Run away fast. Husband was there for rehab. Had a very small bed sore that could have been cleared in a couple of days. Because of their neglect he had a large stage 4 ulcer. It was black skin around it red. You could smell the rotting flesh requiring a very painful surgery. He did not get the proper hydration and effected his kidneys. 2023 I typically only review when I have something positive to say, but the care of a parent is too important not to comment. In my opinion, I cannot believe this place is still operating. I would like to see it inspected as part of a 'sting' operation, for care and especially management. Some events that occurred are too disgusting to include here.'. Place can be hit or miss in the various aspects concerning the totality patients experience overall in nursing care. Some days are better than others. Some staff better than others. Hard to fault anyone in particular though. The fields of rehabilitation, skilled nursing and nursing homes are having troubles over all and while this place is no different they (the staff) manage well. Just make sure you get everything in writing only because they forget, say something different the next time around, and on one occasion will lie (administrative staff!) especially if it's only discussed verbally. So for communications with higher level staff administration I would rate C-. Rehab was excellent and gym was modernized. Med list strictly enforced (weekly doctors visit) which could be good or bad. Marginally understaffed but with what staff is available they can offer more individualized time then most other facilities of the like; yet they can't offer 1 on 1. Honestly everyone's mileage will vary and I suppose it's mainly because individualized care in the medical field disappeared 20 years ago with ever increasing red tape and the privatizing via private equity wrestling control away from doctors and "true" qualified nursing staff. Wish I could give this place 4 stars but it's 3 and a half stars to be honest. Don't knock the score though, if this place was fully staffed and able to give all its employees full time hours, necessary to operate smoothly I bet the communications could then be improved and then it would get its full 4 stars. So this could be a review that's only hampered by the nature of the times of the era felt after COVID. read more

Complaint 1: the miscommunication of the staff and then telling me inaccurate info that I cannot visit my my for 10 days when she first got admitted Complaint 2: the unnecessary, rude comment from the PCA that commented how my mom's urine or poop smelled when they should have answered professionally Complaint 3: when they neglected my mom for 4-5 hours when they just left her in her wheelchair when they brought her back in her room when they should have brought her to bed to rest and should have at least checked up on her once to see if she is ok Complaint 4: when they take so long at everything they do even when they are staffed Complaint 5: when they let a staff come into work sick and handle the patients food Complaint 6: they incorrectly diagnosed my mom with pneumonia Compliant 7: They need better fall prevention for the beds they have, my mom fell once but it would have never have happened if they had better bed rails and maybe bed or floor alarms read more

My Mom was a patient there in the Rehabilitation Dept. for a brief time because she had a bad fall when someone was supposed to be escorting her to the bathroom and claimed she fell on her own, She was rushed to Saint's Hospital where it was determined she had a concussion. she passed away nine days later read more

This place should be closed I CAN'T EVEN GIVE A 1 STAR My Dad was a patient and the care or lack of care was disgusting. The aides are careless. Have no rapore with the patients. My Dad was not bathed properly made to sit 2 hrs a day in a wheelchair foe therapy and he was not suppose to sit long as he had surgery twice for fractures pelvis and developed a huge bedsore which they said was getting better that was a huge lie he was sent home since he was not progressing and I am so glad because I was going to take him home they also ruined his cell phone as it was placed in a bucket that had some water in it. The bedside my dad had was out of control he had to be admitted and have extensive surgery and treatment ... no.bedsore should het to.that point. I could go on and on but I have too much to say. The Palm Center saved my wife's life when they caught a pulmonary embolism and had her rushed to the hospital. That makes them great even though minor things can and do happen. They are understaffed (isn't every facility of this type in this dilemma?) Still, the care for the patients was the very best they could do. The facility passes the smell test, there is none. I visited my wife more times than I can count and all I saw was hustle and work. There was constant cleaning of floors, food deliveries, snacks, checking up on patients for well being, medications being delivered and otherwise the best care that the staff and nurses could deliver. If I needed rehabilitation, I would go there without any hesitation. On the many visits I made, this is what I saw with my own eyes. Before anyone buys into bad reviews, visit yourself. This rehab is not a place I would send my loved one again. Under staffed, bad attitudes on most nurses, although aids were friendly and a bit more attentive, the level of care was really not acceptable. Not impressed with the doctor on-site and lack of communication. PT staff does seem very professional and very capable, which is a plus. Ghost town after 6:00 p.m. with very limited staff to tend to patients needs. My 81 year old father-in-law was admitted to the rehab side on Thanksgiving night after week in the hospital with a bad case of pneumonia and an infection. The next Monday we met with staff to review care plan and were told at least another week due to him still being very weak (he could barely walk and had extreme shortness of breath with very little exertion). That next Wed. (2 days later) we were informed services would end on Friday (I was not notified with a phone call, but found out when we visited Wed. night and found the paperwork on his bedside table. We appealed and it was denied - so he is coming home today thankfully and we will get him stronger at home. Hopefully with some skilled homecare people to assist. Our healthcare system is terrible and I cringe thinking about the people who do not have anyone advocating for them. I have spent multiple hours on multiple days making phone calls to inquire about insurance, other rehab options, calling doctors, calling pharmacies, getting references for home care, etc. We've witnessed multiple room lights blinking for long periods of time without anyone attending to them (under staffed), as well as people actually calling from their rooms for 'is anyone there? I need help!' on more than one occasion. The staff seem detached and burnt out, which is sad really. They are there to care for these people - who are human beings. But it doesn't always feel they are being treated as such. There is no compassion. I will also note the food at the facility is pretty bad (I've tried it myself). It was never served hot, and is not even appealing to the eye. My FIL has lost weight while he's been there, which he cannot afford to. Only after I mentioned supplements did they start providing them 2x a day. That was 4 days into his stay. I was also displeased to find our 'Welcome Folder' and admitting paperwork waiting for me when we visited on Friday evening (the day before he was being discharged). This was the first time we saw any of it and he had been there for 8 days already. They needed it all signed of course. I noted his admission date on each form before signing. This is unacceptable. When I asked the social work person on site, she could not tell me why we had just received it. This explained why when he was admitted we found another patients packet of forms in the room. I guess these don't fall under the HIPAA law?? I gave 2 stars only because of the few people I did interact with who were helpful and not dismissive, and also for the PT staff who I feel are truly doing the best they can with the large number of patients they deal with. Do what you can to care for your loved ones at home. This facility is not a place they are going to improve. read more

If your thinking of Palm manor don't walk away RUN! My father was a resident at this facility for rehabilitation. The Ida was to get him strong enough to go home and take care of himself after a fractures hip. Every time I tried to reach him by phone there was no answer. Then I got smart and called the nurses station to have an aide make sure he could get the phone of was not asleep. Most of the times the nurses station never picked up the phone. When they did they never followed through on my request. After the hip fracture my father had Neuropathy in his leg. He had no feeling. He sat most days in his wheelchair by the heater. The heater was in disrepair and jagged piece of sheet metal was exposed. My dad having no feeling in his leg rubbed up against it numerous times one day until an insightful aide saw the BLOOD covering his calf foot and slippers. He was patched up with butterfly bandages and gauss. I was called an told he ran himself into the heater. Being the engineer that I am when I saw him the next day and the piece of metal was still exposed. I looked and found that a small retaining screw had broken off some time ago. NO EXCUSE FOR FAULTY EQUIPMENT IN A MEDICAL FACILITY. read more

DO NOT TAKE A LOVED ONE TO THIS FACILITY. My mother was in there for less than 24 hours and I took her out. They did not give her all of her required evening meds. When she went to use the toilet in the morning, the seat was soiled with fecal matter. She asked for it to be cleaned and they wiped it off with a wet paper towel. NO SOAP. NO DISINFECTANT. I voiced my concerns with the Supervisor and she stated she didn't know anything about it and she was sorry. I told her it was her business to know about it and that she obviously wasn't sorry since the problems at this facility are systemic and reflective of her and managements careless attitude. read more

Do not take a loved one to Palm Manor or Palm Center what ever they go by. My mother had hip surgery and is elderly. She got worse there. They never answer the call button very quickly. Sometimes it would be 15-20 minutes. Was not supposed to be in wheel chair the first week for long periods of time yet one time I went in her feet were sliding off pedals and she was all slouched up. The food was not very good so most days I would bring her something. I am so sad we ever brought her there. :( Wish I could give it negative star. read more

I don't understand how this facility is still open!! I believe the horrible service to patients starts at the top with Administration and not all fault should be placed on staff members who have no compassion for the patients. There are some truly caring employees who try so hard to do their jobs with zero support from Administration. When they ask for additional help or mention that more aides are needed it falls upon deaf ears. - The facility is short staffed at night, wait times upwards of a half hour for evening meds, assistance getting to bathroom, getting out of bathroom, etc - Overall med list changed to, for lack of a better word, "zombiefy" our family member. She missed out on many daily events due to being asleep. - Windows were left unlocked and open at night in this one story facility when I went to visit my relative. No staff within shouting distance God forbid someone did get in her room at the end of the hallway! - Noone watches the main door at night, noone has to follow the sign in protocol and the daily entry code is verbally given to whoever needs it in order to get in at night (vs pressing enter button and being let in). I was never even asked who I was there to visit when given this code. - Intake and discharge ppwork was completed on the same day (the day prior to discharge), ppwork was backdated, statements entered on the ppwork were not true; especially the one regarding certain family members being present at a mtg that never happened and agreeing to a 21 day treatment plan. - Family members did NOT agree with, nor did they say, 21 days was enough rehab time before patient should go home. We anticipated a recovery time of at least 2 months!! - Day of discharge consists of a ride home from in a van service. If the aide has not had time to help dress the patient they're sent home wrapped in a blanket, even if their clothes are laid out on the bed to be put on. - PALM MANOR IS A REVOLVING DOOR SCAM! THE FACILITY RECEIVES MORE MONEY FOR INTAKES, THEREFORE; PATIENTS ARE DISCHARGED BEFORE THEY SHOULD BE IN ORDER TO MAKE ROOM FOR A NEW INTAKE. - THE TEST OUR FAMILY MEMBER HAD TO TAKE IN ORDER TO BE DEEMED SAFE FOR DISCHARGE WAS RIDICULOUS! THE THERAPISTS ARE NOT LYING, BUT THEY OMIT IMPORTANT INFO SO PATIENTS ARE DISCHARGED PRIOR TO BEING READY. BY OMITTING INFO IT MAKES IT LOOK LIKE THE DISCHARGE TEST HAS BEEN PASSED AND MEDICARE/MEDICAID CAN DENY PAYING FOR ANY ADDITIONAL SERVICES!! - i.e. test questions ask, for example, patient to walk xx distance and check off COMPLETED. The therapist neglects to write that patient had to stop 4 times, sit down to catch breath, was very wobbly on feet, etc. All that matters is the task was completed! I could go on and on and on about the disturbing experience our loved one had while at Palm Manor, but what really scares me are those patients who have no children or noone to advocate for them!! Avoid this place. My Father-in-law has been there over a month and the "rehab" is very poor. He hasn't improved and their business model is to take as much as they can from Medicare and when that runs out (FYI it is 170 a day with partial Medicare!)Now they are kicking him to the curb on Friday because the partial Medicare has stopped. My father was recently ill, he was hospitalized for 3 weeks in ICU before being admitted into Palm. Dad required extensive rehabilitation to get back on his feet. I cannot say enough about ALL the staff, they were great! During his stay dad worked with physical therapy and was able to learn to use a walker again and is now ambulating with minimal assistance. I thank every one at Palm for all the help he received during his stay. I don't typically write reviews but really wanted to share my positive experience with others. Wether an employee, or visiting a friend or your parent staying there for rehabilitation....Palm Manor is not about human relations! The members staying there get lied to and bs told so the senior nursing staff doesn't have to manage her staff! It made me sick today in being asked "May I help you"? From a senior staff member as I came to check into a patient that I befriended there for over the past year! Astonished there wasn't any caring warmth or kind words regarding someone I grew to love very much!!The patient had passed on and the manner of the nurse senior staff? Caddy & Insultingly & Cold regarding one's life passing! But as the patients at Palm say "She "meaning Sr. Nurse staff- is a phony who doesn't give an honest statement to any of us!! The wishes of patients who entrust their lives and their journey through death in the hands of those that? BS the patient and do business as business! HIPPA was created because if establishments like Palm Manor! It is all business and if I have a loved one needing Respectful quality care? It would Not Be in a facility of talking the talk..Shameful shameful best care practices. It's business not entrusting goodness and true respect to those who at the late years in life DESEARVE to have their wishes and rights respected!! I broke several bones in a fall and was transferred from the hosp to this facility IT WAS HORRIBLE to the point after being there for 2 days my family had me transferred to a MUCH better facility. i cannot say enough BAD things about the facility AND staff, if you want to experience hell on this earth that is the place to go!!! read more
