Drugs were never administered on time, and one had to be to eat properly. The food was never what the slip of paper said, with 2-3 items missing plus substitutions were common. (Obviously, they are telling Medicare and the state they serve what is on the slip.) I knew within an hour. The man sharing the room is sitting up in a blood pressure coma when we arrive and never has visitors so I help him cut his food and chat -- until the nurse tells me, "You cannot go inside his room! You are not family!" One example: The nurse brought in an opiod when Admissions was still there. (Later, Admissions tells me, "she was just showing you she had it" -- right, in a cup starting for the patient.) I said absolutely not, it is for severe pain only when Tylenol did not work (the Rx said for pain only). The next day I could not wake the patient until 4 PM. I insisted on seeing the records and they administered the opiod before I arrived. He got severe edema from the opiod and whatever else was poorly administered. I told the nurse, propped his feet, brought in a virgin pina colada (pineapple for bromelain, coconut, lime peel all good for edema), asked for the toilet to be emptied. He could barely move and was worse than in the hospital. The next morning, his toilet was never emptied, the pillow at his feet was missing, his edema was the same. I hit the alarm five times and waited over an hour asking for the help. No one showed but the alarm light in the hall was turned off each time. So I left to stand at the nursing desk waiting for the nurse in charge to acknowledge me. Finally: "my husband has severe edema in his legs, please help." "what do you expect me to do about it?" she says, glaring. I'm frightened. I rush back to the room and his head has no pillow. His feet are lowered and there is a pillow on top of his knees. His toilet still stinks. I call the case manager and rush the patient away as fast as possible.

If I could give them zero stars, I would. Stay away from them and their sister rehab in Clinton!

This started with a 3 fusion neck spine, Inova Alexandria Hospital then Inova Mount Vernon (IMV)…read morethen Alexandria Rehabilitation & Healthcare Center (ARHC). At IMV I had one-on-one with PT (physical therapy), OT (occupational therapy) and SLP (speech language pathology) therapies, eight days, 21 hours. If I am wrong and the first day did not have any therapies then there were "only" 18 hours. That is real rehabilitation. ARHC is not rehabilitation. There were seven full days, the first was Friday and and absolutely no therapy on that Friday. All ARHC therapies are half hours. l add two more just to help things so there are better; that's 4 hours. To make it "fair" I'll chop off one IMV day to get them equal; that's 15 hours. ARHC is 27% compared for the same numbers of days. Rehabilitation? At ARHC I do not know if there is any SLP at ARHC; I did not get any SLP. The PT and OT staff are pretty good but have an environment not easy. Most have two or some even have more than two patients and few patients are just sitting on a chair. The "gym" is small and and the equipment very few, not good. Two exercise bicycles, a new one where the hand bar and pedals have no connection, strange and never to use again. An older and much better but the LEDs all gone so the therapy used their cell phone and tells a few things while someone else is doing something slow and more time. Most of the time were with 3 pound dumbballs, watched and counted but after twice done almost alone. The balance board was also good with the therapy placed to catch if I stumbled or let me between two rails. I've had bowel problems. That cause was not the ARHC staff. The problem was that after the mess it took more than 10 minutes to not get help with the red button. There were about 30 feet from my door with about four staff were chatting real loud (if not screaming) and I left my door not wearing any pants going to the other end to the desk at the hall end. Someone caught me real fast (not physical) and got back to fix it. That helped clean me up but it took more than a day to get the toilet and floor "reasonably" clean but not really clean. The food was the worst I have ever had. To be honest I am very picky and that does not mean that I what I like is what most people do like. But how could every patient get exactly the same thing that every one gets? How well do almost everyone think cooked food that is barely warm is OK. Some do actually have warm parts, but only in the middle and bottom on the plate, or you learn that you can get your plate heated in the microwave with those who bring and take the food. I like oatmeal cereal, when I cook it as the box says and use some milk and sugar. I tried them several times and spit them out. They were mostly cool not warm, sticky, tasteless, some lumpy, other times ground to nothing. I like scrambled eggs; these were OK and I ate all even if not warm. I ate about 20% of what I was willing the entire time I was there. The last full day I had a dizzy like never before and they put me in a wheelchair. They use a cuff and my heart was about 80/50 or 50/80. They sort of said I needed to eat if I wanted to live. I think that's enough. george.shaffer@comcast.net

It took two months to receive reimbursement for the stolen items. It seemed no one wanted to take responsibility for this incident. One case manager did speak with me and I think only her help got the reimbursement process going. Needless to say, we were not pleased with this center.

Evelyn the Director, Lorrie, Michelle & my puzzle buddy Shereen are above the moon AMAZING. The various games we played, getting my nails done, coffee time (great brew) & giving us snacks once a week. The kind gesture was heartwarming. Can't forget the amazing musicians Bill & Anthony who shared their amazing talents with the residents. I truly miss my days of activities in the dining rooms! Shout out to the on-site hair stylist who does hair every other Monday. She worked her magic on my hair. It was tangled pretty bad due to my illness. Blessings to her! My favorite RN's on 1-S, Jennylynne the AM lead & Anita the PM lead (since resigned) treated me so well. Their warm demeanor added in a pleasant stay. I never had to worry or question anything when these two were on staff! The wound nurse Aminah had excellent heeling hands & heeled a wound I had on my side. There were some CNA's who were outstanding & very professional. Allison, Valerie, Rose, Lydia, Hannah, Bernadette, Tamara (overnight) & Blane (overnight) & Mayta (weekends) were the cream of the crop! There were times when I needed certain supplies from Central Supplies. Thomas, Bright (hope I spelled it correctly) & Sean showed the utmost care & professionalism. They were always very friendly! Last but not least Dawn the 1S unit receptionist who arrived mid my stay here was always so polite, friendly & helpful! THE BAD: The food from dietary here needs several upgrades. Not sure what the budget is but it's obvious no care went into the preparation. Requests on many occasions were not honored and when meal orders were wrong there was no accountability. On one occasion I received burnt pizza and when I asked for a replacement I was sent a half cooked chicken breast with egg noodles. However one time I was missing a salad for lunch the kitchen manager made me a beautiful salad & brought it to me. Excellent service recovery! The Kaiser doctor assigned when I first was admitted Dr. Ansari was amazing, professional & caring. The rotation changed & Dr. Khan took over until I left. She was the opposite of Dr. Ansari. I felt I needed a few more weeks of PT & had to file two greivances to prove I needed this extra time. Dr. Khan kept signing off for my discharge despite my requests to stay. The entire 2nd shift staff on 1S was terrible. This was after Anita the charge nurse left. Needing assistance after 1st shift left for the day just wasn't going to happen. Don't call for them 30min before shift change because there would be no one in sight! I didn't know many of the residents by name but there was one who was so kind. This time has been a very different story. The nursing staff has been unresponsive when my mother needs assistance, and because I'm not able to be on-site every day, I rely on being able to reach the facility by phone to check on her and advocate for what she needs. Over the past three days, that has become nearly impossible. I've reached out to the unit manager, the assistant director of nursing, the assistant administrator, and the nursing unit desk -- every contact we were given -- asking basic questions or requesting updates, and I have not received a single callback from any of them in three days. Now, when I try to call, the voicemail boxes for those same contacts are full, so I can't even leave a message. For a family with a loved one in rehab who isn't able to speak up for themselves in the moment, having no reliable way to reach staff is a serious problem -- not just an inconvenience. I want Woodbine to succeed, because I've seen them do better. But right now, families considering this facility should know that getting a real person on the phone, or a callback after leaving a message, is not something you can count on.

My doctor referred me to Absolute Rehabilitative Therapy. I was not looking forward to coming and thought it would be a waste of time. I was pleasantly surprised at the level of Professionalism and knowledge that Dr. Martin displayed during my evaluation. He was able to accurately diagnose my condition as Rotator Cuff Tendinitis. Whereas my personal doctor and the Urgent care doctor brushed it off . I was a little biased that Physical therapy would not help and I viewed it as a useless exercise. I was very surprised that my should did recover and I was able to resume some of the exercises since my shoulder was healing. The exercises, the deep tissue massages and the Ultrasound therapies were great. I will miss the Ultrasound Therapy and the Deep tissue massage the most. The staff is very friendly and easy to work with. The Office Manager Catherine was always available to answer any questions that I had. The front desk receptionist Noel was always efficient in calling me for my reminder appointments. Erica, Irene and Doug helped to make physical therapy bearable with their pleasant demeanor and easy going manner. They also work very well with children who were there for services or the kids that tagged along with their parents. They also had very convenient hours so I didn't have to miss time from work for my appointments. The only thing they could work on is having a dedicated assistant to work with the clients during the appointments. Sometimes I would forget the exercises so it would be helpful to have the assistant stand by to make sure you are doing it correctly. They would correct me when they saw my form was wrong. Sometimes they were busy setting up rooms or assisting other clients. Dr. Martin had the option to either extend my PT or discharge me. I asked him what his recommendation would be. He suggested that I be discharged since I was about 85 percent healed. I was impressed because some doctors would just keep adding in visits to get more money out of the insurance. After 2 months of visits I was happy to be leaving with a stronger shoulder and less pain. I would recommend Absolute Physical Therapy to anyone who needs a reputable physical therapy practice.

Physical Therapy front desk receptionist in Dec2016-17 & the therapist from then was rude by…read morecalling me a liar, stating I need a script from a physician to come to physical therapy (PT) even though I gave them my post operative notes from an approved Medicare surgery. Medicare states that all "original medicare patients" do not need a script for anything except lab work or imagery. Medicares website policy on PT states: Medicare Part B (Medical Insurance) helps pay for medically necessary outpatient physical therapy. (Medical necessary means: Health care services or supplies needed to diagnose or treat an illness, injury, condition, disease, or its symptoms and that meet accepted standards of medicine.) The physical therapy centers "interpret" Medicare's statement as "only a doctor can deem it a necessity" & because PT clinics don't have "MD" credentials, you have to get evaluated by a physician outside their PT clinic for approval of PT prior to receiving treatment. I've received answers from several different customer service representatives at Medicare & their supervisors. They've stated that it's not a necessity to get a script when I: 1. Have an MRI diagnosis 2. Accomplished pre-op PT before Medicare surgery approval 3. Have the surgery approved by Medicare A physical therapist can do an evaluation after surgery & write notes to Medicare that PT is needed after my approved Medicare surgery per the "specific Medicare written guidelines". You cannot find where the necessity must be on a script from a doctor nor it has to be from a physician outside of the PT office. After an MRI diagnosis by a radiologist doctor, I had a July 2021 surgeon visit & an August 2021 physical therapist evaluation in which they both stated physical therapy is not how you treat my injury...only surgery can fix what I have... But yet the PT clinic booked PT appointments even though the injury was deemed "surgical repair only". The PT made my injury worst. Wouldn't you worry about not getting reimbursed for an injury physical therapy is clearly stated it won't help? Which means my Aug-Sep2021 physical therapy should have not been paid for by Medicare since PT was NOT "...medically necessary...". It seems as if interpretations should not include words not written. Clearly the Medicare words written does not state in what form to receive my diagnosis/injury. Which makes this companies assumptions unwarranted. But allegedly PT still has to be booked to "check a box" with Medicare because, Medicare won't pay for surgery unless you try PT 1st even if it's medically written that PT isn't medically necessary for my particular injury. So you're saying Medicare would risk injurying a patient further after 2 medical professionals tell them that PT will not only not make the injury better, but could further injure the patient? Regulations change periodically in any job & Medicare isn't any different. This office might want to keep abreast of how to write evaluations to ensure reimbursement. It just seems as if this company wants i.e. a surgeon to agree with a therapist prior to a therapist eval when Medicare only needs 1 statement from anywhere (MRI, lab work, post-op notes) diagnosing me & then have a therapist professional state how to proceed after the diagnosis.
