The spinal injury left me with a weak leg and I was unable to walk. After three hours of physical therapy and occupational therapy per day for two weeks, I was able to walk with a walker and I'm now at home. (To the cat's great relief.) Following the surgery I was not allowed to bend or twist my back or to lean. The physical and occupational therapists helped me understand what I was capable of and taught me new ways to perform daily tasks using devices such as a reacher to pick things up and a tool to put socks on. Everyone was caring, helpful, and supportive.

They were constantly sanitizing and cleaning the equipment every time it was used. I never felt so safe and had a good workout as well. Although there are some very kind and caring people in this facility, my family's experience is that the majority of them were not very attentive. It was not unusual for her to call for a nurse and wait almost half an hour. It didn't matter if she needed a spoon or she couldn't breathe...same result for both. It was even more common for the staff to tell us they "would be right back", and then never return. One evening Mom was having a lot of trouble breathing and rang for a nurse. Twenty-five minutes later, someone finally came in. Mom asked to speak to her nurse to get some assurance someone would check on her in the middle of the night. The lady told her she would be right back...and never returned. So Mom called me at 10:30 p.m. struggling to breathe and in a panic because she didn't want to go to sleep for fear no one would come if she called for help. I had to intervene to get assurance they would care for her. They gave the medications when it was convenient for them. One day they finally brought Mom her inhaler...6 hours late, even though she kept requesting it. It put Mom in a downward spiral of breathing problems and took her a few days to recover. After complaining, they finally allowed her to keep her inhaler. One time when the aide needed her to rinse her mouth out, she looked around the room for something for my mother to spit in, and she grabbed the little flower vase that was on her nightstand. Very unprofessional and not very hygienic. Another day, I waited an hour for the social worker to meet me for an appointment...only to find out she had forgotten about me. And when I came to pick Mom up to bring her home, her nurse wasn't made aware that she was being discharged, and another nurse failed to call in her prescriptions as he was told to do. The carpet was filthy and the experience was depressing. Unfortunately, I believe this is not unusual for many nursing homes...which is even more depressing. Respect and care for our elderly...that's not too much to ask.

A relative went here to have rehab. We sat with her from 7 am to 10 pm in order to have the aids…read moreand the nurses give appropriate care. The therapists are wonderful and very innovative in the way they can distract someone while doing good rehab moves. The bad thing about therapy in Ohio skilled nursing places is that the person only gets one hour of therapy Monday through Friday. The rest of the time the person is expected to stay seated in a wheelchair. While this building is beautiful, and just about 4-5 years old, the care by the cna's and nurses is quite terrible. Had to report them to the state of Ohio Department of Survey. The food was not always at the temperature it should be. The rooms were not being cleaned everyday. One nurse injured this relative with her fingernails to a 7 inch gash which caused us to send her to the emergency room at Mt Carmel St Ann's Hospital where the ER physician said care there needed to be reported to the Ohio Dept of Health Survey to investigate all their areas. The LPN's have obviously been taught nursing 101 but when it comes to helping one aid to move someone to a toilet they do not. They push the call light and say the aid will be here when they can. There were lots of times when the call light would be turned off within 5 minutes, however the person would have to wait up to 45 minutes to an hour to have someone come in to take them to the bathroom. If one aid was on break or lunch a nurse would not assist the aid to get the person 7 feet to the bathroom. I will say a very few of the aids were smart and helpful, but that is not most of the staff. When in the dining room for meals, the food on plates was almost slung onto the tables and if the person was obviously paralyzed on one side the utensils would be put on the other side out of reach. The aids would then come around within fifteen minutes and ask if the person was finished eating and some of the residents were demented to the degree that the aids just took the plate away. They did not offer to talk to that person why they were not finishing their meals. The tables were not always clean to sit at and the tables were unstable and rocked when touched. When the gash occurred in the abdominal area, the nurse practitioner would not lay the person down to be able to view the area. She said it was not necessary. It was definitely because the person is obese and the area to look was under the abdominal fold. The nurse practitioner said she had other patients to get to and could not wait until the person could be laid down for eyes on review to give best prescription or order to treat appropriately. She said that the wound care nurse would come look at it. The wound care nurse came in the next day and would also not look at the area. Asked if a doctor they said was staffed 24/7 would come look at it they said no. They don't come in after hours at all. NO doctor or nurse practitioner comes on site if needed other than the list the practitioner is to see that day. The people are put on a list and that list is adhered to without any variation. If you are not on the list for the day, you do not get seen. No matter what you wanted to be seen for. We had to make an appointment with her family doctor to be seen. The person went to the hospital and they didn't save a bed for her to come back four days later. They refused to have her come back and so the family and Mt Carmel St Ann's had to find another accepting place. I have a Hospital Administration BS and a State of Ohio nursing home administration certification. This is what you get being in this place. DO NOT SELECT THIS PLACE FOR YOUR RELATIVE. Plaques on the wall and postings in the lobby say they have had an excellent record, but BEWARE. The care is not good here. I had the opportunity to talk with other folks who were paying cash to be there at about $5-6000 dollars a month. They also were not happy with the services provided.

The facility and staff in the mc unit were great back to my grandmother even though she was nonverbal due to Alzheimer's. My interactions and time at the facility were brief but they stood out because of those positive interactions. My 2nd mom who I've been taking care for the last several years is currently a resident at Smith's Mill for rehab. Unfortunately her dementia has progressed so I wasn't sure what to expect from her once she was transferred from the hospital. She's slowly perking up except for when Ashley Everson stops by her room each morning to check on her. She's much more talkative with Ashley and more like herself. That alone was enough for me to leave a review. As former fire/ems, I've seen all kinds of facilities and staff so I came in to sign paperwork and ready to ask a hundred questions. I left without asking any of them because Ashley covered everything I was going to ask and went over things I hadn't thought to ask or even thought about. It's rare to find a facility that is as nice as they appear on the outside. It's even more rare to meet someone like Ashley that genuinely cares about not just the residents but making sure the family members are informed and kept in the loop. When visiting my mom, I saw Ashley pass by the room and stopped her to ask the various units and then asked about meds because my mom said she hadn't been given any pain meds. I took that with a grain of salt because dementia. Ashley could have said she would send the nurse in to talk to me about meds but instead, she talked to the nurse and let me know when the last dose was given and how long before my mom could receive another dose. In that same visit, I was greeted by people left and right while walking to my mom's room. I brought her dinner for her birthday and she was just finishing it when Myshawn came in with dinner and wished my mom a happy birthday. She was slightly confused so he showed her the meal order with her info on it and the happy birthday at the top. Asked how her birthday was and if she enjoyed her dinner then said he had a piece of cake for her. She was all smiles during that interaction with him. Those moments where she's smiling ear to ear while in rehab are fleeting so it means a lot when that happens. I've told a number of friends that have aging family members about my experience over the years any time my mom goes to an SNF so they have options when needed or when they start the process of moving a family member who needs more care. I've since told all but a few of them about Smith's Mill and if they decide to check it out, ask for Ashley because she is not only a font of knowledge when it comes to elder care but she is genuine when it comes to caring about the residents. That and the food has looked delicious every time I've been to visit during meal time. Honestly a little envious of my mom on that front.

I wish we had found this one the first time around. Dr. Deneen is the epitome of professionalism and he really does care about the residents! He introduced himself within 10 minutes of her being there. He saw her almost every day. Every day. The nurses are AMAZING!! One night I was leaving from my mom's and saw 3 sitting around one of the tables having dinner with a resident. They asked how her day was and what she had done. Ami came in and hugged my mom and sat down beside her and told her how much she was going to miss her. That is what you want to see in a facility you are leaving your loved one in. Beth had to call me to set up a care conference and the first sentence out of her mouth on my voicemail was that this was a non-emergency call. Now that may not seem like much, but when you are on pins and needles because your loved one has been very sick and has ups and downs for months; a phone call from the facility they are in usually is an emergency. She really did not have to say that is my point. However, she did and it was very much appreciated. The whole atmosphere is very homey and calm. They have bright call lights above each room and they have signage that shows that at the nurse's station which room needs assistance. Wild turkeys, deer, beautiful places for family to sit inside and outside. It has been impressive from day 1 and I could go on and on, but I do not have enough room. Our mother was taken care of just like we would have and we felt at ease during the day knowing she was there. They care, truly cared about her and helping her get back on her feet at the highest level they could help her get to. We've sadly taken her to other facilities in the area and were disappointed beyond belief. Not at New Albany Rehab. I hope she NEVER has to need that type of care again, but if she does, she is definitely going there.
