When I got upstairs, there were no staff nurses at the counter to direct us to the room we were looking for. The lady who was changing the diaper for the person I was visiting took 10 minutes to change it. The nurses here need a little more practice. The place looked danky and run-down. Patients were screaming all around and crying. It makes me think of places to NOT ever put my parents or any loved ones into. It is really upsetting to see such a nice lady put here. I wish there was a better place for her to live out her life. There is a lot of space for this nursing home to improve on. This is a place for you to leave someone that you don't care about and let them rot because the staff on the 6th floor don't give a shit about them. You can make all the complaints you want and they'll treat them well because you're there watching them but best believe that once we walked away until we come back the following day, he was ignored and we would have to do the whole process again of arguing and letting them know that your loved one isn't being treated well. Imagine if they were treating my father like shit and we were constantly going there to check on him and calling them out on their lack of care, I can only imagine the ones that barely get any visitors, how they were being treated/ignored. Healthcare officials need to make surprise visits to all of these "care centers" and see that the staff are not doing their jobs at all! That there's no type of "care" all all. You cannot bring loved ones back but you can prevent tragedies from happening in the future. 1. My loved one (LO) fell at least four times, so badly to the point where there were open bloody wounds and injuries on the face and legs. LO was crying because they were left for hours in soiled diapers. That only means that the nursing assistants were physically harming residents, neglecting care and supervision. 2. The social work department had care plan meetings without informing me/the family/legal health care proxy when the patient could not make medical decisions for themselves. They also transferred my LO to another floor, again, without informing me. 3. After setting up a date to leave this place, the social work dept denied and delayed discharge to home and could not get a doctor or NP to explain why. I had to find out in the most devastating way, way too late, and not from Cypress Garden. The medical team never informed me that LO had a positive covid test amongst numerous covid positive staff and residents in the facility. There are a multitude other reasons as to why this place is appalling and continue to not take responsibility. I never heard from the administration or care team afterward. My heart goes out to all the residents and families that have experienced horrific, improper care, and negligence from Cypress Garden. The nursing staff on the 4th floor was responsive and the nursing supervisor Chris, in particular, was so kind to my mom. Other staff on the 4th floor were also kind, which I observed during my visits with my mom. The volunteers, the admissions staff (in particular, Cory) and Ms. Park, the head of Social Services were also really great. I did not have any issues with being able to visit my mom. My mom left there in much better shape than when she entered and I'm so thankful for that result. I can't give 5 stars because I learned after my mom left the facility that they wake up residents at 4am to take blood or other samples if they need lab tests. Apparently the reason for that is to gather samples for when the lab opens. That was the only negative thing my mom had to say about Union Plaza.

Most alarmingly, my grandmother has stated multiple times that she is scared of one particular nurse. My mother witnessed this nurse handling her roughly and responding to simple requests for assistance with a clear and unprofessional attitude. Our concerns about this individual were clearly not taken seriously. The situation reached a critical point this week. We last visited on Wednesday, December 3rd, 2025, and she was fine. When we saw her today, December 6th, she was covered in injuries: a black eye, a busted lip, scratches on her neck and arm, a bump on her head, and widespread bruises. She is complaining of head and wrist pain. This did not happen gradually. The facility's response was utterly unacceptable. No one had reported any incident to us. The on-site manager, Mr. Kumar, claimed to know nothing about it, despite being present over the preceding days. He appeared careless and dismissive of the severity of the situation. We had to forcefully press the issue to even get the name of the Certified Nursing Assistant (CNA) assigned to her during this period--the same CNA my grandmother fears, identified as Kimberly "Monique" (last name illegible, as Mr. Kumar only scribbled it). When we insisted on immediate medical attention, the facility prioritized their internal paperwork over my grandmother's well-being, causing a dangerous delay. We ultimately had to call 911 ourselves to get her to the hospital. This is not just "bad care." This represents a systemic failure in supervision, accountability, and basic human dignity. Clear signs of potential neglect and a resident's expressed fear of a staff member were ignored, culminating in a shocking and unexplained physical trauma. BEWARE of this facility. Until they demonstrate a complete overhaul in management, staff training, transparency, and accountability, I cannot in good conscience recommend them to anyone. Overall, our experience with the facility was a positive one. My husband and I visited almost every day during the year she lived there. There were occasional concerns regarding cleanliness and medications, but when we brought those concerns forward, they were addressed. We were especially grateful to a compassionate staff member who called us late that evening to let us know our mother had entered active dying so we could be with her during her final hour. It is because so much of our experience was caring that I feel compelled to share what happened next. While my husband and I sat quietly beside my mother's bed, holding her hands and saying our final goodbyes, the overnight nursing supervisor entered the room. Without introducing herself, she looked at us and repeatedly stated that because my mother's IV had been discontinued, "you are killing her." We were stunned. We had not made any medical decisions regarding my mother's care. We had simply responded to the facility's call informing us that she was actively dying and that we should come if we wished to be with her. Instead of allowing us those sacred final moments, we found ourselves trying to understand why we were being accused of causing our mother's death. My mother passed away approximately one hour later. Families remember these moments for the rest of their lives. The final hour with a loved one should be treated with compassion, dignity, and sensitivity. Regardless of any medical misunderstanding, this was neither the time nor the manner in which to speak to grieving family members. I share this not out of anger, but in the sincere hope that no other family experiences such an unnecessary and deeply painful interruption during one of the most vulnerable moments of their lives. The William O'Benenson Rehabilitation is great if you or your loved one is ambulatory ( you can…read morewalk around or bus around in a wheelchair, eat by yourself, speak). If you need more care than that I don't feel this is the place for you. A lot of people choose Benenson b/c it has an Asian population, the facility is new, and clean... It's more Korean than Chinese, more Mandarin speaking Chinese than Cantonese. I haven't met one true Cantonese speaking staff member. However, the doctor is really Mandarin that can speak some Cantonese (so I hear). Make sure you see the events calender, I felt it was kind of low key only 2 events per day where other facilities typically have 4 in one day. Also, see if you like how the residents typical protocols for a day. They typically have 1 CNA baby sit them all in the dining room during the day while everyone goes in and out for their therapies. If you want to dine with your loved one or bring their own food, you need to go to one of the sitting areas or back into their own room. Most other facilities has a "cafe" setting where at Benenson, you have the food cart come up and then you get served. I felt the nursing was good, but I'm not too crazy about the doctor. I've never seen them before. If you leave your loved one there and they don't really speak for themselves and such; I feel that it is critical that you go there sporadically to any nursing home to make sure you are getting the care you are expecting. In addition, you keep an eye on your loved one where if there's something off baseline, it can be immediately addressed. Who else knows better than you about your loved one. *** For my Asian family readers... I know you can be aggressive and not so passive. I've seen you be aggressive at free events. You even elbow me at most of them. You should fight for what is right and what you want and need. We are the LARGEST population in the WORLD. What I see is a lot of families who only visit on the weekends (provided you are comfortable with that and your loved one can "take care" of themselves to a certain extent). I would suggest people to go a bit more just to really see the type of care you receive. I am a firm believer in PREVENTATIVE medicine, where I didn't feel I received here. They let my grandmother sit around getting septic before sending her to the hospital. If anything the doctor should've come immediately a week before when I complained that something was starting to look fishy and did some labs. In addition to that, during the second admission (b/c you do get bed hold for 14 days after a hospital stay; then you would be automatically discharged). The staff was disorganized where my grandmother was sitting in her own fecal matter for quite some time to the point it was all dried on her. Also, upon request of her belongings, they lost her shoes and custom orthotics (these are typically over $100). To top it all off, after an approval for a dental consult (the previous hospital couldn't get a dental consult). The staff couldn't figure out when the dentist comes into the facility. Where then I had to take on the responsibility myself to fix her problem with her mouth hurting to the point where she was classified "palliative care" and failure to thrive. Hello people... failure to thrive due to you cannot fix the pain in her mouth or throat preventing them from wanting to THRIVE... Also for Palliative care people... make sure you check with each facility what their own definition of palliative care is. There is such a large spectrum that most facilities define it as "hospice" with more than 6 months to live. If you find the right facility palliative care should be practicing preventative medicine, and if your loved one's time happens to come, then it was their time. But they should not be helping them along to getting to the point of no return, just non-aggressive medicine where you do not end up torturing a person with all these needles and such. Another thing when shopping for a nursing home. Make sure they know when all their consultants come in and that they are capable of determining if things are "routine" and if there is a priority it can be pushed up to the top of the list. Most of all, even though there are "scheduled" days that consultants come in, if there are emergency situations, the consultant will come in immediately. Overall, as a family, be active and proactive in managing your loved one's health care. Do not think of nursing homes as day care or long term summer camp. We all have our own lives to live but don't forget about them. In life we all do a full circle being children twice, so as much attention you give your children, you should also give to your elderly (which can also be frustrating at times). We will all be there one day ourselves and just think of how you want to be treated. Also, return to the elderly what they have given to us when we were helpless children ourselves. I'm not even close sorry when I say that this place is akin to what happened during the Holocaust…read more My Grandmother was admitted recently and thankfully was able to leave ASAP. No one should ever be treated the way my Grandmother was treated during her stay there. My Grandmother had a lot of water buildup in her body so medication was prescribed to help her remove that excess water and therefore she had to go to the bathroom often. When my Grandmother needed to go to the bathroom, she would request for assistance but the nurses and 'assistants' there were not willing - they would just leave her there for her to wet the bed. My grandmother was not having any of that so she would just slowly take her time, UNSUPERVISED by the nurses and workers there, and use the bathroom, only to get scolded by the workers there when they find out she has gone to use the bathroom by herself. As part of my Grandmother's stay there, she also had to go through physical therapy. On the first day of her physical therapy session, the workers wheeled her out in a wheelchair and just left her in a corridor. She waited and waited and was never attended to after she was wheeled out of her room. A long while after, she was wheeled back into her room where she waited even longer. No one ever attended her. Since she waited for so long, she eventually had to use the bathroom again, and of course, no one was there to tend to her. She tried to go to the bathroom by herself and the worker that wheeled her in back into her chair did not lock the wheelchair so my Grandmother fell! She is now covered in bruises because (1) she was never got help from the people that work there and (2) the people that work there can't perform their job properly! My Grandmother, during several periods of her stay, was told that if she does need to wet the bed, she should wait every 2 hours. Why, you ask? How dare this facility even suggest such a thing!? I understand that the person that told my Grandmother that was trying to look out for her, but are we serious here? If you want to look out for my grandmother, help her by assisting her to the bathroom, not by telling her a 'convenient' time to wet the bed so the sheets could be changed promptly. We took her home after her atrocious experience and when we brought her home, she was coughing non-stop. We took her to her family doctor only to find out that she contracted bronchitis from the facility! After her atrocious experience where (1) she was told convenient times to wet the bed, (2) where she did not get assistance to go to the bathroom when requested, (3) when she ended up falling because of the incompetence of workers there, and (4) where she ended up contracting bronchitis due to, most likely, unclean practices, where do you expect that person's quality of life to be? My grandmother was the person to raise me up when I was a child - how do you expect me to react when I find out that I can't do the same for her? I read from another review that apparently Medicare.gov rates this place with 5 stars, but those ratings are probably the result of excessive brown-nosing. Please - for the sake of your loved ones - do not allow them to be administered to this concentration camp. Some of the patients there are just waiting for death and the workers there probably expect the same out of all the other patients. This is unacceptable.

My family didn't place dad there blindly, it was by choice, after weighing all the options. We have a family member who is a long term resident there currently as well as a long time family friend who is there too. They love the care they receive and the staffers they receive it from. Their recommendations were well received. From the moment I entered the front door and was greeted by the front desk attendant Lana, I knew dad was in a great, loving, caring and Holy place. I went up to dad's floor, was greeted by every staff member I passed. Not just on my first visit, but every visit. No matter how busy they were, every single person said hello. As time went on, those greetings were by name. It truly felt like dad was being cared for by loving family. My visits only lasted a week or so as I live about 700 miles away, but my experience and my opinions of my dad's treatment are fact based. He could not have received better care anywhere else. The facility was spotless, well maintained and the staff all love why they do. It takes a special kind of person to do this work. Ozanam is blessed with the best. Ozanam is even updating the rooms one floor at a time. It's nice to see the investment back into the facilities. The office staff was extremely helpful, kind and very communicative. They assisted in every way possible, making the daunting logistics of dealing with the insurance companies a less daunting part of the experience. I could not ask for better! I'd like to name everyone I met there who cared for me dad and assisted my family throughout, but I would have to name every person in the public eye as well as everyone behind the scenes there. By God's plan, my dad, Donald B. declined due to advancing medical conditions. He received great personal care and was treated with dignity up to and beyond when he took his final breath. He passed on Sept 22, peacefully in his sleep. The staffers who knew him were as sad as we are. Thank you to everyone at Ozanam Hall of Queens for all you do for every person in your care every day. If you ask for help they walk away or say I am on a break. They leave rooms dirty and if you go to supervision they just cover it up. They do not isolate people during flu season so everyone gets sick. The aides do less than the minimum attempting to feed patients with dementia. This place is hell on earth for the vulnerable elderly.
