WILLING to participate and follow through after treatment? Do they have the support system in place after discharge? Do they WANT to be there? Forced? Court ordered? Quite frankly, I had a terrible experience in Portland. It appeared upper level staff didn't have a clue what is/ was going on. I have NEVER heard the 'F' word so many times in my life!!! That is not part of my vocabulary, and certainly don't want to hear that all day long. Techs, bless them. Seems like at times they are struggling as much as we are. Went to several meetings where we weren't supposed to be. Embarrassing! The house needs some updates, which I pointed out when I was there. Carpet going up the stairs and hallway being one. Why have carpet in a heavy traffic area? Or replace with new indoor/outdoor carpet? Cleaning it at this point is 'pointless' i could go on and on.....,,,, If you have any questions 503-502-9143

THEN it can take another hour (if ever) for them to round up the "correct" personnel to assist, whether it's incontinence care, a sip of water, or help getting out of bed. Miscommunication between staff is routine so the care plan isn't consistent. The roommate of my loved one spread feces all over the toilet, walls and floor; it took three DAYS for staff to clean it up. This facility is a nightmare. That being said, there were two nurses and a physician's assistant who were wonderful. Overall, this is not a place to get better.

I wasn't crazy about the food there, although from time to time there was some good food. Not very often but I don't think that is a huge downfall for the skilled nursing facility. You can always order food in, right? I loved the LPNs that were there, particularly Kayla and Bogale, who always treated me with kindness and a professionalism that was beyond great. THIS IS NOT THE PLACE TO SEND A LOVED ONE IF THEY NEED WOUND CARE OR OSTOMY CARE! I toured the facility before moving my grandma there, and they assured me that they had adequate staffing, great wound care, and were knowledgeable about ileostomies. This turned out to be completely false! I had to come and care for her many times due to their incompetence when it came to adhering the ostomy bag. I also had to purchase several medical supplies needed for it because they said that they could not get them. There is no excuse for this! It is all covered and supplied through her insurance. They would not regularly empty the bag as required, and it would constantly leak. Because they were doing it incorrectly, they were changing it up to 5 times a day, which is unheard of! An ostomy bag holds, on average, for 3 to 5 days when it is done correctly and emptied on schedule. Her wound stayed infected the entire time she was there and grew in size. It became so bad that when we left the facility, her doctor had to make it larger and start a wound vac. One staff member even swatted at my grandma's hand when they were changing her ostomy, and my grandma wanted to help. They were accusing my grandma of "fiddling with it" and causing it to leak. I really should have reported this facility to the state, but I did not have the time or energy. I hope our experience will help others find a better place for their loved one, and that this facility will take a good, long look at what is going on with the care they provide and make changes. I hope they will be honest with themselves and others when they consider accepting patients who need this type of care. I will commend the therapy staff. The only thing this place has going for them is great physical, occupational, and speech therapy. So if that is all you need, it might be an okay fit for you, but be aware of the nursing staff.

    My 93 yo mother who is on hospice with kidney failure fell on Aug 15th and broke her hip. We sent…read moreher to Mira Vista while we got her own home set up with hospital bed. Due to her advanced age and renal failure, she did not have surgery. Hospice told Mira Vista that she was bed bound and that she was supposed to receive pain meds on demand. This morning, Mira Vista personnel pulled her out of bed, put her in a shower chair and gave her a shower. They did not remove her hearing aids first. She was in agony. When it was finished, she asked for pain medicine and was denied, and was told she had to wait until 12 noon (it was currently 7 AM) for her next pill. She was traumatized from pain, shaking from shock, becoming nauseated from the level of pain she was in, and the only way we got her help was to get Hospice to fix Mira Vista's misunderstandings. Not only that, but one of her very expensive hearing aids came out when they washed her hair and is lost! The level of incompetence and lack of empathy is stunning. Thankfully Hospice helped us expedite getting her home set up. She left Mira Vista this afternoon, but she is still traumatized and we are trying to get her stabilized now that she is home. Do NOT bring your loved ones here!

Then there are the few that I question why they work there at all. And if you ever get saddled with those ones I'm so very sorry. It's a cute spot, all in all staff is friendly and pretty efficient. With of course the few bad seeds that make other peoples jobs harder than they already are. I hope things have gotten better since I last worked there years ago. Even with a loved one of my own being a patient I'm sad to see there's still much room for improvement here. I will not be admitting my relatives here ever again. This isn't the first time I've had a bad experience not just as an employee but as a family member to a patient. Those few bad apples really do have more of an impact than i wish they did on the many wonderful people who work there. But the inconsistency that really doesn't make me feel like i can trust this place anymore, and it makes me sad to be honest. Wishing them the best, and i hope no one has to experience this and i truly hope they get the turn around this place needs. Especially for the amazing people there who deserve it.
