This is my second review that I have left for City View postacute in the same stay my first review was towards the beginning of my stay around 7/2025 because I was so impressed by the food service which I'm still very impressed. Today is my Birthday and discharge is nearing, never have I ever thought that I would have received so much love and have been taken care of so greatly that I'm worried about being released out into this dog eat dog world to have to learn how to live on my own again. I'm very grateful to have stayed in a place that is always welcoming & full of love and kindness. In this situation I've been very secure. They say that what you need to heal... Good food to eat in a safe place to get your mind right‍

If you have a loved one going to this place, be extra vigilant! A friend was sent to this facility to be monitored and recover from a multiple fracture on dominant hand.is oxygen dependent, Fall Risk, diabetic, sleep apnea requires a c pap etc. This facility is NOT review patient's Care Plan or basic information!! The aid/nurse took the patient OFF his oxygen, put him a wheel and went down the hall to get his weight. How long can you hold your breath? When family member called to inquire when patient was not connected to Cpap machine, aid/ nurse replied, we only have plugs for the bed, and oxygen. Patient could not sleep, until multiple inquires about the cpap machine. Patient spent one week and returned to the ER due negligence, oxygen Saturation, glucose were never monitored and No insulin given. I don't think the staff was unpleasant and yes I thought they were bus but one week of negligence is absurd . If you have no choice, be vigilant!

Dinner

Awful! I cried when I saw this place that I had to stay at. Absolute filth abounds.I was disgusted to see my bed did not have clean linens . The bathroom was like a dirty storage sp. poop (lots) on toilet seat , along with much soil. bathtub filled with various items, ie: old commodes and other equipment, pillows, and much more. ledge above sink filled with used plastic cups, old vases old toothbrushes and more. The food is near inedible. Last nights dinner was three mounds of different colored mushy stuff. Old, dried food clinging to bedside table. Meds either No delivered or not delivered in a timely manner. I could go on and on. If you're appalled by filth, unsanitary conditions, inept help, and negligent staff, this is NOT the place to come! I'm surprised the health department hasn't shut this place down.

I was left like this for almost an hour till EMT's were called.

Good therapists but lousy experience. Depressing cockroaches, filth, screaming patients because no nurses come.

I was transferred to Lawton Skilled Nursing and Rehabilitation Center from UCSF, and I was highly…read moreimpressed by precise coordination among the Center's staff, by professionalism of nurses and CNAs. Their care for very old and very sick people, some left unresponsive after strokes was full of humanity. I spent at the Lawton Center 2 weeks and all my requests were met with such willingness and smile that I wanted to ask for something else just to see this expression of kindness and benevolence. I would easier trust my own mother to them than to myself, because they were really SKILLED. I asked my friend working in the medical field how such good work of personnel could be achieved? She answered that it could be only the result of responsible supervision of the attentive bosses. I met Lawton Center's Administrator Dan Daly and Director of Nursing Natasha Daragan, who WERE responsible for their crew's coordination and hard work. They themselves are watching everything like hawks and it shows. I appreciated the work of physical therapists who helped patients as a team and individually. The case worker Rudy also was very helpful in complicated and nerve-wrecking dealings with insurance. Absolutely fantastic was CNA Sheryl, who was not only superbly professional, but personally very kind. The society which cares so much for sick and elderly is a pinnacle of civilization. Thanks God that America still has this island of humanity The only bad side is that food is too good for the people who want to lose weight. Tatiana Menaker

I rarely leave negative reviews, but this one matters…read more I was referred to Lawton Nursing & Rehab after a major surgery and confirmed with both Cigna and UCSF that they were in-network. Lawton's staff flat-out refused to even check or speak to the insurer - instead, they rudely denied care and hung up on attempts to clarify coverage. When you're recovering from a serious procedure, this kind of treatment is not only disheartening - it's dangerous. Patients deserve empathy and basic professionalism. Lawton failed on both counts.

The short version: Lidys at Zen Concierge Nursing is wonderful. I recommend her services…read morewholeheartedly! You might think that having your spouse or your sister around is enough, but post-op support is a tough job! If you make arrangements with Lidys, she will provide the level of comfort and care you need, so you can relax and really heal. The long version: My original plan was to have a TT and lipo, and have my adult daughter stay with me for a few days to help. When I read more, and watched more videos by people who had gone through it, about just how uncomfortable/painful and difficult recovery was going to be, I began to doubt my plan. Now that I am 12 days post-op, I can say for sure that working with Zen Concierge Nursing was the best decision I made. I met with Lidys before the surgery, and she advised me on what products to have ready at home. She and her driver picked me up after the operation and I stayed with her for two nights. She (or her fellow nurse Patricia) was with me every step of the way, helping me eat and sleep and stand and walk and use the bathroom. Lidys kept track of my meds, and called the surgeon's office to consult about my condition. She did everything a post-op nurse does, plus talked to me to distract me or cheer me up. Lidys gave me massages, and made me soup and ordered food for me ... she just did everything to help me relax and let my body heal. When it was time for me to go home, she came with me to make sure my home "recovery station" was all set up, and then texted to check on me every day. Post-op lymphatic drainage massages were recommended by my surgeon, and Lidys did those for me, too. Seriously, her professionalism, experience, care, and reassurance have really helped me so much! I highly, highly recommend her!

I went to two different massage therapists before finding Lidys. It is one thing going to a LDM specialist and another to go to someone with a nursing background and understands what your body goes through during the recovery process. I had fibrosis growing in my back and the two specialists I saw before Lidys did not mention it or try to address the problem at all. Thankfully I found her and she was able to get out my fibrosis lumps significantly in one session and completely in the second. Don't get scammed by these other lymphatic specialist that demand you buy 6-10 treatment sessions. She was honest and told me I only needed two treatments which was true. She also recommends stretches, garments and foams that made such a huge difference in my recovery. Thank you Lidys! And to anyone looking for after care massages after surgery there is no one in San Francisco better than Lidys.

This review is for the Dietary Manager named Joanna Sucgang who works at Central Gardens where my…read morefather is a live-in resident there. I wanted to write a review to share how pleased and grateful I am for the dietary care he is receiving. My dad is a strict vegetarian with a very particular diet that he has been on his entire life and I was not sure if he would be able to sustain it after being moved into Central Gardens. So I made a special request on his behalf to make sure he got some of the dietary items he could eat and OMG, Johanna was an angel. She is who I spoke to about my dad's dietary needs and it's been almost a year now and she has steadily gone above and beyond to make sure my dad was taken care of. She goes ABOVE AND BEYOND to make his stay as comfortable and healthy as possible. Her attitude is so pleasant and joyful and that makes us feel good under the circumstances of having to be confined to a bed. How patients are treated is so important and we are filled with gratitude about their willingness to accommodate his dietary restrictions. We have many options for his nursing home care but I am keeping him put there because of caregivers like Joanna. I thank God every day for her because it makes all the difference in a patient's life. Overall, I have had pretty good experiences with most of the staff and nurses there and me and my dad are happy with his care. But I wanted to do a special review regarding the dietary person because that is crucial to a satisfactory stay at a nursing home. Thank you Johanna Sucgang for your concern and care for my father Ron. Warmly, Kelly

Nora Im I have had…read morethe privilege of working with Nora my social worker at Central Gardens for the past five years, and I can confidently say that she is an exceptional professional. Her work ethic, compassion, and genuine dedication to her patients set a standard that is rare to find. She consistently goes above and beyond her duties. Whether it is advocating for patient needs, supporting families, or checking in on individuals even after they have been discharged to ensure their safety and comfort. Her follow through shows how deeply she cares about the people she serves. She also has a remarkable ability to connect with patients, including those who may be more challenging to reach. Her patience, empathy, and calm presence create an environment where people feel heard, supported, and respected. It truly is a gift, and one that reflects the heart of what every social worker should possess. Central Gardens is fortunate to have someone of her caliber, and I am grateful for the years I have been able to work alongside her. I highly recommend her to anyone seeking compassionate, committed, and truly outstanding care.
