I could say a lot of good things, like I did in my original review...but when food is really really an important part of our lives, especially for seniors, this place is a huge FAIL. When we did our initial visit in Oct/Nov 2025 the meal was very very good. They touted their great chef. This was a big factor for my mom choosing Independence Village of Carmel. This time in life isn't always easy for the elderly and food SHOULDN'T be the problem! Shame on you! In 8 months Storypoint should be able to correct a problem like this. Once she moved in in January 2026, that good chef was long gone or it was a facade they created for mom's visit. I can't tell you how many "chefs" they've gone through just since January 2026 but it's been more than you would ever think could be. It doesn't appear they are hiring "chefs" to make their "chef prepared meals" as their website touts. They are hiring people off the street and trying to make them a chef. Mom pays a pretty penny for living here and can't have consistent good meals?!? My mom is resorting to ordering DoorDash and having to buy more groceries for a wheelchair bound 88 yr old who should be able to rely on good meals from the dining room. Does she get a credit in her rent for this? NO. This lack of consistent, good quality meals is a big problem! And a lot of the elderly can't or don't speak for themselves. So I'm speaking. I'm fed up for my mom and the other residents there. After all the research and comparing places to find the best place for her to call home then having to deal with the subpar dining is just too much. This IS a big deal for the elderly. My mom lived at another community for 2 years that truly had a "chef" who created 3 beautiful meals everyday. AND ALL 3 were included in her rent, unlike Storypoint that only gives 2 meals. RARELY, was there a bad one. I wish she could have stayed there but she needs more physical help than they offered. The 5 other communities we visited covered 3 meals in the cost of their rent. Like I started, I could share a list of things we like about IV of Carmel, specifically Corso Care with Elizabeth and her team (they are excellent!) but the unacceptable dining experience day in and day out, knocks out the rest of the list. If you could make the dining experience as excellent as Elizabeth has with Corso Care you'd redeem yourself and truly set yourself apart from the other communities in the area. Otherwise, I'd think twice about moving here.

Seton Specialty Hospital is in my words one great hospital. I spent 2 months there. after having…read moreissues after open hart surgery. The staff were out standing, Meds on-time, treatment by Nurses ,doctors and tech's was beyond the call of duty. Nurses stationed right outside my door. Answering the call button in moments, not ignoring me and my needs. I was in all the area's of the hospital during my time there and I was treated very very well. and I was on Medicaid which in many hospitals means that you are not top priority. The therapy staff fought for me to stay longer and worked with me to get me so I was able to walk out on my own power. And so instead of going to a long-term rehab facility(Nursing-home) I was able to go home . My only complaint is Jennifer, the case manager, telling me our only option to transfer to his next level of care was to go to Allison Pointe. The day after transfer, I went there to be with him and see how well he was settled. I can't believe Jennifer is not aware of how inept that place is. After one day and 2 nights in that horrible place, I was able to make several phone calls and get him transferred to another facility. The facility he is in now says they never heard from Jennifer, although she told me they denied him when she checked with them first. I believe this woman has her own agenda, possibly getting monetary benefits from the lowest ranking facilities. She is not to be trusted. Seton would be much better if they replaced her with a caring individual.

We got a tour of the property on short notice from the director of maintenance, Rick. He was the most down to earth guy and he immediately put my step mom a at ease. The staff and residents were all friendly, the facility is very nice, though they're still finishing up some renovations, there were lots of activities, and the place really felt like community living (not like a hospital). They offer assisted living and memory care as well. My step mom loved it, the cost was reasonable, and we put down a deposit on the spot. If you're looking for a safe place for your loved one to call home this place is definitely one of the best options in the area. Also, the grounds are large and there's plenty of space to walk around. We just lived this place.

My family and I were thoroughly disappointed. I thought they'd have professionals who knew how to work with elderly patients with dementia. Nearly every person who showed up was inconsistent and poorly trained. We had one or two staff members who seemed to know what they were doing, but nothing consistent. We left with the feeling that all they cared about was the payment they'd get for having my dad on their patient list. I have no idea how one might consider their services helpful in the least. Since my father's passing they have not reached out to my mother to offer their condolenses or follow up in any way. Need I say more??

The majority of the times,I'm there; the aides function as baby sitters. sitting at the desk or other chairs minimally interacting with residents. Sometimes the TV is on but most residents are not wheeled close enough to watch or hear. Music is not played, although research clearly shows music is excellent therapy for patients with dementia, which the majority of residents suffer from. I will usually play music on my phone while there and residents visually perk up. A couple times, I've asked the aides to please put some music on to no avail. My sister is in her wheelchair all day from morning till night. When I come in, she is sitting with her head just hanging down. She is verbal and experiences minimal conversation with the aides, as they sit, observe with minimal interaction; virtual babysitting. The room is usually just dead silent. There is an activities lady who comes in during the week days and she clearly makes a wonderful difference for these dear residents. Early evenings and weekends are a depressing atmosphere at Maple Park. Ive spoken with my sister's social worker with minimal results. The aides appear bored and rarely smile with the residents. The staff at American Senior Communities in Westfield takes care of their patients, keeps the family updated, and works their best to get every last benefit you can out of Medicare and Medicaid. The facility director gave me his personal cell phone number; and the head of nursing gave me her cell phone number as well. You will always be able to reach someone. It is a wonderful facility, that will take great care of your family member. One of my dad's nurses actually sketched a picture of him, and shared it with me; she truly cared for my father. I will take excellent care over pretty rooms any day.

I have placed several clients in Prairie Lakes for Rehab for several reasons. The nursing team is very stable with the same team daily, so they get to know their patient very well a know their needs and likes. The most important things in a rehab building is Staffing and Therapy, Prairie Lakes has both. The Executive Director Holly is very involved in the facility with Nursing, Rehab, Assisted Living and a beautiful Memory Care. The building physician is Dr Tetrick, he has been with the building since it opened, he see's every patient himself and he has James a very experienced NP. Just a well oiled building! Let's be honest. Nobody wants to "need" a hospice. It means that you're dealing with end-of-life…read moreissues. In most cases, this will be for yourself or for a loved one. In my case, my father had been admitted to Hendricks Regional Health and was in ICU when we began to realize that serious decisions needed to be made. As his only surviving immediate relative, I was tasked with these decisions while also acknowledging that he was still coherent enough to participate and we'd also previously had discussions pertaining to this. My father wanted to die in his own home. This was a difficult task because hospice requires someone living with the person 24/7 as a sort of primary caregiver. Hospice is a support unless, of course, you actually go into a facility setting. My nephew had been staying with my father, primarily because he needed support himself. However, he'd been keeping a closer eye on my father and was there when he went into a health crisis. The decision was made to try to honor my father's wishes - my nephew would stay in the home and I would provide logistical, financial, and physical support. Despite living about two hours from my father's Putnam County home, we were going to try. Life's Journey came as one of the recommended options from Hendricks. They had a document indicating many of the options. It was clear that the social worker was quite familiar - pretty reasonable since Life's Journey is actually Avon-based. We contacted them - they couldn't come out that night (they usually do) as the intake person was ill, however, we made an appointment and intake came as scheduled. Kudos to Terri for being a strong presence and representative for Life's Journey. We discussed a myriad of issues. Once the decision is made for hospice, it moves quickly. You go from medicating to cure to medicating to comfort. The hospital really becomes irrelevant as major interventions are no longer appropriate. My father was discharged by home with ambulance transport. That same day, a nurse visited the home for a long (about three hours) appointment. I believe her name was Becky - she was amazing with my nephew, a 24-year-old with little caregiving history. He really took to it. We didn't know what to expect. Some said he may not make it to my scheduled appointment with a lawyer a little over a month later. Some said 3-6 months. Hospice starts off with typically 2 nursing and 2 health aide appointments weekly along with chaplaincy support as needed. It increases as one's health declines. Life's Journey offers home hospice, facility-based hospice (in Avon), and palliative care. In some ways, doing a Yelp review is weird. It's hard being "happy" or "unhappy" with care. Generally, I was pleased. Little things bothered me - an aide who no called/no showed twice, a chaplain who never visited despite having been requested to do so, and a last conversation that, if as reported, bugs me. My father ended up passing in a little over two weeks. The nurses were for the most part amazing, the health aide fine when she showed up, and I appreciated that the bereavement director visited my nephew after my father's death (upon my request). We received a lovely gift, a lantern representing light, after my father's death (it went to the funeral home - ironically, arriving late. Somewhat humorously, the batteries died in three days - I started calling them hospice batteries). When my father started his serious decline, Life's Journey responded quickly. A nurse was in the house within a couple hours and agreed that he was declining. They increased his visits, however, it would only be 48 hours later and he would pass. This was a good experience that fell a bit shy of what I'd hoped. For the most part, I'm very grateful as it allowed my father the dignity of death in his home while staying comfortable and being supported. Little touches could have improved the process greatly, however, this was a good experience at one of the hardest times of my life. I would be more inclined to recommend for those in the Hendricks County area - I'm a little less inclined for those in surrounding counties and especially the rural ones as I think that impacted presence. Overall, I'm pleased and grateful. With a few tweaks, this could have been an exemplary experience.

I wish I hadn't been in so much pain and could remember her name. She listened to me, believed me, and called a specialist WHILE I WAITED to help get me in for a consultation. He was amazing, my pain is being managed and I finally have an actual diagnosis and treatment plan. I will only take my family to Monarch from now til forever.
