I'm going to base my star rating on CARE GIVEN and not the other aspects although they will be mentioned. I've never had to explore nursing home options before, and unfortunately like many, when it was needed it was at the time of need so we were ill prepared. I selected this nursing home based on a visit I had years ago and thought the residents were happy and well cared for. I enlisted the help of an elderly solutions "broker" I found online thru google, who was the foot in the door, although I don't know if that was necessary. Like many, I heard horror stories about nursing homes, and have been in several that were subpar visiting relatives over the years. It's believed that many will force you to sign over your house and assets. Surprisingly at OL that is not the case. In fact, they don't ask anything financial. They operate on a base rate and a $310/per month upcharge for every level of care needed. A level of care is based on every 30 minutes a day they tend to you, this is evaluated every month for accuracy so you could be paying a higher level one month and lower the next. Please note that the prices I'm providing here are based on a single room, single occupancy, non-dementia care. Memory care has a different set of pricing, and other style rooms, suites, efficiencies are available. The base rate was $4300/month + each tier of care. The nonrefundable entry fee is equal to one month. They operate on a month-to-month basis. Again, they ask no financial information. My dad lived out the last two months of his life at their Landisville facility and overall, we were very pleased with his care. The pros, aside from the pricing structure is the place is gorgeous, well kept, and the residents and staff seem happy. Residents are dressed and involved. There's a variety of activities every day, as well as outings. Transportation, move in assistance, medication management, all meals and activities, housekeeping, furnished room (if you choose), cable tv, basic supplies such as toilet paper, linens and laundry service are all included at the base level. Kara, who was our initial contact, was very friendly, and it felt like she had our dad's best interest at heart. She really knows how to "sell" their lifestyle, although some of her information was inaccurate. She promises the sun and the moon, however, either by miscommunication or staffing issues, not everything she promised was delivered. Some key furniture pieces that were promised upon check in were missing, but rectified. We were told 181 channels, however my dad complained that some required an additional payment, and channel offerings seemed to change every day. One day a channel would be free, the next it wasn't. We were told a landline phone with his own extension was included, which was a highlight for us given his difficulty in using a cell phone, only to discover we would need to have someone install the line, pay a monthly fee, and purchase a phone. A thorough nursing assessment was also promised when he arrived, but not done till later in the day when I mentioned it. Small things like that were all addressed both worth mentioning. Some things we can't overlook; dad was proud of his military service, and they have a veteran's wall complete with a picture/nameplate. I provided his picture a few days before he moved there, however by the time he passed - two months later it still wasn't hung. I had contacted the activity director on this several times prior. It was heartbreaking to see my dad look for it every day as he passed the wall on the way to meals, knowing he should be up there and wasn't. There was also an issue with obtaining a wheelchair thru insurance, which was a result of something as trivial as the provider needing an insurance number, which they had and never provided them, which once we got involved was addressed. As my brother put it there was a little more "hand holding" on some matters than we expected, however at the time of his passing I thought we worked out a lot of the kinks. What also was disappointing was that we & dad had grown to like the staff a lot, and since we were very hands on as a family communicated with them on the daily. However, when he passed no one reached out, sent a card, or acknowledged his death. The day he passed at the hospital I cleaned out his room, and a few staff came forward to offer their condolences but not key players, which was sad. The care he was given however was very good. They were attentive to his needs and treated him well. His room was spacious, newly renovated, and comfortable. PT & OT therapists work out of that building so getting his therapy needs met was easy. Communication was daily between us and key employees. He was clean, dressed, and tended to daily. Coming here probably gave us more time with him. He is dearly misssed.

    My Great-Uncle has been living at Oak Leaf Manor (Landisville) for about a year…read more I have visited him at least twice a week since he has been there, so I am very familiar with Oak Leaf. The grounds are beautifully landscaped and well maintained. There is plenty of outside seating including two gazebo's. There is also an outside picnic area and nice walkways for the residents. The interior is wonderfully decorated for whatever season it happens to be. There is also a water fountain inside that is quite nice so sit around and relax. The dining room is nicely laid out and the food is good. The residents rooms are spacious and clean. The staff at Oak Leaf Manor is very friendly and caring. If any problems arise, the staff do their best to accommodate you as quickly as possible. There are a lot of activities for the residents, including BINGO, a theater room, scenic bus rides, crafts, and veterans activities. Overall Oak Leaf Manor has a very comfortable friendly atmosphere. If you are looking for a home for a loved one who can no longer live alone, I highly recommend Oak Leaf Manor.

As we were leaving we literally saw a few mice and on top of that the staff did not seem phased by this. The nurse named Patricia was extremely rude and said I quote, "I don't have time for family to bother me about mice". She insulted my sister in the process and would not let us leave when we attempted to discharge her. If you care for your family do not send them here. I also had a resident approach me during this incident and tell me how I am doing the right thing by discharging my grandmother and how this place was dangerous and disgusting as she cried to me.

My review is based 100% on the horrible administrative staff. The absolute worse most disorganized group of people I have ever met! The unfortunate truth is, it spans the entire administration. The business office is a joke, they have zero clue what they are doing. When you call them all they do is give you the run around and if they need to answer a question you will get one of two responses, a lie or something so ridiculous you can't believe you heard it! A counselor called me and asked me what my mom's hopes and dreams were for the future? I started laughing hysterically. She asked me what was so funny? I said, her hopes and dreams! She is dying and jacked up on morphine! I guess her hopes and dreams are to be at peace! I asked her, did you ever look at her file or meet her? She said no! Then, right on cue about 10 minutes later the activities person called me two days before my mom passed away, who was on hospice and could not walk, and asked me what activities my mom likes? She actually asked me if my mom likes walks in the garden, does she like to dance, does she like social activities? After her questions I couldn't help myself, I asked her? Did you actually look at my mom's file? Did you meet her? She admitted no. So then I said, she can't walk! She is 100% bed ridden. So I guess her favorite activity is breathing right now! Her reply was, Ok! Great, I can't wait to meet her! Yeah, really awesome! And now I still have to deal with them and I am still getting the run around! Just plain terrible!

    First off, know that if I could give ten stars for South Mountain Physical Therapy I would. I had…read morebeen suffering from shoulder pain in both shoulders for well over a year. Several visits to well known orthopedic group in the Reading Area did nothing to relieve the pain. All I got from them was always wanting to stick a steroid shot in me and send me on my way. Was told to continue with the exercises that I came up with on my own and I would be fine in a few weeks. Well guess what, not the case. After a few months I INSISTED on an MRI and reluctantly, the doctor ordered it. When I went in for my appointment to see what the MRI showed I did not even get to see the Dr I had been seeing but was handed off to a physician's assistant. He shows me the MRI and he shows me what he called a slight tear in a tendon in the rotator cuff. He stated that he could schedule surgery today and then I would have to through a 6 to 8 month recovery period. He also stated that if I chose, I could try physical therapy for 6 weeks and if that was successful I could avoid the surgery. Of course I chose the physical therapy route. I go to check out and the receptionist stated that the soonest that they could get me in was over two weeks away. What was I gonna do? I scheduled the appointment. A day or two later and I was telling one of my friends what I was going through and how I had to wait over two weeks to get into therapy. He said "Look, you need to call South Mountain, they are absolutely the best!" The next morning I called South Mountain and one of the therapists (Kai) answered and I briefly explained what I was going through. He told me that he felt they could help me. I said, OK, how soon can you get me in. He said "Gary, if one of us has to come early or stay late, we will see you today or tomorrow". Wow, impressive. He said he would have Darien give me a call as soon as she got in and get me scheduled. Darien called as promised and I was set up for a next day appt with Dan. Not only did Darien show true compassion to my situation she also straightened out all of the insurance issues involved. Next day I go in and meet Dan. It was only about 15 minutes into the appointment and Dan tells me that he does not think any of my pain is coming from that tendon tear. At the end of my appt Dan states that he is 85% sure I have something known as Frozen Shoulder (in both shoulders) and that he can get me back to normal through physical therapy. Fast forward six weeks. After going to therapy 2-3 times a week and a lot of hard work on my own at home on off days I am at approximately 95% back to normal. Dan told me to continue with my at home exercises and within a few weeks I should be back to 100% normal. I went from constant pain and very little movement in both arms to just about full movement and NO PAIN! On the last day I was there I asked Dan what his final conclusion was and he stated again that I had frozen shoulder. Now keep in mind, I had 5 or 6 visits with that well known orthopedic group and not once was Frozen Shoulder mentioned. And also don't forget, they were so ready to schedule a rotator cuff surgery that would require 6 to 8 months of recovery time and that wasn't even what was causing my pain! Just a few short weeks of physical therapy and I am fixed. No more pain, back on the golf course, just about 100% movement back. I just wander why didn't they order physical therapy in June of 2020 when I had my first visit? Hmmmmmmmm,,,,,,,, I honestly feel blessed to come across South Mountain Physical Therapy. They gave me my life back. Not only was Dan an awesome therapist to work with, EVERYBODY at the facility that I had direct contact with was as well. That includes Darrien and Christine at the front desk (BTW, Thanks Christine for sending me to Penske for the auto repair, they did a great job!), Kai, Kathleen, Matt and Madison. Everybody there shows that they really care. Believe it or not, it came to the point where I actually enjoyed going to PT! Yes, that's right, I enjoyed PT. :-) In closing, I will say this. Believe me when I say, if you ever need to see a physical therapist, South Mountain is the place to go! You will not be sorry. Thanks again everybody, you are all very much appreciated.
