If you are looking for a clean, professional, loving, caring, and dedicated facility for your loved one; look no further. This is a beautiful space where people go to recover, rehabilitate, and restore their dignity and their lives. My 69 year old mother was a patient here and when she arrived doctors could arguably confirm she should be going into hospice, but we believed in God for her recovery - thankfully the owner Jim believed she could recover also. When she arrived she could not speak, feed herself well, and was physically weak all over. I was SO anxious to leave her in this state, but was immediately assured by staff that she was in good hands, and that she was safe. They nurtured her as if she were their family and I knew then even more that we chose the correct location. Within weeks she saw an eye doctor, physical therapist, speech therapist and occupational therapist. She participated in multiple types of activities (finger painting, gardening, celebrating birthdays, and her favorite - bingo!). After a month my mom graduated from puréed foods to a better diet, could feed herself, had less confusion, and was improving every single day. I am now happy to report she is even stronger, mind is sharper, speech is amazing, and can tackle occupational tasks that this facility assisted her to accomplish. It is the little things that we do every single day that we don't realize we miss until we don't have them. When we find ourselves in this place we need somewhere to go to recover - this is your place! The owner (Jim) will acknowledge you, spend time to speak with you, and the way he looks after his employees and is always upgrading the space shows deeply how much he cares. In my moms time there, they completed a beautiful remodel. Not once did this facility wreak of any smell. Jim - as I write this tears fill my eyes. I am SO thankful to God for your work! I toured this facility when deciding where to transition my mom after a hospital stay for rehab. In short, I met the owner who would do himself a favor by staying out of the marketing aspects of his business. He was pompous and full of himself and put a bad taste in my mouth. It didn't take me long to decide this wasn't a good fit for mom. The place looked cleaned and the workers seemed nice. Most rooms appeared very small though with three beds to a room; not okay in my mind even for a short stay. Do your homework and hopefully, you'll have a different experience considering this place appealed to me as it was close to home and had good reviews. Tours are everything! read more

I wish there were more than 5 stars! The people at Richmond Post Acute are like a family. They are filled with kindness and love while being extraordinarily competent. My wife has Alzheimer's and suffered a fall and a terrible break in her leg. Between their PT and OT plus the nurses and CNA's you could not ask for a more caring staff. This place is top notch. It's clean, they have a caring nursing staff and the food is good! I must have gained 5-10lbs here. As I wrote this, I'm packed and waiting to leave to the next facility that the social workers helped set up for me to discharge to. Richmond Post Acute Care is filled with conscientious, caring, and patient people whose focus is on getting their residents back on their feet, literally. The list of wonderful things to say about this facility is longer than Yelp will allow. My uncle, an 81 year old patient recovering from COVID and trouble walking, said it best, "I love it here. The food is amazing, the people are superb. I could not ask for anything better. It's just wonderful." I agree. He's moving to assisted living, but keeps begging to just stay at Richmond Post Acute because he loves the staff so much. And the food. The meals are incredible. He raves about the "three square meals a day, with sides, and all the fixings, morning noon and night!!" Every time I've been there for meal service, I agree. It's top notch. It's hard not to steal some food off his plate. One night they were making some chicken and the smell of that cooking was better than any restaurant. Really, who can ever say that about hospital food? This is not hospital food. This is food made by people who care, and also know their way around a kitchen. Many times my uncle made sure to get out of bed, and hobble over to the kitchen to thank the staff. If that's not enough, the administration is so transparent, Jim puts his cell phone right there on the website. He was on the phone with me on a Saturday morning, making sure my uncle was getting checked in and answering any questions. The social worker, Roseanna or "Anna" is so resourceful, and she filled out all the county forms for us and introduced us to all the after care team we needed. The nursing team knows me by name, and always remembers my questions, and takes instant action on anything I ask or mention to them. They are so organized and helpful. Jeremy works nights and is just a rock star, but they all really are. I just saw him the most because I often went over after work. If I had to list a negative thing, I would say the parking lot is hard to get out of, so if I wasn't picking him up, I'd just park on the surrounding streets and walk a few houses down, no big deal. You can also park right in front in the red zone if you have a relative who can't walk far. That's as close as the parking lot, and a little easier if you're just picking up or dropping off. Oh, almost forgot -- the physical therapist and Molly, the OT are just amazing. They always updated me on things, gave me items to work on later with him, and continually interact with patients, even after their sessions. When finding Richmond Post Acute Care for my mother during the pandemic was a tough time as mother was in critical condition/coma. After she woke up finding a place that specializes in wound care was difficult as very few places were open in San Francisco at the time. To Richmond Post Acute, I would like to personally tell all who read this review that Post Acute isn't just a place of care but a real family for me ;a powerful force who had my BEST interest at heart. I arrived at Post Acute on my back and unable to care for myself, however every person l met once becoming a patient, was kind ,caring, and giving; allowing me to leave on my feet. As far as staff goes, l would like to say that the battering-ram known as Kimberly, along with Jim Jordan and Bill Jordan , fought to accomplish all the needs of this patient . All of this had happened between 2020-21 during the beginning of the COVID-19 quarantine; which was one of themost chaotic timesin recent memory. This is the second time that my husband has been to Richmond Post Acute Care. After his first discharge and being home for a short time he had a little set back so we reached out to Richmond Post Acute Care to see if he could return. We were so impressed with the care he received here the first time that we did not want to consider a different facility. All of the staff are so well trained and each person really cares about the patients. The staff (Helen, Chrystal, Lonnie, John, Anna, Amy, Chelsea, Marina and Roxy (only to name a few)) all work so hard and go out of their way to see that the patients are comfortable and well taken care of. read more

I never thought I would write a review about a rehab center much less be admitted to one for a good period of time, but of corse you never know what life will through your way. I was diagnosed with a rare muscle disorder related to another disease which doctors didn't really understand; On the positive side they told me there was a good chance I could get back to normal because of how quickly the disorder happened. It was just going to be a long road back. I was in the hospital and eventually got sent to a rehab facility in San Francisco which all I can say; that it was just ok. From there I ended up back in the hospital related to being unable to swallow food. Other medications ensued & I had to go to another rehab center. My brother found Richmond Post-Acute Care. I have to say I'm really grateful he did. That may sound a bit over the top, but my agenda going there was to get well & as at turned out so was there's; and then some in a big way. First, James the administrator & Owner of the facility visited me in the hospital before I arrived to do an assessment; basically to see where I was at. I had to isolate for a time even though I was Covid negative which I had no problem with. I was impressed with the earnestness the facility took regarding the covid pandemic. Brian the lead therapist came to my room right after I arrived to do a initial assessment of me physically. We discussed my case as he had read my file extensively & cause my muscle disorder was so ambiguous; he developed a specific plan of action for me as far as occupational & physical therapy. Chris was my Physical therapist who worked with me on strengthening my muscles & getting them back in working order. I had a long way to go; he just took me step by step. Occupational therapy was my biggest hurdle relating to activities one has to do at home so a person can be functional when they get there. So many things I couldn't do which of corse one takes for granted. Ryan who was my occupational therapist gently pushed me beyond my limits of where I thought I could go. He got me back doing all the activities that we all do & always think we will do until we can't. Happy to report all those things I took for granted I can do now. Ryan also spent a great deal of time in Europe; Germany to be specific as did I; so we had a lot to talk about. Tamika of social services was so attentive & made sure I got to any & all doctors appointments that I had. Bee the speech therapist was fantastic as well; giving me exercises with my voice & helping get my swallowing back; which I'm happy to say is just about back to normal. Kimberly & Georgie who were also attendents there were just always willing to bring me something or help if I needed it. Richmond Post-Acute Care is a place where they know how to give TLC as the phrase goes, but also deal with the people staying there with sheer professionalism. If you do have to go to a rehab facility; I highly recommend Richmond Post-Acute care. read more

I want to recommend Richmond Post Acute Care. My 93 year old mom fell at the beginning of the pandemic. She spent at least 60 days here and it was challenging because we could not visit and monitor her care. However, the Administrator, James Jordan and his team, were available via text and they checked in with us often. I did not want to bother her care team on a weekend but given the pandemic we had concerns about keeping our mom in a congregate living situation. James made himself almost immediately available for a conference call with my siblings. My mom commented that she received the best Physical and Occupational Therapy ever since her stroke 5 years ago. He and his staff are truly dedicated to patient care and go above and beyond to make the patients comfortable. read more

I cannot say enough positive things about Richmond Post Acute Care. James Jordan, who runs the facility, does an incredible job of taking a personal interest in the patients there. He gets to know them and what they need; even making sure they can have their preferences if possible. He communicates with the family, and never makes you feel like a burden if you need to reach out with a concern. If you want your loved one well taken care of when they are in need of a nursing facility, this is the place! My mom was here at this facility for a few weeks just recently. I was very skeptical about putting her in one of these facilities to begin with because of all of the negative things I have heard and seen surrounding nursing homes especially since Covid-19...But to my surprise it was nothing like what I thought. I waited several months before writing this so I could reflect and comment on my experience at Richmond Post Acute. I experienced a COVID induced stroke in October, 2021 which left me with left side paralysis. The hospital released me to another rehab facility where I stayed for only 1 day before checking myself out. Needless to say, it was not the right place for me. Once home, we looked for another facility to place me and found Richmond Post Acute. After further research and communications with the facility, I began my 5 week stay. The facility is well organized and the staff is very caring, very accommodating and professional. On check-in day, the staff came in like clock work to orient and welcome me. The staff has great customer service attitudes and goes beyond their job responsibilities in their care of patients. Their physical therapy team is top-notch and very pro-active in working with patients. In my first week, after having been confined to a wheelchair, their p/t team got me walking with a cane and climbing stairs. After 5 weeks, they enabled me with enough mobility to be able to return home. My continuing recovery will take months to years, but I credit Richmond Post Acute with giving me the tools that I needed in order to begin that journey. If you are in a facility and not realizing the results you hoped for or looking for somewhere to go for the first time, I highly recommend Richmond Post Acute. I regret not being sent there initially from my hospital, so I hope you won't make the same mistake. read more

Jim Jordan and Kim Keller run the facility as if their own family is there. From the first phone call with them, I knew this place was different from the others I had called. Kim worked tirelessly on the insurance approval, including on the weekend, to insure that my mom could be transferred from the hospital as soon as possible. The Physical Therapy team at Richmond Post Acute Care was phenomenal, when something didn't work, they looked for alternate ways to rehabilitate the patient. I would NEVER EVER recommend this Facility to anyone. Due to the constant neglect of My Mother, her health started to decline. She wasn't able to speak, therefore I had to be her voice. I would find her with a very nasty, crusty, dry mouth. I would ask about it, along with asking for water for her. I was told that she was fine and that she was hydrated. I did not feel as though she was in a safe healthy environment at all, so I would go to visit her 3 times a day. I would ask to speak directly to the nurse's and the answers always seemed rehearsed. Everyone would say that she was doing well. They used the same words as they would speak. After a week my Mom was transported to an outside Emergency Hospital due to dehydration and high sodium level. If your Love one is unable to speak, or advocate for themselves, and if you want what is Best for them then, PLEASE DO NOT SEND THEM TO THIS PLACE. read more

After suffering from 2nd and 3rd degree burns which kept me in the hospital for two months, I needed to find a Skilled Nursing Facility to help me heal and allow me to transition back to my home. Thankfully, I decided to be admitted to Richmond Post Acute Care. This is an excellent facility with a professional and caring staff lead my the owner Jim Jordan. From the first day of my stay, Jim went out of his way to make me feel comfortable, providing me with clothes to wear instead of hospital gowns and setting up a daily wound care regimen. Roxie, the wound care specialist is very experienced and provided top notch care with a smile. She showed me that she really cared for me and my healing. Abe and Lani took care of my daily needs with genuine compassion and concern for my health and recovery.
