    Been a while since worked here no matter how far you go up the ladder when feeling like a hostile…read moreor racially divided environment there is no corporate number the way they treat the employees where do I start cna week ha was a joke no activities or parties w/2 pans of chicken for lord knows how many ppl the real party was given by a employee who does all activities out her pocket sometimes employees chip in on their own parties for the holidays not to mention she also pays out her pocket to give the best dining experience ever for the patients yet she can't get a raise which did I mention she pay for all this stuff with no budget or help from the higher up(not much) let's be fair the more money they save the bigger their bonus at the end of the end of the year they take anybody in just to fill beds and keep the numbers and if you got good insurance watch out they will keep you but O might I add release date patient kicked out their room to sit in common area in trash bags & pray someone come get you cause if not healed or insurance jacked or even after appeal you will sit there in the common area with nothing to do til someone come get you even the hospital don't throw ppl on the streets like animals 5 star they had to pay somebody cause dead roaches lay on the floor in hallways etc and poor maintenance guy he don't have much to work with at all he spray but yet they come back every Monday is hell cause they are in a panic about the state coming so the weak links take days off so they not compromised and all the fall guys left in the building don't let them fool you the nurse mgr is a LPN not RN who once told a worker who asked her to grab the wheelchair she is so hands off and don't care she looked and asked the 2 employees who me grab the wheelchair and they were both tied up @ the moment she will sit in her office walk around with her phone and not acknowledge when cna are gone off the floor lights going off she would rather have a worker do others job then have the ppl responsible for it leave off the floor and get missing did I mention she is scared of the employees to the fact other employees have told others how can someone hold a title that likes to harass employees that do their job many employees have left and never came back due to how she treat & talk she does not know how to treat the employees watched one lady leave and never come back because she wanted her to do a shower for the day before she frequently kept asking the patient about the shower because the patient decide she wanted it the next day she felt she was suppose to stop what she doing (leisure) and shower her which was super unfair but she didn't care she will watch you don't sit ever even to chart honey she will get you & activities I feel bad for her she has no budget and no help what is even sad they take her help to put on the floor and she left again with no help by herself so unfair have prn's but yet will short her things she want to do but not able to do at all the pay they don't pay for experience at all if you work here period better join the union they treat their animals at home better than they treat their people then lie to the new patients come you will have a single bed or you can eat like this etc lies lies and lies cause only reason food got half way decent is cause state do in the building anytime no single bed rooms unless you have a medical condition which many times cna's had to tell the patient not going to happen the portion of food given a 3yr old will probably throw the plate you can be the best worker no appreciation I have seen special diet needs not be met although administrator try her best on certain things to get things done not all the time but when the cats away the mice will play I can type for days on this place but this is satisfying for me cause the employees have no voice except certain ones are allowed to talk but if you express concern too much they will strip you then it turns into watching and goes on up they are super petty you can make any excuse necessary for this post but this is the truth and let's hope this is not swept under the rug like they do major issues such as nurse mgr mainly or any other higher up will single you out for your voiced concerns and any opinions not looking for nothing my mental was worth the leave when dealing be prayed up and guards up when working or putting your people in this place not looking for no response their is nothing can be said about this review it is based on facts this is what happen when no one can contact corporate and yes they will be wrecking their brains about this review who cares now you see how I felt when no one cared but yet the ones you are scared of you let get away with god knows what GOD BLESS & I'M always blessed and highly favored & it's ok if ignored I spoke for some of the ppl not all of them miss treated still there Have A Blessed Day

Unfortunately, the laws that should regulate these types of places--where no one is held accountable and no one guarantees the lives of their patients--are far too lenient in cases like the one I recently experienced. My husband is dead today, and I cannot even know why he died. Everything happened so fast that day; there was only one person there helping me and attending to him with great patience, who tried to help him until the very end. And I have to say, instead of thanking this person, the director decided to FIRE THEM; they were dismissed for being a good person. Nothing will bring his life back to me. To them, this is completely normal--that is how they made me feel on that terrible night of his death. There was no one there to help him in his emergency and pain. I made them aware of my concerns weeks in advance; they stopped assisting him, didn't even give him his medicine, and left his food there even knowing that they were neglecting him more and more each day until his final collapse on August 7th. I will never forget that terrible day in our lives. Because of this, I warn you all: patients survive there under constant risk. There are many pitiless people, and the staff only obeys protocols and rules out of fear of being fired, unless this HELL is sued and forced to close its doors, because there is no way in this life or any other that they can ever repay even 1% of the pain they cause to their patients and the family members who love them. Lastimosamente las leyes que deberian regular a este tipo de lugares en donde nadie responde por nada ni nadie garantiza la vida de sus pacientes son muy leves en casos como el que yo vivi recientemente. Mi esposo esta hoy muerto y ni siquiera puedo saber porque murio. Todo paso tan rapido ese dia, alli solo estaba una persona ayudandome y atendiendolo a el con mucha paciencia y trato hasta el final de ayudarlo. Y tengo que decirlo en lugar de agradecerle a esta persona su director decidio BOTARLO, lo despidieron por ser buena persona. Nada me devolvera su vida. Para ellos esto es muy natural, asi me lo dejaron saber esa noche terrible de su muerte. Alli no habia nadie para ayudarlo en su emergencia y su dolor, les hice saber mi preocupacion desde semanas antes, dejaron de asistirlo, ni medicinas. la comida se la dejaban alli aun sabiendo que a el lo fueron desatendiendo cada dia hasta llevarlo a su colapso final el dia 07 de Agosto, Jamas olvidare ese dia tan terrible de nuestras vidas. Por esto les advierto alli sobrevivien los pacientes con riesgo todo el tiempo. Mucha gente sin piedad, y el personal solo obedece los protocolos y reglas por temor a ser despedidos, a no ser que este INFIERNO sea demandado y obligado a cerrar sus puertas porque no hay manera alguna en esta vida ni en otra para que puedan retribuir al menos 1% del dolor que le causan a sus pacientes y a sus familiares que los aman

    Family First Therapy is an incredible collection of therapists that cover speech and OT. We have…read moreparticipated in both here. Ms. Jennifer, one of their OT therapists, uses toys, fun games, physical activity, dancing, music and writing to work on what my child needs. She is very thorough with parents, giving you a breakdown of what they worked on during the session. She usually leaves you with some sort of "homework" to complete before the next session. When we were seeking a tutor for one our children, she had a great recommendation for someone that would mesh with my child's needs. We started speech therapy here when the school district could not work with our daycare/work schedule to provide speech services. Ms. Lindsay has been very personable and made a great connection to my child. She breaks speech sounds down in a way that my preschooler (and me) understands. Just like Ms. Jennifer, she communicates what they worked on after each session. She also provides games to play during the week to reinforce skills. My family, and kids, have absolutely loved all the therapists we've worked with at Family First. If you are seeking speech or OT services, give them a try.

From what I have observed, there appears to be a shortage of staff to properly care for the patients, as well as a lack of adequate cleaning equipment and supplies. The person who cleaned today seemed to only do a superficial job, not because of a lack of effort, but because they simply do not have enough time or the proper materials to clean thoroughly. The CNAs are expected to change, bathe, dress, and make the beds for patients, while also serving meals and later collecting all the meal trays. That is an overwhelming workload for the number of patients they are responsible for. The nurses also appear to be overworked. It is noticeable that many staff members do not seem satisfied with their working conditions, and I understand that the pay may also be a contributing factor. In addition, there seem to be frequent changes in management. It is truly unfortunate because the facility itself is very nice. However, it does not appear to have the staffing levels, equipment, or resources needed to provide the quality of care that patients deserve. I do want to recognize that the staff members are kind, attentive, and quick to listen and respond when concerns are brought to their attention. My comments are directed toward the need for better support and resources, not toward the employees who are doing their best under difficult circumstances.

TWO HOURS she sat sick, and alone. We had to physically go confront them at 9:00PM to get staff to her. They took Her a sandwich. They did not change her out of her hospital gown- they did no medical care and did not even administer ANY of her meds that night nor the next morning.. NUMEROUS calls by numerous concerned family we're unanswered, and not one single call back. By the next morning my elderly mother was begging us to get her out of there and we called to let them know we were coming to remove her to please make the necessary arrangements. Once again FOUR hours worth of phone calls were unanswered and not a single call back. We literally had to show up at the place and threaten to call the sheriff. Unfortunately once we were able to get her out of the place we had to contact them to get an order for home health care which is where we brought her and the administrator of the facility told our Home Care Case Manager they C OULD write the order but THEY would NOT. Even after denying her basic medical/human care they refused to assist my mother in getting HOME HEALTH CARE! They said they COULD but they WOULDN'T.

Before my dad was transferred there, I stopped by there without an appointment to check out the facility. I was immediately greeted and welcomed by the friendly staff and given a tour of the facility. The facility was clean and pleasant smelling. The rooms were very nice and spacious, the shared rooms looked as though the residents had plenty of space, and did not look cramped at all. Once my dad arrived, he was put in a very nice private room with a ton of space. All of the office staff, his nurses and nursing assistants, therapists and doctors have all been very sweet, and do such a wonderful job taking care of my dad. The physical, occupational, and speech therapy my dad receives have been top notch. He had been in another facility a couple months back and the therapy he received was the absolute minimum and didn't help him at all. The therapists at Hawthorne truly care about their patients making progress. The speech therapist, Brittany is so sweet and caring. I get to spend more time with her because I am often there when she comes to his room to work with him on his eating. She lets me bring in his favorite banana cream pies and she stores them in the fridge, so she can feed him when working with him on his eating, instead of the usual apple sauce. When I to go to the therapy gym, it is very busy, and all the therapists are working hard with their patients. My dad has already made a lot of progress in just the couple of weeks he has been there, as they really push him. They all make him feel so welcome and always happy to see him. My dad has dementia, so it can be scary leaving him at a new place, knowing he cannot communicate that well. I visit my dad at different times every day, and every time I go, I always find he has been well taken care of. I also enjoy taking my dad outside in his wheelchair so he can feed the ducks at the pond that is located in front of the entrance. If you are looking for a skilled nursing rehab facility for your loved one, I cannot recommend Hawthorne enough.

    To Whom It May Concern,…read more I am filing this complaint to document the horrific experience my family endured due to the gross negligence and abandonment by LifePath Hospice during my father's final days. This organization utterly failed in every capacity and caused my mother and me immense emotional trauma. From the moment we enrolled in hospice care, we were led to believe LifePath would provide support, guidance, and compassion. Instead, we received only one visit, and only after we called and specifically requested assistance. There was no sustained follow-up, no meaningful contact, and no presence when it mattered most. After intake, we were told someone would visit the next day. I informed hospital staff to contact me if anyone from hospice arrived. No one ever did. I only realized in the chaos of the day, around 7 p.m., that we had been completely abandoned. When a nurse from LifePath finally arrived the next afternoon--after we had to ask for someone to come--it was far too late. My father was already declining rapidly. Over the next three days, as my father actively died, not a single person from LifePath Hospice contacted or visited us again. The only outreach we received was a generic voicemail offering "counseling," which felt like a cruel joke after everything we had endured alone. To add further insult, we later received a survey request--as if our opinion mattered to a company that ignored us in our most desperate hour. We had already emailed our frustrations, yet we were still contacted again. This level of tone-deafness and lack of empathy is disgraceful. This experience was traumatic and completely contrary to the mission LifePath claims to uphold. They failed my dying father. They failed our grieving family. Their absence forced us to lean entirely on hospital nurses, who were not affiliated with the hospice and should never have had to shoulder that burden. We are demanding: A formal acknowledgment of this complaint; A detailed explanation of why this happened; A clear outline of corrective steps to ensure no other family is subjected to this level of negligence and abandonment; Immediate and permanent removal of all contact information from their records. We have asked multiple times and continue to receive unwanted outreach. This organization is not fit to call itself a provider of compassionate end-of-life care. What they did--or more accurately, failed to do--was inhumane.

First, they do not answer the call button. It takes them 15-25 minutes before they come. Both my mother in law and I had to clean him after an accident that he had. Second, one night he wait 3 hours for a nurse to help him. Third, our first night there we asked for water and they took his cup to the bathroom faucet to fill it up. Though the physical therapy department was wonderful, the care facility is not. The blankets smell old and dirty. The head nurse called to tell me when he was going to the hospital and when I asked if I should retrieve his belongs, I was told no. The next day, my husband begged me not to send him back. The place was dirty and the food tastes awful. I went to pick up his belongings and they had already packed it up. I watch as his stuff was put in a closet and asked why it was packed up when I was told I could do it. It then informed them that I need to see the room. I was told I couldn't. I am glad that I did. While walking in the cleaning lady said, " Look, they left these depends. We can use them. I explained that my name is on them and I want them. Also, wanted my husband's charging cords plugged into the wall. Please do not send your family member here if you love them. It was extremely traumatic.
