The CNA was hired to visit 2x a week prior to the time the daily caregiver arrives. The CNA was late or didn't show up at least 60% of the time. When she did, her excuses were ridiculous. When brought to her attention, her response was belligerent and argumentative. She was completely taking advantage of an elderly woman who cannot advocate for herself. All the while claiming to have been here, being paid to have done her job. After being told her services were no longer needed, she showed up with the RN assigned to the case as if she hadn't been told to no longer come. They both told false stories to try to cover for her. It was a very unpleasant experience for the patient and her caregivers. To blatantly ignore your termination and to lie as if your patient and her caregivers have some sort of mental deficit is unacceptable. When you have to hire hospice, I believe you think you'll be getting a compassionate person with morals, not a scammer trying to get paid for not doing a job. Trying to speak with anyone from the company is a nightmare also. They were contacted numerous times to address our concerns and never followed up on anything, from things as simple as proof of insurance to complaints about lack of care. The whole process of trying to speak to someone in charge is a nightmare. I wouldn't recommend this agency if you're expecting compassionate, reliable care.

We opted for the hospice option because it was already spreading around her body. Life Choice Hospice has been there for us with a caring and professional intake session the morning after we contacted them through their website. That evening, Life Choice had delivered the equipment that my wife would need and started transitioning her to her new medications. We have a core care team of three. The names I have been using for them on our CaringBridge page are: The Amazing Nurse Julie (the primary caregiver who expertly changes the tumor's dressing and provides on-point answers, advice and support for both of us three times a week), The Fabulous Jessica (social worker whose visits, calm assurances, and insightful suggestions are truly priceless), and Stephanie the Magnificent (our Certified Nursing Assistant - because like a magician, she can turn my wife's soiled bed linens into clean ones really fast without my wife ever having to leave the comfort of the bed). When my wife woke up in pain at 2 am, we called the Life Choice Hospice 24-hour line, expecting to get an AI or an answering service. A Nurse Practitioner (NP) answered, (yes, a real person), and had the on-call person here in less time than we usually had to wait in an Emergency Room. The on-call (I forget if they were LPN or NP) was professional, caring, understanding, competent, and thorough who took care of her and did not leave until she and I both understood what was going on, why, what they were going to do about it, and what I had to do and watch for as they change her medication dosage and frequency to adapt to her increasing pain levels. We have needed the Life Choice 24-hour services at other odd hours between 11 pm and 4 am, and, while the NP on the phone and the LPN or NP that showed up were different, the pattern of professionalism, care, concern, and thoroughness was the same. When she noticed that I was getting overwhelmed with caring for my wife, The Fabulous Jessica suggested a "respite" meaning that my wife could visit a 24/7 care facility for 5 days to give us both a break. I did not know such a thing existed or could be so beneficial and timely. All three of my wife's core care team continued to care for her while she was in the 24/7 care facility. Life Choice had already contracted with a service award-winning facility, arranged transportation, got my wife settled in there, provided continuity of care by keeping their previous schedule, and coordinated the care my wife received from the 24/7 facility when Life Choice was not there. If you could use the help of a caring, competent, and understanding team of medical professionals like this, please contact Life Choice Hospice and Palliative Care. They accepted both my insurance from work and Jean's Medicare Part A.

We were assigned Emily as our case manager and she was great to work with. However, we interacted the most with Grace who was on-call at the time we engaged them and helped my Dad a good bit during his last couple of days. Grace was super patient with us when we asked lots of questions about his care and hospice in general. LifeChoice also provided a rabbi for us and I think my father really appreciated that the rabbi visited with him for over an hour. I think they provided my father a lot of peace and comfort during his final days. My only regret was not engaging them sooner. Several hours after our call, Christine was at our house to find out more about our situation. Just after midnight that same day, multiple pieces of equipment were delivered to our home and we were trained to use them by the delivery fellow. In the days following, we were visited by several other staff, including Kim who made an 'SOS' call to our house at 6am one morning when I panicked. Unfortunately, she also came over the next morning at 5am after our dad passed. She then interfaced with the funeral home and relieved us of that.
