They did the best they can to make accommodations and help my patient become more independent. She is able to get discharged today after a month of staying there recovering from a hip fracture. As we remember our mom Sushila Amlani on the recent Mother's Day, we are filled with nothing but gratitude for the compassionate care and attention she received at Hillside Senior Care, the former name of Niles Canyon Acute Care. She was there for more than 4.5 years till her passing in Dec 2021, in her late 80's. We can't be grateful enough to the entire staff (from Management/ Administration to floor staff, in all the shifts), who made this facility a home away from home during her entire stay there and welcomed our family with warmth, any time of the day, even multiple times in a day. Not only did they take care of her physical needs, but also provided her technical help to use her cell phone / tablet so she could attend and listen to spiritual discourses, to nurture her soul too!! Some of the staff are fluent in some of the Indian languages, in particular Hindi, that made it easy for our mom to communicate. The entire staff (CNA, LVN, therapists, Social Services, Activities Room, Kitchen, Front Desk, Facility/Equipment/Grounds, etc.) is very caring, warm and considerate. Even during Covid, they did their best, in spite of shortage of staff, personal risks, and all other challenges and issues of dealing with a very dangerous and deadly pandemic. After the new owner and management took over this facility and renamed it, we continue to visit the facility periodically, to express our gratitude. We also continue to refer this facility to our friends for their loved ones. Some of the same staff still work there, remember our mom fondly and always welcome us. The facility is spacious, airy, bright, with all rooms having windows, and a nice garden outside, for walks, family gatherings, and occasional celebrations for the staff and residents. In summary, we are very happy and continue to recommend this facility to our friends for their family members, even after 4+ years because they have continued to maintain the same standard of care, per our occasional drop-in visits. I considered 2 other places before placing my mother in this facility. I am so glad I made the right decision! Nursing staff and administration here including the social worker Malou are caring and kind and We are more than grateful. The only reason I did not give 5 stars was the rude and lady housekeeper. She took it personal when I voiced out my concern. She skips rooms and does not properly clean up. I could see dust, food spills and crumbs including small pieces of trash for days and days on the floor especially under my mom's bed. Sometimes I wipe old spills that have been there who knows how long, considering that my family stayed there for months. I don't think she even mops weekly and it is concerning especially my mom has asthma and she is really sensitive to dust mites. I can't say enough good things about this place! I am the lead caregiver for a Deaf, Blind and sick senior citizen. She was transferred to Niles Canyon Post Acute after a two week stay in the hospital. On her first night, I accompanied her with my supervisor and we were geared up, ready to fight for reasonable accommodations. The reason was because other skilled nursing facilities turned down my patient for this exact reason. Eric, the manager, surprised my supervisor and I by not only being willing to accommodate her, but he was HAPPY to! It was incredible how understanding and flexible he was. There was nothing he wouldn't do! And his team of nurses and other staff were all equally incredible! Eric was shocked to hear that other places had been so horrible, which shows that he really, truly cares about the residents there. She stayed at NCPA for an additional two weeks until she felt well enough to go home. They loaned her all the medical supplies, including a large oxygen tank, tubes and a wheelchair, to hold her over until her insurance gave her permanent supplies. They have activities, meals, a care team, and more all attending to every patient's needs. The entire staff were SO friendly and worked SO quickly! I have had three experiences with Niles Canyon Post Acute (NCPA) over the past four years. People close to me were patients following their hospitalizations, a family friend I was coordinating care for (88 yr old), my mother (86 yr old) and mother in law (92 yr old). My perspective is as a nurse of 45 yrs and from the three different patient stays. I echo all the previous five star reviews. I admire the administrative team that is still in place and has been since before my first contact in 2020. That consistency and teamwork is integral and a huge benefit to their patients. I am talking about Eric-administrator, Hillary- admissions, Malu- case manager, Gemma- nursing, Gloria- PT/OT, Honey Lynn- nutrition, and other department heads. NCPS is a small facility, while the building is older, I have always observed it to be clean and maintained. The grounds outside are a very nice place to sit and enjoy the sun and fresh air. Being a small facility, it is easy to navigate and reach out to each department. They have always been responsive to our needs and very helpful. The staff work very hard and you can see that if you stand in the hall and just watch. The patient population all require a lot of care, motivation and support. They are staffed well and nursing assistants are very warm, compassionate, respectful and show great teamwork. Communicating needs, requests or concerns to nursing were addressed promptly by the staff and even by the DON- Gemma. The staff educated us on Medicare/HMO/etc requirements for the stay. They were advocates with insurance when needed. There is a facility physician and PA/NP that oversee care while at NCPA. When I have had concerns regarding one of my family, they were brought to the physician's attention by nursing promptly and addressed. The food is comparable to what I have seen at Washington Hospital, my family had no complaints. You can request drinks or snacks as needed during the day. Rehab is a transitional place to get you back home and is hard work. It takes time, requires engagement and effort from the patient, is painful at times, and emotionally stressful. It is best experienced with loving family to support and advocate for you on site as opposed to long distance if you can. Set up a care conference if you have concerns or need updates about yourself or loved one. The majority of the patients tend to be older than 60 yrs old with many older. One part of the facility is long term care and the other shorter term rehab. Sadly, in my family I have members who avoid elderly. Society in general I feel tends to do the same thing. So when I meet people that choose to do this hard work and do it well with genuine caring, I have great respect and appreciation for them as they are truly quality people. No one place is perfect. I can say from my many years of nursing experience that hospitals are not either, even the world renowned hospital I worked at for many years. It is how you communicate, respond, resolve and adapt to your patients and family needs. The staff at NCPA get the job done where they can. I feel the staff at NCPA have the best intentions, are approachable, responsive and address concerns directly. I feel patients and family share responsibility too and need patience, tolerance and to communicate their needs. The facility itself is well designed, pleasant, and maintained with well-managed grounds and surroundings. Its manager, Eric Sampson, is always available to listen; Hillary Andres, Director of Admissions, goes beyond providing you the ongoing support; and finally, Malu Agsaulio, Social Services Director goes out of the way to make sure that your interactions with the outside healthcare ecosystem is seamless, always responding with alacrity. The real engine of NCPA's success is their incredible work team--from nurses to the custodial staff--all always responding to whatever that the patient needs, even when their workload spikes because of resurgence of Covid and other seasonal viruses. Im sure it is a great place to people who cannot fend for themselves but it certainly was not for me and had to leave. The care here is decent for this type of facility. What makes it stand out more is the admin staff. They are way better than several others. They actually called multiple times and tried their best to help me with my mother. The admin staff was kind and helpful and tried to help even when the insurance companies were being horrible. The nursing staff was usually sweet to my mom and took care of her. I am glad my mom was able to get care here when she desperately needed it due to inferior care at a different facility. With the declining health of both of my parents over the past few years, we gained a tremendous amount of knowledge about the quality of skilled nursing facilities in the Bay Area. Niles was, by far, the best. The management and staff were top notch. The facility was always clean and well maintained. Being a smaller facility, I found the care to be more personal and the staff to be more patient-involved. The overall atmosphere was positive and much less stressed than others I experienced. NCPA is an excellent care facility. It has a peaceful, airy, walkable, shaded patio grounds with plenty of parking spaces. It also has bright and well-kept rooms. But what makes this facility truly top notch is its leadership team and of course, its staff. The leadership teams which includes its world-class administrators, heads of nursing, heads of social services, heads of activities & entertainments, and heads of PT and dietary dept. whom set the exemplary tone to all of its staff members by demonstrating (on a daily basis) that they are friendly, efficient, caring, altruistic, compassionate and professionals in meeting any needs and shown willingness to resolving any issues (ranging from majors to banal) when it comes to its patients and patients' family members needs. Even though a typical care facility is a hive of activities (control chaos on a daily basis) of each patients needs and activities, NCPA leaderships teams and its diligent staff consistently delivered outstanding care to all of its patients and patients' family members. While at first I would have given less stars because of a difference of opinion as to what "Care" is, Hillside has since followed through with expectations. Family taking care of family is always the best option, but when it can't happen, then a care facility is needed. Having said this, family should not check out of their loved one's life, but visit as often as possible. Those that keep in touch ensure better care for their loved one. I never see a smile on any face at the front desk. From my very first visit I was looked at as if I had intruded. Not a warm reception. But the other folks that work there, the CNA's, nurses on the floor, housekeeping, all have smiles and are very kind. Language seems to be a slight barrier with some of the staff. My family member has had a little difficulty understanding or being understood. The facility seems to be staffed sparingly, so if someone needs attention that's not considered life threatening, they may have to wait until their turn comes around. This seems to be the average these days for a facility of this caliber. Since having contact with the facilities Doctor, my family member is being looked after, it seems, with a little more attention? Different ailments have been detected and monitored. I appreciate that. I would say that keeping the residents reasonably comfortable is the goal. There doesn't seem to be enough staff to ensure everyone gets outside every day when possible. A guest of a resident would have to take that upon themselves. I think It's happen a couple of times with a staff member in the one month my family member has been there. My brother makes sure our family member gets out and a couple of friends once treated my family member to this as well. Shelter, medication, therapy, hygiene (to a point) food and some sort of mental stimulation rounds out the high lights. The care team being caring would be included in that. I believe they do their best with the number of residents they are assigned and the few people there are to take care of them. To recommend Hillside? I would never recommend a care facility if it can be helped. Hillside is okay. My family member has fallen a couple of times, but I don't think it was the fault of the staff. It's a heartbreaking scenario with a dose of helplessness. I think I can recommend Hillside. But remember, be vigilant, be assertive and be involved. My 93 year old mom was a resident at Hillside Senior Care from this past October to January of this year. Her rehabilitation was for a minor fracture to her right pelvic bone that did not require surgery but required physical therapy and managed care. The injury resolved with the passage of time and sessions of physical therapy to the extent that she was able to return to her original base line of mobility with a walker; unaccompanied. The facility is nestled on a corner lot on Mission Blvd at Pickering in Fremont. The facility has a park like atmosphere that adjoins the parking area. Currently, the facility is undergoing some remodeling that for all intents and purposes was needed. The facility is designated for long and short term skilled nursing patient care. The exterior grounds are well manicured and clean. Weather permitting, a nice outdoor area to sit and soak up the sun and fresh air. There is steady interior cleaning of the hallways and residents' rooms. Meals are served three times daily and the quality and quantity of the food is actually pretty good. Meals are prepared and tailored to the residents dietary needs. With regards to the staff and medical attention... I can not say enough about their level of care. The physical therapy department excels in their patient rehabilitation. The therapists take a very pro active approach in patient care with an emphasis on strength, health and safety. Their approach is to each resident in my observation, has always been that of positive reinforcement and encouragement. No resident is forced to do what they are incapable of doing. The nursing and CNA staff are hands down remarkable. Keep in mind that this is not an easy job. Yet, each and every staff member has always been dedicated, cheerful and upbeat. Moreover, each and every resident is shown compassion and respect regardless of their physical or mental condition. The staff calls residents by their first names, and respect the resident's privacy. For example, they knock on doors before entering and provide clear communication of all procedures. In addition to the nursing staff, there is a doctor that makes rounds frequently and you are kept well informed of the provided and managed care. Yes, I recommend Hillside as you can tell and not just from the point of view of how well my mom was treated; this is their standard, that I have personally observed. Also know that upon discharge, you are provided with assistance and information that will aid you in wherever your family member's destination may be. There are to several individuals to single out from staff to supervision that in doing so may make this review resemble an award program. Simply know that ALL have essential roles and perform that duty at a high level. Your family member is in very good and capable hands. Staff is very unresponsive to patients needs. Delayed incontinence change for patients. Medication is delayed or mixed up. CNAs are lazy and complain to patients about the job, that they are overworked and underpaid. The food served is high salt, processed food from packaged boxes and cans. There is no fresh fruit, no fresh vegetables, no frsh mad meat. Patients are not given showers on scheduled days, or patients are not showered at all. Physical therapy is not effective. Physical therapist barely know what is going on with their patients. They can barely handle patients and the needs for recovery. Care plan from care team is not followed and is inefficient. There is no improvement with patients that are in need of recovery. Staff really doesn't show of empathy and care to get patients back taking care of themselves in any levels. The management is not responsible. The CNAs are poorly trained and don't understand the patients. Nursing are probably the only staff members that do there job close to 90% of the time. This place may not be the worst nursing home, or the worst in Fremont but they are not a good place to stay or have a person you know stay there. It doesn't make a difference on how involved you are. It is a 24 hour skilled nursing home with post acute physical therapy. There should be 24 hour care that is efficient, caring, and with knowledgeable staff that can take care 9f their patients in a better way then if they were home. Especially if they charge you an outrageous monthly payment. Most important help better there patients in there condition and bring them value. F.Y.I. pay attention to the physical therapy time amount, exercise type the physical therapist manager says the patients is receiving. Its untrue. There is no real initiative to help and guide patients to learn a new way of taking care of themselves so they may have some kind of normalcy and/or any level of their independence back. Niles Canyon Post Acute is a good choice for a skilled nursing facility. Niles Canyon has a nice botanical garden surrounding the premises. Most importantly is the cleanliness of the facility; the rooms/bathrooms are up to par with hospital standards and the facility is doing a good job maintaining the daily cleaning & sanitizing. The staff is attentive to the needs of patients and tries their best to accommodate requests from family members. Recommend 4 stars to Niles Canyon & once all the renovation is complete & the facility maintains the good standard of patient care, rating could go even higher. This is a second rehab center my loved ones has been admitted to. Compared to the last one, this is way better. facility is cleaner. Many of them are very professional. Especially with the rehab part of it. When my friend was admitted, he was very weak and unable to move on his own. After they trained him for about 14-15 days he was able to walk with the front wheel walker. Everyone is involved and helping me with all the question i have for my friend without any hesitant. They also encourage patient to socialize which I like very much. I don't wish anyone to need for rehab, but if needed, this is place to go. read more

After my hip replacement, I was sent to Hillside rehab center. I was worried before I went there, especially at this pandemic time. I end up stayed there for more than 6 weeks. I have had very satisfied experience there. They strictly followed rules, clean. Nurses, Physical therapist and occupation therapist are all very nice, kind and helpful, they obviously like their work, are doing their best. The food is also very nice, the care center has done studies, so the food is tasty, easy to chew, with right quantity, and with very good nutrition management. My mom was admitted in what we would call a massive crisis. She walked into the hospital for a brain tumor biopsy early one Friday morning and she was delivered to Hillside the following day via ambulance under Hospice care with a life expectancy of fewer than 12 weeks. I did not have time to do comprehensive research before I entrusted them with the responsibility of my mother's end of life. I visited my mom every day (sometimes 2x a day) for the last 5 weeks of her life. I don't know where to begin complimenting Hillside. Let's start with first impressions, the facility is clean and smells fresh all the time. The residents milling about the halls all seems happy and comfortable. Every staff member, even the cleaning ladies were genuinely kind. I even got a tour of the laundry room and it was clean, organized and run by a proud sweetheart! Malu, Lola, Eva and the rest of the team took great care of my mom. They took the time to get to know her and really made an extra effort to make her as comfortable as possible. They made special meals for her and always made sure she had her ice water. They gave her an extra shower and always treated her with dignity during the most difficult time of our lives. As my mom declined the team kept her pain under control. They worked hand in hand with Hospice right to the very end. The ladies at Hillside were like a bunch of loving Anuties taking care of everything so that I was able to just be my mom's daughter and focus on my mom's end of life, not her actual care. It was the worst 5 weeks of my life and these ladies held me up and supported me in ways I did not know possible. I can not recommend Hillside enough. read more

My wife spent nearly a month at Hillside recuperating from a month long hospitalization. She received excellent physical and occupational therapy while there. The therapists were from a company that Hillside hired to provide that service. Having that therapy was the main reason for her stay at Hillside so, in that respect, one might say her stay was a success. My wife's illness required her to have a 5 day hospitalization during the time she was at Hillside. Hillside would not reserve my wife's bed during that time unless we paid $300 per night. That seemed a little unreasonable to me but maybe it's normal within that industry. The facility seemed reasonably clean to me. While not hospital sterile, it was okay. It did not have any long lasting objectionable odors. There were no issues we experienced with broken or unusable features so I would say that upkeep was good. That was especially true of the grounds which were quite pleasant. The most charitable comment I could make of the food was that it was uninspired. In addition, it was almost always served at room temperature. The kitchen had difficulty following dietary direction. It was written on a card but not always followed. All plates, dishes, glasses, and cups were made of plastic. Only the steel utensils were other than plastic. The majority of the staff that did all of the personal service were friendly and seemed to want to be helpful. They were almost always cheerful and responsive to my wife's needs. One difficult issue with the staff is that a large majority of them had accents. They seemed to be from India and some were nearly impossible to understand. The same was true of the management people I met. My wife complained about how noisy the staff would be when entering the room at night. She had an opportunity to compare them to hospital staff who made an effort to be quiet when entering her room at night. At Hillside the staff often made use of their "daytime voice" when entering the room at night. My wife said it was quite jarring to be woken as these people barged into her room while speaking with such loud voices. One other issue that made sleep somewhat difficult is what my wife called "the screamer". She was an elderly woman who would scream in an unbelievably loud voice at odd times during the day or night. The sounds she made were like a person in pain and they would sometimes go on for a half hour at a time. I don't have a large amount of experience with facilities like Hillside. My wife's grandmother was in one for several years and it looked much larger and seemed more professional than Hillside. But that's about the extent of my experience. read more

I apologize for this long post, but please read: If you are looking for a facility that will provide the BEST CARE services for your loved one, then this is not the right facility for you. I would encourage you to continue researching and really find one that you and your loved one will be comfortable and confident in. My 89 years old grandmother (who only speaks Cantonese) suffers from congestive heart failure and other complications was transferred to this facility from the hospital for short-term rehabilitation. Within 30 minutes of her transfer, I realized this wasn't the best place for her. First, none on the staff there spoke Cantonese. Therefore, communication was going to be an obstacle. So I made it a point to be there with her most of the day to help with translation. Second, the CNA's and RNs were not the most polite people and did NOT have the best bedside manners. I witnessed gross negligence and incompetence from these health care professionals who should know to take precaution in my grandmother's situation or any other situation with patients. They rough handled her while getting her in and out of bed. In one incident, the RN literally picked my grandmother up from bed and tossed her onto the commode. My grandmother's feet never even touched the floor. I had an exchange of words with the RN but she didn't seem to care about my concerns. Another incident is the way the RN was giving my grandmother her medication. I knew my grandmother needed to take small sips of water while taking her medications. I told the nurse that but it didn't seem to matter. She took hold of the back of my grandmother's neck and literally forced the water down her mouth. I physically had to push the cup down so my grandmother could catch her breath. The RN handed me the cup and walked out. At this time, my grandmother was still coherent and was able to carry on a conversation with me. It was difficult to hear how sad and uncomfortable she felt at this facility and how they have handled her so far. They were very unprofessional. This broke my heart. At this point I didn't want to complain too much because we were there for a day. I didn't want them to treat my grandmother worse if I weren't there. So, I didn't make a complaint. Two and a half days later, we got a call from the facility that my grandmother had passed away in the middle of the night. We were shocked! I have accepted the fact that my grandmother had passed away. I want to believe that she passed peacefully in her sleep. What I'm having difficulty with is how she was treated the days PRIOR to her passing. The experience she had to go through when family wasn't there. If the RN's and CNA's can treat a human being this poorly when family is sitting right there, then I can't imagine how they are without someone there watching them. It breaks my heart to have seen what these skilled, health professionals can be capable of at these facilities. I encourage you to do your research before settling with one. I couldn't give this place a minus rating so here goes 1-star which they don't even deserve. My dad went to this place to recover and he ended up at the emergency room after few weeks due to dehydration. Really? They often didn't have his gown on, his phone was always put away from his reach, his colostomy bag was never drained on time and the nurse call button was disconnected few times. His belongings such as clothing went missing on regular basis. They do not care for the patients here. After the Emergency visit, we changed the skilled nursing facility from here to Fremont Health Center. My dad says this new place (Fremont Health Center) is way better than the Niles Cayon Post Acute where he didn't like the care he was getting there. He actually came out worse than before. Now after 3-weeks at the new place, he looks 10 folds better. Avoid this place at all cost if you like your family. If you want to get rid of your family rather than them to recover, this is the place. read more

The RNs and office staff are helpful, approachable, and listens to any concerns. . Noticed that some CNAs do really know how to care for their assigned patients; but others just do the minimal things of what their job entails. Day to day care of our family member could be better read more

My mother was admitted into Hillside Senior Care two weeks ago after a spinal decompression and fusion surgical procedure. The staff was extremely friendly, knowledgeable and do care about all their patients. The case worker Malu was very helpful and explained thoroughly the treatment my mom was going to receive. My mother was taken care of properly, given her medication and physical therapy as prescribed by her doctor. We're very happy with this amazing institution. It is clean, they have a beautiful garden, there is enough light and everyone, including the cleaning ladies, are polite and courteous. I highly recommend this place and will take my mom again in a flash. Thank you, Malu and amazing nurses for everything you did for my mom; we will be eternally thankful. read more

My 88 year old mother-in-law, Janet, has dementia and was cared for at home by family for three years. It became too much and she briefly went into a board and care home where she sustained a foot lesion. In January, Janet went into Hillside Senior Care for rehabilitation, where she has remained. We have the highest praise for this facility. It is clean, bright and welcoming. Janet has received excellent care and her foot lesion is almost healed. The staff is astoundingly kind and caring, and they seem happy to be at Hillside. I don't want to start naming people because I wouldn't want to leave anyone out. At lunch I've observed staff members patiently feed entire meals to people who cannot feed themselves. read more
