I visited him three to four times a week, and each time, I found him almost unresponsive, I could not wake him up, which was extremely unusual for him. The wound care he received was the exact opposite of what his doctor had instructed, which was alarming. Additionally, despite his inability to walk, the staff's approach was disturbingly passive. When I asked about his physical therapy, a nurse told me, "If he doesn't want to walk, we won't force him." Rehabilitation is about encouragement and support, not neglect. One particularly troubling incident occurred when my friend repeatedly pressed his call button for help, but no one responded. He then called the nursing station, but still, no one answered. In desperation, he called me, and I had to physically go to Iliff to inform the staff that he needed assistance. This level of neglect was unacceptable. Concerned for his well-being, I immediately contacted his family and urged them to get him out of that facility before it was too late. Meaning, I told his family he might die there, in my opinion if they did not get him out. Thankfully, the family listened to me and they moved him out. He is now receiving proper care at home--he is no longer overmedicated (again my opinion), his wound is being treated according to his doctor's orders, and he is making significant progress with physical therapy. If I could give zero stars, I would. Quality care should never be optional, and basic responsiveness, proper medical treatment, and patient support should be the standard--not the exception. This was not a safe environment.

I was extremely anxious about placing her in a rehabilitation facility, especially because she has dementia and does not handle change well. From the very beginning, this facility has gone above and beyond my expectations. Aaron Rebuck was incredibly welcoming and kind when helping get my mom settled into her room. His calm demeanor immediately put me at ease during our first meeting. Our initial interactions with the nursing staff, Thea and Rose, were equally reassuring--they were compassionate, patient, and so gentle with my mom. At that moment, I knew she was going to be in good hands. The dining staff has been wonderful, ensuring my mom receives a well-balanced and nutritious menu. She has actually gained weight during her stay at The Boulevard, which speaks volumes about the care she is receiving. Physical and occupational therapy also went very well and contributed positively to her recovery. The doctors and nursing staff have been terrific throughout her stay. Tigist, the CNA, is truly a huge asset to The Boulevard team. She has provided exceptional care for my mom and is consistently kind, attentive, and compassionate--not just to my mom, but to all the residents. Milly is another outstanding team member; she is responsible, attentive, and wonderful with my mom. When my mom returns to her memory care facility, I will truly miss the staff at The Boulevard (Willow). We would absolutely return here if she ever needs rehabilitation care again. I highly recommend The Boulevard Post Acute for anyone in need of quality rehabilitation services.

They didn't seem to care. They continue to send me things to the address of the assisted living center my mom moved OUT of to go to the Boulevard and they had my husband down as primary caretaker even though he was in Seattle at the time and she was MY mother. I kept getting calls about discharge even though I made it clear Mom was there for long term care. Communication was non-existent! My mother unexpectedly died after 3 weeks there. Kind of makes you wonder if the medical care was as poor as the administration.

DISCLAIMER: My apologies in advance for this lengthy review. I wanted to document the horrible…read morecustomer service my wife and I experienced from the INOVA FAIRFAX HOSPITAL NICU during a 3.5 month span. The best part of this review is that my wife and I gave birth to a healthy baby girl. Due to her being a preemie, she stayed in the hospital for an extended period where we experienced first hand the broken healthcare system and instances of malpractice within the NICU. Regarding the care of our daughter, it was a frustrating experience from the beginning! There was a great communication barrier between us and the hospital staff, to include front desk personnel, nurses, doctors, supervisors and upper level hospital management. To make a long story short, this hospital failed us, often picking and choosing what protocols they should follow, and not providing key information to us that they promised to deliver. In our initial complaint, we spoke to the supervisor to explain the discrepancy in communication, and all they could do is apologize, and attempted to justify the behavior of their staff. There is also a disparity in the amount of minority (African American) nurses at the unit. The few black nurses that provided care for our daughter expressed concerns with how the unit was being handled. Specifically, one of our daughter's nurses explained her reasoning to transition from full-time to part-time due to her inability to do what she studied and practiced for years regarding taking care of babies. Instead, she explained that at INOVA Fairfax, the nurses are simply tasked with following the doctor's orders even if it is contrary to what the child needs. Although the nurses are the primary care-givers for the children, their feedback is not really valued and that presents a major disservice to the families. In addition, we noticed a big disservice that the nurses were providing. Many nurses were on their phones, reading books and having leisure conversations outside of the infants rooms while alerts were going off that required immediate attention for their assigned babies. There was also a clear incompetence displayed by the night-time staff and nurses compared to the dayshift. Calling at night-time for updates was a nuisance due to the ignorant staff saying that they did not know the answers to basic questions. We would often be advised to just check with the day shift. Doctors and nurses alike agreed that the night-time staff doesn't really know anything and verified that the care at night is often not up to par! Therefore, the night-time staff doesn't really do anything to advance the child. There is also a disparity in the amount of minority (African-American) nurses at the unit. Although the nurses are the primary care-givers for the children, their feedback is not really valued and that presents a major disservice to the families. As it relates to the doctors, it was frustrating due to the many doctors we would see on a week to week basis, all had differing opinions regarding our daughter's care. In most instances, it seemed as if no action was their best course of action instituted. As a result, every milestone that our daughter accomplished was initiated by us. With such a slow trajectory regarding our daughter's care, it inspired us to look into how her development could be expedited to get her home as quickly and safely as possible. Every suggestion that we made was implemented, and our daughter never had to go backwards. That presents a major quandary on behalf of the hospital because the doctor's get paid to make the decisions. Consequently, in this case, we were proposing all the decisions, and they would just say, "ok, then, let's try it." There is plenty more that transpired, but due to limitations I cannot share. However, if you are a minority and are having a baby, you may want to reconsider contrary to INOVA Fairfax.

On occasions when we need a new or temporary PCAs, Grace HHC went extra miles to find and provide us with people who meet my mother's and our family's needs and values. This is a critical element for good healthcare providers. My mother's been with Grace HHC since its opening. Prior to her connection with the company, she has developed Alzheimer's disease and other symptoms, which many people develop as we age. Grace HHC has been finding good senior daycares, worked with her insurance company to provide the right service and hours, and various things to help improve my mom's conditions and provide all her needs, including medical and personal hygiene products. Very pleasant staff at Grace HHC works closely with the family, communicating on a daily bases, as needed. They can do that because Grace HHC is well-staffed with professional nurses and administrators that care about their patients/clients.
