I found the care here, for stroke recovery to be outstanding. I "lived" there for, I believe 5 weeks. The work was hard but worth it. Every day I think I had 4-5 hours of therapy. I learned that through research, it had been determined that that was the optimal amount of therapy to get for the most recovery early-on. On the days that I just didn't feel like it, I really appreciated their encouragement and when I made a mistake for what seemed like the millionth time, I appreciated their infinite patience. I was so lucky to be exposed to very professional staff all around, from the OT to the PT, the ST, and the doctors and nurses. I will forever be grateful that I got to go to this advanced program and receive consistently outstanding care. They taught me tools that I still use to this day. THe communication with my family was also very good all around. At the end, I transitioned to an equally impressive outpatient program with another very knowledgeable team of therapists. I believe I attended 4x a week for several months and worked with committed PTs, OTs, and ST's to improve just a bit each and every day. Honestly, I worked very hard too as it takes a strong commitment on the patient's part to do the homework. If you are willing to work hard you will receive fabulous, up-to-date care with the latest techniques. I also found that the coordination was tight and seamless with the social worker and my team of therapists. They also had a great sense of humor which was necessary to get through this difficult stage of life. I will always be thankful for the care I received and the lessons that I learned in a compact period of time.

Unfortunately, our experience with administration and leadership was very different. We chose this SNF because of the excellent care my mom received at CPMC's acute rehab years ago, but ultimately left feeling like we had made the wrong choice. There was little orientation or communication when we got transfered in which felt very in welcoming. My dad was transferred to his room, asked intake questions, and left waiting to meet his nurse. He was then moved multiple times during his first few days, which was difficult and uncomfortable for him. We also never met the case manager listed on his board. Communication continued to be a concern throughout his stay. During one therapy session, a staff member repeatedly said "tick tock" while my dad was struggling with significant pain, which felt dismissive and lacking compassion. The most disappointing part was discussing hospice. We were simply trying to understand our options and make an informed decision, but staff repeatedly treated our questions as though we had already chosen hospice. A referral was placed before we had made a decision, and a social worker came in that same afternoon said she needed a discharge date. We never saw her again which was very off putting and cold. We were repeatedly pressured for an answer despite explaining we needed the week to get all information needed and to talk to his doctors. At one point, a doctor even told my dad he was going home that night when no arrangements had been made. My dad responds best to patience and encouragement, but the more he was rushed, the more anxious and frustrated he became. We tried explaining this to the therapy team, but never felt anyone took the time to truly understand him or build a positive relationship with him. We understand this is a workplace, but for patients and families, these are life-changing moments. Compassion, patience, clear communication, and simply slowing down can make all the difference. Sadly, we did not experience the warmth and compassion we had come to expect from CPMC.

Their care for very old and very sick people, some left unresponsive after strokes was full of humanity. I spent at the Lawton Center 2 weeks and all my requests were met with such willingness and smile that I wanted to ask for something else just to see this expression of kindness and benevolence. I would easier trust my own mother to them than to myself, because they were really SKILLED. I asked my friend working in the medical field how such good work of personnel could be achieved? She answered that it could be only the result of responsible supervision of the attentive bosses. I met Lawton Center's Administrator Dan Daly and Director of Nursing Natasha Daragan, who WERE responsible for their crew's coordination and hard work. They themselves are watching everything like hawks and it shows. I appreciated the work of physical therapists who helped patients as a team and individually. The case worker Rudy also was very helpful in complicated and nerve-wrecking dealings with insurance. Absolutely fantastic was CNA Sheryl, who was not only superbly professional, but personally very kind. The society which cares so much for sick and elderly is a pinnacle of civilization.     FIRST HAND ADVICE: Lawton is a terrible facility. When I was here for 3 months, a CNA physically…read moreabused me several times, I complained about her a few times, later she was removed from working with me. My clothes were stolen from the closet and they had to reimburse me. The food is terrible and often, patients don't eat it l, lots of wasted food. . They are very short staffed on the weekends and night shifts. Many times, I pushed the button at night time for a CNA Nurse Assistant or Nurse and had to wait up to 30-45 minutes. The Nurses that I had were very good, and only a few CNA's Overall I don't recommend Lawton Skilled Nursing and Rehabilitation. Many people avoid Lawton, because they hear of the bad service that's provided bI don't know how they received good ratings.

I…read morewill tell you that I didn't experience any other this while my father was at the skilled nursing rehab side of the business. In fact, contrary to those reviews, the nursing staff was always prompt and professional. Special shout out to Dennis, the case manager. He was always up to date with my dad's status from the PTs and nursing staff, and exceptional as far as communication and follow up. The place was clean, nice facilities and while parking can be a bit pricey if you go there a few times a day, there is neighborhood street parking that isn't too hard to locate if you don't mind walking a few blocks.

Although they billed for 2 sessions. There were no clocks or calendars provided in rooms so it is very easy for patients to get confused. They only provided 1 dose of medication at discharge so we had to go through a lot of stress to get the dose for the next morning. The only medication my sister was taking was for seizures (very important) so we had to spend hours trying to get more meds (on a Friday night). To add insult to injury , my sister was discharged on Oct 3rd. We only received a phone call from Home Health on October 25th. Home health stated they received referral October 23rd. That's unacceptable!

The nurses and nurse aids helped me with everything. I couldn't go to the bathroom since both ankles had been broken. They took care of me. They have a wonderful pool that I used for physical therapy and occupational therapy. The farm has goats and I was able to visit it on a wheel chair and later on a walker to get around. The staff was from all over the world Indians, Americans, Chinese. It was as diverse of staff as the wonderful city of San Francisco. A true reflection of our great city diversity. I am forever grateful to each of them and this amazing institution.

It turned into a nightmare of months trying to figure out what they were going to do. So many things were made difficult through this administration including now being able to visit my mother. The Sheriffs Department almost always helps to make that as difficult a process as possible. They can't find you in the system, have you been here before, we've had IT issues, missing information.. ... we need to process you again, take your picture again.... Your picture doesn't match. They have "suggested" visiting hours for the hospital and wouldn't let me visit my mother and take her needed items on 04/05/25 at approximately 8:00 am., I had other things to do that day as well and my mother had swollen legs and feet, I bought her shoes to accommodate this issue because a couple days earlier a blood clot was found in her left leg after her Dr's appointment at SFGH..... I NEED THE SAN FRANCISCO SHERIFF'S DEPT, AND LAGUNA HONDA HOSPITAL TO GET IT TOGETHER.... STOP CAUSING ME UNNECESSARY GRIEF AND MAKING IT DIFFICULT FOR ME TO SEE MY MOTHER, IT'S NOT A JAIL THERE ARE NO PRISONERS AT LAGUNA HONDA HOSPITAL... It may be difficult for you to ADAPT...

I did, and after a few sessions Kira had improved my mental and physical state, through physical therapy and exercises she taught me to do at home. She's warm and sunny and will make you feel a hundred percent comfortable and relaxed, which happens to be the therapeutic goal here too. Kira actually doesn't claim to be a miracle worker, but she sort of is.

Between Tiffany and Lauren, they recommended exercises while undergoing physical therapy treatments. Over the course of one year, I reduced the mg and frequency of medication to zero. They are both compassionate, easygoing and patient. I asked a million questions!!!! With their assistance I was able to control my urine, eliminate the accidents and gain back control of my life. If you have issues, they have answers!

    Now that my mom has completed her three-month stay at Pacific Heights Transitional Care Center, I…read morewanted to share how things went and how grateful I am for the support she received here. She has been home for about one month now. When she arrived, she was still recovering from an incredibly serious medical journey, including months without part of her skull after her emergency craniectomy. By that point, caring for her at home had become unsafe for her and unsustainable for us. She had complete left-side hemiplegia, and even getting her to outpatient therapy required us to carry her down the stairs in her wheelchair. Around six months post-stroke, a home health PT even told us he didn't believe she would walk again. Shortly before her admission, my mom began showing some motor return in her left hip. We recognized it at home and through her outpatient therapists, and we knew we needed a place where she could safely and consistently build on that progress. Pacific Heights Transitional Care Center provided exactly that structure. During her stay, the therapy team helped her strengthen that early movement and start relearning the fundamentals of walking: weight shifting, stepping, balance, and confidence. By the time she discharged, she went from total dependence to now walking at home with her walker and the support of one person. She can even navigate stairs with assistance, which once felt impossible for us. She's continuing to improve and is genuinely on her way toward doing these things more independently in the coming months. The staff also took the time to train my husband and me on how to safely support her walking and stair practice, which made a huge difference in her transition home. And throughout her stay, the nurses, CNAs, and therapists created a kind, consistent, and reassuring environment that helped her feel safe every day. A heartfelt thank-you to Jasmine from PT, Joanna from speech therapy, Miranda from OT, and nurses like Massami, as well as Diana and Kenny, who showed her real warmth and care throughout her stay. This facility helped stabilize my mom, build on the progress she had already begun, and set her up for the next stage of recovery. I remain very grateful for the support she received here.

They all failed to complete my healing and help me find my full mobility again. The last place even just dismissed me at the end, saying that was all that could be done and to just go get a cortisone shot. They were wrong. With the help of Jona, and also Tim, my shoulder is making a full recovery. It's taken over a year this time, but they never gave up on me and my healing. I am already able to be more active, and I will soon be able to participate in activities, like tennis, that I thought were never going to be possible for me again. And, the whole staff is kind and supportive, so that I truly enjoy my weekly visits.
