I do not recommend North Park Post Acute. My mother was transferred here from Kaiser after partial hip replacement surgery. The level of care given to my mother by the staff at North Park Post Acute was sub-standard. The staff did not provide the necessary and required support care. Her medical needs were not monitored - although on her chart from her physician and daily requested by family members. She was not given baths/showers, she did not get assistance to dress in the mornings, she did not get assistance to get to the toilet (often took 45 mins+ to get a response from the call buzzer before a nurse/CNA came to her room). The overall care was significantly less than adequate. I discussed my concerns with the staff, but was told it was someone else's responsibility (day staff blamed night staff and night staff blamed the day staff). I even spoke with the nursing supervisor, while she was polite and professional, there were no changes to the level of care my mother received. After a few days, we transferred my mother to a rehab facility in Modesto - the care there is significantly better. I should note, the lobby and check-in desk were acceptable - the patient rooms are not maintained and are in serious need of cleaning (trash on the floor, dirty bathroom: blood from another patients in the bathroom - for multiple hours, that led to blood in the bathroom overnight), and the walls in the patient rooms were in need of repair (very bad patch jobs in large areas of the sheetrock/drywall). Bottomline: Do not send anyone you love or care about here! read more

I was refer to North Park Post-Acute by Kaiser. I stay their for 12 days . The Staff was very friendly CNA, LVN, took good care of me. Pinkee & Dr Kumar very professional. I i enjoyed physical therapy. In occupational therapy..The meals were enjoyable. It would cater to my health NEEDS I also enjoy activities that had scheduled Bingo . North Park Post-Acute Care gave me extreme quality of care, very kind, extremely compassionate, and took care of me as if I were family. The nurses, CNA's, PT's, and OT's all deserve 10+ stars. The team listened to me, and answered each one of my questions. They worked with me daily so that I would be able to go home, and be safe in my own house. I would recommend North Park to everyone. The care and compassion they showed to each and every patient was amazing. I have a few people I would like to give a shout out to. Nurses Kyle, Biddy, Alejandra, Joshua, Zaid, Jocelyn, and all the nurses that helped me during this challenging time. The CNA's Joshua, Gladis, Deana, Louisa, Jasvir, Kiwimder, Kamuii, Debora, Phony, Bell, and all of their co-workers that helped me. The PT/OT department was outstanding. They worked with me compassionately while challenging me during every visit. I'd like to give a shout out to Anne Marie. I hope that's how she spells her name. She was my OT that worked with me every visit, helping me to build up my strength and my upper body, and my balance for when I returned home. She was a kind, compassionate professional, who again treated me just like I was family. There is PT that worked with me most of my days, and her nickname was G. I hope I'm right about that. She had me challenge my range of motion, which helped me continue to improve with my treatment. Again, she was kind, compassionate and treated me like family. I would give this whole facility 500% if I could. There was all so a specialty nurse named Porsche who helped to care for my incision. She is well trained for dealing with wound care. She worked with me multiple times to make sure that my incision was healing properly, and being taken care of professionally. She explained everything to me that she was doing, and she made me feel at ease and comfortable with her work. I want her, thank her for her care, wisdom, compassion. Absolutely wonderful--The nurse Biddy , Joy and Kyle are extremely kind ,their excellent care is very professional. Porcia and all CNR are also very helpful and did what they could to make every patient safe and comfy.. My therapist Cynthia, Monica and Megan has been exceptional. Their goal was to improve my walking and leg strength that I needed to able to works on stairs. I choose NP because of the website information, along with photos of facilities. After 13 days, I am left traumatized by my experience here. Pros: The Veteran staff, (Porsche, Miss Elizabeth, Verna (I call her Mama), Ping, Ashley (PT services) have done all the heavy lifting, as it relates to my wound care, and training me to take care of myself. I can't praise them enough. Cons:The young girls, both day and night shifts, are basically waitresses, emptying my urinals, garbage, bringing my food, etc. No real skill sets learned yet. Th personal urinals are NOT washed after being emptied into the toilet. Bedside manner needs to be taught to all staff. I spoke several times with on staff dietician about my food choices. EVERY meal was incomplete, missing silverware, butter, sugar, etc. when I ask for these items, so much time passes food is now cold and unappetizing. I ask for NO meat, yet Every plate had meat on it. As it reads on the website, you will ideally be placed in a double room with someone else. I was placed with an 82 yo dementia patient who coughed nonstop thru the night, EVERY NIGHT, not allowing me to sleep in any meaningful manner. I had to recommend to night staff to give him some cough medicine to assist him with that. This after 6 DAYS. Th next patient brought to my room was rude, bordering on being racist, and made up lies about me to staff. Constantly complained about EVEY aspect of treatment care (food, medicine, PT, etc.) He watched TV with volume too loud all the time. No consideration for others in the room. The path to healing includes Data gathered by staff, (blood draws, vitals, medicine, food, BUT also includes the environment one is attempting to heal in.) To have screaming patients is NOT conducive to one's healing path. These patients with with advanced dementia need to be in another type of facility, not this one. Patients with wounds from surgery should NOT be subjected to being woken up at all hours of the day and night. On my wing, 3 patients SCREAMED for attention EVEY night! Complete disruption for those of us trying to sleep, trying to heal. Vile cursing at the staff who are doing they best under trying circumstance. This EVERY night I have been here. I CANNOT FULLY RECOMMEND this facility for follow up care. The CONS outweigh the PROS. Buyer Beware! Much crying by other patients screaming to be released. A disruptive environment NOT conducive to your healing process. I too am anxious to be discharged from this facility. I am continually impressed by the staff at North Park. I frequent this facility for my job and it's one of the best facilities I have seen. Nursing staff greet me, receptionist is so cheerful and Social Services Dept is one of the best I've ever worked with. Pinkee and Carina are truly professionals. This place is ran by people with no compassion . There were a few ladies who were very caring towards my father's needs while there , most were stone cold and lacked any type of emotions towards my father when he or myself would ask questions . Example - when is my dad's next med due ? he's in a lot of pain . Answer let me check . 15 min then 25 then 30 minutes and I had to go search for that RN , mind you his call light is on this entire time . Patients in hallways sitting in wheelchairs for hours and hours . One lady 6 hours ! Some are told to calm down or go to your room . It's a very sad and cold atmosphere . Horrible service! Extremely difficult people who don't do their job, financially or care! I have an Aunt that's in that facility and has been their over 9 yrs. Her bank got bought out by another bank firm and the facility did not aware us of the fact they weren't getting paid for approximately 6-7 months! Who does that! Who waits past one month? Then when I come to visit, I'm attacked by their accountant! She even threatened to kick my Aunt out of the facility. My Aunts checks goes directly to the facility for over 9 years. Social Security quit sending her checks to the bank since the bank was bought by the new bank that's how it got behind. Since then, the facility gets the checks, but the accountant wants us to figure out how to get the months back from those 6-7 months! It's made our lives a living hell, just to visit our Aunt! North Park Post Acute has been the most amazing experience we could have under devistating news that my mom had dementia. My mom originally was referred to NPPA to have physical therapy on her fractured hip and several days later couldn't remember what had happened the minute before. NPPA took such good care of my mom the entire 3 weeks my mom was there. They were always in communication with me, and would call me to let me know any changes, or if my mom just needed reassurance she was ok. The PT and OT workers were so patient and kind! My mom referred to them as "My girls." A special shout out to my social service worker, Pinkie... Pinkie has been there 110% to help us through the process of finding long term care for my mom. She was available 24/7 and was probably one of the most caring people I have ever met. I could not have done all the research, and found resources for my mom without her! I would recommend NPPA for anyone looking for re-hab care for their loved one! read more

My mother has been at this facility since 2016, I must say I love the changes since it changed ownership. My mom stopped complaining when they took over. I went from getting two calls a week and having to make unnecessary trips there, to her only calling if she wanted or needed something. Fast forward to March 2023, my mom was put on hospice. I must say that 99% of the staff was awesome!! My mother was on hospice until her passing on June 23rd. I have to say that the entire Admin staff treated us like family, making sure not only did my mom have everything she needed, but also checked on me. The entire nursing staff was awesome, my mom could be a bit difficult at times, but they didn't let that phase them or affect giving her excellent care around the clock until the very end. First, let me start by saying, I chose this location to recuperate and receive rehabilitation after having more than half of my foot amputated. I went based on reviews and ratings. I did not feel safe in this environment. After recently, having a kidney transplant, they still allow visitors and staff to not wear masks which I ended up coming down with Covid while staying at this facility. As a Covid patient, I was made to feel as if I did something wrong. When in fact it wasn't.they did not protect me from getting Covid while I was in their care. They do not enforce, wearing mask. The Nurse who refused to give me a Covid test after requesting twice. After which I was able to get the test after speaking with the case manager. The test came back positive. Had I went with the nurses refusal how many other people could I have Infected with Covid. How dangerous was that. I reported it to the supervisor and to the administrator Tanner. I would like to acknowledge certain individuals for Doing an exceptional job caring for me. The majority of the CNA's have a difficult time understanding what you need due to language barriers. It took twice as long to get a point across to them after sharing my concern with the supervisor of the CNA's. She present me with a job aid photographs that I can point to so that they would understand what we were requesting. This is not acceptable. The property is nothing like the photos in their website. The rooms are disgusting. And unless you threaten to report them to the state, Nothing gets done. This type of VIP treatment should not exist for only those who threaten to report them but should be extended to all patients. My roommate was given privileges that other patients do not get because he had threatened to call the state. He was allowed to have guest coming into the facility after visiting our work closed. He had guest come to the room and stayed after 10:15 PM. He kept bragging that he was given the "golden ticket" Which allowed him to have guest anytime he wanted after hours. My husband was there for 2.5 weeks unable to walk. The quality of care was top-notch, very kind staff wanting to help, all had good attitudes, the different care types, bathing, PT, wound care etc all were very scheduled and good at their job. We were scared at first bc reviews on these places are pretty bad, although this one was 30 min from our home we went with it because the reviews seemed the most positive. We were not disappointed. We may have to go back for a short stint when my husband is ready to put weight on his legs and feet and we will be sure to go back to North Park. Very pleased with their care! read more

What can I say, this place was completely different before it was under new management..once this place aquired new managers/administrations/nee company, it has changed for the better. The entryway is so beautiful with fish tanks for visitors and patients tonlook at. They have a huge dining room and a very nice outside patio. The rehab department is also awesome as they call families once a week to give them updates about how their loved one is doing. The nurses are always there to helpnout when needed also! Families are also able to visit in the lobby or outside of course with COVID precautions which I appreciate their consideration to allow families to visit as other places still don't. Normally I don't have time for this but it only gets to a boiling point that it causes me to take my busy time to downgrade my review. North Park moves people very often for no good reason. This has caused so much stress on those fragile patients and their relatives. I understand sometimes they have to move people who have problems to solve problems within the facility, but I am talking about them moving nice people who are getting along with each other and do not have any problems with their roommates. I asked the social worker to give me one good reason why she has to move people who do not have problems once they get comfortable with their roommates, and she could not give me one. Then I asked the administrator and she said that she is doing to solve problems of other people. This statement does not make any sense. Every time they move people who are comfortable with their roommates and move that person to another room with another new roommate, problems cooccur. My mom has been there for almost 2 years, and they moved her so often that I don't remember how many times. Problems are evident every time they move my mom. I witness it. I am sure the management knows that people do not like changes especially when they are in that age (my mom is 93 years old) and they have medical issues so why they can't just fix the problems by moving people who are having problems? Why they move people who do not have problems to cause more problems for themself, for other patients, for the patient's relatives, not to mention that when the patients get upset, they give the care takers a hard time? And not only that when they move people, they never consult with the patients, their relatives or tell them why or ask them for inputs before moving people. They always sound patients got no choice but move when they want to move people. I talked to a few patients they said the same thing. Mostly people do not have any problems about moving into a new room but having issues with how North Part moves people without notice and the new roommates because they don't know each other. The lady I talked to said in her tears that her roommate who she has been comfortable with got removed while she was gone for a few minutes and then they moved my mom into the room. Now my mom becomes her new roommate, and she is upset, and my mom has to listen to her complaints. This is called change management. I am sure North Park knows what a change management is but just does not care about the patient and those who are impacted. I hope someone begins to look into a better way to solve issues of those who are having problems and leave people who are nice and don't have problems alone. read more

Good Skilled Nursing Facilities are few and far between. Great Skilled Nursing Facilities are even more rare. North Park Post-Acute is easily a great SNF. The staff is attentive and the care they give is genuine. The facility is bright and cheerful from the atmosphere to the people. In an environment where the nurses are the eyes and ears of the physicians, and visitation is limited by a worldwide Pandemic, you can be certain the residents are receiving exceptional care. read more

Don't subject your loved one to this facility! My father was transferred here at our request to be closer to family. Little did we know he was 100% better off in Stockton (which was not ideal)! He was at North Park a total of 3 days with his roommate constantly telling him to "shut up". My father fell off the bed the 2nd night and they sent him to the ER for examination (which is protocol), then would not allow him back in the facility upon being discharged from the ER. Pretty sure this is illegal!!! Ambulance had no choice but to take him back to ER. Nobody called us to explain why he was not allowed back, only saying it was a misunderstanding afterward!!! WHAT?!!! Staff onsite is extremely unprofessional with no sympathy and had zero answers to our questions. The social worker onsite (Beatriz) is the only person who helped us through this horrible situation. She actually got him admitted back to the facility the next day but my family & I declined. I DO NOT RECOMMEND THIS FACILITY. COMPLETELY HORRIBLE EXPERIENCE. read more

This is a wonderful facility and worlds different from a year ago. With the new management company and team, the entire feel is positive and welcoming. In this time of Covid where there are limitations to visitation, they have made it the place to go when you need skilled nursing for your family because you can feel sure they will be given the individualized care they need. Exceptional! That sums up our experience with North Park Post Acute. Our brother/brother in law spent 7 months there with a very challenging set of conditions that required a lot of attention. And attention they gave! Often above and beyond the call of duty. They were constantly on top of his changing medical needs and coordinated all the necessary care. They regularly communicated with us, sometimes even on a daily basis. Because we live quite far away, this was a huge help providing us with the confidence he was being well taken care of and secure. They also bent over backwards to meet his personal needs and requests, sometimes under difficult situations. They continued to encourage and support him even when he wasn't so receptive. Today he is able to be in assisted living in his own apartment when we were convinced he would never get out of skilled nursing. It is only because of Desiree and the entire staff at North Park he was able to achieve this. A huge thank you to everyone! Their level of professionalism, commitment, patience and grace far exceeds other like facilities we've previously experienced with other family members. They should all take a lesson from North Park. So, if you are looking for a skilled nursing/rehabilitation facility, especially with short notice and/or from a long distance, give them a serious look-see. They truly care! read more

My father recently stayed at North Park Post-Acute in Tracy, Ca. His stay was short, however, in that time all the staff we met were very warm, friendly and professional. Especially nurses Nina and Bitty and admissions coordinator Lydia. The facility was bright and clean and always smelled fresh. I especially appreciated the attention to detail in asking all visitors for their Covid-19 immunization record and copying it.
