There is now an Activities Director who is enthusiastic and introducing a schedule of activities for the facility, including memory care. The staffing is also improved, with more full-time aides and somewhat better coverage on weekends, though that remains a weak spot. Nursing has also improved with the addition of an Assistant Director who helps organize and manage the load, and who complements the leadership team. The memory care unit really needs updating with a special emphasis on safety. Falls are frequent among memory care residents, and this unit was designed with little regard for safe landings. The management has always been available and attentive to my concerns and requests, and I've found the staff to be mostly friendly and respectful.

And like others have said, it's not easy getting in touch with anyone there, not even voicemail of staff.

Their staff rate turnover is 71.6% which is "significantly above the national average"; 18 health code violations and $30k in fines paid. There are some amazing staff members, but unless your loved one is in their section, it's probably not going to be good. I visit weekly and have heard a lot of complaints amongst staff. They are over-worked and under-paid\under-appreciated. It honestly makes me sick to my stomach to have witnessed\seen what actually goes on. The first time I went to Bayview for 20 days it was a nightmare. It is dirty, the blanket the aid…read morebrought me head crusty stuff on it so that it had not been washed, they were large roaches walking across the floor in my room when I turned on the light at night. I told the nurse about the roaches and asked for the exterminator. Three days later the exterminator had not shown up. Four days later he showed up and walked around the room and sprayed the perimeter of the room with the spray going all into the air while I was in the room. It simply is not an uplifting atmosphere within which to recuperate. I have all my faculties so I was able to advocate for myself. The nurses would never bring me my IV vancomycin antibiotic on time, which is supposed to be a strict every 12 hours. Once she was five hours late despite my going out to the nurses station and asking over and over and over again. I had a meeting with the heads of the departments at Bayview and they said they would take care of everything. Well, the next day, nothing has changed. If you dislike your family member, stick them in Bayview and walk away.

This facility was originally managed by Civitas and is now managed by Experience Sr Living. It's a very attractive facility. I 'rent' a suite which is one bedroom with bathroom and kitchen. It's expensive (Of course that's in each individual's judgement. In my case, it's base rent (move your own furniture) plus a Level of Care adder so my total is nearly $6000/mo Three meals a day are good choices and consider nutrition importance (ie.fruits and vegetables) i give them high ratings on meals with variety and preparation. Also included is weekly housekeeping/cleaning and laundry. If I had to choose again, I would stay here. I need the care and as a stroke victim, my care is a heavy burden on my wife so she needs support. The staff is well trained and responsive. I've had experience in other assisted living facilities. There have been occasions when I have been angry about issues and I blame the manage. When I started writing this I intended to say, if you are looking for an assisted living facility, stay away from any place managed by Experience Sr Living, but now that I've written it, I'd have to say their pretty good

She answered so many questions and addressed all my fears, allowing for an easier and more comfortable transition for my dad. The staff is fantastic, and my dad loves participating in the activities and has made so many new friends, which means the world to him. The food is great too...kudos to Luis, an outstanding and totally awesome culinary chef! We are so blessed to have found Symphony at St Augustine and highly recommend them for any loved one who needs their services. The stress and worries I had on a daily basis are now gone. All…read morethe staff are very very nice, professional and caring from the administrators, nurses and assistants to the servers in the dining areas.The facility itself is brand new, beautifully decorated, very nice and clean and the unfurnished patient rooms are private and spacious. Also they have a park-like center courtyard, an enclosed patio with tables and chairs also a couple of different seating areas including TV rooms and a private meeting room. The food is actually pretty good too! Three meals a day are served plus some refreshments are always available.
