I urge you to carefully consider this before contracting with any hospice provider. Unfortunately, our experience with Faith & Hope made an already painful situation even harder. In April 2025, my mom suffered a massive stroke. She was placed under Continuous Care (CC), which meant she required critical, around-the-clock nursing support. We were told her case would be re-evaluated after three days. However, the care was stopped in less than 24 hours and we were given only 4 hours' notice that services would be discontinued. At that point, it was clear our needs no longer aligned with Faith & Hope's philosophy of care. We scrambled to hire private help on extremely short notice. That night, my mom was in significant pain & experienced agitation. She needed her medication adjusted and an increase of the dosage frequency to help manage her symptoms. The next day, we made final attempts to Faith & Hope to reassess mom's eligibility for CC, even though the on-call night nurse, when we asked, did acknowledge that we were free to seek another hospice provider. We waited all day for a response from Faith & Hope, but heard nothing. Faced with another evening without adequate care, we transferred my mom to a different hospice. They gave us the right care without hesitation. Sadly, our mom passed away only 4 days later. The only positive experience we had with Faith & Hope was with Nurse Theresa. She cared for my mom with compassion, dedication and a true commitment to patient advocacy. Theresa's thoughtfulness and support helped guide my brothers and me through an incredibly challenging time, and for that we are deeply grateful.

    Hard to believe a medical service can be as attentive as a fine dining restaurant, but that is what…read morethis was. We used Five Star for about five days. Medicare and Optum covered the hospice side. I paid separately for 24/7 caregivers through Sincere Care Services, which I chose because I wanted Cantonese and Mandarin speakers so my mother would be more comfortable. Five Star was recommended by the Optum physician on staff. Separately, a family member recommended them too, through a friend whose father had passed the week before. I checked their ratings on Yelp and Google before moving forward, and I am extremely happy with the service we got from start to finish. This is the point where you decide between more hospital intervention, which can be painful, and accepting the situation and moving to hospice. Hospice is the wedding coordinator for a peaceful death. Things can still go wrong even when you plan everything perfectly, but Five Star was there at every step to handle whatever came up. The rough sequence for anyone about to do this: 1. Decide on hospice. 2. Line up caregivers. They are separate from Five Star, but Five Star will point you to good ones. I wanted Chinese-speaking caregivers and Mark made it happen. 3. Coordinate with the hospital on transportation. 4. Be home to receive the supplies. 5. Be home when your loved one arrives. Have a bottle of distilled water ready for the oxygen machine. 6. Have the nurse and the caregiver both there so you can learn the comfort meds. 7. I used 24/7 caregivers on 12-hour shifts. I made a point of being there whenever a new caregiver came on, so I could walk them through the medications and get their phone number. Nurses came once a day to check on my mother since she was close. You do not have to be there the whole time, but I tried to be there for shift changes. One thing I wish I had watched more closely was bowel movements. My mother had been constipated since the hospital because she was not eating. Another tip: put together a playlist of beautiful music to play in the room. Otherwise it is silence, or your loved one is listening to caregivers talk and scroll on their phones. Even when your loved one is not responsive, they are still listening and drifting in and out. Tell them everything will be okay. Thank them for everything they did for you. I think it helps them go peacefully. Anytime I needed something, Mark was a text or a call away and he always answered. Again, they are the wedding coordinators for a loved one's passing, and they will do everything within their power to help you through it.

Once she was stable, the hospital's palliative care team recommended discharging her to hospice care. I spent a lot of time coordinating with the hospital hospice representative only to be told a day before discharge that their agency could not care for my wife since she had an open and infected surgical wound plus another open healing wound. None of the other major hospice companies would help us either. As you can imagine, this caused us a great deal of stress and anxiety. After making a few calls, the doctor found Burbank Hospice Care who stepped up and took on the challenge This turned out to be a blessing in disguise. The Burbank Hospice team took on my wife's case energetically, managed the difficult wounds with a "wound vac" and pic line, and monitored her lingering pneumonia and other infections. Slowly, over time, my wife rallied with this excellent care despite the fact that the doctors only gave her days to two weeks to live after she was sent home. Burbank Hospice staff provided all of the necessary equipment, supplies and prescriptions, which was monitored by regular visits from the agency's nurses. Whenever I have a concern or a worry, the Burbank Hospice team responds quickly and with compassion. Recently, my wife had another setback after a procedure and spent three weeks in the hospital. Again, Burbank Hospice was there after discharge to oversee her care and she is now resting comfortably. My wife's battle with this chronic illness has been an isolating experience with few visits from family and friends. The Burbank Hospice team is now like family to us. Their care, compassion and responsiveness have taken a lot of stress out of a very stressful situation. We are grateful for their support and would highly recommend them to anyone seeking hospice care for a loved one.

It was really hard as she was young, only 33 years old we requested to have someone come pray for her Rev Hector came made a prayer. We noticed how this was needed as my knees looked that she was at peace. Two days after my niece passed away Rev Hector was able to call to our funeral gave some beautiful words to help the the family with the loss of our loved one I am so thankful and bless with the experience.

Diane the supervisor nurse was very professional and trained us on what to do. The equipment company they used was quick with all the equipment my Mom needed at her home. Heather came to give my Mom a sponge bath a couple of times. She was very sweet and a professional. My mom's caregiver forgot the names of the other nurses but she was also happy with their services. On the last minutes of my Mom's life we called Active Hospice and the nurse we spoke to told us what drugs to give my Mom to make her comfortable and peaceful until her last breath.
