This is the best format for senior living. Each house holds just 10 residents in 10 bedrooms, split between 2 hallways on either side of a central common area with dining space, living area, and an enormous kitchen where staff prepares meals and residents may use space, also. Each bedroom is private with en suite bathroom equipped with a walk in shower. Facility provides twin bed, sitting chair, side table, and dresser. There is space to personalize as well. We plan to add 2 sitting chairs for guests and a larger dresser. There is a large closet, between 5-6' wide. Laundry is done on a schedule and not mixed so chance if items missing is very low. Nurse is available, aids are assigned on shifts and are very capable and friendly. The center publishes activity calendar including social outings and shopping trips. There is a hair salon and manicures and pedicures can be scheduled.

Having my husband at this rehab facility was a nightmare…read more After spending nearly two months out of state for back-to-back brain surgeries and serious complications, we were grateful to have him out of the hospital and closer to home. Unfortunately, after assuring us that they were equipped to care for his extensive medical needs, the care he received was deeply concerning. Some examples: -While in their care, my husband developed bilateral pulmonary embolisms. He was severely short of breath, yet it took nearly 24 hours for him to be sent to a hospital for treatment. Staff attributed his symptoms to COVID and dismissed repeated requests from our family to have him evaluated. When we asked if we could take him to the hospital ourselves, we were told, "You can't take him out those doors." -When they finally called an ambulance to take him to the hospital, they did not provide transfer information, including his medication list and other relevant medical details. -A nurse dropped an IV line on the floor, picked it up, and continued to use it.-Important medications, including pain medication needed to help him participate in PT/OT, were delayed by several hours. -Members of the nursing team blamed each other in front of us for improperly administering his IV antibiotic, which he was receiving for meningitis. -A bag of saline had a handwritten expiration date and continued to be used for two more days. When I asked the head nurse if it could be replaced, she appeared annoyed and said, "We'll replace it if it makes you feel better." -His PICC line was left so dirty that when he was eventually admitted to the hospital for the blood clots, the hospital nurses were afraid to use it because they did not want to risk another infection. We tried more than once to have open, honest conversations with the head nurses about our concerns, but we were met with defensiveness and dismissal. One morning, a nurse told me there was confusion about whether my husband had received his IV antibiotic because it had not been properly documented. When I asked for the head nurse to speak with her about it, she told me there was "no confusion," then turned around and walked out before I could respond. I would not recommend this facility to anyone. My husband was miserable there and said he felt like nobody cared about him. More than once, he was left yelling for help at night to use the bathroom, even though he could not go on his own. Based on our experience, I would not trust this facility with the care of a medically complex patient. We tried to approach these conversations with patience and understanding, but we were repeatedly made to feel like an annoyance rather than a family asking for basic communication and safe care. I also ask that the facility not respond by asking me to contact them to discuss this further. We tried repeatedly to speak with someone who would listen while my husband was there, when it mattered most.

The PT/OT is wonderful. I would say the best in the area. But the aides, the ones who do the hands…read moreon care, for the most part are terrible. There are some who care, but the majority know it is hard to get help and it shows in their attitude. Most of the nurses feel it is beneath them to help a resident get to the bathroom, and since there are not enough aides, you could wait 40 minutes for help. If you have someone there, it is important to be there with them for the majority of the day, or they will not be helped. The staff is overworked and resentful. My husband was there for 12 weeks and by the end he was begging me to get him out of there.

They were attentive to her needs and mindful of her limited vision. They prepared foods that met her dietary needs and cared for her the way I would have while she was at home. I did not have any worries about her during her stay there, which is a huge statement because I'm rather particular about her care.

From the start it was okay, I got placed with a…read moregood therapist and she was with me for a few months, and then it went downhill. she moved to a different place, and wasn't supposed to tell me she was moving, but did for my mental health. so that was fine. then I got another therapist, and she wasn't very caring tbh. I got a long with her and then?? DITCHED. and did it a third time. I also went to the summer camp, and it was a dumpster fire. tons of kids were more worried about dating than the camp smh. it was a cool thing to do yes, but it was pretty bad. every day kids were misbehaving (yes I expected them to) BUT WHEN I SAY MISBEAHVING I mean running out of the camp, hitting, screaming, disrupting lessons, etc. and then past the deadline of people aloud to join, a kid joined and flashed everyone in our class. the staff i feel didn't do everything they could, as multiple girls were crying, and everyone was disappointed. the boys in my class comforted me better than the staff. the staff were also overwhelmed, and sometimes didn't get to issues till days or weeks later. while it was fun, it just wasnt helping and made me worse. they had fun activities, such as field trips, but would take us in commerical vans without telling our parents when or where. thanks for what you did do vital health, but please people take your business somewhere else to help you or your kids...

I have worked at the facility for almost 3-1/2 years, and to say that I enjoy coming to work everyday is an understatement. When you have co-workers that are always there to help, lend a hand, someone to confide in, and treat you like a part of your family, it makes coming to work enjoyable. The best part of my job is the wonderful families that come through our doors. We get to understand all of the great times they have had together with all of the close interaction we have with them. It doesn't take long when we feel like part of their family. I interact with as many residents as possible everyday. I love listening to stories about the "good old days", their families, holidays, etc. I cannot say enough about Perrysburg Commons. A place where work doesn't seem like work, and you feel like you are a part of family.....
