After a head injury caused multiple issues, the Rehab hospital required us to have a plan in place where our parent would have onsite care while Physical and Occupational Therapy would be administered through an home health service. We had heard some good things about Sabal Palms and while my parent was in the hospital, we looked at the facility. It was very clean, organized, and had a very caring atmosphere. Once my parent arrived, there were some complications with his medication causing us to worry and even have to temporarily assign additional 24 hour surveillance. Sabal Palms was very professional and worked with us to assure we had the proper comfort level so that the healing process could take place. With a change in medication, things looked a lot brighter. Although my parent is recovering from the accident, we feel very strongly that care and dignity are being applied at this facility and that this was the best course of action for recovery. Thank you very much, Sabal Palms! (Brett, Sandra, Jaycee, Steve and the other nurses and aides and staff).

I watched residents dancing and singing along with a live performer and memory care associates. I was SO moved seeing residents "come alive" during this time. I can't put in words how blessed I feel that my father is a member of the memory care community. I highly recommend LaRhonda Edwards and her memory care associates.

First of all, I have never been treated so respectfully anywhere I've been in my life I moved in…read morehere in mid December. I'm 61 years old. I moved from South Florida to here because South Florida is not what I moved to in 1988. There's crime there's shootings and one shooting happen about 3 miles from my home. I said I have to get out of here and since coming to grand Villa, I rarely think of where I came from the staff is amazing. The Director is incredibly amazing and more than that. Is this person who didn't have to convince me about this place, but has been the most in touch with me since I've been here and when I wasn't here yet, and we had regular conversations almost on a daily, it was like I would call and if she was busy I could text her and she would respond to me almost immediately never more than three minutes. Amber is a great addition to this facility as well as Tim the Director the restaurant staff and the people what are what makes this place a jewel I use a place for mom as my search engine and I want to tell you that Desiree in Boca is a outstanding representative of a place for mom. And with Amber's help I have really made this my home Palm Coast is a wonderful place to live people say well how long are you gonna be there? I said I feel safe and secure and the people are very nice. I have no reason to leave there's wild turkeys that are walking around the property. I'm not saying there's 50 of them in the flock there's 45, but it makes the place very special And I can't think Amber enough for being there for me and being there for all the residence if you're looking for a place and I'm serious, call Desiree at a place for mom. I am a divorced father and I think Amber and Grand villa for getting me closer to my sons because that's what it's all about right.

Patients said they were not allowed to get out of bed because they were short of staff. Other patients said they were not allowed in the dining room because of staffing shortages. Last week I heard from more than one person about a staff member who refused to let a patient use the toilet prior to their shower. The people who show up for work are working and seem to be doing a good jobs however this place seems to not have enough people to handle their patience's needs. Several patients have not been bathed in a very long time. Some have matted hair. They are fed and given their meds. Some of the overworked staff should get a 5 Star review. Those few employees who don't seem to work quite as hard might do well to remember they may reap what they sow. That being said most of the staff is fantastic and they are Grand Oaks saving grace. Otherwise I would give this place a 1 Star review. This is not an easy review to write because so many good people work here but they are severely understaffed and patients' needs are not always being met.

My father-in-law was sent here from the hospital to recover. He has diabetes, hypertension, severe…read moregout, and celiac disease, all of which were noted on his chart. This place ignored his medical chart and fed him the same cheap food everybody got, all of which was in violation of his medically prescribed diet. His first meal there was grilled cheese sandwiches with turkey, french fries, and juice made from corn syrup. Turkey for gout. Bread for celiac disease, fried salty foods for hypertension, and corn syrup for diabetes. I actually brought this to their attention on and they still fed him the same stuff so we ended up paying to bring his meals for 5 days. If you have any sort of dietary restrictions at all, I'd say their food is pretty much terrible and you should make arrangements to feed your loved one who is unfortunate enough to be there. We also experienced no fewer than 10 different instances where the ear piercing alarms went off on the exterior exit from people push pushing the button and exiting the building without using the code. I understand why the alarm is there, but why even have it if nobody's going to respond? Every time it went off (with a few exceptions) it would go off for anywhere between five to as long as 15 minutes. At one point while we were there visiting and had guests I had to go searching for a nurse to come and turn it off and most of the nurses don't even know how to turn it off because they're not given the code. The manager on duty can do it, but if that person happens to be on the other side of the facility they can take a long time. Meanwhile, the patients are being disturbed. Cannot sleep cannot rest. Can't even have a conversation believe me it hurt my ears and I'm 60% deaf from service related hearing loss in the Marine Corps. The final straw, though really was the PT, which was the whole reason he was supposed to be there. It was not being performed regularly and was not being performed in a meaningful manner other than a couple times when a physical therapist named Chad came. He was awesome. I would classify him as the one true caring person I saw working with my father-in-law. He was friendly. He took the time to actually converse and did a meaningful PT session of 15 to 20 minutes.. on several other days when Chad was not , my father-in-law was given PT that consisted of nothing more than a person coming in for about five minutes and making him get out of bed... which has absolutely zero value, especially when you consider he could do much much more than that at home in a much more comfortable environment. In any case they had them scheduled to be there for two weeks and I could see the writing on the wall, but not only was he miserable and absolutely hated it there. He was getting nothing from it so we insisted on him being discharged after only four days. I would like to express my gratitude for the people in the facility, who actually did provide good care because there were some, and in fact, I don't blame the people who work there I blame the owners of these kinds of facilities that understaff and underpay these hard-working people. Heck, the TV in his room didn't even have a working remote. I had to drive to Walmart and buy a universal remote to get his television working otherwise he would've been in an isolated room with no Wi-Fi because nobody knew the password, and no television and no phone that was within reach because it's on a nightstand behind his bed where he can't reach. So, all in all I'd say The Place is a disaster and not good for somebody who truly wants to get better and go home. Id say you're much better off just going directly home and arranging for in-home hospice because Medicare will pay for it and your loved one will be much happier.

BEST DECISION EVER! She had a 1 bedroom apartment (she moved under great duress, not wanting to leave her home of the past 30 years), but she eventually agreed to the move. The first few months were an adjustment. She wasn't used to people doing things for her. But even then, she was constantly telling me how friendly everyone was. And I found the same thing during my many visits with her where I stayed in her apartment. During her 2.5 years at the Windsor until her recent passing, she eventually loved where she was and all the kindnesses she was afforded. When I'd call, her most common phrase when I asked how she was doing, was "they keep spoiling me here." During her stay I found the staff VERY responsive, that includes the nursing facility, those that helped her down to mealtime, those who did her laundry and cleaned up her apartment, the office staff who kept me apprised of any concerns and helped me when I needed someone to help her with documents, etc. Being so far away, I was terrified of all the things that COULD go wrong. The good people at Windsor proved worthy of my trust. Though she missed her 100th by only 3 days before passing (in the hospital after Windsor staff had noted she wasn't "breathing normally" in a random check and called 911), Windsor staff had been helping me plan her big birthday. I would name names, but I know I'd miss a few and I don't want to do that. All I can say is I am eternally grateful and am happy to recommend The Windsor!
