This place from the very beginning welcomed us in and gave us peace. After my dad had a stroke, he was at a rehab facility. Medicare determined it was time for discharge. They had encouraged us to discharge home, but my dad was still unable to walk, get out of bed, roll over or another kind of independent action at that time. We talked to Tammy at Sage Creek. She was so calming and reassured us that my dad would have the care he needed, they could provide transport for the office visits he needed for his heart surgery and would have the PT, OT and SLP that he needed. We were told at the rehab that a skilled nursing facility was going to just leave him in bed and he would lose any progress he had made with them. This is absolutely untrue for this facility. They got my dad up and dressed every day. He had therapy. The therapists were so patient and kind (Alexa, Vicky, Willy and all the others I'm so bad with names) Mo the nursing supervisor went above and beyond to take care of any concerns that we had. Judy, Tyler, TJ, Brandi, Lindsay, CC, Rosann, Malik and other nursing staff were all amazing. The facility doctors were wonderful with communication and the care they gave. The facility was clean and the food was good with lots of options. Danielle and Nicole in transportation were amazing taking my dad to his many doctors appointments. Paige, our case manager, worked so hard for us. Anything we need to get my dad in the VA Home she came through for me. Absolute godsend! We adopted the staff here as family and we already miss them. 8 1/2 years ago this place was amazing. I even wrote a glowing review. Since my first that time my family member has been in and out of Sage Creek. And this last time I was beside myself. They lost her sling, they didn't answer her call light, they washed her phone. They never put her items back on her tray so they would be easy to grab. Everything was thrown into a chair the was filled sky high. The carpet is disgusting, the place looks run down. If u have another choice...take it. read more

This facility is only for short term stay but if you have other options to chose from, I wouldn't recommend this place. It is a bit deceiving though cause when you initially walk in, it looks like a nice place, the rooms are updated but and yes there's a but.. the nurses aren't attentive to your loved ones in the room. My grandpa is currently in here trying to get rehabilitated to get moved to an assisted living or back home and he even hates being there. He presses the call button and they rarely checkin on him. He had to use the restroom as he's still able to do on his own, with assistance and they told him to use the diaper he's in cause they were short staffed and couldn't take him. I'm sorry but no, that's not right to let your patient "sh*t the bed" cause you can't grab another nurse or CNA for a few moments to help someone use the restroom. I've also noticed in order to get any sort of nurse in the room you have to walk up to the nurses station for them to actual come in and help, since they ignore the call button. I will say though the nurse he has right now has been very helpful and polite with him since I've been coming out to see him. As well as another nurse who was super helpful this weekend, but again they were only helpful when we called upon them at the station. However, the other nurses don't seem to really care much, they just sit around and socialize and they seem to have a little attitude when you ask questions, as we are just trying to get answers when his actual nurse isn't right there. The last few times I was in there the room was a mess. The bathroom was worse, the pee bottle he had on the toilet still had pee in it. He was laying with a dirty stained blanket all day before they came in and changed it. If you're the nurse or the cna look around your patients rooms and see if anything needs to be picked up. Make your patients feel comfortable even though they don't want to be there. read more

I would highly recommend you do not take your loved ones there the nursing staff do not clean the patients at least they didn't cleaning my mom well at all and my mother's in declining since she's been there they say they're gonna do something and they don't and they refuse to give me any information on my mother's health on her well-being and they say it's because she said she told them not to give me any they know who I am. I gave her water. They know I'm her daughter, but she doesn't. My mother does not know where she's at right now and she did when she got there and you would think that they would think and they would say hey wait something's not right we need to give her daughter information about her mother's health and progression or decline and they refuse and also they shove a bunch of wet wipes in her area and left her like that and when she's choking, sometimes she yells for help and nobody comes. She is an obvious slow decline and it's sad and I am brokenhearted. Do not take your loved ones here. A lot of a few people have not made it out if you noticed in some of the poor post, I'm doing everything in my power. I'm calling lawyers and talking to different People. The lady that out of the nursing there she's not nice at all she she's the one that when I called and I demanded certain things be done at the beginning did it but then all of a sudden it changed I don't believe that they're their goal is to is to make people better. I believe their goal is to make people older people just pass out fall asleep and they just check on them every now and then pray for my mother because she is still alive by the grace of God. update my mother does not know where she's at. My mom entered this facility 5 days ago today after being released from St Rose Siena Hospital for a week. This facility is clean with no smells like you have in a nursing home environment. The employees are nice when spoken to, but they do not acknowledge you if you don't say anything. It would be nice if they practiced human kindness here and greeted each person in passing with at least a hello. Quality of care: they only go through the motions from what I can see. The CNA does rounding with patients every two hours doing vitals signs and then leaves the room. I have yet to hear them come in and greet the patient by name, ask how they are, engage in conversation, etc. They walk in, saying they are doing vital signs and walk out. They don't make sure the patient has water or ask if they need anything. Elderly patients need water. Every time I went into my mom's room, she had no water. Like I said, they go through the motions, but they lack in the care dept. Their beds are dated, and for patients that are fall risk, they don't have sensors for when they get out of bed to warn staff. They also don't have bed rails. My mom is a fall risk and has gotten out of bed a lot. My mom is also on oxygen, so when she gets up, she removes the oxygen and then forgets to put it back on creating more problems. The staff does not pay enough attention to their patients who suffer from dementia or altered mental status. Last evening, I entered my mom's room to find stuff laying all over, trip hazards for when she got out of bed. I was shocked that no one was picking up this room for the patients. I addressed it with the nurse, and the CNA came in and picked up the stuff lying on the floor. Again, staff goes through motions but lacks the CARE !!! I believe this place could be great but the staff needs a lot of training in human kindness. Something that is a motto and they sign when hired. Taught the importance of caring for the patients as they would care for their own parents. I hope they turn things around with staff because it is a nice clean facility. In closing, while walking past the nursing station yesterday, there was a nurse or some form of staff member standing at the desk talking to other staff. As I walked by, his language was something I shouldn't have heard. Staff can't have a potty mouth when guests are in ear range. This gentleman looked right at us as we approached to pass him, and still, this came out of his mouth. Again. Boils down to caring read more

Would not recommend this place. My grandma needed more assistance than other patients, so if you don't need much assistance this place may be fine but it was an awful experience for my grandma. CNAs would take up to 3 hours to respond to her. When I was there a CNA walked into my grandma's room and turned off her call light without asking her what she needed. Beena ( I think her name was) told us our grandma presses her call button too much which makes the nurses not want to come help. Paige the case worker/director was very unprofessional and unorganized. There were some very nice nurses, CNAs, activity directors etc but majority were lazy and did the bare minimum. Office staff and nurses are always huddled and complaining while there are at least 5 call buttons on at a time. If I could use 0, I would. Employees rarely answer call lights. I had to get up and out of bed and walk to the nurse station Every time I needed anything. Then there is always an argument over medicaction. some nurses are down right mean and don't tend to your needs. I've been here since Monday, it's now Friday and I have never been given a toothbrush, toothpaste, also have not been bathed, showered or even given wash cloths to wash myself. It's very unsanitary. We were delighted to get in here after seeing the list of Medicare approved choice. Skilled nursing homes can have some pretty dismal decor. This facility is modern and clean. There are 60 private rooms with big, easy access bathrooms. It can be a tough transition coming from a Level 3 in the hospital but he is adjusting well. The intake process can take a while. Be patient and kind to the staff. It goes a long ways. read more

After falling due to osteoporosis that I wasn't even aware this bone disease had progressed bad enough to cause several rib fractures, sacral, lumbar, thoracic spinal fractures along with a femur fracture requiring orthopedic surgery Sunday March10,2024. ST Rose Ortho Floor Rehab, Nursing staff /Doctors excellent until they wanted me transferred to their own rehab on campus that gives rehab to patients 7 days a week . I wound up at Sage Creek Acute Rehab beyond the staff I learned wanted my bed taken down denied the rehab my doctors requested was ignored,, winding up at Sage Creek! Admitted to Sage Creek and there is no rehab open Friday evening til Monday AM! While the floor nursing and rehab staff were extremely helpful though over worked/short staffed and unable to get work done timely! AGAIN Floor Nursing & REHAB personnel great but unavailable weekends so that's wasted days in rehab! Patient Care Coordinator Vaneta Pate is not a true patient care anything as she mismanaged my case to the point I received less rehab and missed important meeting with a third party coordinator because she never told him I was at activities awaiting him so I was passed on by.. for ok to discharge ..thanks to this incompetent HEAD of a department plus Travis Jones: Their Director of Operations was assigned to smooth things along but did nothing but chat with willing other friendly to him coordinators. Vaneta Pate was so rude she made me cry in my room 3times while decent people like Keisha & Ms Doll ran activities for patients in a very positive way . SAGE CREEK ACUTE REHAB is NOT GOOD! If they RELY on their Higher Paid Coordination Staff to run things this will be closed and declared sub par by state as Floor Staff is over spread out while actual patients are neglected by their very own care coordinators who don't care how a patient they just want their insurance company's monies!! Do not go to this rehab! First thing was my Mom arrive by transfer from Dignity St. Rose hospital and they knew she was arriving and they still had a patient in her room when we arrived so my mom had to wait almost an hour in the hallway on a gurney for them to move a patient out and then sanitize the room. This was at 5pm. Once in room they never offered her a dinner. The very next day they notify her that she will be isolated for 5 days. She was hospitalized for 7 days with covid pneumonia and they tested her for covid when she arrived at Sage Creek and it came back negative. The infectious disease nurse said she still needs to be isolated per CDC guidelines. I said that's ridiculous as my mom has already been isolated for 7 days in the hospital. Now this place wants to isolate for another 5 days? I told them she's better off at home. So I went to the CDC website and it clearly states that if you test negative you no longer need to be isolated. My mom was negative! The infectious disease nurse was really rude and said that's what we need to go by and that's what it is. She walked out of the room because she didn't want to hear me tell her that this is ridiculous and that we are leaving the next day. A Physical Therapist came into her room and had her lift her leg up while she layed in bed and then handed her a paper and said she should do these excercise a everyday. This was her PT for the day! This same day she never received a lunch or dinner tray. I had my mom ask if she was still being fed. She got a vending machine like turkey sandwich that the nurse brought her at 1:45pm only because my mom asked for food then the same thing happened for dinner. It was 6:45pm and still no meal. When she asked the nurse she said oh I'll bring you something to eat. ( see attached pics) mind you my mom spent 7 days in hospital not eating much of anything because she has been so sick and weak. Miraculously the next day this IDN comes back in and says if you test negative a 2nd time we will take you off isolation tmrw. I said how did that happen after you just argued your policy was to follow the CDC guidelines? She said we had some internal discussions and decided it would be ok. I said it's too late for that you should've known what you were doing yesterday when we had this discussion instead of arguing with us. We told her she got no real food no real PT so why would we want to stay here. I asked for the director of the facility to call me after I spoke to the admissions lady the day that I took my mom out of here and I still never received a call from him. This shows me he doesn't care that this happened. There are better more organized rehab facilities than this one for sure! What kind of rehab facility forgets to feed there patients meals when staying overnight? Just horrible!! read more

Sage Creek is a caring place for patients and their loved ones. Our family is grateful for the compassionate care that our loved one received. He was a resident for three weeks. They provided physical rehab services for him gently encouraged him to try and move his body more. this facility is clean, nicely furnished with lovely gathering areas. My only concern is that he was not not afforded the opportunity to shower. it seems strange that there was no way to make that available. To me this is a misstep in care. Ending on a high note, the staff was absolutely amazing. They were empathetic, helpful, and knowledgeable. The shining star for me was Doreen! From the moment I walked into the building I knew we chose the right place. She beams with kindness, makes you feel like everything will be ok. Hi I wanted to give a shout out to sage creek. I was a patient here for rehab. Diagnosed with an auto immune disorder, I was not able to walk well. I am younger than the usual clientele. They were very respectful and very prompt with my medical needs and issues. I was here for a short amount of time, but it was very pleasant. I spent three days here in November '22. I would rather die at home than go back here. Staff is supercilious, condesending, and anything but helpful. I was dropped off by a hospital transport team and left in a drafty area I thought was a hallway for over a half hour. Once they checked me in, horrified by the woman's attitude, I found out that this drafty area was actually my room. During my stay there, I was left crying for pain meds for over an hour before bringing them and acting snotty about it. I had a medical emergency that was let go for a full day before an actual doctor was called in, and anther day before my husband and a different doctor managed to get me transported by ambulance back to the hospital. One staff member (I believe her name was Pat) outried LIED--to an EMT no less, someone who had nothing to do with anything except transpo--about a certain medication I know for a fact I had never been given. Said this witch: "It's the same medication I been givin' her for the last three nights!" and then she gave a snotty little shrug. I then barfed this "same medication" right up. (So really? The last three nights? LIAR.) Staff did not empty a used commode in my bathroom for two out of the three days. One staff member smelled so bad it nearly made me puke every time she came in my room and then it would linger for hours. Bed is heinously uncomfortable. Food is inedible. And again, the call button seems to be just a suggestion, rather than something they should attend to posthaste. And just to reiterate...I cannot say enough times how rude the staff is. I would never, ever go back there. Not if held at gunpoint. read more

My wife's grandmother is located here and I love the way they get her up to walk and make sure she does plenty of exercise when needed and the fact that they keep her hair done they bathe her daily all around 10/10 perfect place to have your loved one who needs a little extra care! read more

I could write a novel. This place should have a caution sign with a giant warning label. If you have other options don't choose this place if you don't have other options, and your loved one is here keep a close eye and stay on top of things. Some of the staff is nice... but most of them should have either been swallowed by there mothers or chose different careers. Because patient care is important and so is bed side manor and straight up customer service and I think these people forget that the patients and family are the customers and they are providing a service and they need to be reminded of that. Update: My family's experience with Sage Creek has been a nightmare. Leaving my father at this facility at night was a nerve wrecking and uncomfortable for my entire family. We all needed to be working as a team when it comes to the patients care. When mistakes and incident's like this happen they are putting families in the position to where there only option to them may be taking them home without getting the treatment a patient needs. I shouldn't have to feel like I can't leave my fathers side at all because of thue behaviors of unprofessional staff and lack of communication and attentiveness when it comes to transport or any other situation that may come about. After going off on the staff, Two family members approached me thanking me for going off on the staff. They stated that they felt that there families weren't being cared for properly. And we all agreed we had to be there all hours we were aloud to be there because they didn't trust the staff just as much as I didn't. They told me that they were afraid to say anything because they believed that there loved ones would be treated poorly. This place is unsafe. At one point they didn't even have clean linen in the building. Cna tried to fight me which is why I went off on the staff. Because she didn't even want to help my father turn to his side when he can't move on his own after having a procedure done on his back. And administration is the worst. They don't care about the patients and it is clear. Rocky Tanner is a condescending piece of shit who should not be in charge of these patients care. He's training Harrison chacon, and let me tell you they have no real plans on making this place better. They are okay with nurses and cnas not caring for the patients properly. They'll tell you what you want to hear until they don't like your response for them not actually enforcing there staff to do there jobs. It's disgusting and sad. I pulled my dad from this place the moment they tried to say I was the problem? I was only the problem because I was watching them all not do there jobs and I wasn't gonna let them care for my dad in a poorly lazy manner. read more

Would give negative stars if possible. Staff are unbelievably careless and lack sense of urgency. I have had to request multiple things from the staff that they failed to provide my dad, since he is not monitored well. Caught the staff on their phones most of the time. Had to clean up messes in his room because they couldn't bother to pick up some trash. My dad has been here for almost a month and no progress. In fact, it seems his condition has gotten worse. I thought this was a "rehabilitation" center? He's back in the hospital due to PNEUMONIA as I write this. Staff has lied about his condition and now he had to be taken to the hospital once they realized (aka having my mom beg them to check on him) that he was having a hard time breathing. Great! I have an elderly loved one recovering from a medical episode. The first face you see upon arrival is Doreen, the receptionist. Always greeting with a smile and kind words. Thank you! Kindness goes a long way. There does seem to be a shortage of staff however, in my experience, any requests are completely in a timely manner. The conditions of the staffing at Sage Creek Acute is deplorable I don't know how the company can live with itself knowing that it's understaffed and not considering the patients let alone the nurses and the CNA nurses. All I can think of is that sage Creek must be in dire need of money therefore cutting staff at the sacrifice of the patients who are trying to get themselves better. It's pathetic, when the paitient calls for bathroom help and it takes close to 15 minutes for someone to acknowledge and then left there for another 15 minutes to get help out of the litrene. How unfair it is again to the staff the nurses the assistant nurses running around taking care of two separate wings while trying to maintain. Hire more staff!! read more

My mother had to come here because St Rose Acute couldn't keep her at their facility past 14 days. She was improving and thriving so well from a hip replacement. My mother was here for 10 days and died July 3rd in this facility. I wouldn't place any of my relatives here. The only helpful person was Wendy. Everyone else was to busy in meetings. Because of COVD-19 I could not visit her and had entrust the health care workers to take care of her. The Facility missed her Doctors Appointment and displaced the blame amongst the staff. I also received a 25,000 dollar bill for the month of July , my mother was only in the establishment for 3 Days in July ; but the bill was sent for the entire month, Why? My family never received a sympathy card or phone call from this facility upon my mother's passing. This is just a short summary of the facts involving my mother. read more

Only decent mention is the food - my Mom did enjoy the food. One CNA was great on day shift. One night she was in wet diaper close to 8 hours. Come into her room, turn off the light and back to whatever - cell? TV?? Doing their nails?? The smell - backed up sewer lines - not the normal smell of dirty diapers!! Avoid at all cost. AND, whatever you do, do NOT let them make any Doctor/transport appointments - you will be greatly disappointed. Sitting at home now, waiting for callbacks and missing Orthopedic appointment because of one Sage Creek employee's incompetence. Stay away. read more

This is a new and well designed facility. My Father had brief stay at this facility after receiving a Pacemaker at St. Rose Hospital. Rather then draft a lengthy opus, I will attempt to simplify my review as follows: Layout/Design - A+, Well thought out Staff to Patient ratios - A- Staff Exp/Knowledge - A PT and OT staff are great Nutrition/Diet - A Cleanliness - B, Need better attention to detail regarding debris on floor and items left lying around. Beds - D, They went cheap, beds are horribly uncomfortable. Bathrooms - B, No thought given to providing shelf space or storage for personal items Management - C Definitely reactive not proactive when communicating with patient's family. You need to hunt them down and then they tend to be dismissive when you advocate on behalf of your relative. They certainly communicate amongst themselves with many meetings but that does not actively filter down to the patient or family in our experience In summary, the care provided us more then adequate, perhaps better then most but the management overall seems more concerned about the business model then the clients read more
