Positive: Knowledgeable, qualified, and caring staff, for the most part. The food was not bad, for Hospital type fare. It was better on the weekends, than during the week, for some reason. Most of the Nurses, CNAs, and therapists went above and beyond. They explained what was going to happen each day, as to therapy, linens, tests, medication ajustments, etc. My wife required assistance with the commode, and they were great in helping her. Linen changed when needed. Therapists there are top notch. Negatives: As with everything in life, there is room for improvement. Pine Hill is understaffed, and Nurses are responsible for 10 to 16 patients at a time. The Commode was not cleaned after each use. (Intermittently) The rooms in the facility were way too warm. Multiple complaints by many patients. (This was a 3 week stay in May.) My Wife has Lipidema and needed wound care, which she didn't get at all the first 3 days that she was there, and then it was minimal. She did see a wound Dr. once a week after that, and the wound care improved. Most rooms are double occupancy, and the facility needs to take more of an active role in controlling the TV volume at night when a patient is blasting their TV after 10pm. Makes it hard for the roommate to sleep. My wife is picky when it comes to food, and said that she wanted no milk, cereal, or Apple sauce. She got all 3 anyway. Don't send your loved ones here. As a healthcare professional, this place is embarrassing and the guilt I felt sending my Mom here was tremendous. I would give negative stars if possible. I'll start by saying my Mom is definitely a complicated patient, but if they don't have the means to properly take care of someone then say so. The Medicare 5/5 is a joke. My Mom was here twice and sent back to the hospital in terrible condition from neglect. Second time she went straight to the ICU and almost died. Place smells nice, once you get past the front door however- disgusting. My Mom got a UTI and then colonized because of staff unhygienic practices. Always dependent loops in my Mom's catheter every single time I came to visit. No wonder she kept getting UTIs. The food is terrible- I don't see how anyone can recover being forced to eat the slop they feed you. They never communicate like they say they will. They say they draw labs, but how are your lab values so different from the hospital? When questioned the DON makes it like it my Moms fault, not theirs. When I originally came to get her stuff there wasn't a nurse around to get me her personal medication. I made a special trip to come back and get them only to be told that they "discarded" them. 5 star Medicare facility which is what brought us to Pine Creek. Care is anything but.... PT is excellent, best part of the facility. But attention to care, personal hygiene, response time to call button, staff on weekends, dietary needs met, all at 1 star or less. Example-an upset stomach, rang for CNA, was told to take a drink of water and she walked out. 2nd call for CNA brought someone that actually dispensed meds. Simple things like asking if a face wash or toothbrushing is wanted is non-existent. And getting a shower? Thats a process. Wait times for response to call button can be an hour. Someone will come in and say they will be right back and an hour later show up. Suspect Medicare pre-approval was granted for a set amount of time and discharge was a fight. Communication from nursing staff is poor regarding test results or changes in meds. Would not recommend. read more

My Mom experienced the best care, no matter her age of 92 or her level of care that she needed for six weeks for her fractured hip. Each and every staff helped & focused on her mental, social well being, emotional & physical needs. All also elevated her spirits & provided assistance with her daily needs such as bathing, dressing, grooming, meals, hygiene, housekeeping & mobility assistance. Activity Staff always encouraged participation & engagement, developed a relationship & kept her in physically & mentally active. They always elevated her spirits with various activities. She also enjoyed all the Physical & Occupational Therapists. They were all caring, compassionate & uplifting & empowering her to maintain her independence. It's odd to read such differing reviews of the same place. This is not some high end place that's very costly. You have to expect it's not perfect. Kaiser sent my mom there and she's happy with it. She was at Roseville Care Center next door before, and Pine Creek is vastly better. Could things be better? Of course. She was originally rooming with a very unhappy complainer who was constantly yelling at the staff, plus she wanted the windows open in the middle of the night one night when it was about 38 degrees out. The next morning, they moved my mom to another room without anyone asking them to. The PT staff is always available to talk to you. The aides are all so nice and attentive. They are not always available when you want them. It's just the way things are for low cost facilities. They pay the aides little and it's a thankless job, especially with so many unhappy and hard to please. My mom wears a diaper most of the time. She doesn't mind as she knows staff can't rush to her every time she needs to go. We don't expect high end care. If you go in with that expectation you will be disappointed. I find that when I am nice but firm in what I expect, I get a better response. I know terrible things can happen to a loved one in one of these facilities with neglect and errors. I am fortunate that didn't happen. I can understand people being very upset. But for a decent place for rehabilitation that doesn't smell, with kind aides and a very helpful Naomi in administration, it wasn't bad. The big problem that I had with this place was that they wanted my mother to wet the bed instead of getting her up to go to the restroom. They said it was easier to just deal with her that way. Every time I went all i would hear is them saying they were short staffed. But then when I would talk to other people about that they all were wanting to know who told me that. I told him it wasn't important who was telling me that as much as I needed my mother to have appropriate care. She went three weeks without a shower because of their staffing issues. I had to get really ugly with one of the nurses one night insisting that my mother get a shower that night. After it had been three weeks. My mom was in there about a month and a week before I was able to get her out. She broke her arm and had surgery and was in there to recover but they seem to want to just keep her in bed and not get her up and walk as much as I would've liked them to. I'm hoping and thinking that it would've been a much better experience if they had consistent staffing. They had a lot of training classes coming through and observing. And about each time that I would go they had nurses that were filling in for the first time or filling in from another hospital or location again because they were short staffed. I don't know where to begin! The administration team is beyond unprofessional,it almost seems like a third world country facility. Protocols were broken more than once, emails responses not returned, not even so much as to notify me of my mother's passing. During my mother's last days they went so much as to call the police claiming I had robbed my mother when infact I took her belongings to secure them....( They deemed her incapable of making decisions) (Dementia) They ransacked her room in hopes to find financial information to cover her deductible which they neglected to manage properly. My mom was transferred to this home from Kaiser. I arrived with a duffel bag full of her clothes and items she needed for her stay. I buzzed the door, waited awhile, buzzed 2 more times, a staff member let me in and then walked away without saying anything to me. No one was at the front desk. I waited briefly and saw no one so I started to wonder the halls. My biggest concern was the safety for the staff and patients. No one checked the contents of my bag and let me wonder the halls with it. No one was around to help me. Finally found someone and they took me to my mom. Her bed had hair on it, a stain in the top of the bottom sheet and the bed remote had food all over it. My mom cleaned most of it off but I stopped her so I could take a pic of what she didn't clean. I pushed the red button for a nurse, they finally came after 1 hour, 5 minutes. This was a horrible first impression. My concerns: 1. Safety concern with letting people in with duffel bags and walking away. 2. Waiting just over an hour after pushing the red nurse call button. 3. Hair on bottom sheet along with brown stain 4. Food on bed remote Not sure if they are short staffed or ??? Safety and cleanliness are concerns with this care home. When I left I stopped by the front desk to see if I missed anything. They did have a computer screen for self checkin and I phone number to send a pic of your Covid card. Again, it's all about safety and a self check in a care facility isn't safe when someone can just walk by it with a suit case or duffel bag. read more

Worst skilled nursing facility. My elderly parents have stayed in a few different facilities in the area. Pine Creek should not even be allowed to stay open. The facility is dirty, medications not given on time, rude staff, absolutely no communication, didn't receive admission forms for 3 days after check-in, food is disgusting (cream of wheat hard as a rock), floors are filthy, feces on the floor, patients glasses left on the floor next to big chunk of poop. The staff neglect the patients terribly. Instead of assisting with toileting needs they tell them to pee in the bed. They do not shower the patients. Clothes were taken and never returned even though I stated I would take care of dirty laundry. read more

Unprofessional office staff. They have difficulty keeping aides employed here, so they constantly use registry staff. The registry aides who work here, who don't actually work here, are great for the most part. But as soon as an issue arises which falls under the purview of administrative staff, they will unapologetically screw it up. If my mother wasn't already established at this facility I would move her somewhere else. But it would exacerbate her condition to do so. I would recommend this facility to almost no one. My sister was a patient at Pine Creek for six weeks in April and May of this year. She was treated with nothing but kindness and respect. Everyone was warm and friendly. Her nurses and aides were very attentive, the PT/OT staff were top notch, and the office staff were always helpful to guide us along this difficult journey. Our family and friends were impressed with the cleanliness of the facility. The therapy room was large and well staffed. I have an aunt in this place, my aunt cannot see, the left her with the food just sitting, they closed the door while she was just sitting there for hours. My family went up there in the rain to make sure ate. They left her by herself to feed herzherself, . She cannot see to feed herself, all the food was all over her. We almost had to break the window to get in, my daughter was hysterical feeling like her hands was tied. We trust our love ones to this place trusting they will get the help they need. I am so angry at this place. I can count on only one person and that is Angela her speech therapist, she goes way beyond her call of duty to assist my aunt. read more

Very Frustrating place! I regret taking my mom there! :( she got there right after hospitalization. One of the staff came there to change her diaper while recovery. She put mom in a toilet bowl seat, took a diaper off and left her in a bathroom for an HOUR alone!! Mom was left alone, naked, sitting on toilet seat with her feet off the ground. REDICULOUS! how can you do this to elderly, helpless people??? This is more than ABUSE! "Red Emergency Buttons" are there attached to the bed just for fun, to play with, no function! Mom was trying to call nurse to get up ( High Risk Fall pt), 1.5 hours later of the CNAs showed up. Is it a proper care?? Food is horrible, no shower for 2 weeks, physical therapy is there for 10 minutes the most. I want to CRY OUT! So mad and neglected!!! :((( read more

I would like to give five stars to the staff at PineCreek Care Center. Having a loved one in skilled nursing is not easy. We have all gone thru a difficult time the past few years with Covid. Many skilled facilities and hospitals have had a shortage of staff and it's been very hard not only on the patient but the staff as well. Pine Creek has gone above and beyond to meet the needs of our loved ones in the Community in trying times. Thank you for caring for each patient with love and kindness. Shout out to the Discharge planners that work non stop with all their hearts. Each one of them! All Fantastic. My mom got a terrible infection in their care and almost lost her leg. They do not take care of the patients and the nurses are very rude to the patients. The social Services staff are unprofessional and could care less. They do not answer the buzzer when a patient is in need of help. My mom suffered in their care and dealt with rude and disrespectful nursing staff. There were nurses sleeping with their heads down on the desk and my mom could not wake them up when she was in need of her pain meds. Disgusting place so dirty! read more

My Mother had an elective surgery to replace her knee. Unfortunately the surgical site got infected. Then the hardware got infected and before we knew it she was going back in to have the hardware taken out. The doctor then placed a block with antibiotics where her knee used to be. That meant that she was not able to go back home and my sister and I were left with the task of finding a place for Mom. We toured several building around Roseville and we looked at if it would be possible to take her home with Home Health. After a lot of long conversations, we decided that the best thing to do was place her at Pine Creek Care Center. When my Mom rolled into the facility we were all a little apprehensive. No one really plans for this type of situation. But we had all promised each other that we would give the place a chance. It was not as if everything was perfect. But there was always a sense that the employees really wanted my Mom to get better. The therapists were amazing. My Mom had several therapist that she absolutely loved. We also had great interactions with the CNAs and Nurses. They all cared so much for Mom. We had read a lot of the reviews on here and we had a number of questions for the director when we toured. He was very responsive and did a great job explaining the difference between the Hospital and the nursing home. It helped us have a better expectation. We were very involved with Mom before the surgery and we know how much care it takes to keep her happy. And we know that the staff did as much as they could. I am not sure about the experiences that others have, but I am thankful for Pine Creek and the staff there. read more

Don't do it! My mom escaped from this place recently after a horrifying experience. She was recommended by a Kaiser doctor. I had a strange felling when asking this doctor about some nursing care for my moms rehab ... she kind of went silent and then arranged for transport to this place behind my back. It appears the Kaiser doctors and nurses can get good per high pay per-diem work here for those that are fired or on probation. Here is a list of items she mentioned: 1.)Tried to give her the wrong meds 2.)Put her in a shared room with a psychotic/severely disabled person that yelled through the night, throwing things around the room. 3.)Noticed they have a history if terrible Covid19 protocol, rooming vaccinated and un-vaccinated patients together and then trying to limit visitors to those patients. 4.)Staff refused to let her leave, threatening to keep meds that she needed for her recovery. Fortunately some over the counter meds will work for the meds they lost or refused to administer to her. 5.)The facility is extremely under staffed and many untrained staff with no security or front desk receptionist. That is how my mom was able to just walk out to her ride from a friend! 6.)Staff never bathed my mom, or attempted to clean any bedding. 7.)I can only guess that these actions are more of a scam, to allow/encourage patients to run away or leave early while Pine Creek Care inc bills medicare for multiple patients not even onsite for services. They can do this legally if these patients are not released per protocol. 8.)Notice the boiler plate responses fro the "manager" Todd P. How can this guy sleep at night! 75% horrifying negative reviews here and no state actions! Did I mention that my mom is trained in social service and care facilities policy. She, and I made a poor choice to listen to her Kaiser Dr for a biased recomendation.(Many staff are old Kieser employees). She still very clear thinking for her age, and just assumes poeple will run facilities as she ounce did in a responsible manner. "A friend said the food here was so bad he became bffs with a GrubHub driver. My friend was offered help with a shower only two times a week. No cleaning person showed up for three days. When they finally did , they used some chemical that can best be described as "Eau De Puke." During a ten-day imprisonment there for recuperation and rehabilitation due to surgery, he had ONE session for thirty minutes of Physical Therapy. He finally met the in-house physician after three full days, and that was the only time he saw any physician. The furniture and decor were so deplorably worn and tacky that you can bet they haven't been changed since the facility was built. In the 70s. This is what you get for splitting a shared room with someone who keeps you awake all night because of noises and things he does that he can't control. All that for $350 a day per person. For that price: Marriott. Hyatt. Hilton. Hell, even Motel 6 has better service. read more

Recently my 93-year old mother spent four weeks at Pine Creek for rehabilitation due to a fractured pelvic bone. Because she is 93, I have had the opportunity to spend more time than I would like in rehab facilities due to her age and health. When we learned she was going into rehab again we looked into local facilities close to us. Rehab facilities don't normally receive glowing reviews so we took a chance and selected Pine Creek. We were not disappointed. The staff was accommodating, friendly, and truly concerned with my mothers well being. I was impressed with the way facility is laid out. One side houses the permanent residences and the other side is where the rehab residents reside. There was not the obnoxious smell that so many seem to have. We were very pleased with the care she received. The one thing that could use some improvement is the communications. We were bounced around a bit trying to get information. read more

My experience at Pine Creek Care Center was exceptional in that I was treated with respect and encouraged to recovery! Two months and I was discharged with the independence to function at home with little difficulty! The nursing care and respiratory therapy were excellent, but with out a doubt, the OT and PT therapists put my life back together! Outstanding encouragement with compassion for my recovery was the key to my discharge. I felt they were my extended family and miss seeing them to this day. Kudos to Erica and her assistant, Kirsten and Ricardo and his assistant, Maria. What a joy to work with all of them! Thank You Jessie, Anaya, Thomas and Nadine for always being there for me!
