My mom was there for 20 days. Her care was OK, but the communication from the administration, nursing, PT/OT, and Social work departments was awful. Lack of clarity, lack of follow up and I had to consistently chase them down to ascertain information. Two notable exceptions: 1) The Director- Stacey- was very helpful to me on my mom's discharge. 2) The head of patient recreation-Amy. She was incredible with my mom and frequently tried to help me with getting information and updates- as due to COVID-19, I was unable to visit in person. read more

Dreadful place. Dad sent here for PT after spine surgery. They have no protocol or plan for the therapy, no milestones, nothing...just some random 30 minute sessions while he lies in bed the rest of the day and night, suffering from lack of basic personal care help that he needs. He asked to be released and mom asked for him to be released and arranged for nursing care and PT at home, but they're refusing to do so for 3 more days, citing "paperwork" that needs to be done. Utter nonsense- they're just trying to max out the 6 days that Medicare payments allow, while holding dad against his and mom's will. read more

My mother has been at Powerback several times. But her last visit in September was terrible, it was so bad that I got her out of there. Prior visits were wonderful but this last one was horrific! The food was terrible, cold & inedible. The staff didn't basically help either! read more

Very disappointed with communication between staff and family. No information was given for when they sent mother home. Meds were missing. Over 60 calls to facility in a month. Part time social worker who messed up in home care. After paying 475. for her to stay another day they sent her home with greasy hair. I had no idea what to do with her. Or what she could do. So much for help for the families. No help w social work. No help with the people at rehab. Flying blind. Thank you. That was worth 475. read more

I have been a PowerBack in patient for rehab after a major joint surgery. No one wants to be away from home but the nursing care, occupational/ physical therapy was excellent. They cared about my needs and immediately came to My room when I pressed the call button. As with any facility there are glitches. Whatever my concerns were the nursing staff and executive director herself met with me for a resolution. I am able to return home sooner than expected due to my rehabilitation program at PowerBack. read more

I have never seen such lack of care, compassion or communication amongst staff. Would not recommend this place to my worst enemy.

Nurses and other staff are extremely pleasant. Its clean and comfortable. I would feel safe putting family at this location.

HORRIBLE! Do not trust anyone here. I was given two numbers for the nurses station and both were not in service. They don't answer the main line and the voice mail is full. A male attendant, not a nurse, was obsessed with taking my mother to the bathroom and dressing her even after we requested a female. Even when the administrative staff witnessed an extremely disturbing incident, he's still employed there and apparently allowed access to her. It's pretty to look at but terribly inadequate! This place should be shut down permanently. Maybe they could make some money during the Halloween season. I can't sleep tonight and am just planning to take her out of there first thing in the morning. This would be the perfect place to house the worst criminals. Worse then the death penalty! read more

My elderly father was brought there for rehab. DO NOT bring anyone who is elderly, there. When I arrived 1 hr after my dad was taken there from the hospital, I found him drenched in urine, diaper hanging off, and he was half hanging off the bed. He was a serious fall risk with an alarm on his bed. His alarm went off 7 times and no one ever came to check on him. There was another elderly man that had escaped into the parking lot before anyone even knew. They have no business having people who need extra care and attention in that place. They never gave him any PT, never saw a doctor, it took a whole week for me to get him out of there. They sure did charge his insurance for care he never got. There are many many more scary things that happened there...too many to list, long story short, I had to hire round the clock care to watch my dad while he was in that terrible place. I filled out the survey I got in the mail. Asked them to call me but in 7 months, I have never heard from anyone about my awful experience there. read more

DO NOT SEND YOUR LOVED ONES HERE. Everything about it was awful. Upon admission my mother was told she would be notified one or two days before insurance ran out, this did not happen. The insurance only covered 7 days and she was kept 8 days with NO notification. Her admission paperwork wasn't even completed until the day before she left. When my mother expressed interest in leaving after 8 days, she was told, by the social worker, that it probably won't happen the next day because "she has meetings and doesn't have time to do the paperwork." MAKE TIME, do not charge your patients for an extra day they don't need/want because you're staff is incompetent. The occupation therapy was a joke. I could have had my mom at home and done it with her. The doctor/PA reviewed my mother's blood work on Monday and signed off on it, she had a severely low hemoglobin. NO ONE said anything about it until she got a copy of it upon discharge on THURSDAY! She had such a severely low hemoglobin she had to have a blood transfusion that we had to set up on our own because she looked and felt horrible!!! That is horrible patient care!! What is the point of even doing labs??? Oh and don't drink the coffee, it's inedible. The nursing staff and CNAs for the most part were good. read more
