They transferred him here recently. This is located off of Exit 5 but I learned you can get to it easier using exit number 7. There's a parking lot where you can park at or you can park alongside the road. There's several buildings on the campus. You can enter thru the main entrances and simply check in verbally at the desk by stating who you are there to see. She will guide you where you need to go by just telling her what room number patient is in. By the way, The code to the bathroom is 1,2 and then hit enter. They have a therapy room where someone gets exercise or rehab to a certain degree. The people that work here were nice and helped me find the room, and even talked to me for a little while. I could tell they like their jobs. Nice facilities here though -to say the least.

What you read below reflects my personal observations and experiences. This is what I witnessed: * I observed overnight staff placing 2-3 diapers on some patients, which appeared to reduce how often they needed to be changed. * I observed a nurse speaking with a patient who had been given Haldol and morphine regarding a DNR form. Based on what I witnessed, I was concerned about the patient's ability to make an informed decision at that time. * My loved one developed a bedsore that was approximately 4 inches across and 1/8 inch deep on his back. The wound was severe enough that I could see exposed tissue. * In my opinion, many (though not all) staff members appeared indifferent to patients' needs. * I'm not sure whether the facility has been remodeled since then, but when my loved one was there years ago, there was a persistent odor of urine throughout the building. * In my opinion, the food quality was extremely poor. Of course, there's more, but if you have other options, I would strongly encourage you to consider them. If you must place a loved one here, be a strong advocate for them. Know your loved one's rights. Do not sign anything unless you fully understand it and are comfortable doing so. Be extremely observant. Take pictures. Document everything. Communicate with your loved one and assure them that they are loved and can tell you anything. Ensure that they are not being intimidated, mistreated, or abused. A heartbroken family member.

Thankfully the state has finally listened to so many and have penalized them and placing no new resident sanctions on them for the time being. I had two family members there and during that time saw 6 executive directors and 7 wellness directors. Under each one of them, we saw constant neglect from food services, housekeeping, bugs, incorrect med administring or not at all, unsupervised meds, and not following doctors orders or plan of care. In the last state report, it was found that close to ten residents needed a higher level of care that they were receiving from the Terrace and had to be removed. So revenue has always been the priority over care and wellbeing of their senior residents. Their number one push this year was the renovation, to bring in more residents and to compete in higher sales profit. $$$ SALES $$$$ We had requested several times for carpet to be replaced due to being dangerous for potential falls and was ignored. (see photo) After more than 10 plus meeting over a 3-month span regarding incorrect meds, deplorable housekeeping, unacceptable dietary issues and finally, our family member being hospitalized and going to inpatient rehabilitation due to lack of care from the terrace her health and welfare where too much of a risk to trust with this facility. She hated leaving the residents she became friends with and we worry for them. Below is the list of individuals that I and many other families have addressed and were constantly ignored and knew of problems.
