I am a current rehab patient and laying here trying to receive assistance since my illistomy bag has leaked all over me scalding my skin with poop and was told not to remove it until a nurse looked at it. Would NOT provide me MY supplies to clean it up. I am also to get my pain medication every four hours and it is NEVER provided in a timely manner. This review is for Lorien Mt. Airy AND Lorien Encore in Turf Valley. My 93 yr. old mother was recently at Encore for rehab after two weeks on life support in the ICU due to RSV in addition to COPD. It was not a hopeful situation. Miraculously, she showed signs of improvement and was eventually discharged to Lorien Mt. Airy (due to bed availability). She was there for two weeks before we could have her transferred to Encore which is much closer to our home. Mt. Airy was fine, but not what we had hoped. The staff was attentive and very kind, but the conditions were old and crowded. My mother had to share an extremely small room which made it difficult to move around in and to have company. The hallways were littered with equipment and additional rehab supplies. It was no fault of the rehab facility; they seemed to be short on storage areas. When a room finally opened up at Encore we were thrilled. It was day and night. The staff was extremely attentive to her needs, the facility was full of sunshine and spotless and the staff genuinely seemed to love their jobs. I appreciated the fact that everyone wore a mask, even those working at the front desk. My mother's rehab was slower than we had hoped, but it all takes time. My mother went from not being able to move her leg while being in bed, to walking out of there and resuming her former lifestyle (with the help of nightly oxygen) and is now living independently in her apartment at Heartlands Senior Living. The staff at Encore were always there to answer our many questions and would reach out whenever there was a change in status. I was there daily to visit my mother and although she wanted to go home from Day 1, she was sad to say good-bye when the time came. I can't thank the staff at Encore enough for taking such good care of our mother. God forbid that we will have a repeat situation, but if it were the case, Encore would be our first stop! read more

We used the facility for rehab for my mother. Physical Tgerspy-Occupationsl Therapy and Speech Therapy - Score A - Excellent/Professional Day staff - B/C grade Mid dsy/evening ' C grade ( w/exception of 1 or 2 who did a good job) Evening/overnight. F. Little oversight. Unprofessional and not best of care. Weekend staff - Most Days A/B Food - C/D Cleanliness of Rooms /Bathrooms - D/F There is not a set timely daily schedule for dispersing meds, clean fresh cup of water, emptying trash, cleaning room, cleaning bathroom, getting patients ready for bed. If you had to ask for any of tgese items, often it wasnt well received. Felt like you were bothering the staff. Leave patients on toilet for e tended period of time or told to do in disper. It takes a special person to work in this environment. Skill, caring, understanding and patient. If your not, dont work at such a facility. Its depressing and can be demanding. Its important to know, your csring for someone loved one. You may be in such s fsvility pne day. Care for folks and respect them, like ypu would expect and want. Very disappointed. Did we have 2-3 good csring folks yes. But the majority were awful. Therspists were amaxing A+. Facility could be cleaner and smell better. I csn overlook everything, but not the care. I would NEVER in a million years recommend Lorien Encore's Rehabilitation Facility. I regret to say that my family's experience with Lorien Encore Rehabilitation Center in Turf Valley has been nothing short of horrific. We trusted this facility to provide the necessary care for our loved one, who has multiple serious health issues, and what we encountered was a continuous pattern of neglect, poor communication, and overall disregard for their well-being. Neglect of Basic Needs From the moment our family member arrived at Lorien Encore, we noticed a troubling lack of attention to basic needs. Despite being severely ill, immobile, and diabetic, they were consistently left alone in their room without the proper checks or hydration. On multiple visits, we found them without water, and when we requested it, the staff failed to provide even basic hydration. Additionally, the room was often left uncleaned, with dirty tissues and other waste accumulating without anyone attending to it. Failure to Monitor Medical Equipment and Condition Lorien Encore failed to properly monitor their health, including neglecting to attach an essential blood sugar monitoring device that we personally delivered to them. A week later, it was still sitting unused in the room. This failure to monitor blood sugar is not only negligent but also extremely dangerous, given their diabetic condition. The lack of medical attention to their needs puts them at serious risk of life-threatening complications. Inadequate Dietary Support Our family member is a type 2 diabetic and requires careful dietary management, yet the food provided by Lorien Encore has been wholly unsuitable. Meals rich in carbohydrates and sugar were regularly served, forcing us to bring in our own food to ensure the proper diet. This shows a lack of understanding and care for their medical requirements. Multiple Incidents of Neglect and Hygiene Failures Our family member was left in soiled diapers daily, causing significant distress and exposing them to serious health risks, including infection and skin breakdown. On multiple occasions, they were left in uncomfortable, unsanitary conditions for hours. On one occasion, they were left in a diaper filled with feces for over five hours, and on another, they were left in a urine-soaked diaper for over 11 hours. This resulted in them calling us to inform us that they had chills and were shaking, only for us to discover that they were soaked head to toe in urine. Failure to Provide Physical Therapy Our family member was supposed to be receiving physical therapy during their stay at Lorien Encore, but in the entire week they were there, they received a total of just 15 minutes of therapy. This lack of attention to their rehabilitation needs is inexcusable and further demonstrates the facility's lack of commitment to patient care. Failure to Provide Necessary Transportation The final straw was when Lorien Encore failed to arrange transportation for a critical medical appointment. This appointment was vital for treatment, yet Lorien Encore's lack of follow-through not only delayed care but also placed health in jeopardy. The failure to communicate or make proper arrangements nearly led to catastrophic consequences. To provide more context, our family member's legs and feet were black and purple upon arrival to the critical appointment (to add we had to arrange all transportation and everything ourselves as Encore failed to help in any capacity) and the wounds that were being supposedly treated turned completely necrotic within a week of their admission, when they had arrived with flesh-colored wounds. Overall Disappointment and Concern In summary, Lorien Encore has repeatedly failed to meet even the most basic standards of care. The neglect, lack of communication, and disregard for patient health are deeply concerning. I would strongly urge anyone considering this facility for their loved ones to look elsewhere. The treatment our family member received here was dangerously inadequate, and I fear for the well-being of other patients in their care. I will be pursuing this matter further, as no patient should be subjected to such neglect. read more

Medicare patients going for post surgical rehabilitation in-house care BEWARE! Lorien bills Blue Cross and Blue Shield (BCBS) as a Skilled Nursing Care center and not as a rehabilitation Center. Consequently if you expect BCBS to pick up any deductibles after Medicare they don't! You will be liable for 1000's of dollars not paid by Medicare!! They claim that they didn't know this? Right like no one else has ever found out the hard way? And just what skilled nursing care did my wife get? Well when she needed help to get to the toilet the so-called "Skilled Nurses" most often would just call foe an aide to do this as they walked away! Often the aide arrived 20 to 30 minutes after her initial request and long after she soiled herself and her bedding. Oh the nurses did deliver her meds to her, and I guess that takes an RN to do that. And as far as the 5 days a week of both Physical Therapy (PT) and Occupational Therapy (OT) she was supposed to get. The session were often for far less than the requisite 45 minutes, in which they put her on a stationary bike for much of the time while they worked on their laptops. believe they included in the "45" minutes the time it took them to wheel her from her room to their work site. She didn't receive any truly useful PT or OT until she received at home visits two to three times a week by PT's and OTs who spent the time actually being engaged and working with her. I believe had we hired an in-home care assistant rather than going here she would have receive better care than by going to their Facility.. read more

Encore at Turf Valley was NOT a good experience at all. Extremely understaffed--call button went unanswered for hours. They started painting my mom's room with her in the bed and just piled everything on and around her bed. The fumes alone were awful--had to beg to get her moved to another room. PT and OT were good and some of the staff were great, but many were not. And the administrative end of things was disorganized and did not spell out things clearly at all in terms of costs and insurance coverage And if you asked the on-call doctor a question--we got billed for that separately! Could not recommend to anyone in good conscience. read more

My wife was at Encore Turf Valley for 4 months, two at Skilled Nursing then two at Assisted Living. For the whole time, we found the staff professional, caring and attentive. The facility was always clean, with no unpleasant orders. Physical Therapy and Occupational Therapy were excellent and the reason my wife progressed from unable to use her legs to coming home only needing a walker. The Social Services Office kept us informed of Medicare status and important procedures. Staff would greet you by name every time they saw you. The in-house beauty parlor is very convenient and not exorbitant Every day there are activities for the residents, including exercise, yoga, Parkinson therapy, Bingo, games, and Happy Hour on Mondays, Wednesdays and Fridays. Meals in the AL unit were served in a restaurant atmosphere with multiple choices, including a soup of the day, entrée, salad and dessert. The enclosed courtyard was enjoyable for fresh air and outdoor activities, weather permitting. The General Manager was always attentive and strove to improve conditions. Steve Klein read more

My mother was a patient there recently and the staff was amazing and worked with us through all the COVID restrictions to make sure my mom was well taken care of. When it came time for the end of life visitations they were extremely helpful and made it possible that a large number of family members were able to come and say goodbye. My mother is 94 and needed to transition from another assisted living facility. Lindsay Robey made the transition seamless and help us navigate a few hurdles the day of our move. In addition her care nurses and aides are very attentive and trying to help her settle into her new apartment. read more

Encore at Turf Valley is extremely understaffed. My father is there for rehab and got 15 minutes of physical therapy today when it was supposed to be an hour. He had to wait over an hour for someone to come give him his urinal which was left out of his reach and just tonight had to wait FIVE AND A HALF HOURS for a cup of water. Some of the staff is great, some of the staff is rude. read more

Do not like this place, alot of the staff mostly the younger ones are nasty they keep your elder in bed all day, almost every time I come i have to either call a staff member to help me get my mom out of bed wash, dress, clean her bed and put clean linens on her bed and this at 3:00 in the afternoon and God forbid you ask for a cup of ice or a new oxygen tank for your parent! Forget it! I asked 4 staff members that were just standing around in a nice way if one of them could get me a cup of ice they all ignored me i asked again and again they just looked down at their iPads totally ignoring me like i was invisable finally about the 4th time asking one of the staff looked at the other staff members ignoring me and finally went and got me ice, I felt so humiliated how unprofessional! Over a cup of ice wow! Things have been missing as well clothing, my mom's perfume that I just bought her, phone chargers etc. You might want to wash your own parents clothes as many come up missing.! This place is clean and Beautiful when you walk in and doesnt smell but alot of the staff that work here are rude and ignore you or get an attitude if you want anything I thought it was their job to wash, dress, make the beds and attend to the patient but not here it's just like alot of the other 1 star long term care facilities when it comes to alot of the the staff! I Will be filing a complaint and getting my parent out if this place! Looks can be be very deceiving. read more

My mom just got there yesterday and I was really happy with everything so far, that is until today. They had a guy play music for them and he sang very very inappropriate songs for where he was playing. Many songs about death and WORST of all he sang an original about playing at nursing homes on weekends and how he hated it and doesn't want to turn out like them. He also didn't talk to the other residents well. Very unhappy. But this does NOT reflect the service of nurses and workers. They were all very nice! read more
