My husband and I don't take lightly writing this review but we want to help others be aware of what…read moreyou might face when you move your loved one into a Bonaventure property or any other assisted living facility. My MIL lived with us for 4 years before having a stroke. She was placed in a group home but after a second stroke she needed a facility. I called many facilities but when I called GC I spoke with with Mackenzie she was so nice, understanding and compassionate. I told her all the issues with my MIL: dementia, stroke, macular degeneration, two-person toileting, wandering, and potential sundowner that Salem Transitional Care relating she had developed. She visited MIL at Salem Transitional Care twice. Mackenzie called and stated MIL would be a perfect fit for GC. She assured us that it would take her some time to adjust to moving again but that this was a normal process. She asked us to leave a down payment of $500 to reserve a room. We went over verbally on the phone the cost which was $4250 a month including showering, toileting, laundry, assisting to bed and wheelchair, assistance to dining room. She stated that the great thing about GC was that they never charged over $4500 a month. She knew that MIL was in a spend down period to then transition to Oregon Health Plan. A week before the move Mackenzie called said we needed to come down today to sign paperwork to hold MIL's room. I said "I gave you a deposit". She stated corporate wanted the paperwork signed before the end of the month. I presented that day with my 15-year-old daughter and 10-year-old daughter. I reminded Mackenzie that we did not have power of attorney to sign paperwork for MIL. I could write a check since I was on her bank account, but my signature would not be legal on the contract. She said they would have me sign and have MIL resign once she moved in. Mackenzie reviewed the contract with me, left blanks as she need to figure the pro-rated rent for February based on the day she arrived. She spent most meeting reviewing the section of the contract stating if we had an issue to consider mediation over hiring an attorney. Mackenzie also stated that if for any reason they felt MIL wasn't a good match once she moved in, GC would be responsible to find a new facility for her to be transferred to. We would not have deal with the stress of moving MIL again. I gave her a check for the amount she figured and a week later MIL moved in. Within a week of MIL being at GC I received a call from a team member early in the morning stating Mackenzie had been let go and we were behind on rent. I said "she just moved MIL in and we gave you the rent that we were told". The staff member said the charge was for January 2020. I said "she didn't move in until February". She asked me if I had a contract that I could provide them. I told the staff member "No, Mackenzie didn't give me a copy of what I signed because she was going to have MIL resign since she was her own legal guardian and what I signed isn't legal as we are not power of attorney". She stated they couldn't find a copy of the contract. I told her everything Mackenzie told me at our meeting and phone calls prior to moving in. The staff member contacted corporate and after a few phone calls back and forth everything was straightened around. One morning we arrived to GC to visit with mom. The day shift CNA took us aside and informed us that MIL had not been checked on the whole night by night shift and was covered head to toe in her urine in the am. She said she reported it to the new manager but since Mackenzie was no longer there, she wasn't sure what would be done since the management was changing. In the early part of March when COVID was starting to be an issue, I was called into a meeting with Jordyn to review her care plan. I had never heard about a care plan. Jordyn started informing me about how much they had to do for my MIL. Mind you, we were visiting her daily to every other day and were cleaning her appt, helping her with laundry, toileting etc. He informed me that we would need to pay an additional $3507.00 a month on top of $4250.00. I told him we were not going to be paying more as we were told by Mackenzie that $4500.00 was the max out of pocket. I told him he might want to get with corporate and get this figured out. He then told me that MIL hair needed to be cut because they need the residence to look put together and kept. If the credentialing board comes and walks through their facility and sees residences looking unkept they can get in trouble and it lowers their scores. I asked him if he had made her an appt at their hair salon since we were not able to take her from the facility to get her hair cut. I never heard from Jordyn again and then COVID hit and everything shutting down and we were unable to go to the facility. I assumed that he would be getting back to me so we could reach and agreement on the fees but I never heard from him.

There Were A Set Of Towels, On The Chair, So We Knew Someone Came In Our Room. Went To Look For My Wallet. It Wasn't Where I Left It. I Was Also Missing $169! The Next Morning We Went & Spoke With The Woman That Was In Charge. As Soon As I Mentioned That There Was A Theft In Our Room, She Hurried Us Away From Other People & Into Her Office. I Told Her Exactly What Happened, The Time Frame & How Much Was Missing (8) $20 & (9) $1 Bills. She Told Us That Each Housekeeping Staff Has Their Own Key Faub That Says Exactly Who Goes In To A Room. She Then Notified The Head Of Security & He Would Call Back Telling Her With Who Went In To Our Room & He Would Come & Take Our Statement. He Called Back, Told Her A Name Then She Said (THAT WON't BE NECESSARY) She Said That I Must Be Mistaken. The Head Of Security Never Came. We Were Told That I Must Had Misplaced It. I Did Not. If You Have Parents Or Anyone You Know Living There & They Complain Of Something Missing, Please Do Not Just Dismiss It As A Lapse In Memory. Be Very Aware Of Whomever Comes In Their Room!

I've stopped by unannounced several times to visit our longtime family friend who is living in the…read morememory care ward of Lakeview Senior Living. Although I'm overwhelmed and saddened to see so many wonderful people battling this awful health condition, I'm thrilled to see my loved one getting the care she needs. Every interaction I've had with the staff has been positive, and it appears my friend is well-loved in this facility. I've seen her go from being a loner, to spending more time with other folks in the common rooms. There is a large dining room with ample opportunity for socializing, as well as more private zones spread throughout this massive wing of the building. Residents enjoy relaxing music in the common areas, weekly crafts and special projects managed by Activities Director, Jennifer, a very warm-hearted person who goes the extra mile to make sure everyone feels loved. There are multiple book nooks throughout the building, as well as access to a beautiful outdoor courtyard with a gazebo. I wish our friend didn't need to live here, but I rest easy knowing she is in safe hands. One small area for improvement is it would be nice to have dogs allowed in the memory care ward; my friend's face brightens up every time she gets to spend time with my pug, as well as her son's dog. Overall, I really appreciate the staff at Lakeview Senior Living, and hope everyone has as positive of an experience as my friend.

WVH was SO amazing during the most difficult time for me and my siblings. We called for…read moreconsultation, not knowing whether our mom would qualify for services or not. They qualified her the next morning and within the same day, we had THREE different team members reaching out to make appointments with us to guide us through the process, coverage, shower/bath assistance, meds, medical equipment, social worker assignee, and our AMAZING AND FABULOUS Hospice Nurse, Chantel. I don't know how we would have managed all that was happening so fast with my mom's decline without the whole team, but again, particularly the love, compassion, care, and time that Chantel gave us. Sadly, my mom passed within 2 weeks of setting up Hospice...and there again...they took over handling everything from calling the funeral home, arranging the transportation of respectfully and carefully taking my mom to her eternal home, picking up all the medical equipment, offering bereavement support for a continuing 13 months to my family, and of course....our beautiful Hospice Nurse was right there for us at the most painful moment in the end. Thank you WVH for ALL that you did for my family. xxxooo

Patient and I have gone back-and-forth about deciding to go on hospice…read moreor palliative care and have been weighing my options with companies in the Salem area that handle that and I have really enjoyed the nurses at Willamette vital health, however the ladies who answer the phone are not very friendly they're not very professional and for that reason I have decided to not use this company. When I call it is not a warm greeting. It doesn't make me feel safe and I just don't think that they have the right people working the counter & the desk answering phones, going on hospice or palliative care is a very stressful situation as is and whenever you're not greeted with kindness and warmth when you call a company, it definitely makes you not want to have them involved with your care at all!

I came into the facility expecting the worst, but my experience with Alexis and Shana completely changed my perspective. From the moment I arrived, Alexis was incredibly accommodating and helpful. She helped me locate my mother and even assisted me in finding my mother's phone without any hesitation or frustration. Her kindness and willingness to help immediately made me feel more at ease. Shana also stood out with her positive and upbeat attitude. When families and patients are going through difficult or stressful times, having someone with that kind of energy and compassion truly makes a difference. People staying in places like this really need to encounter staff members like her. Meeting both Alexis and Shana genuinely eased my mind and gave me comfort about my mother being here. Their care, professionalism, and kindness deserve to be recognized.
