No private rooms, three people to a room, beds separated by a thin curtain, patients screaming all night, waited hours after pressing "nursing call button". Now that my mom has completed her three-month stay at Pacific Heights Transitional Care Center, I wanted to share how things went and how grateful I am for the support she received here. She has been home for about one month now. When she arrived, she was still recovering from an incredibly serious medical journey, including months without part of her skull after her emergency craniectomy. By that point, caring for her at home had become unsafe for her and unsustainable for us. She had complete left-side hemiplegia, and even getting her to outpatient therapy required us to carry her down the stairs in her wheelchair. Around six months post-stroke, a home health PT even told us he didn't believe she would walk again. Shortly before her admission, my mom began showing some motor return in her left hip. We recognized it at home and through her outpatient therapists, and we knew we needed a place where she could safely and consistently build on that progress. Pacific Heights Transitional Care Center provided exactly that structure. During her stay, the therapy team helped her strengthen that early movement and start relearning the fundamentals of walking: weight shifting, stepping, balance, and confidence. By the time she discharged, she went from total dependence to now walking at home with her walker and the support of one person. She can even navigate stairs with assistance, which once felt impossible for us. She's continuing to improve and is genuinely on her way toward doing these things more independently in the coming months. The staff also took the time to train my husband and me on how to safely support her walking and stair practice, which made a huge difference in her transition home. And throughout her stay, the nurses, CNAs, and therapists created a kind, consistent, and reassuring environment that helped her feel safe every day. A heartfelt thank-you to Jasmine from PT, Joanna from speech therapy, Miranda from OT, and nurses like Massami, as well as Diana and Kenny, who showed her real warmth and care throughout her stay. My grandma got discharged just this week. She wasn't happy about food and always called me saying "food isn't suitable for her" she doesn't speak English well but then one of the CNA from second floor speaks Mandarin and helped her order Burger and salad from kitchen. She been eating Burger and salad for days and she loved it. I didn't get her name who helped my grandmom but thank you very much to all the staff at the facility. It's a great care that she got from all the staff, now that she is mostly independent again at home. read more

I just spent 2 weeks here an these are my reviews by department, 1 the medical staff doctors nurses and especially the therapists 5 stars***** thank you 2 food even more bland than the hospital when I asked for condiments I was told they only have salt and pepper. the dietician came up the day before I was discharged and I spoke to her about that situation she said that's not true and brought me 2 cups of orange juice. she could not imagine why I was lied to for 2 weeks 2stars** barely 3 laundry services due to an accident I had to send a robe and pair of sweats to the ldry. they never came back. luckily my robe was spotted by my occupational therapist and returned to me still dirty an minus the belt. the sweats brand new and very comfortable disappeared forever. what good does it do to put your name and roomnumber on things. I repeatedly asked to speak to someone in management the never came up to talk to Mr about this situation. laundry 0 stars for incompetent staff management refusing to talk to me and no satisfaction on the missing clothes 00000 one last comment 3 sick people in one room bring earplugs or noise cancelling headset to get any piece again a hurrah to yje medical professionals thank you for your wonderful care John garrett read more

My 91 year old cousin was a patient in July and discharged in approximately 3 weeks and needed medical equipment upon her transfer to an assisted living facility. A completed order form was submitted by Pacific Heights to the medical supplies company. Her original discharge date was delayed which cost us a daily out of pocket payment of over $400 for multiple days. This is due to the medical supply company phone calls to me regarding additional information supposedly needed by them. In the end, the medical supplies company told me that they are 'not in network' with my cousin's insurance'. Shirley Sarmiento, social worker at Pacifica Heights stepped in and ordered a rental ($647) using her own personal credit card . This action was risky, posed the possibility of no reimbursement from the patient and was above and beyond her responsibility. The staff that we met, Cherie, nurse manager, the Physical Therapist and others were professional in the way they conduct their jobs. Later it was confirmed that the Medical Supplies Company, Apria, is indeed 'in Network ' and the needed equipment was finally delivered on August 14. read more

As soon as you walk in you will be greeted with a smiling receptionist and staff seems very happy, everyone greeted you with a smile from the moment you walked in. The care at this facility is exceptional. Their therapy department and nurses are very educated and knowledgeable A++++++. Their social worker is very helpful and uses her resources well. I will not hesitate to recommend this place to anyone needing short term rehab. read more

Unfortunately their facility is unorganized and a few of the nurses are just plain unprofessional and unprepared. They took over 10 minutes to just START the shower for my grandmother because they couldnt get the water temperature or pressure right and because of the difficulties they stood around and took an additional 10 minutes to decide who else would help. I had to step in and provide the shower myself. The next morning there was miscommunication left and right from her morning and night shift nurses and social workers. They didnt even call our family to let us know about the lack of transportation to her morning appointment, instead they decided they would call the appointment place to let them know they had no transport and would just wait and see. In actuality the transport was already set up and was on time. When the nurse called a family to update us she hung up on this family member 3xs because she wasn't reaching the intended person (all family contacts were listed as being approved to share patient info so it was not against HIPAA). Pacific Heights care is a good place for rehabilitation. The receptionist was so wonderful and kind when you entered. This facility was clean and safe. Thanks to all staffs for their kindness and compassion. All the nurse took a good care of my mom. All of CNA worked hard, patiently with their compassion to help out all patients needed it. The social worker was amazing and helpful our needs. Administration staff was nice and kind. Hoping the facility has interpreter hotline for patient who can't speak English like hospital for communication. read more

I have been a patient here for well over two years & have never been through such a horrific and desperate situation in my life. Starting with the CNA's. There are several good CNA's including Mary Lou, James, and Mike. It's rare I have Adelaide but, she's always good with answering call buttons. On the weekends, there are even more registry employees and this includes nurses. Some are excellent; usually the "traveling CNA's some of whom are truly caring people who loveThis facility is seriously understaffed. I feel for the regular staff who I'm sure are seriously underpaid and overworked which means the patients don't get the kind of care they should. They are so short handed that it's the norm to end up with "registry" CNA's which means temporary employees for one or sometimes double shifts. They also have what are referred to as "floaters" meaning those who have two different floors. If they're on another floor, good luck if you need help! read more

A lot of dishonesty here, terrible and inappropriate food, and huge mistakes in dispensing medicines that the patient caught (thankfully!). Doctor actively refuses to talk with patient, her excuse is "I've already read your notes!!!" What if, perhaps, the notes are horribly wrong (as they now are proven to have been) and that's why the patient requested the doctor's help? I pray to god nobody else I know ends up dumped in this facility and seeking competent help with a critical medical issue. Easily they could kill you with overall incompetence. 4 BEDS to a small room, highly noisy and unsanitary, was told this is actually a dumping place for unlucky Medicare patients (though the Patient has private insurance). Also told this is the BEST Kindred location, oh lord! 24/7 the sound of "AYAAAAAAH!!!" echoes through the building. That's apparently Mandarin for "I'm in severe pain, oh my god!!!!" Your heart just goes out to anyone who gets dumped here by incompetent doctors and/or the collaborating insurers. They promise rehabilitation to your face and then spend days blowing you off. They will be leaving you in pain to possibly die from any misdiagnosis, it's like a Roach Motel- they check you in but good luck getting out alive. Oh, and they have no shame and will charge you amply for documentation of their cluster**** care. If this is what we do with our elders and disabled, time to stock up on cyanide. read more

Great location with easy parking and friendly, caring, and well-trained staff. My 91-year old mom was there for six weeks with a fracture + mobility issues and had excellent care. The food is an area for improvement, so I supplemented it with bringing in some outside bites when visiting. read more

I received superior occupational therapy from Marissa Quinones and equally fine physical therapy from Kayleen Rodriguez that was superb in every respect. I was there for over six weeks while recovering from a hip replacement. These two, whether working individually or as a high functioning team, led me from completely unable walk to balancing and walking with relative ease. Other members of the staff were of similar high quality. It's the patient work of professionals like these two that make for successful recovery. read more

I want to say Thank You to all the wonderful staff that cared for my mom while she was there as a patient. The facility was clean and there were no smells or odors throughout her floor (which isn't always the case at some facilities). The case manager, Hannah, was very helpful informing me about what to expect and updating me frequently how my mom was doing. Shirley, in social services, was super helpful arranging at home care to come to my house, after she was discharged. I was so thankful for this as it relieved a lot of stress and anxiety I had about help after leaving the facility. The therapy team was very professional, kind and thorough. The improvements I saw in my mom's mobility was truly amazing. Lastly, a big heartfelt Thank You to all her nurses and nursing assistants for their prompt attention to her daily needs. Ryan, the chief coordinator, has assembled a top notch group of therapists who have so much knowledge of their skills, along with compassion and kindness. Do you or your loved one a favor and look elsewhere. The experience here with my grandmother was out right horrendous! This place did not follow doctor or hospital orders at all. The first day she was admitted they had none of her medications available my grandmother had a major surgery and was on several pain meds and a dietetic. When I called about the medicine the staff was rude and unhelpful told me it might be 2am before it gets there. After all of the non care this place provided she ended up with a serious infection and is not back in the hospital. This place should be down right ashamed of the care they provide. read more

I was a patient at this facility for rehabilitation after a below the knee amputation & wound care October - December 2023. I would like to commend all the staff for such great care and service. Jemma Zosa was the best CNA ever. She should receive an award. Alan Arcella in the Finance Department went beyond the call of duty to help me. Ryan and the physician therapy department are top-notch. Loren Ocampo, R.N., wound care nurse was exceptional. The whole 5th floor staff on all three shifts were phenomenal. Thank you all for not just doing your jobs but going beyond the call of duty and making my stay and recovery great. I give them 5 stars. I would recommend this facility. The rehabilitation for geriatric patients here has 0 success, they will suck up your insurance and have you discharged with their progress reports. Don't bother dealing with any of the case mangers, all passive aggressive. Horrible experience here, the whole family has been left in tears & shuffling for our loved ones placement. The orthopedic surgeon has thankfully given us hope that with a different therapy facility our loved one will walk again. read more

My Aunt B. Caramucci spent 2.5 years at Pacific Heights Transitional Care Center. During her time there the staff was caring, communicative and would inform me of all incidents that needed my attention regarding her needs. My aunt was secure and safe during her time at Pacific Heights Transitional Care Center. It was a God send to my family that our loved family member was in such good hands. The administrative staff was also helpful with my Aunt's case in helping her afford long term care at this facility. At this place some people were friendly some were not they are short handed mabey that's why my parent had to go to this place for a month the day she left to go home everyone seemed very rude they were putting her out so fast like they wanted to get rid of her the people working there are there to help people. Downgrading my review (from 2 stars to 1 star) after being haphazardly discharged without important required meds. The discharge staff told me (verbally and in-writing) that the RX's were prescribed and ready, yet this was not true. The on-call doctor refused to send the necessary (3) Rx after we left, "can't do anything till Monday",more excuses and no problem resolution, just more finger-pointing. Truly horrifying experience. --- Please look elsewhere before trusting this place with the care of your loved one. While the PT and OT staff were very good (and the reason for our stay), it didn't make up for the mismanagement of (what seemed to be) every other aspect of this facility. I spent countless (wasted) hours correcting their mistakes and escalating problems that one would think would be expertly handled in a skilled nursing facility. I sincerely hope we can all expect (and find) better places for our loved ones to rehabilitate after a traumatic medical situation. The jargon and lip service (and brochures/phone-hold messaging about the "joyous" experience) are sadly laughable. Their staff had no idea who was responsible for what, nor where to find or call the people who were needed. The activities listed in the calendar were happening only 25% of the time, and when they did happen, most staff had no idea Where; and not enough staff to escort as promised. The only other + I can give is that it is kept clean, and we were thankful for that. I would give PT/OT 5 stars, and 1 star for the rest of the facility = 2 in total. Keep looking! read more

Where is the diversity ? The facility is in the heart of San Francisco with a mixture of residents but the staff is mainly white and Asian ! How do you house the Hispanic and Black community but have not even a handful of that race working there! Not very welcoming and raises many red flags about management. We are a melting pot and residents other than white and Asian need to also feel a connection to staff that look like them and can communicate culturally. read more
