After I get my 3 replacements (that I'm required to leave there since I was given empty syringes and have to go there for the next month to receive my shot) I WILL NOT BE RETURNING and do not recommend them to anyone if this is how they treat their patients.

After filling out all online forms and bringing in all my records which consisted of multiple heart tests, liver scan and labs and still was still asked what were my meds and allergies.
