The direct support staff is generally Ok so I give them they the 2 stars. The administration is heartless, manipulative & mean. They evicted my father for behavior that was related to dementia on top of depression due to the loss of his wife of almost 70 years. They blindsided us & then lied straight faced to us at a "care team" meeting for our father. My father was supposed to be evaluated at SBU, but while being evaluated at SBU, SJL evicted him. Tossed him out. They never had any intention of letting him come back. Our dad died a month later (4/16/25) & I blame the administration, social work department, head nurse & doctors. They broke the heart & the spirit of a relatively healthy man who was loved by his family but required the care & supervision promised by SJL. We regret ever admitting our father to SJL. I recently spent several weeks at St. Johnland for rehabilitation following an injury. The facility provided excellent care, overall. but the physical and occupational services were outstanding. My mother was admitted to St Johnland post UTI and fall for rehab and possible long stay in Oct 2023 The facility is neglectful boarding on abuse. She fell 4 days after her admission in an unsupervised dining room, no one witnessed the fall. My mother was delirious after hospital admission but after the fall agitated more confused etc. Negative Cat scan for bleed but probably concussion. Sent to hospital next day and then again for low Blood pressure. My mother was sent to KIPP unit where clients are severely demented. My mother barely received PT She was put in a wheelchair where they keep you for 12 hours a day. My mother stated she was pushed by an aide and handled roughly by others. I wanted her out of this place immediately but it was difficult to transfer. Then she got Covid so transfer was put on hold. Westhampton care center then agreed to transfer but St Johnland had her in a dementia unit she did not belong. St Johnland also reported to Medicare that my mother was not progressing in PT which she barely got, so her last 25 days of Medicare payment were denied. This non progression is a lie. Since transfer out of St Johnlandmy mother has not spent 1 day in a wheelchair walks with a walker. Not only was this facility non healing or foster rehabilitation it promotes an institutional behavior where patients are treated sub human and think they are being treated nice if they are provided with an extra juice There were a few kind staff members but on the whole the staff and environment are toxic. The Kipp unit must not be renovated in well over 20 years. The carpet smells, the unit reeks of mold. When you complain my mother hasn't had a shower in 12 days they complain of short staffing etc. The short staffing is none of the clients issues they are taking Medicare/Medicaid and private pay. I have been a nurse for 40 plus years the level of neglect in this facility should not be legal I initially had some sympathy for the lack of staff till I went on line of this non for profit that must post salaries disgraceful. Would give zero stars if possible They are short staffed. VERY short staffed. The staff would complain to us every other day that they didn't have enough staff, not get enough pay, benefits, etc. Not sure why they keep accepting patients if they don't have the staff to support it. Food was awful. My father suffers from heart issues and they constantly would serve him hot dogs and sausage, etc. had several conversations with them about his food restrictions but ignored. No cell phone service. Zero in the entire facility. And on top of that they have VERY limited WiFi. Can't believe that I'm 2021 you don't even offer cell service or WiFi. The two physical therapists were the only positive thing about the entire experience. Jessica & Jenn. Very competent and nice. Too much to write about the consistency in negative experiences we had but the last I will mention is that when I visited my father one evening he looked ill. Couldn't find anyone to help. Finally tracked down a lady who was supposedly the nurse supervisor at the desk. I asked her to call an ambulance and instead she argued with me. That's right. She wasted time arguing with me instead of calling 911. Now I would have just called myself but there is zero cell phone service in the entire facility. Cell phones do not work here. A friend of mine is in this facility after suffering a stroke. They don't take him out of bed in the morning for hours. He has to wait a very a long time for anyone to answer the call bell. He fell out of bed and no-one was there to help him. He laid on the floor for a long time before anyone came into the room and finally got him back into bed. They don't bring his medication at the same time everyday. They are short staffed and some of the aides are very rude. I do not recommend this place to anyone! read more

Took great care of my mother, I great group of people who care for each resident. I highly recommend this facility.

My husband was a patient there in the TBI unit for rehabilitation from a stroke he had in 2010 . I took him out of there after the 2nd week. First night there his clothes ( which I bought new the previous day) were stolen. I don't have many complaints about the actual PT,OT, Speech other than it really was not as intense or often as I was told. Mind you I got there at 10am every morning and left at 10pm every day accept one. I was very observant and also speaking with other patient family members that also had concerns. Staff was quite different when family was there but I saw when they were not. The whole time I was there I took care of my husband because of the lack of care. When I came in the morning he was in sweat pants and pinned in the wheel chair soaking wet. I was told they would get him up to use the bathroom. Never happened. It was obvious that diaper was from the night before when I changed him. I spoke with the head about my concerns with various issues, including my husbands diet. I'm glad I was there because in the dining room people that were not able to feed themselves well were just sitting there. The aide was a total waste. My advice is if they still have that unit put your loved one elsewhere if they have had a Traumatic Brain Injury. My husband had no speech and couldn't tell me what was happening he would cry when I left and would shake his head no and hold on to me. Something was not right! I looked at many places before I chose this place and it was based on what the rehabilitation i was told he would receive. Nope none of that took place at all. I do believe if I was not there every day he would have been pinned to that wheelchair all day sitting in his own dirty diaper... has this place changed? Who knows.... But based on what I have read.... We'll steer clear....if I could give it no stars I would read more

Do not put your family member in this facility. They will intentionally provide lackluster care and cover up any signs or situations of abuse. The facility Is always short staffed except for when they know the state is coming to review them. During this period the facility directors and various department heads come out of their offices to make it seem like they provide excellent service and care but as soon as the state leaves they go back to sitting in their offices and providing the bare minimum of care for the residents. I have worked in this facility for roughly a year and on multiple units. The way managment treats the staff and the residents is absolutely appalling especially since they market themselves as a Christian facility. they should be ashamed of themselves as a facility and as Christians in general read more

This facility is horrible. My 94 year old mom broke her femur and needed rehab after having surgery. We chose this facility because it was recommended to us, we now regret placing her there. She went in for rehab and got Covid due to an outbreak in the facility and moved her to another part of the facility along with the others that contacted covid in her section. Not only did she test positive for Covid, but got an eye infection while there.. I asked the nurse multiple times about her eye and all she did was put cold compress on it. Upon moving back to the old unit, the nurse that was taking care of her when she was in that unit became concerned about the eye, which she could hardly open and sent us to see an Eye doctor , where he prescribed oral antibiotics with eyedrops. Since going back to her original unit she is confused and somewhat disoriented, has loss of memory, and very agitated. The CNA's take over an hour to answer her buzzing for help, so she gets out of bed to go to the bathroom on her own, which she should not be doing. They have no compassion for the patients and the facility is very short staffed. The other day she fell in the bathroom due to the negligence of this facility. Thank God she did not hurt herself. I have complained to the nurses about this issue and still nothing has changed. Most of the CNA's are rude and just dont care about the patients and should not be working with the elderly. I have placed may complaints, yet nobody seems to care or do anything to correct the issues there. I DO NOT RECOMMEND PUTTING YOUR FAMILY MEMBER HERE! read more

I think the main problem is that this is a nursing home, not a rehab center. They don't put forth a huge effort to help you get better, they simply take care of you as you are. And by take care of you I use that term loosely. A friend of mine was in a serious car accident and after many weeks in the ICU he was finally ready to go to a rehab center. His parents made the awful mistake of sending him to St. Johnland. He was there for only a few hours before his vitals crashed and he nearly died. That set him back a few weeks and he had to go back to the ICU for a long period of time. When the doctors decided he was healthy enough to try it again, there was NO way his family was sending him back to St. Johnland. I will make this short. This place has a reputation for being the best for head trauma, including strokes. My mother was placed here and she caught things she never had before. They were adding loads of unnecessary medications to her, against my authority. She ended up having seizures, which she never had before, and subsequently she passed. My Aunt had been at St. Johnland on 2 occasions. Once at 100 and again the following year at 101. Both times, the staff couldn't have been nicer. The PT and OT staff were great with her. I guess it helps when the person is lucid and does as she is told. My aunt is stubborn as a mule and wanted in the worst way to get back home. So, she did as she was told (not easy for her), and in a relatively short time, was back walking and eating on her own. The place was also kept very clean and according to her, the meals weren't bad. read more

His therapists have been caring and take time to explain so much. Monica, Joanna, Denise, Pierre, Jung and so many others have been godsends. Overall as a nursing home it is clean, does not have the typical hospital environment. However, they do seem short-handed. No one answers the nurses station phone, & weekends are the worst- NO ONE is around. Hard to find a nurse or aide when you need one. If they would hire more people the place would be 5 stars. Also, you need to visit alot during the week and speak up if you see something you do not like or it will go unnoticed. Food is very institutional - so plan on bringing in food once in awhile. Social workers don't help when you complain , very defensive. The tour was great, if they delivered everything the woman told us on the tour it would be wonderful, but they DO NOT deliver everything. Their outdoor patios are disgusting, bird poop everywhere even on chairs and there are ants all over the place on the patios. read more

The place is called Saint Johnland Nursing Home in Kings Park NY. It's has to be the most unfair place to try to work for. First a friend of mine showed up on that person first day dressed in street clothes but had there scrubs with them. Then was told they were late for class because because they had to change clothes into the scrubs. Then they said the person was late coming back from lunch. This person was sitting with the class and the teacher. This person came back to class with the others but was told they were late. The person was opening a bottle of small soda. When the person spilled some and was cleaning it up the person was on the ground cleaning it up and that person fell over and they counted this against that person and all the rest. They also said that the person lied on the job application when they said they left last job because of management dispute when the person was terminated. They had no choice on what to put down because it's still has not been resolved with the union. This person begged for them to give a second chance on the next day of class but they said no and they made up there mind. Needless to say this person was very upset. To be treated so very badly on the first day of class is crazy. The person made a few very small mistakes but that's what first days are for. Unless you want the same I would think twice about an offer for a job by them. read more

Since there are only two reviews about St. Johnland and they range from extremely dissatisfied to very satisfied, I thought I'd add my $.02. My brother-in-law sustained a severe traumatic brain injury 5 months ago. Like Jess' brother, he was in the ICU for several weeks, after which he went to Helen Hayes Hospital (an acute rehab center in a hospital setting), where he remained for two months until all of his medical issues resolved and he was ready for sub-actue care. We selected Saint Johnland, despite the fact that it is over an hour drive from NYC, as we considered it to be the best option available. Their Head Injury Unit is small (20 beds) and both the therapists and the nurses are quite good. My brother receives 3 hours of therapy per day (standard in a brain injury sub-actue setting of which there are very few in the NY metropolitan area and the ones that we looked at in the city are overcrowded and much less pleasant than Saint Johnland) . I, too, am very sorry about Jess' friend, but I suspect that after having been in the ICU for several weeks following a brain injury, he needed to be in an acute rehab center in a hospital setting rather than in a sub-acute rehab setting in a nursing home. Whichever hospital released him from the ICU and placed him in a sub-acute setting like St. Johnland, rather than an acute setting like Helen Hayes. made a very poor call. If his vitals crashed after a few hours he obviously still had some acute medical issues and surely was not ready for sub-acute care. While not perfect, Saint Johnland has competent staff, is relatively pleasant (it *is* a nursing home, so it's not going to be The Ritz), and is taking pretty good care of my brother-in-law. read more

There are different units/buildings caring for different needs. I came in almost helpless with most of my muscles atrophied. I couldn't' tell even sit up. The physical therapists rebuilt me-literally. By the time Ileft 3 months later, I was at ease with a walker and starting to use a cane. Physical therapy hurts, no matter who's guiding/pushing you. There are never gonna be enough aides: this isn't a hotel although I saw quite a few patients act as though it was & ruining things for the rest. If there is a negative, it's the 2 doctors who were almost inaccessible &who (mis)prescribed from afar. Big clue to getting longer access inside a facility... if your insurance company doesn't see improvement and especially effort, the facility sends you home. read more

My daughter was a patient last year and it was a terrible experience for her and myself. She had to endure the much much less then mediocre care.Like the aids not answering her call for help and or assistance.And when they did show up it was not minutes but more then acceptable time. I timed them almost every time she needed them. Many times it was past 30 minutes. It was a standing occurrence. One time when I wasn't there an aid was called because she soiled her adult diaper the aid said "no your alright " and left my daughter dumbfounded!!!!! And a doctor openly was critical of me (because I was always reporting these situations) and said an insulting remark in front of the people in the rehabilitation room!! Really ? These were among many,many other unforgivable situations and many times I had to report these situations just purely for principle ! I would never choose this facility again.There is no excuse for this. And if I could have chosen no stars that would have been my real feelings.I was at my daughters side everyday to experience this.These are just a few of daily situations that happened everyday.And of course there were a few good ones among the really bad so called professionals. All that glitters is not gold. We found their effort in the Head Injury Rehabilitation Center to be minimal, in an emergency situation where a maximal effort was needed. Their Medical Director was arrogant and condescending and allowed our family member's neurological situation to go undetected for months. After we were placed in a situation where we had to *insist* that she be evaluated (something they should have been doing on a routine basis,) we were told by the Neurologist that she had "ammonia levels high enough to knock out a grown man." We're not Doctors so we didn't know what was going on, but it is *inexcusable* for this to have gone undetected for months in a facility that claims to have "one of the best head injury rehabilitation centers around. In addition to all of this (and other, less-major but annoying issues) they made her bedsores WORSE when they were supposed to be making them better - - And that very well may have been what ended up killing her. Don't let the pretty trees and park-like environment fool you. Especially not if it's an emergency situation. Simply putting it, they can't be counted on. We are very, VERY sorry that she was admitted to this facility. read more

I absolutely agree with the one-star review. In my opinion, one of the major reasons that Patricia Rigney died is that she was unfortunately sent to this facility after she suffered a major stroke. They failed to give her adequate neurological tracking for over 2 months when she first got there, even though she had just had a stroke and major brain surgery. It turned out that she had swollen brain ventricles - which we now know is a common side-effect of brain surgery and a stroke. Unfortunately, this caused her to sleep through most of the critical period where therapy could have been the most-effective. We were also told, when we finally found it necessary to INSIST that she be given a thorough neurological evaluation, that she had - and I'm quoting word-for-word - "ammonia levels high enough to knock out a strong, grown man." They literally had put the blame on her, and I mean they did that in a snobbish, arrogant way. This in a "head care unit" that claims to be the "best on Long Island." They also made two bed sores which she had received while in the hospital much, much worse - instead of making a solid effort to make them better. We have *no* doubt that they utterly failed to turn her often enough; and we now know that all they would have had to do really, is provide the proper kind of bed for her. Beds which, to all appearances, this supposed "five-star facility" completely lacks. These bedsores caused horrible complications, prevented her recovery and caused her unspeakable sufferering. One recommendation I would definitely make is to fire the Medical Director. She has a horrible, snobbish attitude and makes very little effort to ensure that those who desparately need the care of a good doctor and good nursing care are receiving it. Don't trust these people, and don't let the beautiful grounds and nice building fool you. They *really* let us down. read more

Very sad! Do not believe the 5 stars. Make sure you read the not recommended reviews. My parent was there after being hospitalized for 3 wks as a result of brain surgery. It was less than a week when I found his meds on the floor from the night before and he fell out of his bed. This is in the brain trauma unit. No alarms on the beds,only top guards and indifferent workers. Please review all places way before your loved ones leave the hospital otherwise you have no choice if there is a bed opening anywhere and the hospital wants them out. They have the right to place them in the immediate choices that are available. Know their rights,look up all elder care laws. Also the insurance companies don't always agree transfer for them. So please beware,I learned too late! read more

I'm sorry about your friend in the last review and I'm glad he's doing well elsewhere. I didn't plan to write a review of St. Johnland's; I was just looking for their phone number to call my father who has been there for two weeks. At 83 years old, he needed a nursing center to rehab in after a bad break to his leg. The staff at St. Johnland's was very professional. He received physical therapy every day, and I know other therapies are offered to those who need them. The idea is for the people there to get better to return home. They are taking excellent care of my father, and every day he gets stronger and stronger. The physical therapist working with him is excellent. He talks to him as a partner in his rehabilitation. He makes this crotchety old man work hard every day to return to his former self so he can get home. The rest of the staff is also professional, courteous, and caring. I'm sorry that last person had a bad experience, but it sounds like his friend was only there for a matter of hours and that's just not enough to judge a place. Our dad has been there for a few weeks, and he's ready to come home soon thanks to the hardworking staff there. It's also a beautiful place with large rooms. AND it doesn't smell like a hospital!!! read more

Jess sry to here that, that just sounds terrible. But sry to say but the blame lies on the doctor who released him. Because it sounds like he was not nearly ready to be released. There's no way him only being their a day can be their fault. I've seen a lot of facility's. This one is always clean and smells like a hospital, the staff is always cheerful and responsive. 3 hours of therapy is great for a nursing home. I take care of a resident here as a private duty companion. This place is great, who wants their family member in any facility. But when you have to make a choice I assure you st johnlands is a great choice. Doctors release patients to early out of ICU all the time, it's the insurance company's way of saving money. It's the same thing the guy I'm taking care of us is going threw. Hence why they hired me . To keep an extra look out for him. They knew he wasn't ready to be released but there was nothing they could do. So they hired me as a one on one private duty companion. If you have a feeling a doctor is making a bad decision, than unfortunately you have to take matters into your own hands. Before checking out st johnlands make sure your loved on doesn't need an acute rehab center not a sub acute rehab. This is like the difference between assisting living and a nursing home, assisted living gives much less care than a nursing home gives. That's the difference between Helen Hayes and st johnlands, one is more extreme than the other. An acute rehab gives more care than sub acute, depends on the recovery stage. I would never recommend this place. My mother stayed in the head trauma unit and the aides and nursing staff were below par. The aides do everything on their time no matter what my mother needed, they will not change during meal times which leads to UTI's and other infections. It is in-human to have a system like this. the nursing staff will not help when aides are not around or help you find an aide when necessary. We had to submit a formal complaint when one of the aides was rough and nasty with my mother (we had just stepped outside for them to change her). The head nurse and management act concerned at the time but nothing changes. They are understaffed which doesn't help with the day to day problems but they need to find aides who like their job and are compassionate toward their patients. We did not feel comfortable leaving my mother there alone and therefore made sure there was a family member or friend with her throughout the day. we were already going through enough stress dealing with her accident having a place we had to keep a close eye on did not help the situation.
