While the nurses were generally kind and professional, the conduct of some of the CNA staff was extremely concerning. A CNA made a threatening remark toward my mother, which was completely unacceptable. I also personally witnessed concerns regarding my mother's food. On one occasion, I raised the issue of food being left by the sink rather than being given directly to patients. Before I could even finish explaining what I had observed, one of the nurses acknowledged what I was describing and finished my sentence for me, confirming that this was something they were aware of. Despite replacement food being available, my mother was not provided with a replacement meal when needed. Another incident was particularly disturbing. While I was helping remove my mother from the facility AMA, a nurse clarified that one of the CNAs had been putting my mother in an oversized diaper. I initially did not understand the significance of this until the nurse explained that it was being done to make my mother feel embarrassed or silly. That behavior is completely unacceptable and deeply concerning for anyone entrusted with the care of vulnerable patients. The condition and maintenance of the rooms were also very poor--far below what I would expect from a rehabilitation facility and, in my opinion, worse than some budget hotels. I want to acknowledge that several of the nurses were kind and responsive, and I appreciated those individuals. Unfortunately, the behavior I personally witnessed, along with what was acknowledged to me by staff, overshadowed those positive interactions. The facility needs to seriously address CNA conduct, patient dignity, food service, and the overall condition of the rooms. No patient or family member should have to question whether their loved one is being treated with basic respect and dignity.

The institution claims that he saw a Dr. the first day but only one other time the last day before we took him home. He lost about 20 lbs in this period from refusing to eat the food and getting depressed. There is only one shower forall the patients on the 3rd floor and his surgical incision was supposed to be showered daily. There was no recourse to air our grievances. The Social Worker was hard to reach and not available to help us out. As his wife, I was constantly frustrated by the lack of oversight. I feel that the facility is highly understaffed and overworked. There was no way to reach the patients except by cellphone before 9 AM and after 5 PM. When I spoke to the Management they spouted their great 5 star reviews. But when I finally went to Yelp the reviews were horrendous. I see that they only got 1 1/2 stars. In terms of rehab - in the "gym" people were in wheelchairs heads bent - very sad to see. At least my husband did get a half hour of PT but no communication with family as to progress, etc. As I was leaving one day after my daily oversight visits a patient yelled out to me "WELCOME TO THE PRISON."

My wife…read moreplaced her ninety year-old mom in this facility, with the verbal understanding that her care would be totally covered by Medicare insurance, after disenrolling her from United Health Care. They did not make my wife aware that her mom would have to pay $209 per day for her stay. My wife was too preoccupied with her mom's needs and the appearance that the facility was caring: needless to say, this turned out to be far from the truth. After my wife took her out, Anchor Care made her aware that she owed over $16,000. While my wife acknowledges she should have gotten everything up front, in writing and not trusted a verbal assurance, it is clear now that the facility in not trustworthy. On another note, the care her mom received was subpar, but that's another story. Avoid placing your loved ones in this facility, but if you must, get everything up front, in writing and visit the facility daily to insure that they are providing the care you are paying for.

They have an advertisement which criticizes other rehab centers and…read moresay "They send you to a different therapist each time you visit" My first 3 appointments at Trinity had 3 different therapists. I had a problem and mentioned to the Trinity therapist how I would like to be able to measure and chart improvement in my strength. They had no method other than a guess. When I went to another therapist the first thing they did was pull out a computerized strength testing machine to chart a baseline which to measure my future progress. Trinity said it could not be done. My Sis in Law had a lot of pain and went to Trinity based on advertisement of epat - able to fix you pain "in 3 visits". Expensive and totally useless. They collected the deductible for my insurance and never credited it to or alerted my insurance company. They simply held the money in my account as a credit but never told me I had a credit and never applied it to anything. I was billed for ANOTHER deductible !! I had to chase them to fix it. The thing I hate most in all the world is calling a professional and teaching them to do their job. As soon as I went to another therapy provider, I recognized how very poor Trinity is.
