She had been in the hospital and then a rehabilitation center after suffering a hip fracture at a retirement community's memory unit in Portland's West Hills. That community looked like a high end hotel and was considered the best in the Portland area. It was expensive but I assumed it meant my mom would be receiving high quality care and that was extremely important to me. I quickly learned that a beautiful facility and high fees do not translate to good care. The staff were pleasant but did not seem connected to the residents and took care of their needs in a perfunctory manner. They were also unskilled when it came to working with memory impaired individuals. When I called to check in, they seemed to have no idea how my mom was doing and my mom was rarely bathed. This concerned me and so I ended up paying extra for personal caregivers several times a week. It was at this facility that my mom was intentionally pushed over by an 18 year-old unskilled caregiver and suffered a extremely painful and debilitating hip fracture. After rehabilitation, my mom arrived at Farmington Square. She was crying and clinging on to me as this was her third transition within a span of 2 months. I was not able to accompany her inside as Farmington was abiding by strict COVID protocols. The staff there went out of their way to help my mom feel at ease and helped her get to know other residents. They also reassured me. Whenever I called to check in on her, they clearly seemed to know and care about her and provided detailed updates. When I spoke with her on the phone, she seemed happy. Once limited visits were allowed, they were very accommodating. In June of 2021, my mom's health declined and she was placed in hospice. Initially, I was going to bring her to my home, but I realized her needs exceeded my capabilities. I felt like I was letting her down but after spending time at Farmington with her, I quickly realized that she had wonderful relationships with the residents and caregivers and it was home to her. My sisters and I stayed with my mom 24/7 during the last weeks of her life and we were able to observe the consistent high quality care that my mom as well as all the other residents received. These outstanding caregivers were all emotionally mature individuals who were naturally compassionate people. They were attuned to my mom's needs and she was receptive to their caregiving because they were well-trained and had developed a trusting relationship with her. The staff and administrators even took care of me and my sisters. They brought in trays with coffee, ice water, and snacks and sat beside us as we struggled to say goodbye to our mom. Through these experiences I have seen the very worst and the very best of care for a loved one. Farmington exemplifies the very best by focusing their efforts on hiring extraordinary caregivers, which meant that my mom was always clean and comfortable and experienced the joy of meaningful relationships and feeling loved at the end of her life.

The Pearl provided excellent care for my husband after an extreme spinal surgery. He ended up with…read morean infection at the hospital requiring emergency abdominal surgeries. After all of the surgeries my husband spent a month in the hospital and he was kind of at square one when discharged. When he started at the Pearl he was unable to stand. After his month long stay, he was in good enough shape to slide transfer from a wheelchair to his bed or shower/commode. He was also able to stand for brief periods of time. He progressed enough to join the intensive Rio Rehabilitation Program at Legacy Health. During his stay he received top notch care from Rio staff. They were very caring and met him where he was physically. The therapists pushed him to get where he needed for additional rehabilitation. Thank you Colin and Debra! The nursing staff were caring and as timely as they could be and several nurses were standouts (like Carol and Sherry). The weekends seemed to have less staff so things could take longer. That said, they were upfront about other work they needed to do before being able to help. And when they were helping, they took all the time and care that was needed. The food was very good with several alternatives if the main menu didn't sound good. Bambi was great in helping set up wheelchair transport for appointments, Sondra helped coordinate lots of appointments and helped make Rio happen. Tawnya was amazing at coordinating transition to Rio and backup plans. Penny was the nicest therapy pooch. She lifts everyone's spirits. I highly recommend the Pearl as a skilled nursing facility. (The onion rings are the best ever). The building is older and appliances are tired. But they work. I am content.

While a few individuals on the care team truly made a difference--Damalesh, Shantay, Louie, and a handful of others were kind, attentive, and absolutely wonderful--the overall experience left me shocked and disappointed. The social services director, Sam, is the main reason this facility gets a 1-star review. He was dismissive, condescending, and ultimately responsible for the stressful, manipulative mess that surrounded my attempt to leave--saying that I was "denying care" when in reality I was taking my "care" elsewhere... Had to fight to get basic discharge paperwork (that I asked a half dozen times to get for hours on end), was pressured to sign documents without being given full information, and felt completely stonewalled when I asked for reasonable medical equipment and referrals for in-home rehab. Sam made what should have been a smooth and supportive process incredibly frustrating. As for the food? I'm vegan, and while they made an attempt to accommodate that, I can't live off bread and tofu forever. Toward the end, I could barely eat anything at all--it got that bad I understand care facilities aren't luxury resorts, but respect, communication, and basic follow-through shouldn't be optional. If it weren't for the handful of standout staff members I mentioned earlier, I would've lost my mind here. If you have a choice--keep looking. You deserve better.

I felt absolutely outraged and degraded. Yet, no one seemed to care. The only reason I was here was to rehab from cancer treatment It was much more banal at best. A complete lack of communication between staff members. Often told "oh, I forgot, or no one told me," which is absurd when dealing with people you are there to care for. On more than one occasion, I was brought the wrong tray of food while the staff jokingly said, "oh, it happens all the time", like that's ok. Another issue.. having to put a sign on my door asking people to knock before just walking into my room. It is a common courtesy to knock on someone's door before walking in... it does not matter where you are. Yet, even with the sign, some just walked in without knocking. As far as rehab.. I did better PT in my room. A couple of old bikes and a few weights. It was a waste of my time and dignity. I stayed there two weeks too long. However, that being said.. some of the nicest people you ever wanna meet. All seem pretty stressed most of the time, definitely overworked, probably underpaid. From my personal observation along with asking the R.N's CNA's, LPN's, the rehab staff and others. Each caring for patients with different needs and specifics. The length of time spent that particular resident varies, leaving others under their care basically screwed until they can get to you personally. That's why the one star, because they do try. But ,things definitely need changing as far as respect, patient care, effective communication, the fundamentals and most valuable not treating your patient as you did your last. Each patient are different in their own right, they should be respected as so. As an individual, which we are all and not just another bed filled.

When I toured I witnessed the director ignoring residents and being oblivious to the loud, cold and chaotic atmosphere. The two large common rooms are under furnished and sparse in warmth and decor. The large TV on one wall was turned up so loud that we had to try and talk over it and again, the director Eugene, was oblivious. I watched the director Eugene ignore a resident complaint and turn away from him in a dismissive manner without care or concern or a plan of action. I saw a resident holding onto a wall rail in need of a Walker because it was too painful to walk unassisted and there was no staff available to help her. The environment was chaotic and felt dangerous and neglectful. I ended my tour abruptly and will be filing a formal complaint with the state. It is clear this company operates for profit with no knowledge of how to run a safe and effective Memory Care facility. Horrible. facility!!!
