Some of the people here were very nice but overall their service was lacking : Mom is a diabetic and she was consistently given high sugar content The rooms are 3 to a room and too crowded. When they try to rest the hallways are noisy with staff chatting. Overall not satisfied read more

Summed up: -(proxy's don't matter)They have no care if patient has proxy or not. You will not receive updated unless you bother them and it will be known over phone you are bothering them -(trying to charge extra for things insurance covers) my grandmother was suppose to transition into hospice at the facility. This never happened and they in fact said they needed her full SS check if we wanted her place into hospice (her insurance covered it) they explained to my mom and aunt that insurance pays less for hospice patients they are at a financial loss and they need to cover the rest. Or we can move her at own cost to another facility willing to take her in as hospice. When I called to say that I was her proxy and all calls needed to come to me and that I would have her moved they went behind my back and got approval from a family member(not her proxy) to place her as if she was still in rehab because it pays more!!! They said they planned on charging insurance as a rehab patient but would treat her like a hospice patient so the wouldn't lose money! i said "no" I called our social worker and they called back to say it was a mistake and that all is okay. I plan on filing a report they still charged as a rehab and they basically just let her lay there. -(bedsores)when I did go see her she had developed more bedsores that were left untreated and if you have a family member that develops them good luck having them changed once a day! Her sore developed into stage 4 pressure sore... they did not offer her any medication for her pain when I would call and inquire they would say the doctor said she didn't need anything. She had oral cancer and was in a lot of pain!!! -also in that visit she had an IV that had tons backflow(meaning IV bag was empty so long that it had started draining blood) I mentioned to the nurse that she needed a new IV bag she was never given a new IV bag while I was there. I wish I could've just switched it by myself. By this point my grandmother could no longer communicate. Still no pain meds. Silent tears. My heart broke. -she passed away. Again after multiple times of informing them I was her proxy I was never ever given a call to let me know she had passed. DO NOT ALLOW YOUR LOVED ONE TO END UP HERE!!! read more

This facility does NOT provide adequate care for their patients. It seems all they care about is having bodies in the census to collect revenue for. Actually providing the care is too much to ask. The fastest my moms call button was answered was 75 minutes. Often it was over 2 hours before a nurse would come. Want something more reliable than easily forged reviews? **Update to my original review** The facility did end up reimbursing my family for the lost pajamas, replacement of the lost glasses and a new necklace. While we are happy they "made things right" it is still upsetting our family had to go through this in the first place. The necklace held great sentimental value as it was my grandmothers who passed years ago and he should have had better care from the very beginning after being released from the hospital. Please do not leave your loved ones at this place unless you want to see them deteriorate in a matter of days. My grandfather was admitted with a fractured pelvis, and in less than a week had bad sores, pressure points. The office manager even accepted the delivery of his chair cushion and used it for herself on her office chair without checking that the pillow was delivered for a patient. They also stole his gold chain off of his neck when undressing him, and lost other personal items including pajamas and his reading glasses. Had he not had local family to come and check on him during his rehabilitation, it scares us to wonder what could have happened due to the neglect that this facility provides Another mention is that yelp warns you prior to leaving a review that many positive reviews from the same IP address have been left for this facility meaning they're padding the negative reviews with positive ones from the staff! read more

Santa Clarita Post Acute was an incredible blessing for my mom and our family. From the moment we arrived, the staff showed unmatched care, patience, and love. The CNA's, physical and occupational therapists were phenomenal--smart, experienced, and intuitive. They were gentle when needed and more persistent when it was beneficial for my mom's progress. The management team was attentive and proactive, always prioritizing her care, and the nurses went above and beyond, providing thoughtful, attentive, and compassionate support. We were nervous about what rehab would be like, but Santa Clarita Post Acute exceeded our expectations in every way. My mom felt safe, cared for, and genuinely valued during her stay. She even made wonderful friends along the way! While we're excited for her next chapter, leaving this place feels bittersweet because it truly became a second home. The nurses took really good care of my dad. Everyone was extremely nice. They made sure my dad's appointments, transportation, and meds were on track with his needs. When I took him to his primary doctor he said he was impressed with how well they had managed his meds. read more

* Late to post, this is from an October stay. My mom (83) was transfered to SC Acute from the hospital after breaking her ribs in a fall. I was very afraid that it was going to be some sad, dreary, smelly place. However, to my surprise, it had no odor; it was very clean; and the staff was kind, helpful and very friendly. My mom had physical therapy two times a day and they never skipped a day. In fact, they never hurried her and they really encouraged her. That PT staff was amazing! She was cleaned and changed frequently and her bedding was changed daily or sometimes 2 -3 times a day! (best CNA's ever)! They are very organized and efficient. The place was decorated for Halloween and they had a wheelchair parade which we joined, together with a pizza and ice cream party. And, they offered finger nail polishing on Thursday! The only stressful part was having to appeal her discharge when insurance said she was ready to leave (when she wasn't)! Still, they gave me the number to call and followed up closely (they did approve additional days). Three to a room is tight, but her roommates were very sweet to mom. SCPA is a 99 bed skilled nursing facility. I have been working in this facility since 2015, new management took over in June of last year. While the previous management was a Mom and Pop, and this new management is a corporation, there are pros and cons under each management. One thing good is that this new management has enough staff to provide the best care for our residents, and renovation is happening routinely to improve the aesthetic aspect to match the excellent care provided to all residents, and families. I would rate this place zero stars if I could. Unfortunately Santa Clarita only has this rehab place, which really isn't rehab. They don't help u, if you are in pain they don't try to solve for problems just leave u in the bed. My family member is diabetic and serving them regular soda. Come on people, you put our love ones in there for care and they aren't getting it. Very disappointed with our experience. I will never put any loved one in the place. Also. My two star rating has less to do with staff and more to do with the actual rooming conditions, which I would refer to as human warehousing. My partner was in a room with two other persons that had so little room between beds, that I had to remain standing as there was not enough room to sit on a chair next to the bed without pushing into another beds space. There was no bathroom in the room necessitating a portable potty for a patient. Got my partner out of that place as soon as I could. Reminded me of the horror stories about nursing home facilities. read more

Everyone here treated my mom with dignity, care and respect. I have experience with nursing homes, as my brother is a quadriplegic. I've been to Many. The workers at Santa Clarita Acute gave my mom that little bit of extra Attention you would hope for your 97 year old mom. I could feel the care they Gave her in my heart. It's really something when a nurse tells you, "I HUGGED HER BEFORE I WENT HOME." My mom passed away that night. Very happy with the care my mother received for the last 6 years. I have noticed that since it's under new ownership there are so many more employees working there. The only thing I could possibly complain about is the lack of parking available for visitors. The staff was wonderful to all of my family. Where do I begin to thank everyone? I don't usually write reviews but this facility needs a recognition of excellence! After a broken hip, followed by a successful surgery my father in law was transferred to this hospital. He has been getting great care from the day one. He is always clean, smiling and happy interacting with all the nurses who are very caring and personal. You don't feel like you're in hospital. All the staff, including the office workers are just amazing!!! They know what they are doing and I really believe they love their job. It has been over 4 month since he's been there and we can't think of a better care our loved one could be getting. Thank you all for your dedication and great work. read more

My father was a resident here for a little over a month in 2018 after hip surgery so that he could recover from the surgery and get the physical therapy that he needed. Having him go to this facility to recover was one of the worst decisions of my life and I regret it every day. It's only now that I am able to write this review. In a little over a month he went from being an extremely alert person with a healthy appetite to a confused, malnourished person with head injuries. The facility neglected to supervise him and he fell multiple times, one time suffering head injuries that landed him in the ER. He had several bruises all over his face every time we went to visit him, which was almost every day during the time he was there. After that his cognitive abilities started to decline quickly. The facility also lost my dad's dentures but refused to replace them and he was not able to eat much of the food as a result. He ended up malnourished and in the hospital, which resulted in further physical decline. He eventually passed away after a few days in the hospital. read more

Our beloved relative was a resident at this hospital about 6 weeks. We were aware of the past reviews which were negative. When we needed a facility nearby, my husband remembered this facility was under new ownership. We dropped in without notice, and were impressed with ambience. A tour of the building, including the physical therapy room, gave a further positive impression. The staff in the therapy were attentive to the residents, and spoke with them in a kindly professional manner. Our relative was transferred to this hospital and we visited often. She was seriously ill, and we knew it was simply a matter of time. The staff were always willing to talk with us regarding our concerns. It was a simple matter to talk with the proper person. We are aware of the care needed for an elderly person who cannot communicate. She was always clean and well taken care of. During our visits, we never saw anything that I would see as a negative. This has been my first review in Yelp, and I am doing this because of the positive feelings we had for the facility and staff during a heartbreaking time for our extended family. read more

I will NEVER forget this DUMP my grandmother ended up in (although it's been several years..yelp wasn't around to review it) When I drive by my stomach sinks, because I think back to when we regrettably left her here. She was here a very short time before enough was enough. I remember on several occasions she ended up at Henry Mayo because of a bladder infection and dehydration. Obviously too many people an her needs were neglected. I remember on one occasion seeing a nurse continuously adding food into an elderly mans mouth while he was choking (his head seemed to be permanently tilted back..and he needed to be fed slowly). I also recall the nurse's arguing over it. I went straight to the head of staff to complain about what my uncle & I witnessed. It was heartbreaking. So please really consider this care before leaving your loved one here! Side note: We (like many families) unfortunately were limited in choice's due to her being on medical.. we were very limited on nursing homes. In the end my mother decided to pay out of her own pocket to get her out! It's worth every cent! read more

I can only give a review based on a very short visit to the front lobby. I was cleaning out my parents house and found cases of unopened diapers and bed protectors. The receptionist was very pleasant and then we met someone from the office. She said they would take these items off our hands, etc. just from my first impression, the place seemed clean and didn't smell like some nursing homes I've visited. The one bad thing about this place is their location. It's a very small driveway right before a signal. I'm surprised more cats aren't rear ended when someone enters this place! Then you drive up a steep driveway into a strange lined off small parking lot. There's a few spots marked visitor, a few for doctors, and the rest aren't marked... probably for the staff. read more

Although the staff was very, very nice and alert, and clean, this is an extremely noisy environment - especially for people who are trying to recover. I've only seen a few nursing homes - all medi/medi but never saw one with 3 patients to a room. Some rooms had 2 patients, some had 3. The loudspeaker was so loud I had to cover my ears. My Mom spent 1 night & asked me to come pick her up. There was another patient who was cussing loudly all night. One of the staff was walking down the hall talking/shouting out to a co-worker, so her voice carried all the way down the hall. The call button sound is also very loud & non-stop. It's too bad it's the only one in Santa Clarita covered by medi/medi. read more

If you're reading this, I know this is one of the hardest times in your life. I don't understand why places like this are so discussing, dirty, and able to make so much $$ off if Medi-care and remain open. This first day I had to transfer my mother from Henry Mayo to this location being as it is the ONLY option in SCV that is covered under Medi-Care. Only 2 hours in and I receive 4 calls at 9:30pm that my mother doesn't want to stay there and I need to pick her up. She was outside on the open patio alone and they didn't even know where she was. It's 48 degrees outside,REALLY does anyone WANT to stay there? I know you don't have many options and they are all horrible places. The sheets were stained and discussing, the bedspread had blood stains all over it. The room was disgusting, broken doors, no closet and shared a TV with the roommate. The bathroom OMG so gross and remember this is only 2 hours in. Don't let the LOBBY fool you, and giving 4 stars because the LOBBY was nice should not even be accepted as a review. 0 stars and I'm sorry that you really don't have any options that are decent. They 3 they will give you are all horrible. read more

So it's been a few days since my dads admission. The staff seems nice enough. Although I've only interacted with CNA's. And the nurse who greets me at the front desk is nice. The living conditions seems decent, clean. Unfortunately the air conditioning has not been working since his admission. I understand that some things take time but it's not acceptable to have elderly sit in this heat. Something needs to get done. After every visit I have a headache from how hot it is. My face is super red. I hope something is done soon. read more

THE. WORST. DUMP. EVER. Parent had to recover here and it was dirty, so many things broken (faucets, closet doors, beds, wheel chairs) and the wall paper, wood veneer literally peeling off the walls. Parent was drugged with Xanax every day to the point of losing bladder control. Then they would just leave her in her urine soaked street clothes, pull up a sheet and turn off the light. Often they didn't even offer a meal. This place could easily kill you. The staff is mostly foreign. So don't expect much english. Most of the staff are not nurses. They have maybe one real nurse per shift and indifferent staff that sit around gabbing to each other. They also lie about performing duties that they never performed, like giving essential treatments. We were there in shifts and saw it all unfold like a nightmare. You can see that none of the money is really spent on patient care or better staff. THIS IS WHAT HAPPENS WHEN HEALTH CARE IS RUN FOR PROFIT. read more
