My mother only suffers from forgetfulness; otherwise, she is in good health…read more My mother has been staying here for over a year; I visit occasionally and bring along some pastries or desserts I've made. The nurses see this but don't say anything--they are very nice. However, lately, whenever I visit, I encounter a Vietnamese woman who works in my mother's room; she isn't a nurse but assists the patients. Nowadays, every time I come here to visit my mother, it feels like visiting a prisoner in a detention center. Ms. L. constantly grills me about what food I've brought and what ingredients are in it; she doesn't just ask upon our first meeting--she scrutinizes every time I was here with every single item I bring for my mother. Last week, I brought my mother a small piece of chicken pie (3x3 inches) and three-quarters of a cup of Jell-O--a very small portion, certainly not a full lunch. As soon as she saw me, she began questioning me relentlessly: "What is that food? Did you put chocolate in the Jell-O?" "What kind of pie is it?" She is neither a nurse nor a nutrition expert. She questioned me as if interrogating a prisoner. That wasn't the end of it; after I finished feeding my mother, she came back and asked, "Does your mother eat a lot or a little?" I replied, "Why do you ask that?" She said, "If your mother eats a lot, I'll have her lunch cut off." I felt like she was flaunting her power over my mother and me; I felt we were being abused. It was just a small 3x3-inch piece of cake and three-quarters of a cup of Jell-O--my mother only ate half of it--yet she threatened to cut off her lunch. Who was she in this rehab facility? How much authority did she have to throw her weight around like that? This is America, not Communist Vietnam. At 12:05 PM, I saw a nurse hand Ms. L. a small carton of milk (I guessed it was a nutritional supplement drink like Ensure)for my Mom. I was even more surprised when I saw that, instead of placing the carton of milk on the table--which was completely clear and uncluttered--she stashed it away in the drawer of my mom bedside cabinet !!! It struck me as odd...

Kimberly in admissions was incredibly helpful getting us started. Bambi (Director of RN) and Tammy (lead RN) have gone above and beyond helping coordinate my mom's discharge, along with Eric, the social worker, who's made a stressful process so much easier. Ciya, the physical therapist, and her team have been amazing -- making sure my mom gets PT and OT five times a week, which has made a real difference in her recovery. The facility itself is light, bright, and spotlessly clean, and every staff member we've encountered has been friendly and caring. We're so grateful for the whole team here.

They treat my parents with dignity and respect, and that means everything to me. I highly recommend this facility to anyone looking for quality care for their loved ones. Shout out to Tammy, Kimberly, Kim, Eric and the team for everything they do day in and day out!

Social worker who works here is unlicensed since California Law would require social workers to have a licensed social worker if the facility has 150 beds or more, in which this facility doesn't have. Without a license it's hard to keep the social worker accountable. Social worker here threatened us with laws, yet she herself wasn't licensed and our complaint to the California Department of Public Health for their wrongful actions caused the facility to receive a ticket. The social worker and management here tried to keep our loved one here longer than intended to continue to bill the insurance which was shady. Once Medicare started to not pay as much any more for short term care during the pandemic, then we find it that the social worker kept pressuring us to get our loved one out. The social worker here kept interrogating us if our loved one had 24 / 7 care when the nursing facility here had patients who became infected with Covid and passed and they themselves had elders who had falls within the facility. The owner had both a long term and a short term care facility. Their ultimate goal is to find a way to bill the patient's insurance to the max. or sense of actual care or compassion. Don't keep your loved one here. We had financial elder abuse and emotional elder abuse incidents with them. Luz was an unlicensed social worker and the most abusive and unprofessional social worker I've seen in my life. Let alone she has a thick accent. She acts like she's some sort of law enforcement officer when she wasn't even licensed, and her intent was to keep the elder at the facility longer to keep billing their insurance. She doesn't let the elder get the COVID booster. She acts like you don't have the right to talk to her. The most bit...y, ugly, non-compassionate. and most lazy social worker I've ever seen. Don't be afraid to argue against her. I am pretty sure many have verbally fought back against her. She should go to h..l for being abusive towards family and elders.

My loved one needed to come here on two occasions; on the first occasion she was well rehabilitated, and now on the second occasion she has made some incredible progress toward the goal. The facility is not fancy looking, but they do focus their resources on what it takes to care for your loved one. They have Physical Therapy to keep your loved one moving. They accomplish amazing procedures, sometimes without need for your loved one to go visit a hospital. Key leaders in the care include Kristian (who accurately describes his nursing role as that of "honcho) and of Sia Rasheed, Director of Rehabilitation. Ask them for weekly updates, and they are able to communicate well. There is a house MD, who keeps the medical procedures on track. Two people spend most of their time guarding the entrance. Those are Raffia K. and Mary Alice Puig, who will take your temperature and sign you in while directing you where to go for your visit. Unlike the past, reasonable visiting of your loved one is allowed much of the day. Previously, there was a limit, but no more. Once when they wanted my assistance, with a tricky IV insertion, I visited until 11 p.m. Parking might at first glance seem a problem, but it isn't. There are indeed only four visitor spaces, and those will almost always be full. Don't park in the one space designated for the doctor! However, there is very adequate parking on Country Drive, and also even on Parkside Drive. So, if you want good quality clinical excellence for your loved one, Fremont Post Acute Care is the right choice. An added plus is that two cats are frequently seen near the entrance.

I had fibrosis growing in my back and the two specialists I saw before Lidys did not mention it or try to address the problem at all.
