My mother was forcibly moved to a "rehab facility" by the emergency room director Gina Hocker at Kaiser Hospital Oakland. Kaiser is well aware of the reputation of these facilities as conscientious staff at Kaiser Oakland revealed to me these particular facilities are known for putting patients into dangerous situations with patients being sent to the emergency room with various injuries. The director of emergency services manipulated and in a very underhanded, cold, calculated, deceitful manner did all she could to send my mother into yet another dangerous facility with an even worse reputation than Bay Area Healthcare without my consent .. Shields Richmond .. ordering EMT's to pick my mother up in the dead of night to transport her to this facility. A facility I adamantly refused to consent to have my mother transferred to. Gina Hocker taunted my daughter literally laughing at her being emotional about how her grandmother was being treated. She was extremely unprofessional and patronizing. My mother told them herself she didn't want to be transferred to Shields Richmond. The experience has been very difficult and uncomfortable. My mother is 89 years old.. is diabetic and has suffered several strokes. She was walking and progressing. As a result of a serious fall at Bay Area Healthcare a facility Kaiser highly recommended by Claire UM department manager.. my mother has had a serious setback mentally and physically. Do not allow Kaiser Hospital Oakland the ability to send your loved ones to these facilities. They are cost effective for Kaiser and that's what they are most concerned about.. not the patients. I'm definitely not indicting the entire staff at Shields Richmond but there are staff who are there merely collecting a paycheck. Far too many do not care about the patients and it's very apparent. Several receptionists have routinely pretended not to hear me and hung up the phone in my face many, many, times as has the administrator of the facility Jack Philogene. They feed the patients the same meals over and over again.. leftovers. They've lost several pieces of my mother's clothing. She is not showered regularly.. apparently twice a week if that often. The actual facility smells like the sewer with some areas smelling worse than others. Do not brush her teeth. They immediately lost her bottom dentures and did not want to replace them claiming my mother threw them in the garbage. Sent her to a dental appointment half clothed in freezing temperatures more than three weeks after her dentures were lost by the staff. My mother has been treated practically as a prisoner limited to her bed. I've asked Philogene the administrator that she have a roommate as I'm not comfortable with her being alone and she needs interaction with another person.. Philogene moved a woman into her room who is asleep nonstop.. can barely hear.. is not able to interact... and this was purposefully done. Main contact with staff only for essentials such as meds and meals. Therapy is hit or miss as the therapist is never scheduled to meet with the patient. They ignore patients when they push the button for help. My mother had to yell for help as her roommate recorded the incident. And she literally got the phone and called 911 while the staff ignored her calls for help.. all witnessed by my daughter. I've heard the bell going off myself with the staff ignoring the patient. Shields stops taking calls at 9:00 p.m. which is dangerous as well. No contact after hours. Dr. McFarland never updated me regarding my mother's condition until I threatened to report it. Spoken to the doctor one time while my mother has been at this facility for over a month. I don't believe they're giving my mother her daily medications. The doctor should be on top of this. Patients are forced to attend religious services without the permission of family members. Administrative staff regularly ignore phone calls from concerned family members. The only way this lack of care will hopefully be addressed is if family and friends report their concerns. read more

My dad was admitted at Shields Richmond from Kaiser for his Rehabilitation. Our family was hesitant at first for him to go there base on the location, But we gave it a try. My dad was placed to a semi private rooms has 2 bed that has a direct access to patios with water fountain. The hallways are always clean, uncluttered and so bright. They have good security entrance doors and also surrounded by outdoor cameras which makes your stay safe. I've attached some of their facility photos so people can have an idea whats inside. Shields staffs were very friendly, polite and attentive to their patient needs. They provided an excellent care to my dad during his stay. When he came initially, He was totally unable to care for himself. But because of their EXCELLENT Rehab services, my dad was able to dress, go to the bathroom and walk again with a use of walker independently. Special thanks to their Rehab Director and therapy staffs; Hardworking Nurses, CNA's and housekeeping. I highly recommend Shields Nursing center to your love ones who will need rehabilitation and nursing care. Kudos to Shields team! Tip: please tour the facility first before you let the hospital case managers send your family members to a facility. In that way you'll see the facility and meet the people personally. read more

This facility is not in a great neighborhood, and the building itself looks pretty run down. But if you look beyond the exterior, they've got some of the important things down. To start, the facility is very clean. I witnessed a worker clean a rolling cart from top to bottom - he literally cleaned the wheels. Most importantly, the staff here is lovely. Practically every person we encountered was attentive, helpful, and a human. There are always pros and cons to a place like this, so here are a few more specifics: The food is not the best. Repetitive and a bit dull. That said, during the week when the main cook was there, the veggies were fresh and delicious. Speaking of delicious, you should check out the Tacos las Palmas food truck around the block. Not related to Shields, but quite a local gem. I'm surprised we didn't catch Covid during our stay. I can't expect the residents to stay masked up at all times, but some of the staff are a bit sloppy with masking as well. Some wear it under their nose and some pull it down to talk. I can see how an illness would spread very quickly once introduced. The security of the place seems good, and although the neighborhood isn't nice I felt safe going to my car every night. Parking is terrible! The tiny lot is mostly reserved for staff - which I do not object to. But it can be tough to find parallel parking on the neighborhood streets. There is no HIPAA in a facility like this one. With two people to a room and some hard of hearing, doctor visits, physical therapy, and medical conversations are not private. The onsite doctor made no attempt to conduct visits in private, and I'm not sure it would have been possible if she tried. While we had a good stay here overall, there were a couple of notable issues. The staff naturally has to abide by all doctor instructions on a patient's chart. Knowing that, I would have appreciated if the onsite doctor was better about updating that chart. For example, the staff would wake up the patient to give her melatonin to induce sleep. Several times this resulted in her being unable to get back to sleep. We repeatedly requested that the notes for this medicine get updated, but it took days. Once the doctor leaves for the evening or weekend, she is gone and cannot be reached. That seems inappropriate. This happened with several medications. Do not expect a schedule if you stay here. You will not know when a physical or occupational therapy session will occur, or when the doctor might pop in for a visit. You really can't do any planning or set expectations. You just have to wait and go with the flow. The staff can be slow responding to the call button, resulting in patients sitting in their own waste. I don't necessarily blame Shields for this, as the staff is in high demand. I never saw anyone lazing around or ignoring patients - everyone was working hard. They just can't keep up. But when they do come, the staff is really great. Everyone is very respectful of the patients and treats them with kindness when cleaning or bathing them. This makes a huge difference for patients who are forced to give up their dignity due to injury or illness. I especially want to call out people like Muhammed, the nurse Maria, the nurse Tenzen, Marlett, and Olga. They were delightful and there are many more. The people are the most valuable asset Shields has. They made all the difference during our stay. We did have one unpleasant incident where staff tried to move the patient without consent or conversation. It was a scary moment for the patient. On the positive side, the administrator was responsive, professional, and empathetic when we brought the matter to his attention. I suspect he is the reason the facility is so well run. Overall, I would recommend this place. Provided the current management and staff are in place, Shields is a good choice for recuperation. read more

I want to give an outstanding review to the staff, nurses, doctors and therapists who helped my loved one recover after a stroke. The Shields Rehab Center works together as a team, and they love what they do. Each person treated our family member with respect , expert care, and affection. He came to learn everyone's name and was given a loving send off when her was discharged. Visiting family and friends were given training on how to help with recovery and apply for needed services. Horrible place! Don't send anyone here. Those people are only there to collect their check they don't care about their patients at all! My mother in law passed away inside this facility supposedly off a "fall". They didn't notice until later. I tried speaking with the "manager" about this situation but conveniently she was always "busy". Before my mother in law accident she told me that every time she pressed the button for a nurse to help her it took 45 mins to an hour for someone to come. Also 1 time she told me that she yelled for help and a nurse yelled back no one is here. Trash place for real man!!! read more

The staff at Shield in Richmond all worked very hard and tirelessly to get my mom feeling good & strong enough to get discharged home. My mom was sent to Shield after having ACDF surgery and having a NG feeding tube inserted on the recommendation of Kaiser. The reason for Kaiser's recommendation to this facility is the fact that they have Occupational, Physical Therapy, & Speech Therapy. With these three therapies it was possible to get my mom strong enough to go home after 6 weeks. A huge thank you to Aaron the patient social worker, Dr. McFarland, all the therapist-especially Marc & Norman,, the amazing speech therapist, the front desk staff, all the smart nurses-especially Zelli, the hardworking CNA's-especially Silvia, and all the janitors. I was skeptical--because like everyone I want the best care for my dad. So when he went to Shields Richmond I wondered if it was really a good place for him to do rehab after his hip surgery. To my surprise he was very content there and even wanted to stay longer than the hospital was willing to pay. He found the meals decent and sometimes really great (and he is a very healthy and picky eater). The physical therapists are just the best. He looked forward to that everyday. He loved it. He slept on a hospital bed at home but he found the beds at Shields much more comfortable. As a family member I was content that I could always call and get the front desk to direct me to whatever help we needed. His social worker was always responsive also and did his best to help at all times. From Shields my dad will transfer to assisted living. Shields is helping every step of the way. Pleasant, caring staff and warm atmosphere. Not a super-modern, slick building, but fully adequate and homey. Most notable for my stay of three weeks (serious injury from bad fall) was the outstanding services provided by their Physical and Occupational Therapists - five star all the way, staffed six and a half days a week. Their "gym" (therapy room) is bright and active. Food is adequate, and abundant, but not up to the standrds of foodie snobs. read more

If you are seeking care for a relative or friend, do not use this center. It is overcrowded, patients do not get enough food, it is not run with consideration for fellow humans. Please do not consider having anything to do with this institution. Possibly in years past the word ethics could have been applied to its management/owners, but not any more. Avoid this place as a place of employment or of health care. Please be advised that the only reason there is one star up there is that the website won't let you rate something without a star. I'm very satisfied with my care here at Shields Nursing Centers. They have the best Rehab services. Avie, Stacy and Diana are very excellent therapists. They offer therapy services including weekends. I definitely recommend this facility to all people who needs therapy. I received quick because of them. I received excellent care and courteous services at the physical and occupational therapy at Shields Richmond. I highly recommend these to my friends, relatives and other people who needs short term rehabilitation treatment. Avie, the facility director of rehabilitation and her staffs always gave their patients above and beyond services. They will treat you like their family member and explain to you their goals for your physical therapy. They offer therapy services even sat and sunday. My husband, John, has been at Richmond Shields since May of 2018. I highly recommend this Skilled Nursing Facility (SNF), providing you are looking for 'priceless' caring in a loving and family atmosphere, combined with a skilled, knowledgeable staff. Not only has my husband been made to feel "at home", so have I, his wife. The staff is like no other I have had experiences with, especially considering this is not the first SNF my husband has been in. I understand when they first started they were rated poorly. Since then, Richmond Shields has grown into a SNF that is recommended by doctors, nurses and patient advisories, which is how I came to discover this wonderful place. Thank you, Shields! My mom's stay was a bit rough because most people who work there aren't trained in the United States and was still learning but they were all super nice. 1. First night there was no cpap machine we had to bring ours from home people stop breathing at night who has sleep apnea so it's good tp have it. 2. The nebulizer machine they had was so ancient/old that you had to tap it in order for the Albuterol med to get into her lungs. I did this in front of Dr McFarlane and she didn't seem to think having to do so was abnormal. Not sure Kaiser knows what's going on. 3. The first night mom told them she's allergic to the blue bed protectors and they still still use them so she got bed sores. 4. This place has an order and needs to be updated badly, everything's really old. 5. The lady physical therapist is very abrasive to patients beware and advocate for your love ones. Mom was in severe pain one day, so the dr told her it was ok to pass on therapy. The therapist said "are you going to brush your teeth? bathe? change clothes if they tell you not to? I had to switch mom from her(Hernandez or Rodriguez) I believe. 6. They washed mom up good, fed her well, gave her meds, washed her clothes. This place is good for people who don't have good insurance or are severely overweight, most nursing homes have a weight limit, so thank God for this place. 7. You get what you cannot pay for but for the few weeks she was there know one died. The reviews on here aren't all true, judge for yourselves it could be worst. 8, Saddest part of when I did visit was seeing all those 80 or 90 plus year old patients in one room barely could keep there heads up and in wheel chairs. I have no idea who would ever leave a loved one here. My mother was in this hell hole for all of 12 hours before we decided to take her out. She hadn't eaten, hadn't been given her meds and even though she was a fall risk, I found her sitting in a room alone on the edge of her bed when I went to visit her. My mother's words to me as soon as she saw me were "this is a bad facility". When I asked the staff why she had been left alone, I received several different answers: "I don't know", "She's not my resident", and my favorite "we are understaffed". But that's not the kicker, as I was signing my mother out the "activities director" kept trying to ask questions. I was overwhelmed at this point and I kindly told her that my mother wouldn't be answering anymore questions because we were leaving. Her reply? "I have a fu*kin' job to do"... and then proceeded to be sarcastic and cynical and threatened to fight me. All of this in front of my mother who has just had a stroke. The nursing director and nursing home supervisor did apologize to me but said nothing to the rude and unprofessional staff. Please do not send your elderly parents or family here if you actually care about them. This home says they can hold up to 84 people but the rooms are small and stuffy and some have 3 beds to a room. It looks like an old orphanage out of a bad 1950's film. Thank God my sisters and I check on my mother. I could not imagine ever leaving her there for any length of time.
