My father was released from Osborn Hospital in Scottsdale AZ on 7/15/26 & admitted to Osborn health & rehabilitation in Scottsdale, AZ on 7/15/26 in the evening. He passed away 7/28/26. Upon release he was talking and walking with assist and stayed an extra 3 days in the hospital to ensure he was his normal self and was ready to start activity rehab. He had pneumonia and an infection that cleared prior to leaving the hospital when he returned to the hospital upon evaluation they noticed it returned.The first night he was in the rehab facility he fell out of his bed around 4:00AM and a call was placed to my stepmother from Nadeera around 5:39AM stating he fell and has a slight bump on his bed and he was normal. My stepmother went to the facility around 8:30AM after work and my father was incoherent, staring at the walls and had a temperature of 80 degrees.I spoke with Chanelle on 7/19/26 stating his vitals was normal and she released information to me and I wasn't on the contact list, she placed me on the contact list after she gave me his medical information in which my stepmother didn't authorize and Bodie stated my dad spit his food and turned his head away during an attempted 1:1 feeding session. My father was completely incoherent and not responding to touch or anything so that's not true. He was left in a hypothermic state for 4 days, no food or water and an IV was not administered until day 4 or 5 of his stay. My father was not in a position to do any physical, occupational,speech or activity therapy. He immediately declined in less than 12 hrs of being there and his ICU doctor kept asking us why they (Osborn rehab) sit on him for 4 days and not send him back to the hospital right away (right across the street from the rehab is the hospital less than 30 seconds). THe rehab doctor spoke to the ICU doctor and stated my father never fell and we have a voicemail stating he did. My father got transported back to the hospital on Monday 7/20/26 and went to trauma for MRI and Scans which came back clear, after the evaluation in trauma he got rushed to ICU due to septic shock in his heart/lungs, and severe coma, lack of oxygen and immediately had to be incubated on a ventilator. Once my father fell they never gave him an XRAY or CT scan to check on his health status which is also on record. We also have photos and videos of our fathers condition/ health which was utterly disheartening. I walked into his room and immediately knew something was wrong and he needed immediate medical attention. The aide Chanelle stated he had no xray done and his vitals were normal which is untrue. MY father body temp was 80 degrees and we found out from the hospital his actual temperature due to a catheter being inserted into his groin for an actual temp. SO how did they get a temp of it being normal? The nurse stated my dad was normal after the fall he was non-responsive, blood shot red eyes, not responding to touch or conversation and was naked no clothes and a made up diaper, a mat on the floor to brace his fall had a feeding table on it locked in place. My dad was in no position to move anyway but he is a fall risk and the table was completely unsafe. My father experienced a severe decline in health and experienced malpractice, pain and suffering, gross negligience, delayed calls to 911 un an emergency, sepsis (leading to septic shock), sudden decline, neglect, malnutrition, inadequate resources (no clothes on my dad, no warmer to provide warming blankets which was a delay in adequate care. The rehab kept insinuating to my stepmother she could take him to the hospital herself and my dad was completely not in the position to move he needed an ambulance. The rehab sttd he was there under doctors orders and nothing could be done until the doctor came in. The facility wanted my stepmother to take him herself so they wouldnt be liable for the lack of care they provided my father while in their care. I would give all the earthly things in the world to have my father back with us, he was aiming to come home nothing about his stay was leaning towards him not fully rehabbing and coming home. My family and I are truly upset that due to sudden decline and delay of calling 911 is what contributed to him passing away. He was up and talking and walking when he got there and less than 12 hrs my father was in a position of a person who was left at the top of a mountain and froze to death. The staff is only there for a check and every person there needs to be fired. My family and I are currently taking legal matters and are currently represented by a firm that truly believes justice will prevail in our favor. Honestly, one star is too much zero.

I understand rhe intake questions for alcohol are pretty intense but her demeanor was not kind. At one point I stuttered because I wasn't sure how to answer. She repeated it again in the exact same way, but louder, and another nurse stepped into further clarify. The lady who stepped in was the only person who showed an ounce of respect for me as a human. As the process continued I spoke to a couple different staff and not one looked me in the eye. I displayed no anger or hostility so I'm not sure if that's company policy but it certainly didn't read as empathy in a vulnerable period. While we waited the last who did the line of questioning brought back my boyfriend and asked if he was my drinking buddy. They told me blood pressure was super high, handed me a 5000$ bill (not their fault, but showed no interest in helping me find help elsewhere or working with me for payment plans or anything). I asked to leave and they said surely. Literally. I hate to complain but I was so horrified and thank god I wasn't there for suicide because that wouldn't not have helped after being shown zero empathy, or respect as a person for that matter. My heart breaks for the next person who enters that facility and are treated this way. There is hope and other facilities. I went to a different facility the next day and was treated with the utmost respect and made me feel proud for pushing through and sticking up for myself.

The first time, the nurses were cold and dismissive. It felt as though they had no compassion for what our family was going through and treated my child and me as an inconvenience. The doctor wanted to discharge my child without explaining their condition or providing any clear guidance about next steps. Sadly, our next visit was much the same. The doctor seemed eager to release my child as quickly as possible, offering minimal care and almost no resources for our family. After crying and begging- we were finally provided with some resources for aftercare. But in their brochure, they say that they will provide many resources to customers. In my experience, you have to beg to get this. Caseworkers ignored my phone calls and emails--many of which remain unanswered even now. To their credit, the nurses and security staff do seem to have received some training since our earlier experiences; they were at least more courteous this time. But overall, it was still a heartbreaking experience. My heart goes out to any parent who has to bring their child here. We've been to several mental health facilities, and I'm sorry to say that Banner might actually be one of the better ones--which says a lot about how far our mental health system has to go. Our children and families deserve so much more compassion, care, and communication than what we're currently given. I urge Banner's leadership not to respond with a generic apology, but to truly invest in compassionate care. Focus less on profit and more on humanity. And to the parents of a child struggling with mental illness--please, take notes during every interaction. Write down names, dates, and details of each phone call. Get your caseworker's direct number (if you can). Sadly, you may find that you have to advocate alone more often than not. The hardest lesson I've learned from all of this is that, for the most part, I am my child's only consistent advocate. I wish that weren't the case--but for now, it's the reality.

The staff - since being here I have interacted with 10+ people and EVERY SINGLE ONE of them have been sooooo nice!!! A lot changed during my time there, and I think it's only fair to share the full picture. First, they did eventually allow me to get back on methadone, which made a world of difference in my experience. That was honestly the turning point for me. I still believe moving forward it should be handled on a case-by-case basis, based on what the doctor feels is best for each individual. The only difficult part was that they didn't allow me to go above my starting dose, even though that wasn't my therapeutic dose. For me, 70mg is ideal. But I was honestly so grateful they let me back on it at all that I just dealt with it. I think the limitation probably comes from fear of people trying to increase doses, which wasn't the case for me. Before I left, one of the directors, Sawyer, actually met with me after seeing my initial review. He wasn't trying to get me to change it -- he genuinely wanted feedback on what they could improve, and I really appreciated that. It showed me they do care and are working on making positive changes. There were things I really liked about Hopess, too. The staff was overall very sweet and kind. The med techs were fast, efficient, and always respectful. I also liked my room -- not all rooms are the same, but I really loved mine. Even though my experience started off rough, a lot of things improved during my stay. I'm grateful for the parts that helped me, and I hope my honest feedback continues to help them grow and improve the program for future clients.

They offer constructive feedback and are quick to tell you what you need to hear - not what you want to hear. The staff overall truly have your best interest at heart and want to see you succeed. Thus far, treatment here has been a remarkably empowering and fortifying experience. They're recognition and approach have pulled me up from darkness into an optimistic state.
