They didn't follow my doctor's orders. They ignored a piece of safety equipment that was important. I am visiting with attorneys to rectify additional surgery and time caused by this intentional gaffe. I wouldn't send my worst enemy there!! read more

I was recently discharged from my first experience at any rehabilitation center. I really did not know what to expect, but I was pleasantly surprised with the facilities and the staff. I hope I never have to go back again, but if I was in need of their services, Old Mill is the first place I would go. read more

My husband was admitted to UNMC ER with a virus on 4/13/25 and when he was ready to come home he was unable to walk due to the weakness from the virus so he was admitted to Old Mill Rehabilitation on 4/18/25. The physical therapy was great and actually got him walking enough to come home on 5/9/25. Unfortunately I can't say that about the CNAs, doctors and nurses. 4 days before he was to be released to come home the Doctor who only comes once a week found a baseball sized abcess on his bottom. He had never had bed sores before so I don't know how the CNAs and nurses taking care of him for the last month did not notice this abscess before it got to be that big. The Dr said she tried to drain it and started him on an antibiotic and would see him next Monday. Fortunately he was released on Thursday and came home Friday morning May 9. Also fortunately we had a home health social worker come in to set up his treatments at home and she said he needed to be rushed to the ER immediately. The Drs in the ER scheduled immediate surgery. The abscess had gone through his groin and across his entire stomach and was now a flesh eating bacteria. It took several surgeries to clean out the bacteria. He spent 10 days in the hospital and was then moved to a skilled nursing facility where the were supposed to have "skilled nurses" to take care of the surgical area. he was moved to Hillcrest rehabilitation in Bellevue NE. His care was less than acceptable at Hillcrest where he eventually got pneumonia and a blood clot in his lung from their lack of care. He had been coughing all day every day for weeks and I had asked that they give him something to help it - like an antibiotic or breathing treatments which they didn't. When we finally got him home he was only home for 4 days and he had to be admitted to the hospital again where he died on 8/8/25. If Old Mill had sent him to the ER when they found the "baseball" sized abscess all of this could have been prevented. Old Mill has great physical therapists but that's all they have. I don't recommend either Old Mil or Hillcrest for care for your loved ones. If you can set up care in your home - that's what .i would recommend. I wish I had known it was available to us and it would have probably saved my husbands life. read more

Disappointing. That would be my one word review. Old Mill Rehab came highly recommended by the staff at the Med Center when my mother's top two choices were unavailable. The issues we dealt with over the course of her three week stay (be prepared for a 20 day stay, as that is what Medicare will cover and they make sure to have concerns up until the 20 day limit) included: 1. lost paperwork from the hospital 2. rehab equipment that was not wiped down 3. a night nurse who passed out in her room 4. an unwillingness to connect and disconnect the ice cuff for the knee she was rehabbing 5. A missed meal 6. missed dressing changes for knee incision 7. neglected doctor's orders The staff was friendly and the facility is bright and well decorated. The common areas are very nice but the furnishings in the rooms are scratched and showing a lot of wear. Travel was provided to and from doctor visits, although it was 15 minutes late to her appointment. Old Mill Rehab would not be on my list for a family member, especially if you are not in the Omaha area to advocate for your patient. One word, disappointing. read more

I stayed there after surgery. I found the daytime staff either incompetent or having a really bad attitude. The way they handled me with the wheelchair caused a hematoma and a second surgery. I came back after the second surgery and the bed was made with the bloody sheets still on it! The care taker on the night shift was wonderful. I wonder how they kept her there. Now a couple of months later I have scabies. The only place I could have gotten it was there. read more

I couldn't get my mother out of there fast enough! They left her in a wheelchair for 8 hours her first day there. Things did not get better they made her wait for over a half hour most times when she would call for assistance to use the bathroom resulting in her sitting in soiled clothing. The food they served was processed crap that made her ill most days plus full of so much sodium they had to start her on water pills to remove the retained water. We would bring her food, flowers, miscellaneous gifts we were lucky if she got them within a few days. The staff was not very kind and should not be in the line of work they are in. After reading other reviews I don't know how this place is still open caring for families most precious members. It was heartbreaking my mother had to experience this! ZERO STARS! read more

Both my grandma and grandpa have stayed here. Overall this place seems nice and the staff have been friendly and helpful. I'm told the food is good and they give you plenty. I know no one wants to go here but if you have to it sounds like a nice place to be for awhile. read more

Bottom of the barrel facility that is mismanaged and does not pay enough to attract competent staff. They play the pass the buck blame game when things go wrong. Things constantly go wrong. Dirty dishes in the cafeteria. Pain meds go missing. Draw your own conclusions. If there was a true emergency I do not trust that they would respond appropriately. There are real problems at this facility. You have been warned. read more

I took my grandma to old mill after she was hospitalized for breaking her hip. The staff treated her so well she did not want to leave. The facility was clean. The care she received helped here to quickly get on her path to recovery. I would recommend this place to anyone. Obviously, no one wants to find themselves in this situation, but if you do, this is a good place. P.S. the food is great! read more

My mother is there right now, and the place, except for the physical therapy, which is extraordinary, is the pits. The building, when you enter and walk around, is beautiful. The rooms are large and have plenty of chairs to use. There are many private rooms scattered around that allow families to relax, visit and let kids be kids. Having said that, the nursing care is very, very poor. The nursing staff sits at the far end of the hall and does, well, I don't know. My mother has pushed the call button many times, and it has taken almost a half-hour to get a response at most times. (Makes trying to go to the bathroom a challenge!) Granted, my mother isn't the nicest of people on her best day, but rehab nurses should be used to grumpy old people, or perhaps they should go into another line of work. It appears that the nurses, aides, and other support staff are temp hires. Perhaps some permanent nurses that have an investment in their jobs would be a great idea. So, for physical therapy, this place is terrific! The nursing staff is a big no, at least for me and my family. read more

My mother in law was put in quarantine for two weeks upon them accepting her for physical therapy. She is fully vaccinated for Covid-19, and the two week quarantine will prevent the therapy she was admitted to receive. When the quarantine order was questioned, "It's standard procedure" was the response. She was moved home immediately. read more

So far, I am not pleased with this facility. Finally after day 5, they showered my mom. Five days!! Yesterday, we overheard the aide outside of her room complaining about my mom's weight (she has had cancer and takes meds whose side effect is weight gain). Communication is poor, phones are not answered, and clearly the staff has ussues. I will be attempting to speak to the administration on Monday. No one should be treated this way. I am truly disappointed. read more

If I could give this place zero stars, I would! My mom came here because she was recovering from surgery. We chose it because it was new and because it's not a nursing home, only rehab. When she checked in, they talked a good talk about the food and the staff and the opportunities for residents, like daily bathing. During the four days she was there, no one ever offered to help her take a shower, so my sister and I had to do it. They didn't change her bedding or empty the garbage. The food was bland, despite their claim of gourmet food. During my mom's stay, she developed a fever. The staff would not take us seriously not call their on call physician until we insisted. She did had a bladder infection, which they never would've found unless we had been there. Her hospital discharge instructions, which they went over when we checked her into Old Mill, states that they needed to check her incision site twice a day, yet they never checked it once in the four days she was there. My sister and I stayed with mom almost around the clock, so I can say that we are pretty good judges of their neglect of my mom. After four days, we informed them that we were checking out my mom, and they turned very rude. They refused to give her outpatient PT orders or exercises to do at home. Her surgeon told her that she would only need to stay there a few days until she got over the "hump." Yet they wanted to keep her as long as possible to make as much money as possible from Medicare. It is obvious that they are only interested in making a profit, and they have bare bones staff for this reason. read more

I am extremely disappointed in this facility and just want others to know the experience my family has been through. My father had a stroke and was staying here for regular PT, OT and ST, as well as ongoing care due to his incapacitated right side. Most of the people are kind and on the initial walkthrough, everything appeared fine. However, he has now fallen 3 times under a CNA's care, has been to the ER twice due to the falls (falls caused more swelling. He is a stroke patient - head trauma could be fatal). The nursing staff does not always dot 'i's and cross 't's - they do the very basic. Give meds, talk to patients, do charts. The CNAs don't seem aware of how to handle stroke patients and my family is almost terrified to leave him alone there given his already serious falls. And on top of it all, they have never once apologized, shown concern, or followed-up. It's not conducive to good rehab care. read more

They were so attentive to my needs. They would refill my ice water before I even had to ask at times. They made sure that I kept up on my pain meds. Majority of the time they would answer my call light in a matter of a few minutes or less. Sometimes they were like ninjas and I wouldn't even know when they came in my room and refilled my ice for my leg. Lol. There were times that as soon as I hit the call button they would be in the room with whatever it was that I was going to ask for aftwr knowing my routine. Every person truly cared about my needs. I broke my right leg and my left ankle was sprained, so physical and occupational therapists had to get creative on my treatment. They pushed me, but with care and concern and they made sure to not do so beyond my limits. I couldn't have asked for better treatment. My range of motion doubled and tripled and I left at about 2 degrees from normal range in my right knee and about 3 degrees in my ankle. Granted I am in my early 30s, so age was on my side. I liked that visitors could come past visiting hours, but the doors are locked so they just had to ring the bell and look at the camera. And I was allowed to come and go as I pleased (within reason) as long as I was with someone, but just had to let staff know when I was leaving and coming back. And it had to be as long as it didn't interfere with therapy. Food was decent, and I loved that I was able to have a glass of wine or a beer with dinner. Since they didn't serve Diet Dr. Pepper, they even sent someone to the store to buy me a 12 pack to keep in my room. Occupational therapists came to evaluate my home and made sure I could handle everything needed at home for my release. I couldn't have asked for a better experience. And although I am glad to be back home, I am going to miss the people who helped me... I may have to go back and visit, just to say hi. A lot of the individual care givers and therapist's philosophy was they treated everyone how they wanted their mother or themselves to be treated. In conversation with my occupational therapist, she indicated how sometimes she had to advocate on behalf of the patient for care to the insurance companies. And that says alot. Instead of just accepting a denial, they would fight for what the patient needs. I recommend them with no hesitation. read more

My wife was there for several months for physical therapy. The therapist were all very professional and CNA's seemed to take a personal interest in her care. The meals were hot and good, even sampled several impressed on the quality and quantity. Commented could have used some salt but that is part of health care, reduce salt intake. Staff friendly and helpful. Made sure she received any medication when she was to get it and made sure she was taken to any doctor appointments. read more

Old Mill Speech Therapist Killed my mother!!!!!!!! She is not worth the title !!!!!! I hope she goes to He!! She should be charged for Medicare fraud !!!!!! She charged Medicare for 45 minutes a session and never I mean never spent 45 minutes with my Mother!!! Besides that she never EVER even tried to help my Mother!!!! She just put her on some on some STUPID diet and THIS IDIOT THERAPIST was told over and over along with the rest of the STAFF My Mother is NOT DRINKING OR EATING!! My Mother gave everything she had in Physical Therapy and more!!!! Instead she lost 27 pounds in less than 3 weeks!!! Everything she worked so hard for down the drain!!! She ended up with an IV for fluids and a feeding tube for food !!! I am begging everyone PLEASE NEVER take your Loved one to this place!!!! They LIE with everything they say!!! In the end they should never have accepted my Mother!!! They have people right off the street working there!! The couldn't give you a Tylenol correctly!!! Of course you have to give them ONE STAR just to write a review!! They had a 10 page "contract", staff could not explain rules, a "nurse" came into my room and said "you need to take off your clothes so that I can see all your skin and make a report to the doctor". When I refused, he became angry. Not 10 minutes later there was a knock at my door. Before I could say anything another man came sauntering in to write a schedule on my wall. I found another nurse and told her I was leaving. She said, "You cant." When my husband came back we walked out. I called the doctor who said"I dont want you to stay somewhere you dont feel safe". The day nurses are good . Night staff has left my dad in his wheelchair until 11:30 at night when asked to go to bed .They don't want him to get out of bed or bathroom without help but never answer the call light .I don't think his room has been vacuumed for a week .I have found pills on the floor that got spilled .Bed broke on Saturday had nails sticking out of headboard reported it I put little straws over the nails (they are still laying on the floor) They are supposed to have lift chairs was told they have 4 we have asked for one the whole time 4 different people no chair .Went up to have dinner with dad I called and let them know 2 of us would have dinner . When we got there the young lady that brought out the meals got smart and said we just can't show up we are suppose to let them know (we did 2 hours ahead of time ) Now their P T and O T dept is good but the rest of the time he sits in his wheelchair Its like no communication between staff . (See previous review below for more information.) Lucky us! My husband had another orthopedic procedure which required a three week rehabilitative stay. We, of course, chose Old Mill again. When we arrived it was like a family reunion. There are few new staff members (that's a good sign to me--they are careful who they hire and the people who work there are happy, even though the standards and expectations are very high), so nearly everyone who greeted us remembered us and greeted us with hugs, laughter, and memories of my husband's previous stay last November. What a welcome! One interesting point was the physical therapists knew my husband's level of motivation (high) and worked him as hard as the surgeon would allow. The head of occupational therapy was new and didn't know hubby's history. It took her a while to realize how motivated my husband was and eventually shared her pride in his work ethic with him. The nursing staff is outstanding, truly. They bear the burden of the work. What professionals! The maintenance folks, nutritionists and their staff, even housekeeping were always friendly, kind, helpful, and went out of their way to honor any request. (Once I saw the head administrator, Dave, vacuuming. I commented that it was the first time I had seen a head administrator of any organization do housekeeping. He said if he saw something that needed attention and he had the time to do it, he did it. I mentioned this to several staff members and they concurred that Dave was as good as administrators get. It starts at the top, right?) Everyone was sorry to see my husband go, great progress was made, and as much as we think this is the best rehabilitation center in the Omaha area, we hope only to visit and not stay again! When I checked in, they wanted my medical power of attorney. I told them to call the hospital as I had given it to them. I found out that the hospital had forwarded it to them in the papers that came with me. Later they called my medical power of attorney and told her that she had to bring the document to them. How did they know her name if they did not have it? They told me that I had to have a TB test when I checked in. I told them that I was current on TB testing, to check with Immanuel Hospital where I had it done. Later they came to give me the TB test, but I would not allow it. The mattress had a six-inch rim around it. I was having trouble getting my leg in bed after hip surgery, now I had to lift it six more inches. The mattress was so soft, I woke up after 2 hours with a very sore back. I called for someone to help me onto my side. The aide started pushing on me. I had to instruct her on the proper procedure. The toilet paper was mounted about 8-20 inches from the floor. I could not bend over, so they did move the toilet paper on top of the grab bar. The physical therapist gave me one session of therapy. She had me doing one maneuver, which I found out later, was not allowed by my doctor until later. The instructions were included in the paper work that came with me. The food was bland. In particular, the Brussels sprouts were so overcooked, there could not have been any nutrients left in them. I knew that I could not recuperate here if I could not sleep, so I practiced getting in and out of bed all night and called my friends to pick me up and take me home. I signed a paper saying that I was leaving against doctor's orders, and then they totally ignored me, even though I should not have been allowed to walk unattended. When I checked in, I asked if they would use my own medication (the hospital had) and they said yes. When I was getting ready to go, I asked for my medication, and they said that I could not have them. After some strong language, she produced them. When I got the bill, they charged me over $200 for medication. I reported it to Medicare as fraud, but they got away with it because Medicare treats it as one charge instead of individual items. read more
