Very displeased with service from Synergy Home Care for numerous reasons. After several months on…read moreservice for bathing attendant Monday through Friday, today September 5, 2025 I discontinued service. Previous calls to Synergy with complaints were not addressed. Attendants would come into my home and pull out their cell phones and play games instead of having any eye to eye contact or any communitcation with me as their client.

You need to review and check on the actual people who are the caretakers.They use my parents home…read moreto do their crafts and play games.They don't watch my mother when she's sitting in her chair give her crap like cookies when she needs a snack and don't use a plate (lazy piggy).They are told to follow a schedule for getting her up to make sure she eats and takes her meds on time.They change the meds to be given to a different time that's fits their needs ( stupid these need to be given at a certain time for a reason!! I'm a pharmacist I know).They don't bathe her like they should or make her brush her teeth and keep giving her sugar and crap instead of nutritious meals.Wonder why she eats like a horse when I go to visit her.Quit sitting on your asses and do your job or find another.What the hell is this company hiring? Training..... appears non-existent.This is Synergy Wisconsin to be clear Waushara county.Its bad when you know the other caretakers are commenting on the shitty ones your parent is paired with.Get it together or get a lawsuit.Certain caretakers should be paid less,Than the superior ones.Do your job ! Follow the plan.My mom had several falls because of lazy incompetence.Oh I guess I should post a photo of the caretaker sleeping in the chair snoring with her arm hanging down when we came back from the grocery.Hope she didn't miss my moms calls ( and quit lying).When we go to visit they think they can shirk their duties and are more interesting in what we r doing than what they should be doing.Piss poor people do your JOB!Tired of feeding them too.

If you're looking for a first-class facility to care for a family member, and a professional,…read morecaring and attentive staff is important, then I recommend The Forum at Lincoln Heights. My mother has lived at The Forum since 2010; first in Independent Living, then Assisted Living and now Skilled Nursing. The facility, food and activities have been consistently excellent and the staff has always been professional, attentive and friendly. The staff's actions the evening of February 20, 2017 however deserve special recognition and are the most recent example of their dedication to serving residents. A tornado struck that area of San Antonio that Sunday evening at about 10:30 pm. The night staff responded quickly and efficiently, as the entire staff was recalled, to calmly and effectively evacuate resident, many to safe areas within the facility. This task was especially difficult given the large number of older and infirmed residents and the time of night, especially on a weekend. The facility received moderate damage, the weather threat passed, and no residents were harmed. Clean-up and repairs began the next morning. I visited The Forum on March 6, 2017. Repairs continue but operations have returned to normal. It surprised me to learn that many residents, my mother included, still did not realize the extent of the danger or damage. I conveyed my thanks to Kristi Allen, Director of Community Relations, but words can not adequately express the gratitude my family and I have for the staff. This was, quite simply, another example of the excellence we've come to expect from the staff at The Forum at Lincoln Heights.

I am still very dissatisfied with the care my father got…read more However, I tried to amend the part of my review that references the director; I do not like how the unit is run or her tone to patients but the words I used were unnecessarily harsh. Unfortunately I cannot change the review but am apologizing for the part that was likely hurtful. The intention is to have the Memory Care staff improv, not to be solely attacking.
