I highly recommend this place. This is my dad's third stay here, and we've had another positive experience. Yes, sometimes you have to be patient because the nurses are very busy, but the majority of them do a fantastic job and truly care about their patients. A special thank you to Nautica, Cindy,Alexandra, Nicole,and Desmond for the outstanding care and compassion they showed my dad. I also can't say enough about the physical therapists, Dawn and Mariah. They were absolutely incredible and bent over backwards to make sure my dad received the best care possible and was comfortable every step of the way. Overall i would rate approx 70% which is excellent for a nursing care facility. By husband was there for a respite visit, several of his items were missing & couldn't be found. A brand new black neck pillow & a pair of black hand cones. As well staff might consider taking a complete thorough assessment on new admissions. My husband is total care with Vascular Dementia, he won't drink on his own & of course he can't feed himself. Being given a sip of water throughout the day wasn't priority for staff. Feeding while food is warm wasnt a priority. The building was bright & clean & most of the staff was friendly. However, the patients must be the main focus & concern. He was found on the floor several times & it wasn't an urgent matter for the staff. Hospice care was notified. Afterwards my husband was welcomed back for another respite visit on another unit. My father was just admitted this afternoon from the hospital for rehab. Granted he has only been there a few hours and the staff we encountered were nice, BUT the condition of the building is horrible! 85 degrees out and no air conditioning in the room. Told we were not allowed to open the windows. Carpets are badly stained, walls beat up and the place smells. We are going to do everything we can to get my father moved to another facility ASAP. read more

My one piece of advice if you are considering Shelby Nursing Center for your loved one. DON'T. My mother spent approximately three weeks at Shelby. The status quo for the staff is "I don't know" or "I haven't seen her yet so I am unable to assess what is going on" or misrepresenting the amount of food my mother was eating. I spoke directly with Nurse Practitioner Paula (I was out-of-state for the first week my mother was at Shelby) who reassured me that my mother was eating 50 to 75 percent of her meals, but family members reported that my mother was eating only a forkful or two, with limited water intake. Even when we reported to another nurse and Nurse Practitioner the lack of eating, we were brushed off. One nurse was more concerned that we sat on an empty bed than the state of my mother. The Nursing Director also claimed not to know my mother was not eating. Additionally, Charlotte in the 200 wing of the rehabilitation section is rude and uncaring and should not be working in a rehab facility. My mother continued to rapidly decline, and it is my understanding that after two days of not eating the facility should have transported her back to the hospital if they were unable to care for her. This did not happen. She was also moaning constantly. We had to have her admitted to the hospital ourselves, where she received in-hospital hospice care. Realize that if you admit your loved one to this facility you will need to advocate for your loved one continuously. State of Michigan Ombudsman 1-800-882-6006 Select Option #1 then Option #3 My mother was discharged from the hospital to this center, while it was the third choice on our list it still had decent Medicare ratings.... Boy was that wrong. Within 3 short days, my mother's health significantly declined and went unaddressed. By the 5th day I demanded EMS to be called even though "she was fine". Apparently a blood pressure of 70/50 is thriving. My mother was severely dehydrated, had blood in her stool and we were not informed, fluid overloading her lungs and had an infection on the verge of being sepsis. On top of her being put in diapers unnecessarily, she was left in a bed with urine soaked cloths for hours at a time because of their lack of response time and laziness. My mother was 1:1 assist to the bathroom due to fall risk after back surgery, but no where lacked the ability to get there or needing diapers. I gave the Administrator the opportunity to discuss this matter privately, however have not received a phone call back. I will make sure I make as many people aware of the safety & quality of care issues, lack of compassion, and inability to properly manage a facility know. It's quite apparent based upon LARA though the State of Michigan reporting (see pictures attached) that complaints are a common occurrence for this facility. read more

My 105 yr old father was recently a patient in Shelby Crossing Rehabilitation facility on 21 mile . He had suffered a fall. The care and rehabilitation was personalized and excellent. The nursing staff was prompt to assist and he is leaving the facility in much better condition than he has been in several months. read more

The one star is because PT was good to my brother. Most of the nursing staff and assistants were awful. They were negligent, uncaring, and rude. if you need critical meds Do NOT go there. My twin brother was found unconscious one time due to the poor management of his insulin. There were excuses for why it took 3 hours to get him his insulin, but I was assured that it wasn't normal. it happened a few times. When he unexpectedly passed away at 67 years old last week I stood outside for 10 minutes waiting for someone to let me in after telling me someone would be right there. I was let in because of a transport person with a patient saw me and I yelled to her. She went and got someone, who actually took her sweet old time getting to the door. They may be short staffed but when I consistently saw workers standing around the desk who wouldn't even look at me every time I was there, which was almost daily for the past month, I believe they just didn't care. read more

My sister fell in the middle of the night on a Saturday in November 2024 while in rehab for hip pain. She was not taken to the ER nor X-rayed. We were not informed until Monday. When we insisted on her being taken to the ER since she was complaining of greater pain, a displaced hip fracture was found, requiring surgery. I would not recommend this place for anyone needing medical care while rehabbing. And I am not angry (as suggested by another reviewer), just pretty disgusted by the standard of care. Don't be misled by the new decor. No complaints about the rehab staff, they did what they could . read more

Rehab staff is awesome/ which is the reason we're there/ strictly for the rehab/ our family member had to have emergency brain surgery due to a bump on the head & being on a blood thinner.. it caused a brain bleed. So we needed to have some intense physical therapy. The quality of care is substandard in every other area / our family has been there 19 days and not once have sheets the sheets on their bed been changed .. which we were made aware of on day 17 .. there have been a few CNA's that work their tales off ( I would recognize them by face/ but I'm sorry I cannot give you names .. for the most part the nurses have been awesome / on the afternoon shift only .. not all, I'm saying most. Make sure your family has a DPOA ( necessary in all facilities now ) I don't know where they hired their kitchen staff from / the food is terrible .. always cold, portion size is a child's portion & the taste is absolutely horrible, they offer substitutions, but good luck getting any, we kept being told, I just started maybe you need to talk to your nurse ( this is directly from a kitchen staff member/ twice ) We've had to bring our family member food & snacks everyday. This is not acceptable for any client !! The social workers do not follow through/ funny how our Neurosurgeons office got through in less than 5 minutes, they're telling us our family member now has Covid .. left our family member in a room with the door shut until our Surgeons office made a phone call ! As soon as the call was made our family member was moved to a private room ! Are N95 masks being worn .. no .. Covid is not something New, it is an upper respiratory infection that's been around since 1968. Seems awfully strange our family was asked if they want to still go to physical therapy .. so you're telling me you are willing to infect all other clients, not acceptable. And why are you putting clients admitted with Pneumonia in a room with a client recovering from Brain Surgery ... absolutely unacceptable .. Uh oh .. another phone call in the middle of the night, they want to give our family member a Covid 19 vaccine when they supposedly tested positive yesterday morning !! DO NOT PUT YOUR Family member in this facility .. is insurance fraud a possibility here, Hmmm .. pay attention folks !!! Your family NEEDS YOUR SUPPORT !!!!!!! Mistake after mistake has been made since day one, paperwork was mixed up and our family was told they signed a DNR order ( it was a serious mistake, and the person that made it should have been immediately fired ) We were told the mistake had been corrected, but continued mistakes are occurring . If in fact our family member has Covid, they are not following CDC guidelines they are telling our family they need to stay an additional 15 days ... CDC recommendation is 5 days with no fever present. This is public knowledge folks !! read more

Reviewers tend to be angry, and given the general state of nursing-home care, my expectations of Shelby Rehab Center were full of trepidation. BUT, I was very favorably impressed during my father-in-law's month-long stay. The receptionists are pleasant, the facility is clean and pleasant (I appreciated TABLECLOTHS on the dining room tables, and live music during FREE ice cream socials!). They'll happily label your laundry for you, and the rehab facility is just plain impressive. The very BEST thing is the staff. Dad's main nurse, Moses, was AWESOME; knowledgable, welcoming, diligent. The rehab people and social workers are great. I appreciated so many friendly and helpful people there! (The night and weekend staff could be sparse and a little... uhh... lackadaisical... but that's honestly what I expected to see across the board, given prior experiences elsewhere.) If anything needs improvement, it's communication. Therapy staff contacted us once or twice with great information, and the head of therapy made time to answer my extensive questions, but it'd be great if families and patients knew to expect a weekly update call, and if therapy schedules could be posted on patient whiteboards ahead of time (Dad complained of having naps interrupted at random times). But overall, a very happy experience with Shelby. Thanks to everyone there! read more

First off my orthopedic surgeons, Dr. Lindsay Maier & and Dr. Fitzgerald we're outstanding. My nurses, nursing assistants were empathetic and compassionate. My Physical Therapy and occupational therapy were awesome. Specifically, my at home, Physical Therapist, Kyle, was exceptional. He encouraged me with a can-do attitude. IF YOU CARE ABOUT THE HEALTH, SAFETY AND WELL-BEING OF YOUR LOVED ONES PLEASE READ THIS BEFORE ENTERING THEM INTO THIS FACILITY! My father came here after being in the hospital after a fall. He has very bad knees so his mobility is not the greatest, he went into this facility with hopes he can get the PT he needs to gain more strength so he could become more independent and hopefully not have any more accidents. Well let's just say he left this facility in WORSE shape than he entered it. For starters when he was transferred there he was not receiving the medication he needed and was prescribed in the hospital. One of the medications he needed to take was a water pill for swelling in his legs. He was in the facility for weeks before he was given this medication. This led to IMMENSE swelling in his legs, which made them heavier than normal, which meant he was unable to move his legs as well as he could before. Eventually this also led to an accident that took place in the facility. He was laying in bed and the weight of his leg slipping off the bed led to him falling out of bed and hitting his head on the nightstand next to his bed. He landed on the call button and was unable to move himself enough to actually use it. He screamed for at least 30 minutes before ge was actually able to get a hold of the button. Once he pressed the button he had to wait an additional 30-60 minutes for someone to even come to his aid. After sustaining a head injury and what he felt like could be a fractured wrist and ribs no medical attention was sought. He was in their care for three days without ever having his injuries addressed. NO MEDICAL ATTENTION WAS EVER GIVEN AFTER A PATIENT FELL AND HIT THEIR HEAD. How is that even okay? My father eventually signed himself out and was transported to the hospital for medical care. The staff here did not care. The nurse we spoke to tried to empathize but I honestly don't believe she cared. My father spoke about the nurses coming in and out of his room smelling of heavy marijuana. If you are working in a medical facility "taking care" of elderly and disabled patients you should not be high while doing it. Please do not send your loved ones here. This facility is beautiful but looks can be deceiving. read more

Worst place in Michigan... I would never ever send my parents to that home.....Very poor run, nobody gives a damm, like your not even there.....had to tell 3 people about things that needed to be in the room......any still no body listened......they have the worst nurses there, and all just a very shady place where most employees if not all, even managers, only care about money and nothing else. took all day to get release to get over to another location. had an employee walk into to my dads room and wasnt even suppose to be in there and pulled down his bed sheets and you dont want to know...... took all day to even talk to someone in admissions.....like the place was deserted......horrible place....dont waste you time or money to go there...there are better places....that place has really changed form what it use to be..........POOOR SERVICE....!!!!! read more

I don't often leave reviews, but this is definitely needed after the HORRIBLE experience my father and family experienced at this place of "care". Two "nurses"(if that's what they call themselves) treated my father/family like a bag of trash. My father came to said location after a hospital stay for "rehab" to gain some strength before he came home. We've had CONSTANT issues with two "nurses" that were supposed to be providing care for my father. He cannot feed himself and they were told that on multiple occasions. So why is his tray of food sitting there with his medicine on it? Noone there to assist him. He presses the call button, they turn it off without seeing what he needs. Let him sit with a blocked catheter for 12 hours after continually pushing the button for help. This went on all day. These 2 "nurses" treated my father like SHIT. I will not tolerate that. The state has been notified and I pray noone ever has to put a loved one here. I'm not here to listen to the excuses either. THERE ARE NONE. These are only 2 examples out of 50 I could put here. The head nurse was helpful it seemed. Let's see if she puts her words into action. State has been notified. read more

As of now, it's so/so. They do need more staff. Anytime someone there has to use the bathroom and help with shower the residents has to wait a very long time. The food for them is ok, but you'd think with the money they get from insurance, the food quality would be a lot more. I hope they hire more people to assist! read more

I was here at the beginning of the year. Since then this has been sold. It was a part of Beaumont Hospital. Back in March there were three aides per room. Someone brought the food and took it away. Had a person who gave showers. Now it's all on one aide per room. The aides and nurses are overworked. 12 hour shifts which they like but let's face it a long day. I've had to wait for an aide to assist me to the bathroom. When you're old that becomes an eternity. Ask for pain med and the nurse forgets the request. The food isn't as good as what it was under Beaumont. I needed therapy for my broken ankle. Got that, been here a month. Cannot wait to go home! A bad roommate, constantly complaining. Screaming when the aide isn't fast enough. I've requested a room change. I'm on "the list". If I need a facility again it will not be here. Yes, the people are terrific. Just overworked. More cleaning people than aides. A shame this had a fine reputation when it was a part of Beaumont. read more

Would give them a zero if I could!!!!This is the worst place I have ever encountered!!!!!!! Very poorly staffed. No continuity between shifts. No communications from Drs. Only proficient staff are the Therapists. That's if your patient survives the horrible medical care!! My husband has been there for two weeks. He is worse now than when he came. Has been left alone to scream for help for over 45 minutes at a time! They have call lights that aren't answered for more than an hour. They don't bother feeding the patients that need help and remove trays after 30 minutes even if patient hasn't eaten. My husband has sat in a wet diaper for over 8 hours! They also falsify their daily records. yesterday my husband had a respiratory level of 83%. The tech said she had to get the nurse. She showed me the screen which did say 83%. Nurse never came. 2 Hours later, someone came in to give him his medicine, and went to get last O2 report, and said it was written up as 93%. . If you ask to see who in charge, no one comes. Long story short! Don't send your loved ones here unless they can take care of themselves!!!!!!!!!!!!! read more

the worst place for care, treatment, rehab. 80 year old grandpa admitted for abt 2 months. showed no care and just try stealing money and insulin from patient. if i could give 0 stars i would. dont care for patients. tried no physical therapy w patient leading to weaken legs causing falling now. tried keeping insulin from patient after he was admitted, only giving us the pen and nothing else. after we called they said they don't have it, even the manager. we had to call the pharmacy for them to show that is was delivered there. just a very shady place where most emploees if not all, even managers, only care abt money and nothing else. asked an employee at a desk w another employee abt something i needed done in the room cause he made a little mess and i needed like another cloth or blanket and goes " that isn't my area" highly highly advise taking loved one else where for care instead of here!!! read more

BEWARE THIS PLACE IS AWFUL!!!!!!!!!!!! My father was sent here for rehab after a fall. Let's just say after being there for a couple weeks he's in way worse shape than he was when he went in. He fell out of bed a couple of times and we were never notified. When he fell out of bed he had to wait 45 mins for someone to come help him. He busted his face on the nightstand, hurt his wrist and ribs. He was never checked out by a doctor after the fall. His head and eyes are black, blue, and swollen. He was complaining of a headache too. If you saw how awful his face looked you wouldn't believe that not one person tried to get a doctor in to see him! ALSO He needed help to get up to go to the bathroom and they didn't get him up in time so he ended up urinating on himself and they stood there and laughed at him. This place is very unprofessional. Some of the workers even smelt heavy of marijuana. Which is very unacceptable while working with sick people. I don't recommend this place to anyone unless you want your loved one left to die. read more

My mother suffered a stroke while here. Complained daily of symptoms and no one did anything, it took them a week to have her see a doctor. He immeditaley sent her to ER. She missed the time to take the stroke medication and now will have more complications. They left her in the bathroom unattended for 45 minutes at a time and they lost her laundry and other belongings. No elder should suffer like this. Incompetency and neglect. read more

When Beaumont owned Shelby, I was there for therapy. It was good. Since the new owners, it has really gone downhill. Went from five stars to one star. The aides run in and turn the light off and never come back to help the patients. Everything gets taken including a good wheelchair. When you complain, they give you the toughest oldest wheelchair in the place the patient can't possibly operate. Many employees are quitting so no one cares about their job and do the absolute least amount of work they can get away with. Aides are not trained on the lifts and hurt the patients. read more

If I could rate a negative star I would! My mother wanted to do her rehab at Shelby because they took such great care of my father a few years ago. But, unfortunately my mother did not receive the same care. No nursing staff, the aides do not care to help those that need assistance, calling Shelby to talk to a nurse or supervisor is impossible. They leave patients sitting on bed pans for hours, the "gourmet food" they advertise is a joke, cold dry food at every meal, if a meal is delivered. Getting water was a battle. Nursing staff doesn't know how to administer medications, they let patients with dementia wander the halls and enter other patient rooms. My mother was there for a month and was only given 2 showers, because, no one had time. They delayed progressive physical therapy by a week, because nursing was too lazy to enter doctor notes into the system for PT. Also, my mother required an X-ray for a follow up appointment, and they didn't give the X-rays to my mother on her way out to the appointment, causing her to have to be re-xrayed at the doctors office. I could go on and on. Family members need to stand up and be the voice for their loved ones. nursing homes should not be able to continue on this way. read more

I am quite happy with the treatment my father has been getting at Shelby Nursing Center, the people treat him like family according to my Dad. I really need to make note of some of the staff members names, as I want to make mention of the kindness I have seen them show to the patients. My mom is in rehab at this facility. From the first day we got her checked in, she has had a positive experience and so has our family. The staff is attentive, friendly and helpful. If you have questions or problems, they will do their best to assist you as quickly and as well as they can. The physical / occupational therapy area is a lot larger and so much better than what I imagined it to be. The facility is clean and well kept. Couldn't have been happier with the staff and facility during my mother's stay for the last weeks of her life. They were very attentive and always available to assist Mom and answer questions my sisters and I had from time to time. The facility is clean and doesn't have the typical odors associated with nursing facilities. They have ice cream socials everyday and entertainment from time to time to make a sad situation as bearable as possible.
