I needed ADA title IX forms for work so that I could miss work whenever I was having a mental health day or if I needed to come in for those treatments. Apparently, in very small print on their website, they say that they do not do any disability forms which I think is really absurd when you have a mental health condition that's considered to be a disability. This was a massive waste of my time because now I'm starting back over to find a psychiatrist. I would not recommend them to Anyone.
