My Mother-in-law received fantastic long term Hospice care at her assisted living facilities from all her Nurses that were assigned to her. When we had to move her to different Assisted Living facilities due to her needing higher levels of care, because of her severe Dementia, they followed her to each facility. They continued to provide her with fantastic care of her for well over a year. Their nurses were nothing short of fantastic. Their Social worker also assisted us with applying and obtaining Arizona Long Term Care Assistance when she needed it, as she ran out of money towards the end. I can not stress enough, that not all Hospice Care companies are the same in the Phoenix area. Sapphire Hospice is Top Rated compared to other Hospice companies that service our elderly. I learned this when other assisted residents relatives, would see the incredible A+ service my mother-in-law was receiving from Sapphire Hospice Nurses and CNA's as compared to their relatives receiving substantially less long term Hospice care with other Hospice care companies. She had severe dementia and she liked to wonder off the premises a lot. The care she received from all the staff was truly amazing!!! Staff physically saw her 2 to three times a week and they phoned me with an update every time they saw her. They were very patient and professional. When my mom needed to be seen every day for what ever reason they did so. For a short time period my mom was at a very very bad memory care facility that I was not aware as to how bad they were. So for three weeks the only care my mom received was from Sapphire Hospice!!! The facility was not feeding, bathing, giving my mom her medication or doing her laundry. If it wasn't for Sapphire Hospice staff my mom would have died at that facility. With Sapphire Hospice staff assistance I found a group home for my mom that actually cared for her. After couple visits, I was referred to his PA Dr Moon. During one visit Dr Moon noticed I had signs of Parkinson, and sent me back to the MD. I was getting worst and did not want to wait 2 additional months, but front desk staff refused to change the date and seemingly did not inform the doctor; I felt front staff was taking decisions on their own. I have other problems that handicap me, and staff will not mail me copy of report insisting I pick up although I am unable to drive there and my email is not working. The office seems to not know about handicap laws. Not a major complaint but to advice on office staff. Doctors are only as good as reflected by staff.

We were set up to move my Mom into Pennington Gardens this Monday, but was suddenly informed last…read morenight that my Mom can no longer move into the facility, ever, based on incomplete data from their nurse. Over the past month we have searched for a facility to take care of my Mom due to a continued decline in memory and reached out to their director Susan and was able to secure a room in their "memory care" wing of the facility. During the process, their nurse met with my Mom and had contacted her PCP for information and up until yesterday they became concerned that my Mom was on a antibiotic for a medical condition that she no longer has (tested negative early April). When I asked what my Mom's PCP told them I was informed they had left a message an they did not respond. In literal a few hours we went from moving in on April 6 to "sorry, we can never take your Mom, but can help you find a place". Please note that we have already given the notice at my Mom''s current facility. The worse part is that I never spoke with the nurse. All of my Mom's health concerns and denial came their director on site contacting me. The question becomes: do you want your loved one in a place where their director and nurse can make decisions like this in a vacuum and apply them to your already stressful situation.

Hospice provided transportation to Dobson home using a third party transport company. The transport van was not meant for transporting humans. We live in Arizona and the temperature was over 100 degrees when they literally shoved my father in the back of a non air conditioned van. When I say shove I mean aggressively slamming him into the van repeatedly. If I was there this would not have been allowed to happen but unfortunately I wasn't. What happened to my dad was abusive and careless.
