I came across a text message in which she expressed inappropriate views about patients at her current place of employment. As someone working in the healthcare field, particularly with individuals experiencing behavioral or mental health challenges, it is deeply concerning and unacceptable for her to refer to patients as "crazy." Patients of all ages come to your facility seeking genuine behavioral and mental health support. These individuals are acknowledging their conditions and actively seeking professional help, which should be met with compassion, dignity, and respect. I have grave concerns regarding the lack of empathy and professionalism displayed by this staff member, as this behavior appears inconsistent with the standard of safe, respectful, and adequate patient care that should be upheld. I respectfully ask that this matter be taken seriously to help preserve a culture of professionalism, compassion, and accountability within your organization. Patients of all ages deserve sincere care, understanding, and support during vulnerable times in their lives. To patients or family members receiving services here: I reported a HIPAA violation on 4/30/2026 and was contacted by the Quality Management Director, who stated they were investigating the matter. I was told I would be CC'd on an email regarding the investigation, but I never received any follow-up communication. I called again during the week of 5/11/2026 to request an update, as this is a serious concern involving patient privacy. At this time, based on my understanding, no accountability action but a signed form was given regarding the involvement of the employee. BAD. Ok--This whole organization is a huge mess. I was a patient with the clinic in Flagstaff for an entire decade. Why would I do this? Why wouldn't I just go elsewhere for my services? The answer? If you only have ACCESS--the two choices in Flagstaff are 1) Bad-SW 2) Worst-TGC. There is only one individual who still works in Flagstaff (who was there) when I started. I usually had a completely NEW case manager, at least once, often twice a year. Three of my case managers told me that SW was a horrible place to be employed due to the fact that they pay their CM's badly and give each one a huge caseload. A few of mine were very nice, but quickly left due to this reason. Many of the LOWER level employees were excellent. But the problems lie with the UPPER management. They are horrendous. Most have obtained their degrees from what are called "schools of last resort." Places that students go because they can't get into good institutions. In the whole ten years I was a client there, no clinical director, or second in command ever made an attempt to assist me in any way. In fact, it's hard to even get their last name, or a phone extension or email address, if you have a concern. They go out of their way to not address any issues. Also, if you want to really see what a "great" organization these people or running, go online to the court docket for Maricopa County where they are headquartered. It's true that they have been around several decades, but the numerous times they have been sued by former patients is astounding. Also, please read all these Yelp reviews. I found in each of them something I also had a problem with in my own personal care. They think of nothing of leaving you (high and dry) when it comes to prescribing or REFILLING medications. If I hadn't had an understanding primary care physician, I would have had to present myself at the (hospital) emergency department two or three times a year. Also, they do a horrible job of keeping your personal HIPPA information private. I was very careful with any ROI (releases) I did sign, but they often "chatted" with other organizations in Northern Arizona, when I told them I did not want them discussing my health information with others. In Behavioral Healthcare there is something called "Need To Know" Does an employee working at the organization have "a need to know something?" Why was the front desk receptionist knowing my private information? Why was the Peer Support Person discussing (with me in the car on transport to the clinic) something I told a staff member in therapy? SERIOUSLY? If your in the Valley, please do yourself a favor and walk or RUN elsewhere. If you're needing help or treatment in Northern Arizona, move to the valley or Tucson. Don't waste several years of your care deteriorating at this poor excuse of a behavioral health organization. PLEASE HELP OTHERS BY REPORTING YOUR ISSUES TO ACCESS OR STATE LICENSING ORGANIZATIONS. Reporting concerns to SWBH, to Health Choice or to Care 1st, is a huge waste of time. They are only concerned about protecting themselves from liability. Star program is a joke and very much is a flop house. It's poorly run and the PC is just there for a check and only is onsite for 4 hours a day. Also, it should be said that the staff for the most part are very rude, have no clue of what recovery is and or should be. They just throw a body if they are down a staff and occasionally. leave them with only one staff person. I feel more should have been done after we lost a peer whom was not being checked on and was post op. I truly have no trust in Southwest Behavioral Health Services or the STAR recovery program. I should also be said that they make you see only there providers PCP, Therapist, Case managers etc. to charge your insurance more money. It is also very dirty and unclean as well as unsafe. This is the second time my appointment was canceled at the last minute because the doctor is now seemingly unavailable. They will call you right before closing and ask to reschedule at minimum two or more weeks later with no earlier availability. This has been inconvenient and poorly managed. So I have been here since may and it's been a horrible experience. Their is no flow of communication nobody's on the same page, their is definitely a who you know type atmosphere. I've been physically attacked by both roommates, leading up to both times I was assaulted, I had reported to sbh staff about the living situation not working out the first roommate would lock me out of the apt, be engaging in drug motivated groups in the living room with his dealer and friend, and I was told I need to provide pictures, "ok yes let me take some pictures of a drug dealer and his friends in the act" that's when I realized they didn't care, why would you want me to risk my well being to show you a picture when you could've just did a pop up visit and seen for yourself as a result of their lack to provide effective handling I was attacked while walking up the parking lot by 1st roommates friend/dealer because sbh told him what I was saying, 2nd roommate would constantly steal from me I didn't have money to buy a lock as they said they don't provide one the person they put in to live with me would bring random couples home there was a time he did this and the couple broke into a huge fight , and I was able to do what sbh said they needed and I recorded the incident, and again they just shrugged it off, so I went and got a restraining order, after the judge granted it, the pc2 from sbh acted like this was all brand new news to her and very rudely asked why I got a R/O and what I said to the judge I told the judge same thing I reported to them repeatedly, also even after sbh knew the situation with 2nd roommate they left me their to be violently atacked 2weeks later by that roommate and a friend he brought with him . there's so much much to say. So be careful if you seek assistance from sbh and if all possible find a better program read more

It has been near impossible to get off of hold or speak to the right person needed, from day 1. Then I finally got an intake slot on Zoom with a person who does...intakes? Needs assessments? This person deemed me unfit in some way for them to even conduct an intake interview with me (for psychological health counseling MANDATED by the court for my DUI conviction. So, the help I needed and literally legally needed was met with a door slammed in my face. For most businesses that would just be their prerogative, but for a mental health and substance abuse counseling provider?!? What?!? If zero stars was a thing, I would give them one. I have a friend that wanted to quit drinking and needed detox for his 20 year history of heavy alcohol use. They needed medical clearance, which I understand, we got that at the local hospital. Got back to SWB in Kingman Arizona. They accepted him into a "chair" while waiting for the doctor to make rounds in the morning. I got a text from him at 10:45 saying that SWB was discharging him. How is 16 hours freaking detox. That doesn't even get the alcohol out of the freaking body. It takes approximately 1 hour for a healthy liver to process one shot of alcohol. My friend drank approximately 1 gallon of alcohol per day. Just to get the alcohol out of his body is a 4-5 day process. Then the body will then go into withdrawals. This is not a safe practice. I will NEVER recommend SWB for anyone that has an addiction and wants help. read more

Both I and the pharmacy have tried to contact the Mesa office a total of 6 attempts with the staff. The staff keeps hanging up on us after lengthy hold times. After unsuccessfully trying to get a hold of them on the phone, I sent an email requesting someone contact me regarding getting some urgently needed help for my brother. I finally got a phone call and was told that, in order to set up an appointment for him, I needed to call the main number at 8am. Well, it's Monday morning and I called at 8:06. It's now 8:54. These people are a massive joke. And not even a funny one. They think just because their clients require assistance from them that they from then on out own those people and have complete control over every aspect of their lives. I have a couple of friends who receive assistance through them for housing and both of them are treated like prisoners. The southwest case workers or whoever come into these men's homes and tell them that they are not allowed to have guests come visit and they can't have pets. Now mind you they make these rules up to suit their control issues as they feel like it. It's like they want to completely isolate these men from having any kind of companionship or happiness. And today was the straw that broke the camels back for me. Myself and my husband adopted the dog that was living with these men and he is having issues adjusting to being away from the only home he has ever had so since we live close by the one remaining tenant of the apartment was allowing the dog to come there at night and eat and drink. He was doing so because the dog won't eat or drink anywhere else. So today these pieces of sh** went into the apartment and took my dogs food his food and water dish and all the bones and treats that I had in just bought him for Christmas along with his food storage container. Almost 200$ worth of brand new stuff for my pup just gone. This has to be illegal as all hell. There has to be something that can be done about these people. And either I want all my dogs stuff back or somebody better cut me a check. If not I'm sure I can find other ways to recoup my money from the pieces of sh** one way or another. I Cannot fully explain how terrible my year and a half were at this location, and I'm worse off than when I started. If you have to go, for the love of God avoid Sotoyia Fairley at all costs. She is not just a terrible "care provider", but also a vindictive and evil person. And I say evil because anyone who lets another human being suffer for months on end without listening and trying to offer a solution is truly evil. I even tried to move to a different care provider due to her negligence, and they scheduled me with her again without my knowledge or consent. She refuses to listen when I tell her the diagnosis is wrong, she treats me like a junkie just trying to score drugs (even though I'm sober), I have to repeat myself constantly, and we stayed on the same course and diagnosis trying medication after medication that wouldn't help at all. She aparently knew me and what I was feeling better than I did, and after months and months of trying her way she still would not listen. They put me through hoop after hoop just trying to get a simple ADHD diagnosis, and dropped the ball time and time again where I no longer had any patience for their negligence. They even gave me a referral for a psychiatric specialist (for a ADHD diagnosis!) and after 2 months of waiting they finally had me call to check up on it, and they had no record of the referral and said they would have to send it again. They scheduled me talk therapy with a intern because they don't take mental health seriously apparently, and I had to take a month off of work (costing me thousands of dollars) because I had a severe breakdown due to my symptoms getting worse, not better. They still didn't even lift a finger to help me until months later and I had no choice but to return to work and suffer through it. Truly one of the worst experiences of my life, and no person should ever have to feel hopeless and ignored by their care provider. If I can help just one person avoid this place and the nightmare I had to endure then this review will be worth it, please do yourself a favor and avoid this inhumane establishment at all costs, you do not matter to them. read more

I went to the center on 4420 South 32nd Street, Phoenix , AZ 85040. It was one of the worse experiences. The employees there we all robotic. Very cold, no emotions, and not experienced enough to be giving advice to people with issues. The environment is exactly what you would expect in this area. I wasted 3 appointments/many hours of my time. I even went out of my way to have a video appointment in the office with a Np to be ask so many questions. I said too much to these people who had no care in the world for my well being. They didn't take any of my history on this medication in consideration and made up their mind on what they thought was right. No in between. I have been on this medication for 11 years, maybe longer now. You can't just tell a person to go off just like that. No warmth or emotions. It was strange for some one who has been to many of doctors for a variety of issues. More related to minor issues. I felt like I was in an interview that took too much time. They couldn't even give me a bridge/medication until I found someone who was more educated. My wonderful PPC helped me for 3 months until I found someone else and I did. This new center I found is lovely, caring, and experienced. They had no hesitation on keeping me on the same meds. They've been around for 30 years and the doctors and nurses were so caring and I felt content and pleased with the outcome. They did not give me any issues or made me feel I was in an interview. I did have to fill out many questions and I was asked many questions but they understood my needs. They didn't try to scare me off the meds. I'm very educated and experienced with this medication. I felt more educated then the doctors at Southwest behavioral health. Knowledge wise. I'm very aware what I'm going through and they surly did not take the time to actually dive deep into what was really going on with me. It was awful and a waste of my time. But now I found a lovely place in a nice area that fits well with me. It is structured with serenity and care. Multiple times I have been told that my appointments were labeled as no shows and that I had to complete a 10 min thing about no shows before scheduling an appointment. The thing is though, I didn't miss any of those appointments! Now, I just recently began my new job which led to me having to call to reschedule the appointment I had for the 1st time and it was on the day of my job training which I said to the rep on the phone that I know me doing this would label as a no show on me. Then the rep on the phone said how I have too 2 other no shows and won't be able to reschedule just yet....what???? Again I had to tell the front office how this is ridiculous for 1 on how many times now I am hearing this incorrect information and 2, had to inform them that all of my appointments marked that way were NOT no shows and that I actually did have my appointments. So the front office rep said she rescheduled me since the other appointments weren't actually a no show. BUT today when my appointment was supposed to be, I never received my scheduled call for my individual counseling...so I called the office. Apparently, I was never actually scheduled!! On top of that, she couldn't schedule me because my counselor was booked for an entire month and apparently she can't schedule me a month out. I am beyond beside myself with all of this!! To me, it seems like dealing with this place alone you are in need of even more mental/behavioral health services than ever. Ridiculous! Went to an intake appointment and met with a staff member that was not a nurse or doctor, to go over my concerns and current med list. I stated I did not want to change meds. I go in today, a week later to actually meet with the doctor who tells me their office (as in every doctor and nurse who works for them) does not prescribe two medications that I can't function without. No exceptions, they just don't. Thanks for wasting my time. The doctor then says to check with reception, they have a list of other clinics I can contact. The receptionist literally has no idea what I'm talking about. So I say forget it, I'll figure it out. She does give me a flyer with information on a place that writes psych meds for 10 day supplies in emergent like situations. I call and they don't write for controlled substances- the meds I take are. In conclusion, nobody there knows what the f*ck they're talking about. I do not recommend them as they set me back a week in my search for finding a new doctor. I'll update this if I end up in a psych ward because of their incompetence (once I get out...). read more

I called the SW Behavorial - Erickson Center on Wednesday, 9/16/2020 in search of a therapist that specializes in BiPolar II, PTSD and Grief (spouse death). I provided my health insurance information to the assistant and was assured someone would call me back to schedule an appointment. Since I hadn't heard anything I called the center today. I was told the person handling my insurance info was out of the office due to an emergency and would not return until Thursday. The operator assured me someone would call me on Thursday!!!!!!!!! THURSDAY!!!!!!!! At times, mental illness can be a gushing, life threatening wound. Can you imagine going to the ER with a life threatening wound only to be told you would need to come back a week or two, later!!! My husband is 69 years old and recently contracted CCovid. He was hospitalized four 1 week. When he tried to return to Southwest clinic for dosing he was told he needed a doctor's clearance. He received that from his primary care provider upon request. After submitting the correct forms to Southwest they decided they wanted more paperwork from his cardiologist, a medical clearance ri be exact. We obtained the clearance from the cardiologist, however its still not enough. He hasn't been dosed in two weeks and is extremely sick from the lack of methadone. He's 69 years old, has a liver transplant, has heart issues, and now is being denied methadone treatment for no good reason. He's done everything they've asked and then some, still isn't being tested fairly. The PA had asked for everything he could possibly want and is still not dosing Michael. I guess our next stop is the lawyer's office. If my husband dies because these people are introducing so much unneeded stress and pain into his life, I guess a law suit would be in order, however no amount of money will replace my wonderful husband. He does not deserve to die because someone is refusing to be sensible and listen to the doctors who know what they are doing. Had the unfortunate experience of having to go here with my daughter. The first day we were told that the appointment would take 2 hours. We arrived at 730am for an 8 am appointment. We were finally seen at 930. And didnt get out untill almoat 11am. The woman who did the intake was nice enough. The second visit we arrived at 936 am for a 10 am appointment. My 10-year old Asperger's Syndrome son has had the great fortune to work with a therapist from Southwest Behavior Health Services. Our therapist is very flexible in terms of meeting at locations that are more convenient to our home. For example, we live in the South Mountain area, so she sets up our appointments closer to home versus the 3rd St. office. Our therapist calls us before every scheduled appointment, not only to confirm the appointment, but also to get updated on our son's activities and life to better leverage life activities in his sessions. The value of using Southwest Behavioral Health is that they are access to a wide-range of resources and can help pull in resources when necessary. read more

I'm flabbergasted that Southwest Behavioral is still in business after reading many of the reviews here and on other sites. Their lack of professionalism, integrity and patient care is deplorable. How they have not been shut down by the state is a mystery to me. My primary care physician and my mental health providers are both at the same location off 28th Drive in Phoenix. I would rate my primary care physician a 7 out of 10 and that would be Dr deck , I believe she means well however I think the systems in place at Southwest hinder her ability to actually do her job correctly. Now on the mental health side this is a clear disaster and should be shut down. They have an abundance of turnover for their staff and providers. Many have complained about the process of making a mental health appointment, which is to call same day to try and make an appointment and usually all the appointments have been taken within the first 15 minutes of the day. This in itself should send up a red flag that you cannot make an appointment a week, 2 weeks, 3 weeks out. My first psychiatrist was there for about 60 days then after repeatedly trying to schedule an appointment and complaining while I was there to see my primary care physician they put me in a room and up Pops on the TV a psychiatrist very unprofessional borderline comical. The counselors are horrendous, Young unexperienced or just don't care. My first counselor was a 20-something year old with less skills than my dog. I was then given Anna who in the beginning was okay however after needing documents from her she disappeared off the face of the Earth. She gave her email and told me I could reach out to her at anytime however after 10 emails 10 phone calls I've never heard anything from her. As of this date I've been waiting over 60 days for a call back from management to try to schedule me an appointment with a new counselor and psychiatrist. Dr deck is the only one who calls me back and I feel bad for her that she's stuck in the middle dealing with these incompetent management people because she's told me on multiple times that they would be calling me back they'll call you Thursday at 12:30 they'll call you Monday at 10 they'll call you Friday and I haven't heard anything from anyone at Southwest Behavioral in over 2 months. I have also been trying to get my documents in order to go seek care elsewhere however that request has also been neglected buy Southwest Behavioral. I have filled out the paperwork to request my medical records literally more than five times. I I'm told they cannot find them and do not have them, this is after I have physically filled out the paperwork in front of a staff member and handed it to them. I've called and spoke with the so-called manager lady that's a 28th Drive location in Phoenix however she was arrogant ignorant and basically could care less about my needs. I've also called the main office in Phoenix and left several messages for the person in charge and have yet to hear any response from them as well. Southwest Behavioral incompetent and unprofessional establishment run by incompetent leadership who hides behind their phone. I recommend anyone with concerns about Southwest Behavioral to call the Arizona medical board and file a complaint Yelp is great to educate potential patients however to see real change change or at least to hold those accountable you need need to contact the Arizona medical board. read more

I just want to say that the front office at the location on Broadway is AWSOME!! they are very helpful they go above and beyond. they treat you very kindly when you arrive to an appointment. If they don't know an answer to your question they try their best to give you an answer. the girls in the front office greet you with a friendly smile as well as the Gentleman in the office. For a business dealing with bipolar, schizophrenic and other serious disorders, patient care is truly abysmal. Their system for mental health care appointments is to call in at 8am for a same day appointment. That's the ONLY option. So right away you can see that they assume every patient is a worthless member of society with no job, otherwise why have a system that, on receiving ongoing care, would get you fired from almost any job? Then, if you do call exactly at 8:00:01, says "2-minute hold" then 8 minutes later says "hold is currently 5 minutes" then when they finally answer "all appointments are already taken" Really? Southwest's policy is you call ahead 48 hours for an appointment. I call at 8am to then be on hold for 50 minutes. After finally talking with someone, they tell me they don't have any appointments for the next two days. So, I have to start the whole process all over again. Because of this, I have to call for a bridge for my 10 year old son (ASD) and after leaving numerous messages for the MA to call me back, another day goes by, more calls to be made then, the idiot finally calls me back. She tells me that she will send in a med bridge until we can get an appointment made. She didn't get those meds filled, and the same process starts again. Once we do make an appointment, there policy is that if you are 10 minutes late you can't be seen and that is fine, I understand that, but to get there on time and having to wait for 40 minutes past our appointment is unacceptable. My son has an attention span of a nat, so he has a meltdown. Three months later, I find that we have three days left, so I call. Same freaking process. This time I go to the center so I can see her mug and make sure she does her job. "I will send the meds over" All weekend we didn't have meds for him, 4 days later, because of the MLK holiday, they see us on the spot on Tuesday, and the Dr said once her computer is working she will send us the meds to the Pharmacy. Nope, another day without meds. I tell them that I want a different MA, she sucks. I want a different location but it's too far. Excuses after excuses from the staff. One lady said "We sent them over" Wrong!!! another said "They were mailed out yesterday" Yeah right. I am going to fight them legally. A child without his meds is criminal. I will call the insurance company in the morning and file a claim, while talking with our lawyer. I'm done!!! read more

When my granddaughter was seen in Buckeye and Kingman, care was excellent. Now that she has moved to Bullhead City, her care is less than desirable. Not consistent, too many excuses, lack of compassion by staff. read more
