    My brother is well taken care of. The staff is great and respond quickly. He's only been there a couple days but so far so good!!!!!

It is with how our family was communicated with, how my concerns were characterized, the repeated documentation delays, the financial consequences those delays caused, and the poor transition of care. Families are already under tremendous stress when a loved one enters treatment. They should not leave the experience feeling more divided, financially destabilized, and unsupported than when they entered. While my husband was in treatment, I called the facility extremely upset because he had entered rehab without telling me where he was going. I was suddenly left managing our children, household, bills, and everything else on my own. I was asking for basic information I needed to keep our household functioning, including login information for bills. Instead of accurately relaying the context of my calls, a nurse apparently told my husband that I was only concerned about money and wasn't concerned about him. That was absolutely not what I was communicating. I was then told to leave him alone and find my own support group, and my calls were not being passed along. My husband later told me that staff had given him a very different narrative about my calls and encouraged him not to engage with me. This created even more conflict during an already incredibly difficult time for our family. The administrative side was equally frustrating. Documentation was repeatedly completed late. SDI paperwork delays resulted in us going an entire month without payment on THREE separate occasions. When a family is already dealing with someone being out of work and in treatment, timely disability documentation is not a minor administrative issue. Those delays had real financial consequences for our family. Finally, the transfer of care was absolutely terrible. After such an intensive level of treatment, I expected significantly better coordination, communication, and planning when transitioning care.

    Only now, more than four years later, have I found the emotional strength and moral courage to…read morepublicly speak about what my family experienced at San Joaquin Valley Rehabilitation Center. My father entered this facility with documented "good rehab potential." Until only weeks earlier, he had been ambulatory and self-sufficient. Yet almost immediately, Dr. Cha repeatedly dismissed his worsening condition as simply "old age." What we witnessed felt less like compassionate rehabilitation medicine and more like an elderly patient quietly being written off. One physician later intervened with IV hydration and steroids after recognizing a serious decline in my father's condition, and we saw noticeable improvement. Yet shortly afterward, according to staff, the IV was removed despite my father struggling to swallow even water. Communication with the family was deeply inadequate despite staff knowing my father spoke no English and communicated only in the Calabrian dialect of southern Italy. He was therefore heavily dependent upon proper medical oversight, attentive observation, and honest communication with family members advocating on his behalf. The medical records later obtained by our family contained troubling inconsistencies, contradictory notes, omissions, incorrect dates, and even the wrong gender in one report. Most heartbreaking of all was the feeling that my father was no longer being treated as someone worth fighting for. Despite ongoing weakness and COVID symptoms, he was discharged to a skilled nursing facility instead of receiving higher-level hospital care. Two days later, he was dead. After reading numerous prior reviews of San Joaquin Valley Rehabilitation Hospital, I was deeply disturbed by how many experiences closely resembled our own concerns regarding patient care and Dr. Cha's medical oversight. Multiple reviewers described issues involving medication management, poor communication, dismissive behavior toward family advocates, questionable treatment decisions, and inadequate patient care -- concerns that strongly parallel what is documented in our records. My concerns are not speculative. I possess extensive medical documentation, medication records, physician notes, timelines, and related evidence that I believe demonstrate serious negligence involving Dr. Cha and, by extension, San Joaquin Valley Rehabilitation Hospital. What is especially troubling is the consistency between our documented experience and the accounts already publicly described by multiple other families here. Because these matters are supported by substantial documentation, I am willing to privately share relevant records with legitimate investigators, attorneys, licensing authorities, journalists, or families seeking information regarding this facility. If others experienced similar issues and wish to compare records or discuss concerns privately, they may contact me. Patients and families deserve transparency, accountability, and safe medical care. I would never entrust a loved one to this facility.

Before the first person got there in the morning, My dad fell cause they didn't go help him after calling out 3 times. (Heard by his roommate) He was found wet as well as his bedding, due to lack of staff to help him! He had back surgery and they know what he's in there for, it's been a week! He's 86 yrs old, thank goodness he has a room mate that heard it all. Don't leave your loved ones unattended! At first they didn't want anyone staying. All of a sudden they r making allowances! Not consistent from nurse to nurse, they don't document properly. No effective communication between nurses, patients and family. Thus we, the family, r the advocates for our loved ones. The family is supposed to be learning how to help him when he gets back home, after healing from back surgery. What good r the nurses when they don't instruct or practice what they're supposed to be teaching the patient so they could be independent again at home. Care quality and staffing need improvement big time! Don't take them there if u don't have to. Be there no matter what! Thanks for reading this.

We…read morewere very pleased with their care AFTER my grandpa was put with a one on one, due to a fall being there two days after hip surgery at 94 years old. My mom had kept telling them he wants to get up he thinks he can get up and walk also having vascular dementia. He was stubborn and kept wanting to get out of bed nothing was done for that and he ended up falling, how long was he there on the floor we the family aren't sure. I was in the area that night and when I got the call to go check on him he was sitting in a wheel chair no blankets, no socks, right in front of the nurses station. I was really upset and asked them why was he sitting here cold and no socks or blanket. I asked if he could have socks on and the nurse said "sure" so why didn't they put them on him? I put some and checked him out made sure he wasn't seriously hurt. He was sore on arms and back but he said he was ok. After that incident he had a one on one CNA 24/7 due to being a *fall risk* . That is when my mom who was there daily as she had been and learned all the CNA's who would be taking care of grandpa for his stay there. We were lucky to get some very special people who really showed they cared for their patients! Ones who took the time, who listened to grandpa and who would converse with him. He was one strong old man and he was a character, we are saddened he passed there with my mom and I by his side. The dementia only got worse and aggressive and he lasted two and a half months. We are so please with the CNA's that we got close to Kay, Bianca, Niezel, April, Mylee, Manpreet, Robeil, Kobe, & G. Most importantly one who always made us feel comfortable leaving at the end of the day was Jennifer (PM shift). She would make sure he was cleaned and comfortable. She was amazing and made this experience at this facility comfortable, that grandpas was in good hands. We truly appreciate you and all you do for your patients. For your patience, for your care, for your love, for the concern and for the Laughs! Thank you all so much!! You can see the tiredness on their faces, the patients who get overlooked, it is sad to see them get neglected and not saying purposely but it happens they get overlooked to be changed or helped with feeding (which was observed while there). We need more people in that career who have a heart not just do it as a job because the attitude and the looks and their mannerism shows they feel other wise and for a family who has a loved one in there for whatever reason, who have to deal with a person who doesn't want to be there isn't a good thing. Observed the LVN's being rude and snappy with family. We really cared for Dona, and Fatima. Thanks to them! My son also made some friends who we plan to go back and visit soon! Alan, Charles!

They took horrible care of my grandpa and…read morewhen we finally moved him out he was severely malnourished to the point where he couldn't even open his eyes. They refused to feed him because he was unable to feed himself and would mark it as refusal to eat when really he couldn't feed himself or give himself water. We finally moved him to a new home, and the nurses and hospice could not believe how badly he was neglected at horizon. The only good thing about horizon is RICO! He was awesome !!!!

They said oh but it didn't touch the ground when it fell. continued pushing w/c out and left me with no call light. finally after 3 hours a nurse came in and I asked to be put in bed. well after lunch physical therapy ( female with glasses and long black hair came in and asked me if I'm ready to get up for therapy. I explained what happen. Guess what , she said that my leg didn't touch the ground. and then told me ( so your refusing therapy) I said no I'm not , I can't do it because of the pain caused by the negligence of your coworkers that were here today. well she put down that I refused and next day I'm told I'm down for discharge. Not one nurse, Doctor, case manager, OT, PT, ever looked into this situation. I told all of them and it was totally ignored. here I am still suffering , I had bars and screws in my leg when they dropped it (yeah maybe it didn't touch the ground but it was not suppose to bend. Come on San Juaquin Vally Rehabilitation Hospital. Dont your employees know what can happen if they are in a rush. I just had surgery in trauma center and had bars and pins in my leg. Everybody knew and nobody cared. it was ignored . I cried every night in pain. i was discharged because they said I refused therapy the same day of my injury. It doesn't take a genius. I WAS IN PAIN get it together, I will never refer somebody there. it is not safe. it shows that they don't care what happens to you

I…read morewas there to get rehabilitated after a serious illness that required a long hospital stay. My experience there was 100+% positive. Everyone there was very nice, kind and professional in the way they treated me. They were all on my side and supported me. This ranged from the janitors through the executive director. This was especially true of the nurses and CNAs. I did come in with a positive attitude. That was reflected by everyone I was involved with. Whenever I had a question it was answered. Requests for help from the nurses and CNAs were promptly answered. I was well taken care of. The stars of the place were the rehab team. Every weekday I was helped to work with weights to build up my strength and stamina. Also we worked hard on walking. I was constantly encouraged in my desire to build up and expand my abilities. They helped me get my life back and for that I am grateful. I saw the same excellent care given to the guys I shared the room with. The physical facility was well kept up, neat and clean. If I ever had to this again, I would want to come back.

"could hive this place MINUS I would. DO NOT & I REPEAT, DO NOT, put anyone in here If you love them. This is place is DISGUSTING! They LIE, SNEAK & COVER. Upon arrival, we were NOT greeted by Staff (Admin nor a Social worker) we weren't spoken to about how things worked there, we werent given a tour, no Welcome paperwork. Nothing was explained to us. My Mom was treated so badly here. They had people from MAINTENANCE (a man who had a brain injury from an accident) brushing my Moms hair, FORCE FEEDING HER, food that was too hot. On days my Mom did NOT want to shower, they LIED to her saying they called her Daughter (ME) & that I told them to make her shower. My Mom cried all the way down the hall (my mom had a stroke and was immobile). When these issues where brought to up to her Social Worker, they then sent in Psyche to do an eval (which no phone call was made to me about-which by law they are supposed to do), tried making it out that my Mom was SUICIDAL (another lie). This place is SICK! I YANKED MY MOM OUT as soon as that happened. If my hands werent so full, i would of filed a complaint with the State. Do NOT TRUST these people with someone you love. I wont even get into the things we witnessed happening with other patients in this Facility. Their license should be pulled! Im sure Im missing a lot of other things we went through, Im doing this review on a whim. And Nurses WILL drill the Families for info, then later use that against the patient and make it seem like the patient gave them all of the information. They play DIRTY!!     I was transferred to CSTCC in June. I was on the subacute side so there was ALOT of dinging and…read morebeeping from the medical supplies. I was here for 6 months and mostly got good and fair treatment. Staff RNs were helpful and courteous however the LVNs tended to be more robotic and too busy to introduce themselves and explain to me what they were going to do. It was mostly "here's your meds!" I enjoyed my stay there for the most part. Activity staff were great like Willy, Hillary, and Jennifer. From top administrator Leslie always had time to talk to me AND LISTEN. She always had the brightest smile. Pt rehab program was very efficient as well. I would recommend this facility to anyone that needs the care.
