    I am looking for past clients who stayed at Sierra Care Clovis for post surgery rehab. My mom is having a colon tumor removed and resection, she will need to heal and rehab in a SNF, would like feedback on this facility, has anyone stayed here?

That he wasent being fed or offered food and he looked near death when leaving. I was told that he refused physical therapy and food. After one week at a new facility he has gained so much weight! He looks happy healthy and is doing much better! They send me photos of him eating all his food and pt said he is going great. I'd hate to say what I think would happen if he stayed at this facility another day. I add that I was lied to by staff and I would not send your loved one here if they have any sort of a mental injury.

Had this happen before, and luckily the night shift nurse gave me her cellphone number before so I could check in when stuff like this happens! I am really disappointed in my loved ones care, and I am already scared to post this. They treat them like an animal just because they are a jail convict! They tell me how cold the food is! How rude the management is! And they told me they have non certified staff members that are giving them showers and doing their care! I have been begging my family members jail case worker to move them facilities! The case worker even told me how many complaints they received from Grand Villa from other clients! Why do they keep sending them here!!! Dont let the beautiful building deceive you! They dont even have proper nursing staff! My loved one told me they cant keep staff! They have gone through nursing directors left and right! The nursing turn over is ridiculous! All the managers quit because of the owners! The owners are just in it for the money and dont actually care about the patients or employees! They pay their front desk worker minimum wage she told me last time I visited it! Of course they would pay their staff bare minimum if it meant they can pocket a profit! I would give negative stars if I could! Absolutely disgraceful to call this a care center! Shut it down! I will be calling state!

    I've stayed here for 4 weeks recuperating from a severe sprag to my right ankle. Was not broken…read moreonly strained. I fell from a 2 ft step trying to reach a high shelf. Had to call 911 & was taken to St Agnes for xrays & ultrasound, spent the night then was brought here to Oakwood. I rate them 5 out of 5 stars, clean & safe. Food is great! Staff is caring & readily available. Physical Therapists are top rated in my book. I'm stronger than before my injury & plan to continue working out to get even stronger. Occupational Therapists have taught me how to prevent future injuries & ways to strengthen every day practices. I give top rating all around. God Bless them all.

    Adding to yesterday's (6/24/26) review…read more Oakwood had ne pick up my husbands belongings. Today they called the hospital inquiring when will my husband be back at their facility. You told the hospital you didn't want him back for whatever reason. ( let's see being quiet, being cooperative etc). They had me pick up his belongings per protocol and they don't have storage. (Bologna Ham and cheese). No one can call me back or explain what's going on when I followed the policies and now you want him back. (Very fishy) This is a terrible place with unprofessionalism and character flaws. They don't care about the people they just want the amount of people so they can get more money. This place should be investigated and restarted with caring people because this isn't the place. You can't treat people anyway kind of way and expect them to take it. We weren't treated fairly or courteous and now you want him back. Stupid. I tell you there's something suspicious going on there.

We…read morewere very pleased with their care AFTER my grandpa was put with a one on one, due to a fall being there two days after hip surgery at 94 years old. My mom had kept telling them he wants to get up he thinks he can get up and walk also having vascular dementia. He was stubborn and kept wanting to get out of bed nothing was done for that and he ended up falling, how long was he there on the floor we the family aren't sure. I was in the area that night and when I got the call to go check on him he was sitting in a wheel chair no blankets, no socks, right in front of the nurses station. I was really upset and asked them why was he sitting here cold and no socks or blanket. I asked if he could have socks on and the nurse said "sure" so why didn't they put them on him? I put some and checked him out made sure he wasn't seriously hurt. He was sore on arms and back but he said he was ok. After that incident he had a one on one CNA 24/7 due to being a *fall risk* . That is when my mom who was there daily as she had been and learned all the CNA's who would be taking care of grandpa for his stay there. We were lucky to get some very special people who really showed they cared for their patients! Ones who took the time, who listened to grandpa and who would converse with him. He was one strong old man and he was a character, we are saddened he passed there with my mom and I by his side. The dementia only got worse and aggressive and he lasted two and a half months. We are so please with the CNA's that we got close to Kay, Bianca, Niezel, April, Mylee, Manpreet, Robeil, Kobe, & G. Most importantly one who always made us feel comfortable leaving at the end of the day was Jennifer (PM shift). She would make sure he was cleaned and comfortable. She was amazing and made this experience at this facility comfortable, that grandpas was in good hands. We truly appreciate you and all you do for your patients. For your patience, for your care, for your love, for the concern and for the Laughs! Thank you all so much!! You can see the tiredness on their faces, the patients who get overlooked, it is sad to see them get neglected and not saying purposely but it happens they get overlooked to be changed or helped with feeding (which was observed while there). We need more people in that career who have a heart not just do it as a job because the attitude and the looks and their mannerism shows they feel other wise and for a family who has a loved one in there for whatever reason, who have to deal with a person who doesn't want to be there isn't a good thing. Observed the LVN's being rude and snappy with family. We really cared for Dona, and Fatima. Thanks to them! My son also made some friends who we plan to go back and visit soon! Alan, Charles!

They took horrible care of my grandpa and…read morewhen we finally moved him out he was severely malnourished to the point where he couldn't even open his eyes. They refused to feed him because he was unable to feed himself and would mark it as refusal to eat when really he couldn't feed himself or give himself water. We finally moved him to a new home, and the nurses and hospice could not believe how badly he was neglected at horizon. The only good thing about horizon is RICO! He was awesome !!!!

After my…read moreconcerns were raised, my mother's missing item was returned, and the staff made a genuine effort to ensure she felt comfortable and supported. That meant a great deal to her and to our family. My mother is in rehab and relearning how to walk, and she now feels safe, respected, and cared for. The rooms are clean, the food is genuinely good, and most importantly, the staff have been kind and attentive, which has had a very positive impact on her mood and recovery. I appreciate the steps management and staff took to address the issue and restore trust. Based on how things stand now, my mother is happy with her care, and that is what matters most.

The nursing staff managed her care exceptionally well, ensuring she got to and from dialysis appointments safely and experienced no complications during her stay. Their knowledge, skill, and compassion exceeded our expectations and gave our family great peace of mind. We are also grateful to the social services team for coordinating a smooth and successful discharge plan. Their support helped make the transition home much easier for my mom and our family.
