The visit will be on time (they want to bill your insurance!) once you get an appointment but that can be difficult I finally had to call the discharge social worker at the cardiac rehab facility to ask questions. She was appalled and called the sales representative at Black Stone, that's when the phone rang and folks started showing up (after signing up with them and 4 days of silence) I feel sorry for people, especially the solo elderly who have to deal with these folks. No process (except how to get paid), no communication and no accountability. In an industry this big and important to people's lives, this can be a lot better

This is the absolute worst company I've ever worked with. Our local VA contracts with Care Tenders…read morefor home health care services, such as Occupational Therapy, Physical Therapy and nursing. This company has no set schedule for home services. We never knew when people were coming until the same day they wanted to come to our home. In fact, 2 therapists literally just showed up, assuming it was a good time to work with my father-in-law. One therapist called our home at 9:30pm and 10pm on 2 different occasions. I reported these incidents to management 3 times, to no avail. Nothing changed. Also, Care Tenders discontinued occupational therapy and physical therapy without contacting us to let us know. We had no idea until they just didn't show up and I had to call the company to figure out why. That's when they told me that his services had discontinued. I can't emphasize enough how awful this company is and I would never recommend this for any loved one. Disclaimer - we did have a wonderful nurse names "Z", who did let us know when she was coming ahead of time and always showed up on time.

Without obligation she is checking to see if the scooter will be covered by Medicare. The prices of the scooters were very reasonable. Only 3 were available for tests driving. The brand was Golden a high quality scooter. Especially known for their comfort. We have a van fitted with a lift. Theresa was very helpful in measuring the scooters and van to see if the scooter will work in the van. Thank you Theresa for a positive experience.

By not including any less than complimentary comments a clear picture is not being presented. My mother (and my father until his passing) lived in the Seasons community for 11 years - 9 years at Seasons and 2 years at Courtyard. The years in independent living was tolerable - Courtyard, not so much. The lack of staffing, particularly at night, was very concerning. When you have one nurse on duty and a scattering of aides handling 3 floors of elderly residents, this is makes for a less than ideal assisted living arrangement. Care conferences were never held, repairs requested for her apartment (broken tile in bathroom - possible trip hazard), thorough apartment cleaning as stipulated in lease agreement each took over a year to achieve. Other than an ideal location there is nothing ideal about Courtyard. My mother at age 96 passed away recently and no one in the management of Senior Lifestyle had the humanity to even utter condolences to our family. Concerns regarding the facility were also voiced to the President of Senior Lifestyle in their Chicago headquarters and has also been completely ignored. Would NOT recommend this facility to any family looking for an assisted living facility.

My family was very pleased with my dear mother's stay at Seasons for short term rehab.  From the…read moremoment she arrived until she was discharged, she was treated with the highest level of professionalism and compassionate care.  The facility is very nice, very clean, safe, and we felt very safe with the covid protocol that was in place.  Her room was lovely and everyone was very accommodating and responsive.  All of the staff/administration were very helpful, approachable, knowledgeable, kind, considerate, and went above the call of duty to make my mother's stay a great experience.  We appreciated the friendly and hard working housekeeping staff, food staff, and all of the workers whom we encountered from day to day!  The food was good/prepared well and extra care was taken to ensure I was served meals with regard to my nutrition restrictions.  Heartfelt and special thanks to the Therapy Dept., Nursing Staff/Medical Team and the Social Worker!  They worked tirelessly to provide the highest level of care that we have received anywhere and it was absolutely the primary reason for such a great and successful outcome.  We would highly recommend Seasons to any family looking for great, quality care!

I have had the worst experience with this employer. The owner/administrator has lied about my…read moreemployment with her company to Ohio unemployment/job & family services. She told them that I was working out of town, when I was there because my daughter was sick and in the hospital. So I would not get unemployment payments. She keeps saying that I work for her company which I do not, due to the fact that they only want to give 1 to 2 clients per day. And you only get paid by the client not by the hour and Achieve Rehab & Home Health doesn't pay all employees mileage only select employees. This experience has been horrible, the owner/administrator doesn't care that I have no income while seeking employment and soon will lose everything because she continues to appeal all approvals by Unemployment/Job & Family Services. If employee are treated this way How would clients be treated
