and helped us out alot with all the information she had. The social worker was amazing as well. The office staff was fast and helpful.
