Please read about Immune Mediated Hemolytic Anemia. We brought our 3-year-old dog, Charlie, to Dansville Animal Hospital for worsening lethargy and loss of appetite. Bloodwork, urinalysis, and X-rays were performed. They wanted the x-ray to rule our pyometra, but after speaking with a ER vet later, we learned ultrasound would have been more useful for evaluation. While I understand everyone approaches cases differently, I wish the advantages and limitations of each options were explained before I spent hundreds of dollars. Ultimately DAH felt pyometra was unlikely and that Charlie appeared to have an atypically presenting urinary tract infection, causing acute liver injury. We were also informed that she was also 'mildly' anemic. Because Charlie is young and had never experienced significant health issues before, this was alarming. When I asked why a UTI would be causing liver abnormalities, as my experience in human healthcare did not lead me to understand the vets conclusion, I felt that the explanation provided was still vague and did not adequately address my concerns, even after I sought clarification. Charlie received B12, antibiotics, anti-nausea medication, and fluids. This cost just under $1k. Although it was a significant expense, we were hopeful she'd improve. We were instructed to either return Monday or seek urgent care if she didn't improve over the weekend. Unfortunately, she continued to decline. She became increasingly lethargic and ate less and less. Concerned, we sent her medical records to two veterinary urgent care facilities and sought input from another general practice veterinarian. After reviewing only her laboratory results, all of them advised us to bypass urgent care and seek emergency treatment immediately. We were told her anemia appeared severe enough to require a blood transfusion and that a condition called Immune-Mediated Hemolytic Anemia (IMHA) needed to be ruled out. The condition causes the body to destroy its own red blood cells, and it's not rare, it's the most common autoimmune condition in dogs. By that point it was Sunday, and several emergency hospitals were at capacity. Instead of driving approximately one hour to Rochester, we ultimately had to drive nearly two hours to Ithaca to obtain emergency care. There, our nightmare was confirmed, they agreed with the IMHA workup, needing hospitalisation, and a blood transfusion. We were quoted an estimated cost of $8-10k. The emotional impact of this experience is difficult to put into words. Charlie is not "just a dog" to us. She is a beloved member of our family, and she has the most vibrant personality. She came into our lives during one of the most difficult periods I have ever experienced, shortly after my friend ended their life, and has been a constant source of comfort, companionship, and joy. Facing the possibility of losing her at only three years old was devastating. What concerns us most is several veterinarians, notably another generalist vet, recognised the severity of her condition simply by reviewing the lab work, yet we were never warned that her situation could be life-threatening or that emergency evaluation should be considered. Medicine isn't perfect, and diagnosis can be difficult, but we believe better communication about abnormal findings and potential risks could have changed how quickly we sought emergency care. Her presentation was textbook. My goal in writing this is not to attack, I genuinely hope this is constructive and encourages greater vigilance about IMHA for DAH. No family should have to go to bed worried they'll be woken up to a call saying their baby didn't make it through the night. I considered giving 2 stars because the vet techs were wonderful, but this experience was overall traumatic and I feel that in human medicine, this would have been viewed as medical negligence. Charlie is still recovering, and we don't know what the future holds. We are grateful for all of those at Colonial Animal Hospital.
