I would not recommend this facility…read more When a facility is unable to have sufficient staffing levels, it is impossible to provide care in a timely manner. (On one weekend all the aides called out for the overnight shift, so an aide was pulled from another unit to help. With 40 beds in Mom's rehab area, it was tough.) Aides should not have to hunt for supplies when preparing to change a patient. Large sized briefs, barrier cream and garbage bags should not be in short supply in a nursing facility. 95% of the employees here are good souls. Their heart are in the right place, they care about their patients and attempt to do the impossible. The kitchen team is incredible. They offer so many options to patients, produce delicious food and are incredibly responsive to requests. As someone who stayed with their loved one - leaving only to shower and briefly nap - I have the following recommendation to caregivers. - Keep a careful eye on prescribed medications - Visit frequently - Check skin integrity frequently - Engage with the dietician and ensure personnel delivering/recovering food are aware of current meal plan protocols. Dietician does not reach out to patient/family to inform them how things are supposed to work. You will need to ask. - The PT program is not a rigid structured time setup. It is effectively first come first serve. Which may result in the therapist who is familiar case being unavailable when the patient is available. - lack of visibility into prescribed medication as well as report of medications actually dispensed, needs significant improvements. Key personnel administering medication have experienced significant unexplained turnover, which is somewhat in step with facilities personnel charges at all positions. When Mom was last released from a hospital, our family opted to bring her directly home instead of risking another experience like the one at Island Nursing.

    I am reporting this place to the Dept of Health. My family member was here for a couple of weeks…read moreand I was so happy when they left. Calling for a nurse in this place is almost impossible. After many many attempts to get medicine on time it becomes an impossibility to find out why they don't respond. One nurse tried giving my family member the wrong medicine. Thank God they would ask about what they were being given. Their own nurse attended to wounds once every 3-4 days when their wound was to be taken care of 2x a day. This place is terrible and if you need rehab or nursing care, DO NOT GO HERE.

The conditions and treatment my mother was subjected to under your "subacute" care were not only negligent but, in my view, bordered on medical malpractice and predatory financial manipulation. I. Administrative Misrepresentation and Insurance Coercion Upon admission, I was pressured into transitioning my mother from her established UnitedHealthcare Dual Complete coverage to straight Medicare. I was assured this was a "procedural necessity" and that Chana from Thryve65 would seamlessly restore her original coverage. I now recognize this for what it is: a tactical maneuver to prioritize the facility's payment security over the patient's continuity of care. As of April 6, the promised restoration of her plan was denied, leaving us with an unauthorized AARP Medicare plan. This bait-and-switch regarding a vulnerable senior's insurance is legally and ethically reprehensible. II. Gross Negligence and Safety Violations Despite my mother being a documented high fall risk with dementia, the facility failed to provide the most basic safety equipment. Upon arrival, there were: * No bed alarms; * No guard rails; * No walker (denied under the guise of "waiting for assessment"). The suggestion that the facility's solution for a fall-risk patient was to "put a mattress on the floor" is a staggering admission of a lack of professional standards and a failure to provide a safe environment. III. Chemical Restraint and Medication Mismanagement Most disturbing was the unauthorized and aggressive sedation of my mother. In response to her "sundowning," the staff administered a cocktail of Ativan, a Haldol shot, and an additional 50 mg of Seroquel the following morning--the latter of which was not prescribed by her primary physician. I arrived to find my mother slumped over in a wheelchair, incapacitated by what can only be described as chemical restraint. Over-medicating a dementia patient to simplify staffing duties is a violation of patient rights and basic medical ethics. IV. Deplorable Sanitary Conditions and Staff Incompetence The facility is antiquated, filthy, and staffed by individuals who appear wholly untrained. I was forced to personally bathe my mother because the assigned aide was "new" and clearly inexperienced. My mother was left naked in a cold shower room with barely lukewarm water--an experience that was both humiliating and physically dangerous for a senior. Conclusion Medford MultiCare is a facility that masks its lack of care with administrative pressure and medical mismanagement. I wouldn't entrust the care of a dog to this institution, let alone a parent. Be advised that I am documenting these failures for a formal report to the New York State Department of Health and the Better Business Bureau. Your "care" is a joke; your staff is clueless; and your facility is a liability

    This is a horrible place. My mother fell at least 15 times. Once, I found her lying on the floor…read morepassed out with her head on the bottom metal piece under her bed. I have photos . They lost her upper dentures and her remaining teeth rotted out of her mouth. She ended up getting a UTI and they weren't able to treat it well and she ended up dying. She was 75. After I begged for her to have a chest xray they told me it was a holiday weekend . they waited an additional two days before sending her to the hospital. I spoke directly to the Dr at Medford multicare and said i thought she had pneumonia. I don't think he cared because she died 2 days later. The Dr at the hospital told me of this were his mom he would let her go. She was very very sick. Completely septic by the time she got to the hospital. This place is pure negligence and the residents are constantly sitting in their own feces. They do try to make things fun and some of the staff seems like they cared. The PA Kristen was very good
