In the middle of the night!! Family was told it was a drill. Why was there a drill at night? No one called person listed as first to call about the fall. Not notified of abrasions on pts knees. Nursing assistant allowed to speak to family inappropriately! With family witnesses in the room administrator sides with staff. Due to conflicting stories family not allowed to see family member who is now at his limited defense's. The only thing 4-5 stars is the building!

Being able to stay in the same complex is a bonus in not having to adjust to a new environment at a time you are more vulnerable. The facility offers many amenities for every type of lifestyle. There's an onsite gym/pool, chapel, library, beauticians, extension of the MOWA art museum. They offer many excursions around Milwaukee and being on the bluff above Lake Michigan offers a great location to watch lakefront festival fireworks all summer long. In addition to the regular cafeteria style dining, St. John's offers a higher end dining option with Taylor's. The service at Taylor's is impeccable and really gives you the feel that you are out for the special dinner overlooking Lake Michigan. The apartments (at least the one I visit) in St. John's are a little dated but the common areas have all been updated and the facility continues to expand. Keep up the good work St. John's, the facility, staff and amenities seem to be top notch.

It started off with a cup of vegetable soup that, with the addition of just the right amount of crumbled up Ritz crackers, warmed my soul to the core. Next came a deliciously prepared grilled baloney sandwich with melted cheese on a toasted brioche bun. This sandwich took me back to my youth, much the way Ego was transported when he ate the ratatouille dish created by Rene in the movie. The masterpiece was made even more amazing by perfectly seasoned, fried shoestring potatoes. The climax to the meal was a masterful rendition of cake and ice cream presented in the form of a heavenly piece of devil's food chocolate cake topped with the creamiest of peanut butter frosting that would make a Reese's peanut butter cup weep. All of this was paired with a scoop of spumoni. An amalgamation of the most delicious chocolate, pistachio and cherry ice cream. The likes of which I have never experienced before. This experience would not have been possible had it not been for the flawless execution of our guide, Sherry, through this culinary excursion

I also found out that Medicare has cut off funding to this place, not once, not twice, but thrice! How bad do you have to be for Medicare to say they're not going to pay you anymore? And what happened to "Three Strikes and You're Out"? I would think they should have closed this place down by now. I don't know how bad you have to be before the government shuts you down, but I would think getting cut off by Medicare 3(!!) times would be enough. Everything I have seen personally at this dump certainly confirms the low ratings. I have absolutely nothing good to say about this place. The Facility: The place itself is a dump: dirty carpets, dirty floors, dirty windows. Broken down equipment too, including room furniture and wheelchairs. The place stinks too - literally and metaphorically. Wear a mask, even though they aren't required anymore. Just to help block out the smell. The Care: I don't even know where to begin here. Patients are left out in the hallways unattended for long periods of time, as if they're just bags of yesterday's trash. Call lights left on for hours without response. I'm not sure some call lights ever get answered. Heaven forbid, you have any kind of emergency. Employees are more concerned about getting their own meals rather than making sure patients are fed and properly nourished and hydrated. Medication delivery to patients is spotty. I heard some patients complain that it took forever to get their meds or they weren't exactly the meds they were supposed to get. The Staff: First of all, I know when you are rated 1-star by Medicare and are rated in the bottom 20% of facilities nationwide, you're not going to get the "cream of the crop" or "best and brightest" for employees. You are going to get the dregs, people who can't get a job at better facilities. And that's what we have here. Staff are generally rude, unprofessional, unhelpful and defensive. Better not to deal with them unless you absolutely have to. But the real problem here isn't the line staff. It's the management, although I hate to use the term "management" for these people as they clearly don't have a clue how to manage. The head honcho is so useless, it's not even worth time commenting. If you want to find nursing "management", and I use that term very loosely, just look on the patio. They spend a lot of time there feeding their faces with junk food and smoking. Nothing gives me less confidence in so-called "health care professionals" as when I see them routinely engaging in such slovenly habits. Quite frankly, their Backwater Rube-i-tude is written all over them. I would guess there isn't more than a sixth-grade education level in the bunch. How they got nursing licenses is beyond me. From a Cracker Jack box would be my guess. I have, however, asked the State Board of Nursing to look into the licensing situation at this place, just in case. Quite frankly, I wouldn't trust these people to clean up after my dog, let alone be handling healthcare for some of the most vulnerable populations. I would also recommend that you document everything. In any way you can. There is a problem with truth at this place, so it's best to have documentation to counteract it. You will probably want to get a lawyer too as the level of health care knowledge, and especially health care law, is very low here. Have an expert in your corner. But really, it's probably best to save yourself all the bother, and just avoid this place in the first place, if you can. Also, as I was doing research on this place, beyond all the stuff I mention above, if you read some of the stuff on the State's website, this place has been assessed thousands of dollars in fines, slapped with numerous enforcement actions, and the record is littered with alleged patient neglect, alleged medication errors, and even alleged wrongful deaths. Pretty much anything you can think of, and just another reason this god-awful hell hole needs to be shut down. Summary: This is a trashy place, run and staffed by trashy people, providing trashy "care". This shoddy little sh-thole should have been closed down a long time ago. I can only hope for the sake of the patients that it is shut down. And soon! If I could, I would give it Minus 10 Stars. There is absolutely nothing good about this place. Stay away from it if you can.

    Abusive and neglectful staff. This was one of the worst experiences in my life. I was completely…read moreignored at first. Nobody came when I was pressing the call button. I was yelling at the top of my lungs for help while pressing the call button for over 2 hours. Ended up calling 911 from land line, as I thought my bladder would burst. (cell phone battery was dead) Two staff then arrived. One screamed profanity at me for calling 911. I could see into the bathroom fifteen feet away, but they refused to help me get there. They threw me a giant square thing that felt like a flat diaper and insisted I pee on it instead. Without even allowing me to wipe, the woman grabbed the leg I just had operated on and lifted it forcefully into the air, grabbed the diaper sheet, and threw my leg forcefully toward the bed. It was incredibly painful. I had been stripped of my dignity. I told the woman I needed my pain medication and water. The temperature in the room said 89 degrees. I had been crying and yelling for help for hours. The woman hissed at me, then unplugged my call button and land line phone and both left, and shut the door!!! Nobody returned for another 3.5 hours. I was so thirsty and couldn't ever remember feeling that panicked before. I was facing the wall clock. By then, it had then been over 9 hours since the last time I had anything for pain and this was one day after a knee replacement. I had been given a dose 1.5 hours before my transfer to the rehab facility. My teeth were chattering from the pain and I was shivering. The lady literally broke the pill in half to give me a half dose of what the doctor prescribed. She told me she didn't have to give me any more medication if she didn't want to and there was nothing I could do about it. I was told I should not have called 911. She was punishing me. She was pissed. Scared for my safety, and having no call button or operating phone, I somehow got up from the bed and onto the Rollator seat and pedaled my way to the center of the building where there was a phone. Crying and shaking, I called 911 again and insisted I was removed from the facility immediately. I was returned to the hospital. Doctors were completely aghast by the mistreatment. It took some time to get my shivering from the pain and shock from the trauma under control. I am still in shock over what happened. I was scared. I thought I was doing the right thing by signing up for rehab after my surgery. Turned out going home where I live alone was safer. This horror story spanned seven hours of my life. If I could give this negative stars, I would. Nobody should be treated like that.

Ultimately, it became clear to me that VMP does not like staff who speak up and their primary goal is to better cover up issues rather than actually fix them. They are currently under corrective action from the state and have a one star rating. The HR manager slipped up today when I pressed her and admitted she had conversations with the CEO, COO, and therapy director and that they were concerned that I would be one to report issues that I saw and that was the primary reason that they decided to rescind my job offer. Prior to this conversation, I was given a bunch of other conflicting reasons and lies were made up, including claiming that I refused to accept the pay they offered me even though I only learned in that conversation that they actually got exactly what I asked for approved. Holly (HR) and Lisa (HR manager) were the two people I've been dealing with for the last month and I would recommend that you get everything in writing or record your conversations with them in particular to protect yourself (WI does not require you to let them know you are recording them FYI). Holly said so many lies it made my head spin and the few times I had hard evidence that she was lying her story would change entirely and she would say that she "made a mistake" and "misspoke". Lisa said that Holly followed company policy, even when I pointed out that Holly was even caught in a lie in a conversation we all had on the phone together. Lisa also claimed that no one has ever said Holly is unprofessional, implying that I was the issue, but in the exact same sentence said that Holly admitted to being unprofessional with her lack of communication with me and follow through of actually calling me when she said she would (for reference, on several occasions she would tell me she'd call me on Friday, and then we didn't speak again until the following Thursday only because I would call her). Ultimately, the therapy department is understaffed right now, they told me how desperately they needed my help, and offered me the job on the spot, but they would rather operate understaffed than risk having an employee who advocates for the residents. I am passing this information along to the state too since they are still under corrective actions, but feel it's important that potential employees and residents are aware of the depth of problems here. Other past employee reviews on different websites also echo that anyone who speaks up about treatment of residents is fired, so be prepared to file a whistleblower claim if you take a job here. As someone who rarely reviews places, it really means something that this place made me concerned enough to warn people on several review platforms. I knew that VMP had high turnover in staff, but now I know why.

Since my father's passing (after my mother was already gone), I have been struggling for eight months with the accounting department. For one thing, they didn't close my father's account (under Alexian's contract) with Time Warner Cable and I had to deal with TWC directly in person to do so. They also sent an unauthorized debit to a closed bank account, incurring overdraft fees to me. Meanwhile, Alexian owes me money at this point for overbilling by their own admission, but they still haven't repaid me. My emails and phone calls are not returned by the ever changing staff, promises are not kept, and the whole situation has been exasperating. The administration is very disorganized and completely unprofessional. I can't believe I'm still having to deal with this so long after my father's death.

Mom was not able to walk unassisted; aides would leave her laying in bed for hours. Continuously short-staffed, the facility routinely relies on contract help. Once upon requesting help to use the rest room, a staff person helped her sit up / perch on the side of her bed w/ feet dangling, then got called away; Mom sat like that from 9 am-3 pm. More examples... Mom expressed urgency to use the restroom; they told her she was too big to handle and that she should hold it or just go in her diaper and they would clean her up later. She had no other option than to go; the aides left her in her waste for 6 hours, starting a horrible skin infection from which she never recovered. Initially, she could use the restroom with help; however, it was routine for "busy" aides to let her sit on the toilet for an hour or more. Aides also did not help her re-position her clothes nor wheelchair for easy transition from the toilet, causing her anxiety. Mom heard staff talking about her in the hall and then - to her face - aides called her a moose. Aides would also place her phone and picker-upper stick so she couldn't reach them. Mom was initially tested for her glucose levels before each meal; once she was moved to the Lodges, they stopped checking her glucose because they said they had no standing orders - and administered insulin anyway, dropping an insulin-dependent diabetic's glucose to a level of just 60 on multiple occasions. They also said they didn't have staff with the credentials to take her glucose levels regularly. At least twice, Mom did not get her breakfast until noon and, many times, got cold meals. Mom had only 6 lower front teeth left and needed mechanically-assisted meals - she could not chew meat, for example. However, she got chicken and hamburger that she couldn't chew so she was slowly starving. She lost 25-30 lbs and became very weak within about 90 days. Mom was forced to leave short-term rehab because they said she didn't need daily PT/OT; they felt three times/week would be beneficial. But that was mysteriously dropped once she got to the Lodges. Mom said she never saw a doctor in the three months she was there. It is a mystery as to why there were no orders for glucose testing for an insulin-dependent diabetic nor for the OT/PT that staff said would be helpful and provided. Note: the directors of nursing seemed very capable, but the day-to-day care was unprofessional and disrespectful unless it was a contract staffer. We asked the directors of nursing to check in with Mom; however, Mom feared retribution from floor staff and did not share all the details of her negative experience with the aides. After sharing about one employee, the directors confirmed the employee was a known problem but felt they had to keep her due to being so short-staffed. The last few days of her stay, aides left Mom in bed 24/7. She was schedule for transport to an assisted living facility at 1:00 pm. That day, aides did not get Mom to the bathroom all morning. They threw her belongings in a clear plastic bag vs. using her suitcase, brought her lunch late (needed to accommodate the transport), and refused to pack her CPAP machine. They said they planned to leave her parked in her wheelchair in the lobby for two hours until the transport driver got there. During Mom's stay, she became depressed. Her mental state and memory declined; upon reaching the assisted living facility, she had tremendous distress resulting from Linden Grove. She lost the ability to mobilize a new wheelchair, had multiple significant skin infections caused by negligent personal care, and was physically debilitated. Altogether, it is clear to me that Mom's stay at Linden Grove was exceptionally detrimental to her physical, emotional, and mental health. In my opinion, the deterioration of her health as a direct result of Linden Grove's negligence ultimately precipitated her premature death. Bottom line: do not allow anyone you care about be placed in this facility.

    Negative stars!!!!!!!! In my opinion and experience,…read moredealing with Linden Grove Communities-The Courts Waukesha was like a nightmare that never ended, the worst two years of my life. The memory care unit has the most vulnerable residents and I have seen animals treated better. I have a full-time job, but this quickly became my second and not as an employee. I wish I could have had the opportunity to move my mom sooner. There are better places out there. Like countless others, It was one of the hardest decisions I had to make for my mom, placing her care and giving blind trust to strangers, but boy did they sell me in the beginning. I quickly learned how poorly it was managed, starting at the TOP, a constant revolving door of staff, never knowing who is in charge. The caregivers that cared were left without the proper tools or assistance to do their job. Overworked, underappreciated and ultimately many left throughout this time. The others seemed to be there just for a paycheck, more interested in their smart phones than the residents. A certificate of any sort is not required to work in a CBRF facility. I could list all the terrible quality of care issues and occurrences, but this review would be even lengthier so here are just a few. When I couldn't be there to do ALL of the care for her, she was left without being provided basic daily needs. My biggest issue was her medication not given when scheduled or sometimes at all, leaving her in a state of fear and agitation. She would be left in the same clothes multiple days in a row, not being provided a weekly shower which resulted in a horrible peri-area rash. It was only treated daily if I was there to apply the ointment. Meals not provided on time and often served cold. (I regularly helped serving) Her room was not cleaned, bedding changed, or laundry washed. I was constantly informing management, filing grievances as to what was happening and the horrific things I witnessed, including documentation and pictures. There was never any accountability, just excuses and finger pointing. Be patient I was told, give the new director of nursing and charge nurse time to get acclimated and implement the procedures. Instead of that happening, there was just a revolving door of both, each trying to reinvent the wheel without follow through. It got to the point where I didn't know where else to turn so, I contacted the state. That helped for a hot minute and then it was back to business as usual. Their mission statement "We strive to bring dignity, vibrance, and love to the lives of older individuals and the families we serve throughout the Midwest." My mom was not treated with dignity, I don't know how they sleep at night or are even able to continue to operate. They should be ashamed of themselves.
