The menu said roasted Brussels Sprouts and what came you could flattened with your fork. I think they boiled everything. The food is very salty. The CNAS were so overworked. If you needed to see the Nurse (which there was only 1 for up to 18+ patients) it could take 2-3 hours. The same for the CNAS. My surgery wound got infected. Nothing was done for 10days. It would be looked at every few days and the comments were-oh this is getting redder, or this has gotten longer,-or,now its oozing. I never saw a Dr, it was all over the phone. I need to have a 4th surgery as the infection didn't respond to the antibiotic and finally the medication was increased but too late. The building was in such bad shape. To get into the shower room you had to go over a torn floor. The wheels would get caught and it was dangerous. They sold the building so they spruced up with painting the room. However, you could see where it was sloppily done. The only helpful people in the building were Kaylene (the PT) and Todd ( OT). They were awesome. Very supportive, caring and would answer all questions. However they were often overworked due to patient loads. Bottom line: DO NOT SEND YOUR LOVED ONES TO ANY AVAMERE CENTER.

The CNA's staff are really hard workers especially the new ones are very professional and the best in the west, but the experience with one of the night-shift nurses unfortunately has been continuously unprofessional and unpleasant since the beginning of time of stay without mentioning names :( The facility is somewhat basic but if having a better attitude night-shift-nurse, patients and families will be satisfied as if it was a heavenly service and consider it a five-stars-facility. We support your business and value your hard work and hope to convert this to 5 stars by the end of the stay.

When I went in, worker after worker just walked right on be me. I ended up going to the nurse's station and they just said he hadn't arrived yet and I could wait in the lobby. No welcome or offer to show me around. After over an hour he hadn't arrived so I went back in with his stuff. They just told me his room number without showing me where I could put his stuff, so had to go back and ask. Care was terrible. Our dad said it would take them over 30 minutes to come in for help to the bathroom or anything else and would just watch him struggle to get out of bed without helping. They basically left him in bed, including to eat. He finally asked if they could help him get out of bed in the morning so he just wasn't laying there all day. Only one option for meals, which he mostly did not like. He lost over 15 pounds in 2 weeks, and left very weak, which lasted several days after discharge. He was in the hospital for one week prior, but was eating regular meals. I brought him a toothbrush, but he never once brushed his teeth. Hygiene needs were not tended to. Therapy was supposed to be everyday, 5 days a week, but lucky to get one session every other day. Their answer for his bathroom needs was to put depends on him. When he was discharged he was to have a home health nurse to go over 3 times a week. It turns out the home health agency Sapphire sent the orders was at capacity so they let Sapphire know. It's now been 1 week, with no home nurse. Nobody from Sapphire has notified us and do not return our phone calls. They seem to be there to provide the bare minimum of care.

    The Pearl provided excellent care for my husband after an extreme spinal surgery. He ended up with…read morean infection at the hospital requiring emergency abdominal surgeries. After all of the surgeries my husband spent a month in the hospital and he was kind of at square one when discharged. When he started at the Pearl he was unable to stand. After his month long stay, he was in good enough shape to slide transfer from a wheelchair to his bed or shower/commode. He was also able to stand for brief periods of time. He progressed enough to join the intensive Rio Rehabilitation Program at Legacy Health. During his stay he received top notch care from Rio staff. They were very caring and met him where he was physically. The therapists pushed him to get where he needed for additional rehabilitation. Thank you Colin and Debra! The nursing staff were caring and as timely as they could be and several nurses were standouts (like Carol and Sherry). The weekends seemed to have less staff so things could take longer. That said, they were upfront about other work they needed to do before being able to help. And when they were helping, they took all the time and care that was needed. The food was very good with several alternatives if the main menu didn't sound good. Bambi was great in helping set up wheelchair transport for appointments, Sondra helped coordinate lots of appointments and helped make Rio happen. Tawnya was amazing at coordinating transition to Rio and backup plans. Penny was the nicest therapy pooch. She lifts everyone's spirits. I highly recommend the Pearl as a skilled nursing facility. I came here with a lot of medical needs including a feeding tube and pump, 24x7 oxygen concentrator and a lung suction device. Not a small feat to support me all day and night but The Pearl was up to the task. I've made friends with many of the staff and I look forward to returning to do some volunteer work if possible. I totally recommend The Pearl if you are in need of rehabilitation services like I was.

    If I could give zero stars for how this discharge was handled, I would…read more I came to this facility after a motorcycle accident, hoping to recover safely without putting extra stress on my partner. While a few individuals on the care team truly made a difference--Damalesh, Shantay, Louie, and a handful of others were kind, attentive, and absolutely wonderful--the overall experience left me shocked and disappointed. The social services director, Sam, is the main reason this facility gets a 1-star review. He was dismissive, condescending, and ultimately responsible for the stressful, manipulative mess that surrounded my attempt to leave--saying that I was "denying care" when in reality I was taking my "care" elsewhere... Had to fight to get basic discharge paperwork (that I asked a half dozen times to get for hours on end), was pressured to sign documents without being given full information, and felt completely stonewalled when I asked for reasonable medical equipment and referrals for in-home rehab. Sam made what should have been a smooth and supportive process incredibly frustrating. As for the food? I'm vegan, and while they made an attempt to accommodate that, I can't live off bread and tofu forever. Toward the end, I could barely eat anything at all--it got that bad I understand care facilities aren't luxury resorts, but respect, communication, and basic follow-through shouldn't be optional. If it weren't for the handful of standout staff members I mentioned earlier, I would've lost my mind here. If you have a choice--keep looking. You deserve better.

    What can I say about this place? The fact that my concerns were not met when I felt completely…read moreviolated, disrespected, and shamed while having to take a shower, while a male staff member sat just inches away from where I had to take a shower. I was told that if I didn't take a shower at that moment, I would not be able to take one the next day. There were enough female staff on duty that one of them could have come to the shower instead. But yet, being humiliated. I still have trauma from that night. Only one staff member approached me about what happened. I was not apologized to by Hussain nor by anyone else. I felt absolutely outraged and degraded. Yet, no one seemed to care. The only reason I was here was to rehab from cancer treatment It was much more banal at best. A complete lack of communication between staff members. Often told "oh, I forgot, or no one told me," which is absurd when dealing with people you are there to care for. On more than one occasion, I was brought the wrong tray of food while the staff jokingly said, "oh, it happens all the time", like that's ok. Another issue.. having to put a sign on my door asking people to knock before just walking into my room. It is a common courtesy to knock on someone's door before walking in... it does not matter where you are. Yet, even with the sign, some just walked in without knocking. As far as rehab.. I did better PT in my room. A couple of old bikes and a few weights. It was a waste of my time and dignity. I stayed there two weeks too long. However, that being said.. some of the nicest people you ever wanna meet. All seem pretty stressed most of the time, definitely overworked, probably underpaid. From my personal observation along with asking the R.N's CNA's, LPN's, the rehab staff and others. Each caring for patients with different needs and specifics. The length of time spent that particular resident varies, leaving others under their care basically screwed until they can get to you personally. That's why the one star, because they do try. But ,things definitely need changing as far as respect, patient care, effective communication, the fundamentals and most valuable not treating your patient as you did your last. Each patient are different in their own right, they should be respected as so. As an individual, which we are all and not just another bed filled.

My case worker immediately told me not to look at yelp reviews, instead look at their score on the Medicare site. I wish I had not taken her advice. I chose this place based on its Medicare score and regretted it the moment I arrived. Before I go into all that is wrong with this place, let me say that every one of the staff are extremely nice. The night staff are a little loud, but still very friendly. Unfortunately, that is the only good thing here. Now, the bad. Food - food is the worst I've ever eaten. I mean, the hospital food was really disgusting; this is worse. I've spent a lot of money on door dash the week I was here. Cleanliness and appearance - the place is rundown, the carpet is stained and things are falling apart everywhere. The thermostat in my room seemed to only have two settings - arctic frost or Sahara hot. Nothing in between. Facilities - The showers in the bathrooms do not work. They have one shower area for the entire wing, thus you are only allowed two showers per week. There is only one small outdoor area to sit, it is on the resident side. It is small and rundown. The only "common area" on the short stay side is behind the nurses desk and consists of two tables, usually occupied by staff. There is a larger dining area on the resident side, as well as a very small seating area. There is a good sized activity room with a tv and dvd player, some books, puzzles and two board games. The books are random and old, the art supplies are old and not kept up. OT - the tech was very nice but the OT program is poor. They had me do a few arm exercises and then had me sort a tray full of washers, bolts and beads into little plastic cups. Seriously. Oh they told me a story about this woman Lois who was a successful stock broker who married a really good looking guy. They had 3 children and she decided to stay home and raise the kids until they were teenagers before she went back to work. Then they asked a bunch of questions like what was her name. I was preoccupied though. I wanted to know how Lois and the really good looking guy paid their bills for the 16 years or so she wasn't working. PT - again, poor. My PT consisted of getting in and out of bed, walking with the walker and getting in and out of a wheelchair. I was supposed to learn how to use crutches to go up and down stairs because I have six stairs to go up to get to my front door, but they only had three crutches in the whole place; two were for the jolly green giant and one was for Tom Thumb. So the skill I needed to learn most couldn't be taught. Summary - I'd like to say don't come here it's horrible, but I'm sure there are a lot of places worse. If you end up here, you're looking at laying around in bed 23.5 hours a day. If you do get out of bed, you're pretty much on your own. Activities consist of bingo twice a week, movie night once a week and snow cones once a week. This is clearly a nursing home for people in the latest stages of life; the rehab was probably added as an afterthought to generate more revenue. I wonder where they send younger people who need in-patient pt after a hospital stay. Because I was probably the youngest person there - and I'm 65!

There were a surprisingly high number of patients waiting to be seen but we got in. Lindsey did the first set of X-rays and I was so impressed with the care and communication she gave the both of us the whole time. We even got a sticker of her cat miss Norris and we were told they are a hot commodity. I believe it. I hate that I didn't completely catch our nurses name working with doctor Washington- Kayla, Kaylee, Caitlin?! I'm so sorry!!! But she was so great throughout the whole night with my daughter and even gave her a brown bear at the end with a splint and cast on the same arm my daughter left with one. Ded. Dr Washington you were also very kind and thank you for doing your best to put the broken bones into place. We had a long visit but the care was superb so 5 stars to the whole staff. So so impressed and made the emergency visit less painful. Also note to the world- monkey bars are horrible and I shake both my fists at them.

I gave birth to my daughter, Kinley, by C-section on April 10, 2026, which was much earlier than her estimated due date of May 4. As a first-time mom at 36 years old, I was nervous, emotional, and honestly scared stepping into such a big and unexpected moment. But from the minute I was checked in there, I felt so thoroughly, thoughtfully, and genuinely cared for. What could have been overwhelming turned into a beautiful, peaceful, and deeply supported birth and recovery experience. I did not have any hospital bags so early and they had everything I could need and more! My stay was four days in a beautiful private room, and I am so grateful for how comfortable, safe, and cared for I felt the entire time. The hospital itself was clean, calm, and beautiful, and that really mattered during such a vulnerable and life-changing experience. It gave me such a sense of peace. What stood out most to me was the incredible staff. Every single nurse, doctor, hospital staff member, and person who came in to check on me or my daughter was kind, gentle, respectful, and compassionate. I never felt like just another patient. I felt seen, cared about, and truly looked after. I especially want to recognize Sydney and Jill, who were both amazingly kind, fun, attentive, genuine, and just wonderful human beings. They helped make such a scary and emotional experience feel smooth, safe, and even joyful. They were so patient with me as a nervous first-time mom healing from a C-section, and they were incredibly encouraging and informative without ever making me feel silly or behind. I felt empowered and encouraged. They taught me so much, helped me feel more confident, and brought such warmth into the room. I truly hope other moms get the chance to have nurses like them. I also want to mention my last nurse on discharge day, who helped with the car seat and made such a sweet final impression on me. She was wonderful, helpful, and caring, and it meant so much to end our stay with that same level of kindness and support. Most importantly, the care my daughter Kinley received was amazing. Watching how gently, carefully, and lovingly she was handled and brought into this world is something I will never forget. That means everything to a mother. I felt like both of us were in such good hands the entire time. I could not have asked for a smoother experience, especially considering how unexpected it all was. Providence St. Vincent gave me a birth story and recovery experience that I will always look back on with gratitude. Thank you for helping make one of the biggest days of my life feel beautiful, safe, and full of care. I wholeheartedly recommend Providence St. Vincent to any expecting mother or family. I will always remember how loved and supported we felt there. I know I will never forget this day and the days following my swift and smooth discharge with my new baby daughter, but I will ESPECIALLY never forget the amazing women who helped me through walk into motherhood for the first time with such care and compassion. It was truly more special than I could have ever even planned. The office also moved things along so quick and organized I never even had a chance to feel overwhelmed, was just able to focus on loving and caring for my daughter.
